The U.S. Food and Drug Administration revealed in a letter to healthcare providers that it's investigating numerous medical device reports describing patient infections and other possible contamination issues potentially associated with reprocessed urological endoscopes. Dr. Larry Muscarella delves into more detail on reprocessing best practices and some of the challenges therein. He also shares his key takeaways from the FDA letter — for clinicians, regulators and healthcare consumers alike.
This week, Prof Marco Bruno, Department of Gastroenterology and Hepatology at the Erasmus University Medical Centre in Rotterdam is explaining about the problem we have with scope contamination. Last year his department published a Nationwide Study of bacterial colonization of endoscopes. The study included 74 Dutch Centres and reported that 17% of duodenoscopes and 13% of EUS scopes were contaminated! What can we do about this? With Marco, I discuss the current situation and possible solutions, like new endoscope designs, continuous monitoring and culturing of scopes and the training for reprocessing colleagues.

Endoscope drying times and the use of an alcohol rinse has become an area for debate and emerging research is increasing with each passing day. Inadequate reprocessing of flexible endoscopes led to 30,000 patients being called for follow-up testing between 2005 and 2012, according to one estimate. Contaminated endoscopes can spread infections like HIV and hepatitis B and C, and even in the absence of an outbreak, media coverage of a recall can damage the facility's reputation. ECRI Institute's James Davis and Jackie Ferenschak discuss why endoscopes are difficult to reprocess effectively and strategies for helping staff succeed. In ECRI’s Top 10 Health Technology Hazards for 2019, we identified mishandling flexible endoscopes after disinfection on the list as a potential source of danger that we believe warrants attention. This week on beyond clean we speak with Jenni Gibbs, Manager of Central Sterile Department at Altru Health System. She began her work in healthcare in 1989 doing inpatient nursing. She holds a Bachelor of Science degree from the University of North Dakota. Gibbs has a background in project management and Lean. As CS manager, Jenni developed a dedicated scope team and a career-growth track for her employees to include paid certification education and technician levels. In May 2017, Altru’s Central Sterile department moved to centralized, organization-wide endoscope reprocessing program. Altru was the inaugural US facility in June 2018 to sterilize duodenoscopes using hydrogen peroxide and ozone technology.
