On today's special bonus episode, recorded live from the Road to Majority Conference in Washington, D.C., put on by the Faith and Freedom Coalition, I was able to sit down with John Spagnola, President and CEO of Veterans Strategic Solutions. One of the main items John wanted the audience to know is about informed consent and that our veterans need to know it is not just their right, but it's required. Veterans Strategic Solutions was created to form alliances and strategic coalitions, irrespective of political affiliation, in order to enlighten, inform and increase awareness and to enhance the survival of the veteran public, who individually would not have the strength nor the power of such influential alliances. John has also formed relationships with Congress and Committee members with the intention of bettering the lives of veterans and their families. As a result of the above relationships, John has been instrumental in linking a Congressional Sponsor and a Senate Sponsor of separate bills. Substance abuse contributes to around half of all maternal deaths in Alabama. Dr. Huh is joined by Suzanne Muir, associate director of the UAB Department of Psychiatry and Neurobiology, and Carolyn Webster, MD, a physician within the UAB Division of Maternal-Fetal Medicine, who are part of the leadership team for the Comprehensive Addiction in Pregnancy Program (CAPP). This program, which treated its first patient in 2018, offers pregnant women and new moms a unique web of support, ranging from standard prenatal care and education to peer support, counseling, and recovery services. Webster and Muir discuss the combination of challenges to addressing addiction in pregnancy and how their program is creating a sense of community and safety for these vulnerable families.
In 2009, Liz founded the nonprofit Horses Healing Hearts. Liz personally experienced the struggles of growing up in a turbulent alcoholic home. At the age of 24, Liz lost her mother to liver disease. During the last ten years of her mother's life, it was the support of mentors and horses that gave her the strength and inspiration to survive. Motivated to “pay it forward” Liz created HHH to help the children standing where she once stood. In her Executive Director role, she oversees the daily operation of the charity in close collaboration with and under the guidance of the HHH Board of Directors. In May 2016, Liz was invited to testify before the Bipartisan Task force to combat the Heroin epidemic. In support of the Comprehensive Addiction and Recovery Act, she shared the detrimental effect of severe trauma these children may survive but then carry into adulthood. A prevention advocate, Liz urged lawmakers to create a more strategic approach providing services to ALL family members impacted by the family disease. Liz is a national subject matter expert and sought-after presenter on topics such as Children of Alcoholics/Addiction, Equine Assisted Therapy, and turning adversity into empowerment. Some of her presentations include: The ACEs (Adverse Childhood Experience ) Youth Eastern Summit 2017 (the same lecture forum as renowned experts as Dr. Vincent Felitti); Drug Endangered Children's Conference (Reno, NV 2016) and EAGALA National Conference (Kentucky 2016) (evidence based model). Medication assisted therapy (MAT) for addiction has been available since 2000. Up until 2016, physicians have been the only healthcare providers able to prescribe buprenorphine for opioid addiction. On July 22, 2016, President Obama signed into law the Comprehensive Addiction and Recovery Act (CARA). This is the first major federal addiction legislation in 40 years and the most comprehensive effort undertaken to address the opioid epidemic. CARA incorporated many needed resources to better manage patients with addiction, but one specific aspect of the legislation was focused at nurse practitioners and physician assistants: providing greater access to care by easing the restrictions on who could provide MAT after completion of continued education. Nurse practitioners and physician assistants caring for patients with chronic pain and addiction need continued education and empowerment to accept this level of responsibility. This session will look at the history of addiction, past therapies, current therapies, and the future of addiction, specifically how it intersects with chronic pain management.

Some highlights: Ending today is the comment period for modifying HIPAA. The Health & Human Services Office for Civil Rights is looking for information on how HIPAA laws may discourage or limit information sharing needed for care coordination or the evolution to value based care. The Department of Defense proposed rule adding PTAs and OTAs as eligible providers--the APTA is aware of the errors in this rule, and will be submitting comments for language and terminology changes. Those comments are due by February 19th. The Pain Management Best Practices Interagency Task Force is looking for feedback on its draft report. This is a federal advisory committee established by the Comprehensive Addiction and Recovery Act of 2016 to propose updates to best practices and issue recommendations that address gaps or inconsistencies for managing chronic and acute pain. If you work with the chronic pain population, please submit your comments by April 1, 2019. The APTA provides form letters for comment. The APTA has released their 2019 Public Policy Priorities. It also is a nice discussion to have with other parents. When you're trying to bring up the conversation of ... Instead of an accusatory way of, "Whoa, have you cleaned out your medicine cabinet? Are you watching your child?" For me, it's been a great way to sort of talk about, "Oh, I have this great new client. Have you heard about this product?" I never even realized that I should be cleaning out my medicine cabinet until I had that. For me, it's been a great segue into having conversations with my daughters' friends." Lisa Mortier, Capitol Hill veteran lobbyist in studio with host Brian Wilson Brian Wilson: Lisa Mortier is a Capitol Hill veteran on a mission. As a federal Lobbyist for DisposeRx she walks the corridors of power in Washington with a simple message - drug disposal is key to solving the opioid crisis. Everyday on Capitol Hill, she encourages Members of Congress to consider legislation that deals with the issue of disposal of opioids - employing her years of experience, her extensive understanding of policy matters and the heart of a mom. Lisa Mortier: We first started talking about this a little bit under the Obama administration. We had the Comprehensive Addiction and Recovery Act. . . President Obama: To combat the heroin and prescription drug epidemic that is plaguing so many of our communities. Lisa Mortier: and the 21st Century Cures Act. . . Senator Mitch McConnell: This is the product of a couple of years work, we know this is not the end of the fight but it's one heck of a beginning. Opioid Epidemic Declared a National Emergency Lisa Mortier: Starting in 2017, we did start appropriating some dollars for it, but it really didn't take effect until August of 2017 when the Trump administration declared it a national emergency, President Trump: We can be the generation that ends the opioid epidemic. We can do it. Lisa Mortier: And, That started 90 days of action. The first lady had a round table discussing opioid epidemic. First Lady Melania Trump: It's effected more than 2 million Americans nationwide. And sadly the number continues to rise. Lisa Mortier: The president appointed his very trusted advisor, Kellyanne Conway, as their health czar. Kellyanne Conway: With the Presidents leadership and the First Lady's involvement we are confident we can help those in need across this country. Lisa Mortier:The Department of Justice started cracking down on synthetic drugs entering the market. What really, I think helped push this ahead, as well as when Secretary Azar came into power as the Secretary of Health and Human Services. He declared this as one of his top pillars and priorities. Secretary Azar: Mr. President thank you so much for the confidence that you have bestowed upon me and the incredible department you have entrusted me with. Lisa Mortier: But you still need the resources, so you still needed Congress staff, and Members of Congress jumped on it because they were hearing from all their constituents. Brian Wilson: One thing I know about Washington, and it gets a bad rap in many ways, that if people back home are screaming about something, it gets the rapt attention of Members of Congress. Is that what's happening right now? Lisa Mortier: Yeah, and unfortunately, it's not just screaming, it's crying and they're seeing the pains of their constituents every day. And in this political environment where everyone is so divided and everyone is targeting certain people for their votes, unfortunately or fortunately, this issue crosses all demographics. And Members are really seeing this as something that they can do to help their constituents on an individual level and really make a difference for all these families in crisis and communities.

"We are treating the symptom and not the disease, and the disease is prohibition." -- Patrick Heintz regarding the Comprehensive Addiction and Recovery Act of 2016 (CARA)With experience in both corrections and substance abuse counseling, Patrick Heintz has worked with incarcerated populations for over 20 years. Beginning as a child care worker in a maximum security Department of Youth Services facility, he spent the majority of his career at the Hampden County Sheriff's Department as a counselor/corrections officer.As a substance abuse counselor licensed by the State of Massachusetts working in a variety of corrections settings, he was witness to what he calls the “revolving door and intergenerational nature of the offender population”. Patrick explains, “Early on in my career, it became apparent that to a large extent, this revolving door phenomenon was a direct result of the prevailing laws associated with the war on drugs. It became a daily frustration that no matter how service-oriented and well intentioned treatment attempts were in a corrections setting, nonsensical drug laws such as mandatory minimum sentencing, school zone violations and other punitive consequences of drug arrests kept us from being optimally effective.”As a human service worker in corrections, it was obvious to Patrick that substance abusing and addictive personality disorders were more mental health issues than a law and order problem. He experienced an avalanche of realizations upon first hearing a LEAP presentation where the speaker pointed out that after 40 years of being at war with drugs, the percentage of the population abusing drugs remains at approximately the same level as in 1971, when the war on drugs began.
