In this JCO Precision Oncology Article Insights episode, Miki Horiguchi summarizes two articles: “Biomarker-Driven Oncology Trial Design and Subgroup Characterization: Challenges and Potential Solutions” by Wang, et al. published on June 7, 2024, and “Biomarkers in Oncology: Complexities in Biomarker-Driven Studies and Statistical Analysis” by Uno, et al. published on July 22, 2024. TRANSCRIPT Miki Horiguchi: Hello and welcome to JCO Precision Oncology Article Insights. I'm your host, Miki Horiguchi, an ASCO Journal Editorial Fellow. Today, I'll be providing summaries for two articles.   The first article is a review article titled, “Biomarker-Driven Oncology Trial Design and Subgroup Characterization: Challenges and Potential Solutions,” by Dr. Jian Wang and colleagues. Biomarker driven clinical trials represent a key component of precision medicine, focusing on tailoring treatments to patients based on specific biomarkers. By identifying and targeting therapies to patients who are most likely to benefit, these trials aim to improve treatment outcomes and reduce adverse events.  The article highlights several important points to optimize biomarker driven clinical trials. The authors first reviewed US FDA approvals in biomarker defined subgroups and conducted an in-depth analysis of key regulatory considerations. They developed an innovative decision tree to guide designing biomarker based clinical trials. In addition, they clarified the statistical challenges, including ones found in the all-comers study design. The authors found that most of the US FDA approvals are being restricted to the biomarker positive subgroup, indicating that observed treatment benefits in the overall population are heavily influenced by the biomarker positive patients. This raises concerns as the treatment effect in the biomarker negative subgroup may be smaller but still clinically meaningful. Additionally, achieving adequate statistical power for the biomarker negative subgroup is often not feasible. These factors could limit access to the treatment for biomarker negative patients who might benefit from it. To address these challenges, the authors introduced various statistical methods and conducted numerical studies to compare the performance of several of these methods. They found that a promising approach is a Bayesian Dynamic Borrowing Method that leverages evidence from the biomarker positive subgroup to evaluate the treatment effect in the biomarker negative subgroup. The authors emphasize that any statistical method used for subgroup analysis must be prespecified. Proactive engagement with regulatory authorities and alignment with the guidelines before finalizing study designs and analysis plans are also essential.  The second article is an editorial which accompanies the first article, "Biomarkers in Oncology: Complexities in Biomarker-Driven Studies and Statistical Analysis” by Dr. Hajime Uno and Dr. Miki Horiguchi. In this editorial, the authors introduced additional statistical considerations that can further enhance informed decision making based on the results of biomarker driven oncology clinical trials. Specifically, the authors raised three key points to consider.   Number one is controlling the type 1 error rate. The qualitative assessment of a new treatment involves a statistical test, while regulatory decisions consider the totality of evidence rather than evidence based solely on P values. Statistical tests play a crucial role in determining treatment benefits in each of the three analysis populations, that is, the biomarker positive, the biomarker negative, and the all-comers population. The type 1 error rate of a statistical test is the probability of rejecting the null hypothesis when it is actually true. The threshold for the type 1 error rate is conventionally at 0.05. The threshold value can vary depending on the situation, but maintaining the type 1 error rate at the nominal level is essential to ensure the reliability of the conclusions drawn from a statistical test. Any inflation or deflation of the type 1 error rate from the nominal level can lead to significant issues in regulatory decision making.  Number two is choosing robust and interpretable quantitative summaries of treatment effect. Statistical tests provide a binary outcome aiding regulatory decisions like drug approval. However, quantifying the magnitude of the treatment effect is more informative for clinicians and patients when assessing the risk benefit balance of the treatment. Therefore, the choice of a summary measure to quantify the between group difference is also important. Dr. Wang and colleagues use the Cox Hazard ratio in their study, which is the most common summary measure in oncology trials. Yet this measure relies on several assumptions. Specifically, when it is applied to biomarker driven trials, the proportional hazards assumption must hold in both biomarker positive and biomarker negative subgroups.   In addition, when a stratified Cox analysis is used to integrate the hazard ratio of the two subgroups to derive the hazard ratio for the all-comers population, there is an underlying assumption that the hazard ratios from the biomarker positive and biomarker negative subgroups are the same. These assumptions do not usually hold in practice, and violations of these assumptions can compromise the interpretability of the estimated between group difference and its generalizability to future patient populations. It has also been discussed widely in both statistical and clinical journals that the hazard ratio is difficult to interpret because of the lack of absolute hazards from the treatment and control groups. To address these limitations, Doctors Uno and Horiguchi suggested using alternative summary measures, including restricted mean survival time and average hazard with survival rate, which do not share these limitations and offer more robust and interpretable results than the conventional hazards ratio approach.   Number three is using coherent statistical analysis models for the three analysis populations. In the first article, Dr. Wang and colleagues introduced a Bayesian Dynamic Borrowing approach. The primary analysis of their approach borrowed information from the biomarker positive subgroup only when analyzing the biomarker negative subgroup. They did not perform the borrowing when they analyzed the biomarker positive subgroup. The accompanying editorial highlights the potential bias introduced by this asymmetric approach. Specifically, suppose the treatment effect in the biomaker positive subgroup is pronounced, but that in the biomarker negative subgroup is weaker. In this case, their asymmetric approach produces a more favorable result for the biomarker positive subgroup compared to the symmetric approach, where each subgroup follows the information from the other subgroup. Providing a convincing rationale for using an asymmetric approach or conducting a sensitivity analysis with a coherent approach for both subgroups would be required.  To conclude, biomarker driven oncology trials are diverse and complex, requiring a tailored approach to statistical analysis that considers the unique characteristics of each trial. The Bayesian approach represents one useful analytic approach, but might not be a universal solution for all biomarker driven studies. Further discussions among stakeholders, such as those from regulatory authorities, clinicians, and biostatisticians will stimulate further research on the optimal design and analysis methods for biomarker driven clinical trials in precision oncology.  Thank you for listening to JCO Precision Oncology Article Insights and please tune in for the next topic. 与HORIGUCHI堀口咖啡的两位成员录制本期播客的时候,堀口咖啡刚刚庆祝了上海外滩源门店的四周纪念日。 回顾日本母国,堀口咖啡也已经成立了30多年。在日本这样一个咖啡深入人们日常的国度,堀口咖啡从创立之日起,就极其重视生豆品质,追求鲜明原产地特色的深烘焙咖啡,在日本业内也被称为日本精品咖啡的先行者。堀口咖啡采纳了不同数字来代表咖啡烘焙的不同风格和种类。每款都按经验以时令不同产地咖啡豆混配而成,首先采用酿制红酒的理论混拼,确保长年能享用到独特香气的咖啡。不论是日本,还是上海的顾客,当人们步入堀口咖啡充满复古风味的精致门店中,都会在此处品尝到用1-9数字命名的不同风味咖啡豆。2021年9月,堀口咖啡正式入驻TSUBO衡山坊概念店一楼。相处4个月以来,我们看见了堀口咖啡认真而严谨冲煮咖啡的一面,也看见了他们与客人们交流时随性又可爱的一面。TSUBO RADIO:The Last Table第三十五期,我们邀请了来自HORIGUCHI COFFEE堀口咖啡的Joe与Taylor,一同加入圆桌讨论。本期主题: 喝咖啡时候的心情很重要|堀口咖啡。欢迎大家收听。嘉宾介绍JoeHORIGUCHI COFFEE堀口咖啡品牌经理,致力于推广日式咖啡文化。中日混血,自小在中国香港和日本东京两地生活和成长。提倡人们去觉察品尝咖啡时的当下心情,为自身那一刻的心情来挑选合适的咖啡。TaylorHORIGUCHI COFFEE堀口咖啡店长,负责HORIGUCHI位于上海外滩源和TSUBO分店的门店日常管理工作。大学期间就在咖啡店打工,毕业后自己曾经短暂从事平面设计工作,因更喜欢咖啡带来的随性和纯粹氛围,随后在兰州本地开了一所咖啡馆,后来来到上海加入了HORIGUCHI。MaxTSUBO户外品牌主理人、品牌创意总监;同时也是摄影师、健身爱好者。作为自幼在澳大利亚成长的华人,Maximus曾就读金融与哲学双专业。早在成为TSUBO品牌主理人之前,便是户外文化的先行者。本期内容节选Host:Amber嘉宾:Joe, Taylor, Max03:00Taylor跟其他咖啡店相比,我们比较特殊一点是使用号码1-9来区分每款咖啡豆,没有将产地特别明细化,当然我们所有配方都是公开的。主要原因是希望客人挑选咖啡以口味为主,而不是以产地。比如1号是明亮的果酸和果香,如果客人喜欢果酸和果香,就可以选择这一款。 咖啡属于农作物,即使同一产地,每个产季的咖啡豆都会不一样。像埃塞豆可能到了一些特殊产季,质量不是特别好,我们就需要进行替换,以此保证每个号码的咖啡豆保持原有的调性。08:00Max我从小在澳大利亚长大,那里最受欢迎的还是奶咖。澳大利亚主要有两种移民,一部分是历史上英国当时最早去的,另外一部分是第二次世界大战期间跑过去的难民,很多来自希腊、意大利。这两个国家的人日常都是习惯饮用咖啡,并且他们偏好咖啡味道苦的、重的,也就是浓缩咖啡,再加上澳大利亚的牛奶品质又很好,所以就出现了澳白、拿铁。 15:00Joe相比中国,日本会更早接触到咖啡。目前日本的精品咖啡店还是比较多。不过跟国内不一样,比如说某一个咖啡品牌,一来到上海就很多人会过去打卡。日本人还是比较喜欢去他/她家里附近的咖啡厅,而且这些咖啡厅的出品都已经很好了。因此人们不太会集中去追某个咖啡品牌。54:00Max我经常出差去意大利,渐渐才知道,Latte是意大利语,其实是牛奶的意思,在意大利,Caffè Latte才代表我们经常说的拿铁咖啡。在意大利,牛奶咖啡其实是早餐的饮品,如果你中午或下午跟店员要一杯牛奶咖啡,对方会觉得很奇怪,就像我们中午跟服务员要一份豆浆油条一样。 01:04:00Taylor比起其他行业,从事咖啡行业,或者说服务行业,工资相对来说不是那么高。比如说我大学学习设计,现在做设计可能待遇会更高,但是咖啡相对更加自由。咖啡里头还有一些匠人的东西,还有咖啡领域的专业知识。我认识的一直从事咖啡行业的人,不论是做Sales、CS、培训还是其他,包括非常专业的咖啡师,相对来说都会更随性一些。01:16:00Joe挑选哪一款咖啡?我觉得看客人当时的心情,心情很重要。有时候你早上看到一位美女,心情非常好,你可以挑选一款2号Flowery & Juicy。如果你刚刚失恋了,心情苦涩,你可以挑选一个8号或者9号非常Smoky的。我们的咖啡豆都有专业配方,但最主要的还是你今天的心情。本期背景音乐Hip Hop Lo-Fipixabay.com

But in Japan, it's a little more complicated. On Valentine's Day, only women give chocolate, and not just to their partners, but to their male colleagues too. Critics say the practice sucks all the fun out of Valentine's Day and instead turns it into a dreary duty where women risk offending co-workers if they leave someone out. Others say "giri choco", which translates to "obligation chocolate" is a little misunderstood, and besides, it's slowly fading as women opt to give chocolate to their friends instead. 'Obligation chocolate' Of course, giving chocolate on Valentine's Day can also be a romantic gesture. Women will often give "honmei choco" or "true feelings chocolate" to their partners. But giri choco is more about expressing appreciation to male colleagues. A 2017 survey by multinational firm 3M found that nearly 40% of female respondents planned to give giri choco to a co-worker. The ones they admire would get chocolate, while the incompetent ones could buy their own treats. "In other words, it could be seen as one of the few opportunities for women to exercise power over men, resisting prevailing gendered norms," says Sachiko Horiguchi, an anthropologist at Temple University Japan. More than two decades later, this might seem a little less appealing to Japanese working women. "I am not sure if these professional women feel obliged to 'exercise their power' through giri choco gift giving," says Ms Horiguchi. Chocolate battle Last year the practice attracted an unexpected critic in the form of Belgian chocolatier Godiva. The company took out a full page ad calling for an end to giri choco. "Valentine's Day is supposed to be a day when you tell someone your pure feelings. It's not a day on which you're supposed to do something extra for the sake of smooth relations at work," the ad said. They followed up this year with a tweet to Yuraku Confectionery, the makers of Black Thunder, a low-cost chocolate and self-styled "king of giri choco". Serotonin, is another neurotransmitter that is integral for some of the most intense and positive emotions we feel in our daily lives. www.jolsid.com Before we jump into the technical side of Serotonin, I want to share some of the benefits that it has in your life and how it indirectly impacts your success. Also, how Serotonin is Different than Dopamine. Appropriate levels of Serotonin  promotes the feeling of comfort, contentment and well-being. It has also been called a “happy” neurotransmitter.The  deficiencies in serotonin can lead to depression and fear. Feeling contentment and sense of well-being is responsible for continuous progression. If we lack contentment and sense of well-being in our daily lives then we opt to live daily on instant gratification lifestyle or things that provide thrill and excitement in the moment. Even though instant gratification is healthy for us in the moment, it tends to create addictive behaviours which could eventually lead to habits that can be destructive for our long term growth and well-being.   For example If we start eating 2 sweets  per day and each time we eat them we get a combination of thrill, benefit and exhilaration for that specific task at that moment (not in the future but at that very moment). This is also simplified to instant gratification that makes individuals become addicted to substances or certain destructive habits. Because the combination of thrill, benefit and exhilaration in the moment sends a wave of Dopamine in our brain telling us that we are happy and excited in the moment, which is not necessarily bad. But If we repeat this action every single day eventually 2 sweets or the reward will become the new baseline and we will not feel the same feeling of happiness that we used to. Now we will have to  increase the stimulus or in this case those sweets in order for us to achieve the same level of excitement or happiness we felt with the 2 sweets initially. That’s where we lack the feeling of contentment and get busy in the chase of that emotion that we felt the first time.   Serotonin is the neurotransmitter that rewards us when there is slow progression and it keeps us motivated though the journey as opposed to seeking the reward now and abandoning the path of growth and slow progress.   Now let's talk about science and history of Serotonin. Serotonin was first discovered by Vittorio Erspamer in Rome in 1935. But it was not isolated and named until 1948 by Maurice M. Rapport, Arda Green, and Irvine Page of the Cleveland Clinic. Serotonin is a neurotransmitter released by neurons in the brain. It's made from the amino acid tryptophan, which is a component of protein that we consume in our daily routine.  Tryptophan can be found in foods like Brown rice, Turkey and Chicken. The majority of Serotonin in humans is present in the enterochromaffin cells of the digestive tract, where it is involved in regulating movement of the gut.   Effects of Serotonin Relaxation A "feel-good" hormone, serotonin helps people to feel relaxed and content. According to Macalester College. Low serotonin levels are linked to anxiety and depression. Like many hormones and brain chemicals, serotonin functions best when it's at optimal concentrations. Normal levels of the chemical promote relaxation, but be cautious that extra Serotonin won't necessarily increase feelings of well being--it may just make the individual feel sleepy. But one of the 2010 studies on excess serotonin viewed sleepiness as One of the benefits of excess serotonin. As sleepiness, which is not necessarily a benefit at the wrong time of day, is certainly welcome at night.   This study in the scientific journal "Sleep" notes that if the brain's serotonin transporters are misshapen, patients experience insomnia. Further, individuals who have difficulty sleeping have long been advised to try a glass of warm milk before bed. Milk, like many sources of protein, contains the amino acid tryptophan, which signals the brain to make more serotonin and which may produce feelings of sleepiness. There is also some speculation that the familiar "Thanksgiving Day coma," wherein people feel tired after a big meal, may be due at least in part to high levels of tryptophan found in turkey meat.   Serotonin receptors are also being considered as targets for drugs useful to treat psychosis and cognitive impairment in schizophrenia according to the research conducted by  H. Y. Meltzer, B. W. Massey and M. Horiguchi. The conclusion of their research was and  quote “Multiple 5-HT receptors contribute to effective treatments to reverse adverse effects of NMDA-RA which model psychosis and cognitive impairment.”   How do you know if you have low levels of Serotonin? Some of the symptoms of low Serotonin are: anger carbohydrate cravings and binge eating constipation digestive disorders feeling overwhelmed insomnia joylessness low self-esteem migraines poor cognitive function There are also Serotonin blood test that you can request from your Licensed Physician or family doctor. And they can discuss potential treatments if needed.   Why should Entrepreneurs, Creatives and Athletes care about Serotonin. Entrepreneurs, Creatives and Athletes ideally deal with high amounts of stress. Which can eventually result to low levels of Serotonin, which can adversely impact your performance and can even eventually make you give up on your goal, aspirations and dreams. Especially if you experienced combination of stress and loss or poor performance at the same time. Entrepreneurs, Creatives and Athletes success is not in the moment but is a slow process or journey. Sometimes they are even called futuristic when it come to planning their lifestyles and their goals. The Hammer MMA Radio crew are on the way to Montreal for UFC 186: Johnson vs. Horiguchi.  Joining us on that trip, as well as on this episode, are Fightland contributer Shawn Smith and The Hammer MMA Canada host Rob Veevers. This is an extra-long two hour episode that begins with a huge preview of UFC 186. There hasn't been a ton of mainstream interest in this event but we break down the entire card from the UFC Flyweight Championship main event to the Fight Pass exclusive Women's Strawweight fights. Following that Rob Veevers takes over the middle segment of the show to discuss MMA in Canada, including a likely Titan FC event in June, a Global Warriors event in Ontario next month, and some discussion as to why live MMA has essentially disapeared from the province of Ontario. Finally we wrap up this massive episode with a full recap of last Saturday night's UFC On Fox: Rockhold vs. Machida, and a preview of Friday night's Invicta FC 12: Kankaanpaa vs. Souza.
