    Recently Kate and I attended this conference in Brazil. It was 4 days jam packed with information in the sexual health field. People from all over the world attended and from many different professions. Many urologists, sexologists, a few nurse practitioners, occupational therapists and many more. I have attended this conference in previous years and it is always exciting to update skills and think about what we can incorporate into our care to assist more people in their intimate lives and cancer journeys.    Today we are going to discuss the big takeaways we got from this years conference. In future episodes I will discuss some of these topics in more detail but for today is an overview.   I chose to attend mainly presentations around prostate cancer rehabilitation, peyronies disease and relationship issues.     179: Robert- Everything he has done is hard won! A prostate cancer story. Spice up your rehab!

    This is a very special episode featuring none other than Victoria Cullen from “A touchy Subject” and Robert, a longtime patient of both Victoria and me (Melissa Hadley Barrett). Robert had an open prostatectomy in May 2009 and has been on a journey of self-discovery ever since. He initially saw men's health Dr in Brisbane for 4-5 years until he went searching for other options. He injected twice a week for years a mysterious mix … he did achieve an erection, but this was not ‘wife friendly” as no spontaneity. He was only 57 years old at this stage and disappointed with the loss of what was previously a good intimate relationship. Unfortunately, he received advice that vacuum pumps were a waste of time and used injections for rehab, this became costly and tedious and eventually he ran out of steam 2016 and gave up. He felt shame about loss of function.    Along the way he met various health professionals whom he asked if they could help and usually, they didn't respond well until he was eventually sent to a sex therapist who convinced him that outercourse was the way of the future. Finally in 2021 he was introduced to Victoria's work by a physio he saw to assist with incontinence issues, and he hasn't looked back. Through Victoria, he met me and Tess Deveze and continued to spice up his rehab and life.   This is a story of perseverance, patience, resilience, and of finding pleasure and joy. But what happens if your pelvic floor is too tight? Well, you can have too much of a good thing.   Today we talk to Archie, a 29 year old guy who has a tight pelvic floor. This caused him issues in the bedroom. When your pelvic floor is too tight it can be a contributing factor to sexual dysfunction in both men and women. For women it usually results in pain and in men erectile dysfunction can be an issue. Professor Chris Coombes and Dr. David Dangerfield perform this surgery to return erectile function to men post prostate cancer treatment. Following Dr Dangerfield's TV interview, I was inundated with requests for more information so here it is.   Those of you who are longtime listeners of The Penis Project may remember Episode 46-way back in August 2021 where Jo Milios and I interviewed Dr. Dangerfield on this life changing surgery, and following in that we interviewed a patient who had been through the surgery (Episode 83).     Today on The Penis Project Project podcast I speak with Louise Hug. I work closely with Louise as she sees many of the men that I see pre-prostatectomy and she does women's pelvic health too, so we cross paths quite a bit. Sometimes men tell me snippets of their consultations with Louise, and she has some great tips to share and ways of explaining pelvic floor training. So, for today's episode I have asked her to share them with you.  About Louise:   Pelvic Health Physiotherapist  MClin (Pelvic Health and Continence)  BSc (Physiotherapy), APAM, MACP  She completed her Undergraduate Bachelor of Physiotherapy at the University of Notre Dame, then commenced work at a private practice shortly after graduating. In 2021 she completed a Post Graduate Certificate in Continence and Pelvic Health Physiotherapy, followed by completion of Clinical Masters, majoring in Pelvic Health and Continence Physiotherapy. Following Graduation, she worked with Dr Judith Thompson (Physiotherapist PhD) who I have interviewed previously the podcast.    She worked musculoskeletal physiotherapy for eight years treating neck, back and pelvic girdle pain with both hands-on and gym-based treatment. In 2015 she developed an interest in core activation, breath work and muscle balance for rehabilitation, subsequently completing a Cert IV in Pilates.   In her early sporting life, she was a full-time gymnast until the age of 13 and enjoyed tennis, pilates, swimming and barre. These experiences gave her an understanding of injury management and prevention. This is what motivates her to assist others with their return to daily life and sport, not only post-injury but also after life events such as vaginal delivery and caesarean section, as well as gynaecological, breast cancer and prostate surgery.    She has completed several courses upskilling in Women's and Men's Urinary Continence and Ano-Rectal Dysfunction expanding her knowledge, leading to a passion in the field of Pelvic Health. Jonathon thought he would get prostate cancer, as his family had a history of it, and he was proactive in getting a diagnosis and watched it for 4 years before finally being advised it was time to remove it in December 2022.   This is an excellent, honest conversation with this lovely couple where they discuss their fears, treatment and recovery journey.     Men's health and the importance of having a regular primary health care practitioner Today we speak to GP Dr Lloyd about all things men. So when should a man start having regular checkups? According to Dr Lloyd, “No set age but the sooner the better. “ Men and women are different, women have time points in their lives where they have to see a GP such as hormonal changes, childbearing, breast checks etc. but for men it is different and often health is treated like firefighting. Men just go to the Dr when they have a fire that needs putting out. In this episode we look at why starting earlier is a better option for men, the health system, and their loved ones. Dr Lloyd says: “So medicine's complicated, it's fascinating, but it just shows how one thing, one issue needs a whole wider approach, not just deal with the problem, and brush under the carpet and move on. I think that's what guys do wrongly, isn't it? They'll see a GP; we call it firefighting. So, you quickly put that fire out, and actually, there's an inferno going on behind.     154: The Solo Sidekick - ‘The Eureka' prostate patient

    In this episode, we revisit Anthony the Eureka patient from episode 112 and 113. Anthony was diagnosed with prostate cancer in August 2022 at age 53. He was determined to get through it with the best possible outcome. He decided to document his journey in film and post on YouTube, in the hope of helping other men and their partners in the same situation. Jo Milios interviewed Anthony in December 2022 to hear all about his early days recovering and today we will see how he is going another year down the track. He is now cancer free, with almost no side effects.  He has also written a song about prostate cancer which we play at the intro and outro of this episode.   In my experience many men tell me tell that their life has been improved by their prostate cancer journey from various aspects, improved health, rekindling of past hobbies and interests and a deepening of relationships.     Peyronies Disease Explained Did you know that prostate cancer survivors have a marked increased risk of developing Peyronies disease? The prevalence of diagnosed PD has been estimated to be 0.5% in adult males. PD is thought to happen in about 6 out of 100 men between the ages of 40 and 70. It's rare in young men but has been seen in men in their 30's. The incidence of PD increases after prostate cancer treatment; however, studies vary in statistics. Malcolm candidly shares his post prostatectomy experience, elevated Gleason scores and the need for ongoing radiation therapy post-surgery.  He also reflects on his lifelong role as being the one standing strong when everyone else falls down, and the significant adjustment it was to allow himself the rest and care he truly needed. Think on the bright side it's all “POSITIVE” from here on!!!! Touch base with your local HIV organisation. They will have counselling services and/or a Peer Navigation programme or be able to point you to one. In Perth there is POWA(Positive Organisation WA). Find an HIV+ Peer and get peer support or even a group of peers. In perth we have a monthly brunch for People with HIV. Even if you don't think you need counselling engage with one, its surprising how even when you think you have things sorted how the discrimination affects you. Engage with your specialist/Dr around your treatment options you don't have accept the first one they suggest. It's your own body you decide what goes in it and there are multiple HIV treatment regimens available now with different side effects what is best for me is not necessarily the best for you.
