David Okita has flown helicopters on Hawaiʻi Island for many decades, first for emergency responders assisting with fire department search and rescues and then later for conservation organizations like The Nature Conservancy and the Department of Land and Natural Resources. David reflects on how flying for him has been a family affair, growing up on Hawaiʻi Island beginning with his dad in the 1970s, and now with his son as his assistant. He speaks to the unique relationship between helicopter pilots and the forestry and biology field crews he supports–one of mutual respect and admiration for the skills and hard work required to build fences, monitor ecosystems and conserve endangered species in the remote wilderness.

Hey daar lieve luisteraar, Lou hier. Nog steeds vanuit het hart van Miracle Town in Costa Rica. Deze week gaat het in the Miracle Town Radio Show over Identiteitscrisis en het afscheid (moeten) nemen van oude Identiteiten. Ik had zelf niet in de gaten hoe sterk mijn (oude) identiteit van moeder (en daarna nana) is, terwijl mijn kinderen allemaal volwassen zijn en mij niet meer nodig hebben zoals toen ze klein waren. Dit op zich is natuurlijk logisch, maar ik had niet in de gaten dat ik hén nog altijd nodig had als bevestiging van mijn bestaan, terwijl ik een ander bestaan aan het opbouwen ben..! Dit blijkt een klassiek fenomeen bij Identity Shifts. De zgn IDENTITY DESTABILISATION OF THE OLD REFERENCE SYSTEM. Wanneer je stopt met jezelf te reguleren via externe bevestiging, voelt het systeem tijdelijk onbeschermd. Niet omdat je zwakker bent geworden, maar omdat je een oude regulatiestrategie niet meer gebruikt. De oude identity reguleerde zich via - nabijheid - bevestiging - respons van belangrijke anderen (In mijn geval dus mijn kinderen) De nieuwe Identity reguleert zich via: -interne stabiliteit -zelf referentie -structurele identiteit (Bij mij de Empire State Being) En precies op dat overgangspunt voelt het soms alsof je ‘bloot' bent. Niet omdat je minder stabiel bent, maar omdat je stabiliteit nu van binnenuit komt in plaats van van buitenaf. Dat is de identiteitscrisis. Niet het verlies van jezelf. Maar het verlies van de oude manier waarop je jezelf stabiliseerde. Wanneer je afscheid neemt van een oude identiteit kunnen er momenten zijn waarop je zenuwstelsel nog reageert vanuit de oude structuur. Niet omdat je perse terugvalt maar omdat je zenuwstelsel aan het leren is dat het zichzelf nu anders stabiliseert (of moet leren te stabiliseren) Je stabiliteit is dan niet langer afhankelijk van hoe anderen reageren, maar van wie je besloten hebt te zijn. In deze radio show weer reuze aha's bij mezelf hoe het vasthouden aan mijn ‘core' identity als moeder/nana mijn succes in de weg zat. Verder een gesprek over hoe je als Identity verder moet zonder je partner en wat te doen met al die ‘negatieve emoties' als schuld, schaamte, boosheid, verdriet etc. Want die zitten je manifestaties toch in de weg?! Dat hoeft dus helemaal niet. Hoe? dat hoor je in deze show. Het is een heel eerlijk en transparante show geworden, waarin ik de schellen die me weer eens van de ogen zijn gevallen openhartig met je deel. For our february episode we finally complete the 70s by discussing our favourite songs of 1974! You can expect lots of prog, much more krautrock than expected (particularly from Tracey), some funk classics, sparkly glam bangers, and most of the members of The Byrds.We've each chosen our 10 favourite songs of the year and sent them over to Colin's wife Helen, who put the playlists together and distributed them so we were each given a playlist of the 20 songs from the other two hosts, along with our own 10. We then ranked the playlists in order of preference and sent them back to Helen, who totalled up the points and worked out the order. Time-boxing wasn't new to him either—when you're flying, you only have so much fuel, so deadlines aren't arbitrary constraints but physical realities that demand disciplined execution. In this episode with Christian Boucousis, we also discuss the brief-execute-debrief cycle in detail. In this segment, we also refer to Cynefin, and the classification of complexity. Alignment: The Real Purpose Behind Ceremonies "It's really important to make sure everyone understands why you're doing what you're doing. We don't brief, execute, debrief just because—we do it because we know that getting everybody on the same page is really important." One of the most valuable insights Nate brings to his work with software teams is the understanding that Agile ceremonies aren't bureaucratic checkboxes—they're alignment mechanisms. The purpose of sprint planning, daily stand-ups, and retrospectives is to ensure everyone knows the mission and can adapt when circumstances change. Interestingly, Nate notes that as teams become more high-performing, briefings get shorter and more succinct. The discipline remains, but the overhead decreases as shared context grows. The Art of Knowing When to Interrupt "There are times when you absolutely should not interrupt an engineer. Every shoulder tap is a 15-minute reset for them to get back into the game. But there are also times when you absolutely should shoulder tap them." High-performing teams understand the delicate balance between deep work and necessary communication. Nate shares an aviation analogy: when loadmasters are loading complex cargo like tanks and helicopters, interrupting them with irrelevant updates would be counterproductive. But if you discover that cargo shouldn't be on the plane, that's absolutely worth the interruption. This judgment—knowing what matters enough to break flow—is something veterans develop through high-stakes experience. Building this awareness across a software team requires: Understanding what everyone is working on Knowing the bigger picture of the mission Creating psychological safety so people feel comfortable speaking up Developing shared context through daily stand-ups and retrospectives Why Veterans Make Exceptional Scrum Masters "I don't understand why every junior officer getting out of the military doesn't just get automatically hired as a Scrum Master. If you were to say what we want a Scrum Master to do, and what a junior military officer does—it's line for line." Nate's company, Form100 Consulting, specifically hires former military officers and senior NCOs for Agile roles, often bringing them on without tech experience. The results consistently exceed expectations because veterans bring foundational leadership skills that are difficult to develop elsewhere: showing up on time, doing what you say you'll do, taking care of team members, seeing the forest through the trees. These intangible qualities—combined with the ability to stay calm, listen actively, and maintain integrity under pressure—make for exceptional servant leaders in the software development space. The Onboarding Framework for Veterans "When somebody joins, we have assigned everybody a wingman—a dedicated person that they check in with regularly to bounce ideas off, to ask questions." Form100's approach to transitioning veterans into tech demonstrates the same principles they advocate for Agile teams. They screen carefully for the right personality fit, provide dedicated internal training on Agile methodologies and program management, and pair every new hire with a wingman. This military unit culture helps bridge the gap between active duty service and the private sector, addressing one of the biggest challenges: the expectation gap around leadership standards that exists between military and civilian organizations. Extreme Ownership: Beyond Process Management "To be a good Scrum Master, you have to take ownership of the team's execution. If the product requirements aren't good, it's a Scrum Master's job to help. If QA is the problem, take ownership. You should be the vessel and ownership of the entire process of value delivery." One of Nate's core philosophies comes from Jocko Willink's Extreme Ownership. Too many Scrum Masters limit themselves to being "process people" who set meetings and run ceremonies. True servant leadership means owning everything that affects the team's ability to deliver value—even things technically outside your job description. When retrospectives devolve into listing external factors beyond the team's control, the extreme ownership mindset reframes the conversation: "Did we give the stakeholder the right information? Did they make a great decision based on bad information we provided?" This shift from blame to ownership drives genuine continuous improvement. Building Feedback Loops in Complex Environments "In the military, we talk about the OODA loop. Everything gets tighter, we get better—that's why we do the debrief." Understanding whether you're operating in a complicated or complex domain (referencing the Cynefin framework) determines how tight your feedback loops need to be. In complex environments—where most software development lives—feedback loops aren't just for reacting to what happened; they're for probing and understanding what's changing. Sprint goals become essential because without knowing where you're headed, you can't detect when circumstances have shifted. The product owner role becomes critical as the voice connecting business priorities to team execution, ensuring the mission stays current even when priorities change mid-sprint. Guest Air Accident Updates We look at some air accident and incident updates in the press: NTSB Chair Slams FAA for Ignoring Repeated Warnings, Says Midair Collision Was Preventable In this C-Span video, NTSB Chair Jennifer Homendy slams the FAA, saying that it let its guard down and ignored repeated warnings that ultimately led to the deadly midair collision near Washington Reagan National Airport on January 29, 2025, that killed 67 people. “We should be angry, because for years no one listened. This was preventable. This was 100 percent preventable,” Homendy says. Passengers on JetBlue Plane That Plummeted When Flight Computer “Glitched Out” Sue Airbus for Negligence The JetBlue A320 experienced a sudden “uncommanded loss of altitude,” initially attributed to a software glitch that Airbus believed could result from intense solar radiation. This led to an emergency worldwide recall in November 2025, with an estimated 6,000 A320-series aircraft affected. Three passengers on that flight are now suing Airbus for allowing an “unreasonably dangerous” glitch in the system to go unchecked. Those passengers question the solar flare explanation and claim that JetBlue was aware of a recurring issue with the Elevator and Aileron Computer (ELAC) system. No defect found in switch of jet grounded by Air India – regulator Pilots on an Air India B787-8 noticed that during engine start, the fuel control switch did not “remain positively latched in the run position when light vertical pressure was applied”. This happened twice. The third time, the switch operated normally. The Directorate General of Civil Aviation (DGCA) said that when the switches were operated according to Boeing’s recommended procedure, they were found to be “satisfactory”, staying at run instead of moving to cut-off. The DGCA reported that, when incorrectly handled, the switch would “move easily from run to cut-off”. Three United Airlines Flight Attendants Seriously Injured After Air Traffic Control Failed To Alert Pilots Of Turbulence The NTSB has released its final report on the February 10, 2024, incident where a United Airlines 777-200 experienced a sudden drop, seriously injuring three flight attendants and throwing several passengers and an unsecured baby into the ceiling. The NTSB concluded that a contributing factor was air traffic controllers’ failure to alert the pilots of turbulence in the area. TSB Canada Releases Update on 2025 Toronto CRJ-900 Crash Investigation Photo Credit: TSB Canada. The accident occurred at Toronto/Lester B. Pearson International Airport (CYYZ) on February 17, 2025, when the CRJ-900 regional jet attempted to land and struck the runway hard, causing the right wing and tail section to separate from the fuselage. The plane flipped upside down and slid to a stop. The wreckage was transported to a hangar while the right wing, landing gear, wing box structure, and related parts were taken to the Transportation Safety Board of Canada (TSB) Engineering Laboratory in Ottawa for testing. The metallurgical analysis of the wing and landing-gear fractures is now being examined. Recordings from the flight data recorder, cockpit voice recorder, quick access recorder (QAR), and the enhanced ground proximity warning system have been examined. Data from prior flights on the same aircraft were also compared to the accident flight for context. Evidence was collected from NAV CANADA's Toronto surface movement guidance system and the airport's closed-circuit television footage. A full meteorological review, including wind data and soundings, has also been finalized to understand weather conditions at the time. Simulator sessions have been run. Aviation News Under Pressure American Airlines CEO Robert Isom Tells Staff That Everything Will Be Alright In New Video Message In a new video message to staffers, American Airlines chief executive Robert Isom says, “As we look forward to 2026, it's with a lot of excitement and confidence. I know we're going to do better financially and operationally. We have a plan to be solidly profitable this year, which will mean good things for our customers, our shareholders, and all of you.” Isom articulated strategies concerning the customer experience, hub facility investments, maximizing the power of the AA network and fleet, new routes, new frontline team members, new aircraft, AAdvantage program changes, and doing a better job selling the AA product. Trump's U.S. Transportation Secretary Sean P. Duffy Doubles Down on Purging DEI From Our Skies, Calls on Airlines to Affirm Pilot Hiring is Merit-Based The FAA is issuing a new mandatory “Operations Specification” (OpSpec) requiring all commercial airlines to commit to merit-based hiring for pilots formally. The mandate (New Mandatory OpSpec A134, Merit-Based Pilot Hiring, for Certificate Holders Conducting Operations Under 14 CFR Part 121 [PDF]) is effective February 13, 2026. U.S. Transportation Secretary Sean P. Duffy said, “When families board their aircraft, they should fly with confidence knowing the pilot behind the controls is the best of the best. The American people don't care what their pilot looks like or their gender—they just care that they are most qualified man or woman for the job. Safety drives everything we do, and this commonsense measure will increase transparency between passengers and airlines.” ALPA Statement on Pilot Training and Qualification Standards Capt. Jason Ambrosi, president of the Air Line Pilots Association, Int’l (ALPA), issued a statement that included: “All ALPA pilots are trained and evaluated to the same uncompromising standard regardless of race, gender, or background. A pilot’s identity has no bearing on their ability to safely operate an aircraft. What matters is training, experience, and qualification — and on that front, there are no shortcuts and no compromises. “At ALPA, our work to advance aviation safety never stops, and as always, ALPA airline pilots remain ready to safely transport passengers and cargo to their destinations. Safety is, and always will be, our number one priority.” Portland International Jetport logs busiest year in its history The Portland International Jetport had a record year in 2025, with more than 2.59 million passengers traveling through the airport. The previous record was 2.44 million in 2024. The Jetport says it became the first airport in New England to surpass its pre-pandemic passenger levels in 2023. Southwest Airlines Turns a Corner as Activist Investor Elliott Walks Away Elliott Management, an activist investor, began to acquire shares of Southwest stock in mid-2024. It bought enough shares to gain board representation and dictate the Airline's strategic and financial changes. As a result, we saw paid assigned seating, baggage fees, expiring travel credits, and adjustments to its Rapid Rewards program. Southwest also looked at asset sales and balance sheet strategies to fund share buybacks. But in late 2025, Elliott began reducing its stake, and by early February 2026, its ownership dropped to about 9 percent. Two Elliott representatives have resigned from the board at Southwest. Southwest Airlines Now Faces a Flight Attendant Backlash As Hand Luggage Woes Grow Southwest moved a dedicated overhead bin for crew members to the back of the plane, and flight attendants are not happy. Passengers sitting at the front of the plane who have paid more or have elite status find that bin space is an issue.
