Best practices for hospital-based initiation of medications for opioid use disorder: A consensus statement JAMA Network Open This survey study used a 2-round Delphi process to develop expert consensus on best practices for hospital-based MOUD initiation for patients with OUD, with a goal to provide guidance on changing inpatient addiction treatment in response to increased synthetic opioids in the unregulated drug supply. A total of 42 expert clinicians participated; clinicians were considered a national expert if they had cared for at least 100 hospitalized patients with OUD in the last two years. There was consensus that buprenorphine and methadone initiation in the hospital setting were appropriate, with less support for hospital-based naltrexone initiation. Access to Medications for Opioid Use Disorder Among Veterans With Homeless Experience in Permanent Supportive Housing JAMA Network Open This cohort study assessed 10,110 US veterans with homeless experience and opioid use disorder (OUD) residing in permanent supportive housing (PSH), in terms of what factors are associated with receipt of medications for opioid use disorder (MOUD).  Only 17% received MOUD within 12 months. Greater behavioral health engagement was associated with MOUD receipt, while older age, race minority status, and prior inpatient hospitalization were associated with lower odds. Medicaid Managed Care Plan Alignment With State Substance Use Disorder Treatment Coverage Requirements The Milbank Quarterly Medicaid managed plan coverage for medications for alcohol use disorders (AUD) and opioid use disorder (OUD) varies across states but is generally lower in Republican-leaning states. Researchers conducted a national survey to evaluate if these differences in coverage were due to variation in state policy or variations in Medicaid managed plan alignment with state policy. Researchers found that while Republican-leaning states were generally a little less likely to require coverage of most or all medications for AUD and OUD and place limits on prior authorization, managed plans in Republican-leaning states were much less likely to follow state requirements. In Nederland bedragen de kosten van mislukte medtech-startups jaarlijks honderden miljoenen euro's, terwijl veelbelovende technologieën stranden bij de overgang van universiteit naar markt. Die kloof probeert de Venture Challenge te overbruggen met een tienweeks trainingsprogramma voor Life Sciences en MedTech-startups dat al bijna twintig jaar onderzoekers omvormt tot ondernemers. Oud-deelnemers haalden samen ruim een miljard euro op. In deze aflevering van BNR Beter bespreekt Nina van den Dungen hoe je als wetenschapper een medisch bedrijf opbouwt en waarom het Europese investeringsklimaat dat moeilijker maakt dan het hoeft te zijn. Te gast zijn Chretien Herben, programmadirecteur van LifeSciences@Work en medeoprichter van de Venture Challenge, en Bram Schermers, CEO en medeoprichter van Sirius Medical en voormalig deelnemer aan het programma. Chretien Herben richtte de Venture Challenge op na zijn eigen ervaring als startende ondernemer, waarbij hij botste op universiteitsbeleid, IP-conflicten en commerciële tarieven voor laboratoriumgebruik. Het programma bestaat uit twee intensieve driedaagse bootcamps, vijf weken validatie-onderzoek en een eindpitch voor investeerders. De winnaar ontvangt 25.000 euro, maar Herben stelt dat de echte waarde zit in de gedwongen keuzes, het coachnetwerk en de commerciële blik die deelnemers ontwikkelen. Bram Schermers deed in 2014 mee met wat nu Sirius Medical is geworden: een magnetisch navigatiesysteem waarmee chirurgen niet-voelbare borsttumoren tijdens een operatie precies kunnen lokaliseren. Het bedrijf is actief in 26 landen en groeide het afgelopen jaar 100% op jaarbasis. Schermers won de challenge destijds niet, maar noemt het programma toch essentieel voor zijn vroege ontwikkeling als ondernemer, met name voor het leren prioriteren en de commerciële druk verdragen. Verslaggever Stijn Goossens is voor de reportage langs bij het Innovation4Health-congres in het Beatrix Theater. Hij spreekt daar met de nieuwste winnaar van de Venture Challenge: Joric Oude Vrielink met zijn bedrijf Syntric Medical. Joric vertelt waar hij met zijn bedrijf aan werkt en waarom de Venture Challenge voor hem relevant was. Dat voorstel werd niet erg serieus genomen, of zelfs als schrikbeeld afgeschilderd.       Maar hoe ging dat in 1973, toen autoloze zondagen wel werden ingevoerd? Hoe reageerden de Nederlanders, en was het achteraf eigenlijk wel nodig? Muḥammad ibn ʿAbd al-Jabbār al-Niffarī (c. 10th century) was a little-known but profoundly influential early Sufi mystic, best known for his enigmatic work al-Mawāqif wa'l-Mukhāṭabāt (“The Standings and the Addresses”). Living during the formative period of Islamic mysticism, he left no biography of his own; what survives is his dense, visionary prose, which later mystics regarded as spiritually radical and difficult.Al-Niffarī's spiritual insight centers on the experience of divine encounter beyond concepts and language. He describes mawqif (a “standing” before God) as a liminal state where the self dissolves and God directly “addresses” the seeker. His writings emphasize the paradox of divine proximity and incomprehensibility, insisting that true knowledge of God arises not through doctrine or reasoning but through annihilation of the ego (fanāʾ) in the overwhelming presence of the Real.He also explores the limits of language and consciousness, using cryptic, almost aphoristic statements to suggest that ultimate reality cannot be grasped by ordinary thought. Mark Parrino has been involved with the delivery of health care and treatment for opioid use disorder (OUD) since 1974. As the president of the American Association for the Treatment of Opioid Dependence, Inc. (AATOD), he works with treatment providers across the country to develop and improve treatment protocols. In December 2022, AATOD worked with the National Association of State Alcohol and Drug Abuse Directors (NASADAD) to initiate a first-of-its-kind census of all patients currently receiving treatment from government-certified opioid treatment programs (OTPs). Their findings, based on responses from over 1,500 OTPs nationwide, show the breadth and distribution of addiction treatment in America, and are the product of almost fifty years of medication-assisted treatment (MAT) in the United States.
