Guideline evolution is driven by ongoing research; recent studies like PeriOperative ISchemic Evaluation-3 Trial (POISE-3) and trials such as OPtimisation of Peri-operaTive CardIovascular Management to Improve Surgical outcomE II (OPTIMISE II) trial may influence recommendations on hemodynamic management and the use of tranexamic acid. Future guidelines are expected to integrate machine learning models for risk prediction, addressing complex patient phenotypes and interactions. Discontinuation vs. continuation of renin–angiotensin system inhibition before non-cardiac surgery: the SPACE trial has challenged existing practices regarding the discontinuation of ACE inhibitors or ARBs, highlighting the dynamic nature of medical guidelines and the continuous incorporation of new evidence to enhance patient care. Hugh Bonneville was the first voice on air, followed by a wide range of local people from around the area and our presenters Joff Lacey and Claire Vennis. Enjoy that little moment of local broadcasting history as our team marks four years on the air, and hear Jack and Tod reflect on their role with the radio station here in Petersfield. This piece looks at the DanGer Shock Trial which examines the efficacy of the Impella device in reducing mortality in patients with acute myocardial infarction and cardiogenic shock. We explain how the device works, its implantation process, and the significant findings from the trial, including a notable reduction in six-month mortality with a number needed to treat (NNT) of eight. Despite higher rates of bleeding and renal filtration therapy in the Impella group, the trial provides strong evidence supporting its use in high-volume centers with trained personnel, emphasizing the importance of careful patient selection and timely intervention. Presented by Andy Cumpstey and Joff Lacey with their guest Vasileios Panneudales, an interventional cardiologist at Brompton and Harefield Hospitals.
Recorded exclusively at this year's Evidence Based Perioperative Medicine (EBPOM) World Congress in London this piece looks in detail at High-Intensity Interval Training (HIIT). The discussion also explores the integration of experimental medicine from bench to bedside, emphasizing the multidisciplinary approach involving surgeons, scientists, and anesthetists. Presented by Andy Cumpstey and Joff Lacey with their guests Colleen Dean, Lecturer in Muscle Cell Biology and Malcolm West, Associate Professor in Colorectal Surgery and Prehabilitation Medicine at the University of Southampton, an honorary appointment as a consultant colorectal and complex cancer surgeon at University Hospitals Southampton NHS Foundation Trust.
Evidence Based Perioperative Medicine (EBPOM)'s incredible World Congress in London continues and TopMedTalk is there bringing you exclusive interviews with delegates, guests and key opinion leaders. Here we have a much needed discussion regarding regulating artificial intelligence (AI) at the point of care, and the need for proper regulatory frameworks to ensure its safe and effective use. We stress the importance of clear policies and clinician training for AI implementation, highlighting the potential risks of misusing AI tools due to insufficient understanding of their outputs. How do we get transparency from AI manufacturers regarding the data sets used and the intended applications of their algorithms? Is there a need for independent verification bodies to assess the robustness of AI tools? The conversation concludes with a call for collaboration among clinicians, manufacturers, and regulators to harness the power of AI in healthcare while safeguarding patient outcomes, underscoring the exciting yet challenging future of AI in medicine. Presented by Andy Cumpstey and Joff Lacey with their guest Lee Fleisher, Emeritus Professor of Anesthesiology and Critical Care and Professor of Medicine at the University of Pennsylvania Perelman School of Medicine, Founder and Principal at Rubrum Advising, LLC, Senior Advisor of the Bipartisan Policy Center and FasterCures of the Milken Institute, Senior Fellow of the Leonard Davis Institute of Health Economics and Visiting Fellow of the Duke-Margolis Center. This piece provides "a thumbnail sketch summary of the fluid debate". Challenging the "prevailing wisdom" regarding fluid management, the kidney and volume overload; "as a young Doctor you sort of got the impression that if you got the kidney to handle a lot of fluid somehow it would be happier". Sodium and volume overload are in fact issues and extra work for the kidney and have tended to cause more difficulties than they solved. This podcast is crucial to understanding Enhanced Recovery After Surgery (ERAS). The ideas outlined in this series all lead up to it. In short, the length of stay as part of a patient's post operative recovery should be less than 3 days with over 90% of your patients mobilised on day one. This in depth discussion relates to the consensus paper from the Perioperative Quality Initiative (POQI), see link below. It focuses on: outcomes, length of stay and DREAMS (Drinking, Eating and Mobilising) within hours after major surgery. These are all measures of compliance and success in an Enhanced Recovery Program. Monty Mythen and Joff Lacey with Mike Grocott presenting alongside contributions from Dr Julie Thacker, Colorectal Surgeon from Duke University, President of the American Society of Enhanced Recovery (ASER) and Dr Tim Miller, Anesthesiologist from Duke University. The POQI website is here: http://www.poqi.org For further CME read the paper in Perioperative Medicine: https://perioperativemedicinejournal.biomedcentral.com/articles/10.1186/s13741-017-0062-7 -- Love this, want more? Perioperative urine output is a vital sign which can have a very significant impact on outcomes. This discussion emphasises the importance of monitoring it and using the data accordingly. It then moves further into permissive oliguria within an enhanced recovery protocol. This shows no difference in creatinine values and patients are not reporting that they feel uncomfortable or thirsty as a result. Presented by Professor Monty Mythen and Dr Joff Lacey featuring guests Dr Tim Miller from Duke University Medical Centre and John A. It's about "optimal analgesia" rather than aiming for an entirely "pain free" approach. America's opioid crisis brings the importance of this concept into sharp relief and this episode of The Perioperative Coach reflects on the pain management discussions had by the American Society of Enhanced Recovery (ASER) and its members. Presented by Monty Mythen with Dr Joff Lacey and Mike Grocott, Professor of Anaesthesia and critical care at the University of Southampton.
