Mark Parrino has been involved with the delivery of health care and treatment for opioid use disorder (OUD) since 1974. As the president of the American Association for the Treatment of Opioid Dependence, Inc. (AATOD), he works with treatment providers across the country to develop and improve treatment protocols. In December 2022, AATOD worked with the National Association of State Alcohol and Drug Abuse Directors (NASADAD) to initiate a first-of-its-kind census of all patients currently receiving treatment from government-certified opioid treatment programs (OTPs). Their findings, based on responses from over 1,500 OTPs nationwide, show the breadth and distribution of addiction treatment in America, and are the product of almost fifty years of medication-assisted treatment (MAT) in the United States. I spoke with Mark about his census results, as well as the history of MAT, and specifically methadone, treatment in America. You can see the full report here. Episode 152: Making Space(s) for Arts and Culture in Libya In this episode, Hadia Gana, founder of Bayt Ali Gana, and Faraj Alsileeni, Cultural Manager at Tanarout Organization for Libyan Creativity, share their experiences opening community spaces for art and culture in Libya. Providing two generations of perspective, the speakers find commonalities in their trials and successes. While budget remains an elusive problem and support from governing institutions can be fickle, both Gana and Alsileeni have found strength in building community networks including within their own families. Above all, the leaders seek strategies for sustainability. Reflecting on Libya's past and present, they raise the questions: To what extent could traditional tribal systems be integrated into this new burgeoning cultural sphere? How to convince the public that art can be a powerful tool for change? And amidst the current political uncertainty, what are the spaces in which ongoing cultural education can take place? Faraj Alsileeni is the Cultural Manager of Tanarout Organization for Libyan Creativity, an arts and cultural center in Benghazi, Libya. Opened in 2015, Tanarout offers exhibitions, theater performances, workshops, a book circle, a cinema club, and an open arts space for the community. Alsileeni is also a Resource Center Officer for Civil Society at ACTED Libya. He is an active cinema blogger and cinema researcher, recently presenting his first paper, “The Libyan in the Others Screen,” at the cultural policies symposium of Axiology magazine at the Libyan International University. He is a fellow researcher with Network for Arab Alternative Screens. Hadia Gana is a pottery artist and founder of Gana House for Arts, a cultural space in Tripoli that includes an archive and museum of Ali Gana's art works and art research. Für Rita ändert sich die Situation drastisch, als im Raketenwerk Briefbomben explodieren und dabei auch Hani, in den sie sich verliebt hat, verletzt wird. Im Affekt greift sie selbst zur Waffe. Friedrich Hellberg – unter den Nazis vielbeschäftigter Flugzeugbauer und im Nachkriegsdeutschland ohne spannende Perspektive – will unter der Leitung von Professor Willy Messerschmitt für Nasser einen Überschalljäger entwickeln. Seine Frau Ingrid und seine beiden Töchter Rita und Petra sollen bis auf Weiteres bei ihm in Kairo bleiben. Nur der Sohn Kai, der mit dem Vater im Clinch liegt, weil er sich gegen die atomare Aufrüstung der Bundesrepublik einsetzt, ist als Student in Hamburg zurückgeblieben. Trotz ihres anfänglichen Widerstands gegen ihr neues Zuhause ist Rita von ihrer neuen Umgebung immer mehr fasziniert. Folge 4: ABC-Waffen. Seitdem bekannt geworden ist, dass die deutschen Wissenschaftler in Kairo für Nassers Rüstungsindustrie arbeiten, halten Entführungen, Drohungen und Attentate die deutschen Experten in Atem. Im Raketenwerk explodierten zwei Briefbomben. Auch Ritas Freund Hani wurde schwer verletzt und liegt im Koma. Die Deutschen in Kairo geraten immer stärker ins Visier der Geheimdienste und der internationalen Presse. Ein Bericht der Jerusalem Post sorgt zusätzlich für Aufruhr. Auch für Rita hat sich die Situation in Kairo geändert. Sie hat ihre Stelle als Sekretärin im Raketenwerk verloren. Bei ihrem Abschiedsessen zerstreitet sich Johnny Schwartz, der inzwischen als Reporter in Kairo arbeitet, mit Ritas Kollegen. Rita hinterfragt immer mehr die Vorkommnisse um sie herum und beginnt, selbst zu recherchieren. Sie schließt sich mit Johnny Schwartz zusammen und versorgt ihn mit Informationen aus den innersten Kreisen der deutschen Experten. Zusammen versuchen sie herauszufinden, welche Waffen hergestellt werden sollen: Handelt es sich um atomare, biologische oder chemische Waffen? Parallel arbeitet der CDU-Abgeordnete Franz Böhm in Bonn an einem neuen Entwurf für ein zweites Ausführungsgesetz, dass es Deutschen verbieten soll, sich im Ausland an der Herstellung und Verbreitung von ABC-Waffen und Raketen zu beteiligen. In der Wüste beobachten Johnny und Rita einen weiteren Raketenstart, bei dem Max durch einen Unfall schwer verletzt wird. Doch war es wirklich ein Unfall? Aus dem Krankenhaus erhält Rita schreckliche Nachrichten, und schließlich macht sie auch noch eine Entdeckung, die ihr Vertrauen in Johnny zutiefst erschüttert. Friedrich Hellberg – unter den Nazis vielbeschäftigter Flugzeugbauer und im Nachkriegsdeutschland ohne spannende Perspektive – will unter der Leitung von Professor Willy Messerschmitt für Nasser einen Überschalljäger entwickeln. Folge 2: Raketen für Nasser. Der Ingenieur Friedrich Hellberg ist dem Aufruf von Präsident Nasser gefolgt und mit Frau und Töchtern nach Kairo übergesiedelt. Zusammen mit anderen bundesdeutschen Spezialisten soll er für die aufstrebende Vereinigte Arabische Republik Düsenflugzeuge und Weltraumraketen bauen. Seine Tochter Rita hat auf seinen Wunsch hin begonnen, im Raketenwerk als Sekretärin zu arbeiten und gewöhnt sich schnell an das privilegierte Leben der deutschen Experten in Kairo, das so gar nichts mit der spießigen und biederen Wirtschaftswunder-BRD zu tun hat. Sie genießt die Ausflüge in den mondänen Club Maadi und entdeckt das aufregende Nachtleben Kairos. Spätestens bei der Militärparade zum Jahrestag der Revolution wird der Weltöffentlichkeit deutlich, dass die Deutschen nicht an Weltraumraketen, sondern an Rüstungsprojekten arbeiten: eine Nachricht, die vor allem in Israel hohe Wellen schlägt und die Bundesrepublik in Schwierigkeiten bringt. Es häufen sich Gerüchte über ein Arbeitsverbot für die deutschen Experten bei ihrer Rückkehr nach Deutschland. Eine Entführung und anonyme Drohbriefe verunsichern Friedrich Hellberg und die anderen Experten. Wird die ägyptische Regierung weiterhin für ihre Sicherheit sorgen können? But with a fractured and underfunded system of OUD treatment, as well as stigma around drug abuse in pregnancy and siloed maternal health care, how can pregnant people access this lifesaving care? Dr. Stephen Patrick, pediatrician and former senior policy advisor at the White House Office of National Drug Control Policy, returns to the podcast to talk with Lindsay Smith Rogers about a new report that lays out suggestions for lifting barriers to OUD treatment for pregnant people. Only 5% of clinicians are licensed to prescribe buprenorphine, a medication used in medication-assisted treatment. Many people who seek treatment are forced to get medication through the black market. There are two main problems that telemedicine solves in regards to getting a patient into a healthy long-term treatment: getting patients to opt-in and getting them to stick with treatment. “Telemedicine is great for allowing people to get treated privately, and do so in a way that fits into their schedule. This final (for now) episode discusses the ‘other' patient situations concerning perioperative times. How do you manage patients who have an undiagnosed opioid use disorder or are on very high doses of chronic opioids? What about patients who are opioid naive but who are high-risk for development of an OUD? These patients may be those who, personally, do not want opioids for any reason either due to a history of another substance use disorder or those who just do not want opioid exposure. Someone has finally found a way to address opioid misuse and addiction among people with chronic pain. In this show, Dr. Eric Garland, the world's leading expert on the use of mind-body therapies to treat opioid misuse for pain will discuss the recently proven effectiveness of “MORE” – “Mindfulness-Oriented Recovery Enhancement.” Dr. Garland will define opioid use disorder (OUD), its relationship to chronic pain and what makes opioids so addictive. He will discuss the treatment components of “MORE” ‘the differences between this and other treatments and the research design used to test this treatment model. In the second segment Dr. Garland will invite the audience to hear and feel what it would be like to be in a Mindfulness-Oriented Recovery Enhancement Group. In the back and forth with Host Dr. Phillips, he will consider why the group format matters and why he believes a significant number of those who received this treatment to reduced their opioid use without being specifically directed to do so. Given the danger of Opioid Use Disorder, the number of people suffering, the importance of these findings and the need for available effective treatment, this becomes a show everyone needs to hear.
