A., Schabrun, S. M. Abstract: Transcranial magnetic stimulation (TMS) has been used to examine the inhibitory and facilitatory circuits during experimental pain and in chronic pain populations. However, current applications of TMS to pain have been restricted to measurements of motor evoked potentials (MEPs) from peripheral muscles. Here, TMS was combined with electroencephalography (EEG) to determine whether experimental pain could induce alterations in cortical inhibitory/facilitatory activity observed in TMS-evoked potentials (TEPs). In Experiment 1 (n = 29), multiple sustained thermal stimuli were administered over the forearm, with the first, second and third block of stimuli consisting of warm but non-painful (pre-pain block), painful heat (pain block) and warm but non-painful (post-pain block) temperatures respectively. During each stimulus, TMS pulses were delivered while EEG (64 channels) was simultaneously recorded. Verbal pain ratings were collected between TMS pulses. Relative to pre-pain warm stimuli, painful stimuli led to an increase in the amplitude of the frontocentral negative peak ~45ms post-TMS (N45), with a larger increase associated with higher pain ratings. Experiments 2 and 3 (n = 10 in each) showed that the increase in the N45 in response to pain was not due to changes in sensory potentials associated with TMS, or a result of stronger reafferent muscle feedback during pain. This is the first study to use combined TMS-EEG to examine alterations in cortical excitability in response to pain. These results suggest that the N45 TEP peak, which indexes GABAergic neurotransmission, is implicated in pain perception and is a potential marker of individual differences in pain sensitivity. Abstract: Background: The angular gyrus (AG) is involved in numerous cognitive processes, and structural alterations of the AG are reported in many neuropsychiatric diseases. Because abnormal excitability or connectivity of such cortical hubs could precede structural alterations and clinical symptoms, approaches assessing their functional state are needed. The combination of transcranial magnetic stimulation (TMS) with electroencephalography (EEG) can provide such functional readouts by probing how the cortex responds to direct stimulation. Objective: To characterize TMS-evoked potentials (TEPs) elicited by AG stimulation, determine optimal stimulation parameters, and identify TEP biomarkers of AG function. Methods: In 19 subjects, we recorded AG-TEPs using four TMS orientations and three intensities and compared their spatiotemporal features using topographic dissimilarity and microstate analyses. We also explored the link between AG-TEPs and TMS-evoked muscle activity. Results: Early AG TEP components of interest (P25, N45) showed topographic variability dependent on stimulation parameters. The P25 topography was sensitive to TMS orientation and less to intensity, whereas the N45 topography was highly dependent on both coil orientation and intensity. However, TMS-evoked muscular activity was also dependent on coil orientation and the dominant topography of N45 was strongly related to this muscular activity, indicating that the component may reflect somatosensory-evoked responses to this peripheral activation. Conclusions: The earliest AG TEP component P25 likely reflects neural processes triggered by direct AG activation and could provide an index of local excitability. N45 must be interpreted with caution as it may mostly reflect peripherally evoked activity. Coil orientation can be optimized to minimize muscular contractions. Abstract: GABAA receptor (GABAAR) - mediated inhibition participates in the control of cortical excitability, and its impairment likely contributes to the pathologic excitability changes that have been associated with multiple neurological disorders. Therefore, there is a need for its direct evaluation in the human brain, and the combination of transcranial magnetic stimulation (TMS) and electroencephalography (EEG) might represent the optimal tool. TMS-evoked brain potentials (TEPs) capture the spread of activity across the stimulated brain network, and since this process at least partially depends on the GABAAR-mediated inhibition, TEPs may constitute relevant biomarkers of local GABAAergic function. Here, we aimed to assess the effect of GABAARs activation using TEPs, and to identify TEP components that are sensitive to the state of GABAAergic inhibition. In 20 healthy subjects, we recorded TEPs evoked by sub- and supra-threshold stimulation of the primary motor cortex (M1), motor-evoked potentials (MEPs) and resting-state EEG (RS-EEG). GABAARs were activated (1) pharmacologically by oral administration of alprazolam compared to placebo within each subject, and (2) physiologically using a sub-threshold conditioning stimulus to characterize the effect of short-latency intracortical inhibition (SICI). In supra-threshold TEPs, alprazolam suppressed the amplitude of components N17, N100 and P180, and increased component N45. The pharmacological modulation of N17 correlated with the change observed in MEPs and with the alprazolam-induced increase of lower {beta}-band RS-EEG. Only a reduction of N100 and P180 was found in sub-threshold TEPs. TEP SICI manifested as a reduction of N17, P60 and N100, and its effect on N17 correlated with the alprazolam-induced N17 suppression and {beta} increase. Our results indicate that N17 of supra-threshold TEPs could serve as a non-invasive biomarker of local cortical excitability reflecting the state of GABAAR-mediated inhibition in the sensorimotor network. Furthermore, the alprazolam-induced increase of {beta}-band oscillations possibly corresponds to the increased inhibitory neurotransmission within this network. Edge TEP IP can be on the same subnet and VLAN as the local hypervisor TEPs. This is beneficial for collapsed cluster topologies where Edge Node VMs are running on vSphere Hosts which are configured for NSX-T, as Transport Nodes. There are a lot of topologies where there is a collapsed Compute & Edge or Management & Edge Clusters where this feature is beneficial. In this interview, we talk to Dr Jane Foster, educator and creator of MYTERN - a program designed to help people change their language around emotion and regain control! TERN = Take Emotional Responsibility Now. As an educator, she was concerned about the false narratives that were, and still are, embedded in our culture, disrupting people's lives and even ending them. She wanted to create a simple, every day language that could be used by everyone to build new positive narratives, eradicating stigma and judgement, enabling people to regain control of their mental and emotional state. A language that could replace blame, judgement and retaliation with empathy, compassion and responsibility. The culture of having to be happy and positive all the time is unrealistic and almost impossible to maintain - especially now. In this interview, Dr Foster talks about - her PhD project where she sent one text message per day to university students over a ten week period. She had astonishingly positive results in the test group, including increased psychological well-being, life satisfaction and purpose. They also had decreased psychological distress levels and there were several students who said that the program prevented them from committing suicide. - how MYTERN can be used in schools, homes and occupational settings. - how the program revolves around metaphors such as roads- you can be on a red (rough road) or a green (smooth road) in terms of your emotions. They are not necessarily "bad" and "good" roads- and we don't want aim for always being on those smooth roads. The red roads can help us with our resilience, as we will always come up against challenges in life. The idea is to be in control - whether we are on the red or the smooth roads. - how you can be out of control on red OR green roads. You can be on a green road and out of control - eg taking alcohol / drugs. You can also be in control on a red road - eg angry but responding in a calm way. - how being out of control in MYTERN means that you are hurting yourself, hurting someone else or hurting something else. - how the language can be simplified to - "are you on a red or green road? Are you in or out of control?" - how the analogy of the steering wheel works - YOU are in control of the car (your emotions) and how you respond. Other people can not make you feel a certain way. "YOU made me feel this way" is giving the other person all the power. and making yourself a victim. - MYTERN teaches you how to empty your glass - how little stresses build up over time and fill our glass. Minor stresses can help build health and resilience. - how people of different ages can use the MYTERN model - from young children and adolescents to adults (even corporations) - how sign language can be used to indicate when you're feeling out of control (extending the fingers, flexing the thumb across the palm) - the acronyms of PETs and TEPs.  - PETs are Personal Emotional Tools that help us regain control esp when we are on red roads. A PET could be going for a walk, taking some deep breaths, patting a pet, reading a book, cooking a nice meal, dancing etc. Adults will often choose alcohol, exercise or eating chocolate - which are fine in moderation, but in excess can become a TEP. - TEPs are Triggers for Extra Precaution - ie the events/situations that cause someone to feel out of control. For example - parents are fighting, losing a computer game, bad drivers, feeling unwell or going to school/ work. Calling it a "TEP' rather than a "trigger" means that there is no judgement. It's a made up word so that there is no connotation associated with it. Understanding your own TEPs (and what PETs can help in that particular situation) gives you skills to help bring yourself back into control. - how these tools can help lay down new neural pathways and some of the changes that Jane has seen over time within groups using the MYTERN model. - the different packages available on the MYTERN website - eg therapist pack, teenager pack, family pack, adult pack. Deze week hebben we weer te maken met zo’n gastspreker. Den Teps komt ons gezellig vervoegen om wat te keuvelen over het Vijverfestival. Hij zorgt namelijk voor de programmatie en zit al 7 jaar in de organisatie van het evenement. Wij kennen hem als de papa van Anders (en Kirsten), onze homie, maar ondertussen is hij wat figuurlijke papa van iedereen. But I thought they couldn’t aspirate? Right….but there is still dysphagia. See all that residue? This affects quality of life. If you have to swallow 10 times per bite, you might start to wonder if you energy could be better used on other pursuits. When pressure gets so built up in the pharynx that patient’s TEPs have leaks after 1 week, we have to get creative. There is limited research on this population, but what we are starting to hear is very interesting! Laurie Wennerholm shares the latest research and current best practices for assessment and treatment of dysphagia in a population we never used to treat for dysphagia.
