Comparison of buprenorphine treatment by virtual-only vs. traditional clinicians  Journal of Substance Use and Addiction Treatment  Few patients with opioid use disorder (OUD) receive medications for OUD (MOUD), such as buprenorphine. Given that many patients, particularly those in rural communities, do not live near a clinician who prescribes MOUD, telemedicine could increase MOUD access. However, clinicians and policymakers are concerned about the quality of the virtual-only model.  This study compared the care provided by virtual-only versus traditional clinicians.  The authors found few substantive differences in who used virtual-only care. For example, patients initiating buprenorphine with virtual-only versus traditional clinicians were equally likely to live in urban areas. Patients initiating buprenorphine with virtual-only clinicians had more visits in the 6 months after the initiation and were more likely to be on buprenorphine at 6 months, but had fewer days of continuous buprenorphine treatment. Adjunctive psychosocial interventions and opioid abstinence among patients receiving buprenorphine: A randomized clinical trial JAMA Network Open This randomized clinical trial of 340 adults with opioid use disorder (OUD) taking buprenorphine examined the comparative effectiveness of 2 psychosocial treatments—cognitive- behavioral therapy (CBT) and peer recovery support (PRS)—on opioid abstinence when used as adjunctive therapy to office-based buprenorphine treatment in adults with OUD.  Researchers found that neither PRS, CBT, nor a combination of these psychosocial interventions yielded differential effects on opioid abstinence. De Ethereum Foundation, de drijvende kracht achter het op één na grootste cryptonetwerk, gooit het roer om. De stichting schrapt 54 banen, ongeveer een vijfde van het personeel, en verlaagt het jaarbudget met zo'n 40 procent. Achter die cijfers zit een principiële keuze. De Foundation stapt over van een model waarin ze jaarlijks ongeveer 15 procent van haar schatkist uitgaf naar een endowmentmodel, waarbij een organisatie vooral teert op het rendement van haar vermogen in plaats van op het vermogen zelf. Universiteiten als Harvard werken zo. Het doel is om de jaarlijkse uitgaven richting 2030 terug te brengen naar ongeveer 5 procent. De koerswijziging past bij de filosofie van medeoprichter Vitalik Buterin, die de Foundation liever ziet als één van meerdere bewakers van Ethereum dan als het centrum ervan. In maart 2026 legde de stichting een Mandaat vast, een document van 38 pagina's dat deels als grondwet en deels als manifest dient. De prioriteiten zijn samengevat in de term CROPS: censuurbestendigheid, openheid, privacy en veiligheid. Het werk wordt voortaan verdeeld over vijf clusters. De afgelopen maanden vertrokken ook meerdere kopstukken, onder wie Hsiao-Wei Wang en Tomasz Stanczak, terwijl onderzoeker Dankrad Feist naar Stripe ging. Een deel van het losgelaten onderzoek wordt opgevangen door ETHLabs, een nieuw initiatief dat gefinancierd wordt door grote ETH-bezitters als Joe Lubin en de treasurybedrijven BitMine en SharpLink. In de prijsanalyse bespreken we met Tim Stolte de stand van de markt. Bitcoin zakte terug richting de 60.000 dollar en raakte even het laagste punt van deze bearmarkt, terwijl het sentiment op extreme angst staat. We kijken naar de koersen van MicroStrategy en het financiële product STRC, die opveerden nadat Michael Saylor aankondigde bitcoin te verkopen. Ook goud, zilver en de aandelenmarkten hadden het zwaar, en we vragen ons af hoe de Amerikaanse centrale bank met een nieuwe voorzitter naar de markt kijkt. Tot slot het Nederlandse cryptoplatform Knaken, dat al een maand op zwart staat. Het Rotterdamse bedrijf, opgericht in 2017 en uitgegroeid tot zo'n 30.000 klanten, staakte zijn activiteiten omdat het de vereiste MiCAR-vergunning niet kreeg en die ook nooit zal krijgen. Knaken koos voor een eigen afwikkeling zonder rechter en zonder curator, maar volgens het Openbaar Ministerie verloopt die niet ordentelijk en stopte het bedrijf met uitbetalen. Het OM vroeg daarom in het openbaar belang het faillissement aan, terwijl de FIOD onderzoek doet naar mogelijk strafbare feiten en op 29 juni doorzoekingen verrichtte. Een kerncentrale in Groningen, dat zouden we nooit doen. Oud premier Rutte zei het letterlijk in 2021: "er komt géén kerncentrale in Groningen". Maar wat schetst de verbazing; sinds vandaag ligt de optie toch weer op tafel, zo heeft het kabinet vandaag besloten. Volgens de beheerder van het electriciteitsnet Tennet is het zelfs dé beste optie, beter dan de optie Terneuzen in Zeeland. Maar kunnen we dit de Groningers, na alle beloftes, nog aandoen? Daarover een debat in cafe Miles in Amersfoort tussen SP-kamerlid Sandra Beckerman en VVD-kamerlid Alisha Müller.

Oud-finansminister Calle Schlettwein het kommer uitgespreek oor die Stad Windhoek se kontantlose parkeerstelsel en voer aan dat dit teen die Bank van Namibië-wet en die beginsels van finansiële insluiting is. Die stelsel, wat deur die private kontrakteur KeyPlot Investments bedryf word, vereis dat motoriste slegs vir parkering betaal met bankkaarte, KPI-uitgereikte parkeerkaarte of die KPI-mobiele toepassing. Schlettwein sê kontant bly 'n wettige betaalmiddel en verbruikers moet ingevolge die wet ‘n keuse hê.

Nergens ter wereld komen zo veel reizigers genieten van groene landschappen, mooie regio's en cultuursteden als hier in Europa. Meer dan 500 miljoen van hen bezochten in 2025 de landen van de EU. Maar liefst tien procent van de Europese economie draait op toerisme. Maar dat succes komt niet vanzelf. Er zijn veel uitdagingen. Klimaateffecten, overvolle steden, nog te weinig duurzaam transport, gebrek aan nieuw jong talent in de sector. Jaap Jansen en PG Kroeger praten erover met twee Europese topexperts op het European Sustainable Tourism Mobility Forum deze week op Breda University of Applied Sciences. Daarna volgt een bijzonder gesprek met een levende legende uit de Europese politiek, de Oostenrijkse oud-Eurocommissaris Franz Fischler. Sleutelfiguur daarbij is Mona Bjorklund, directeur beleid bij 'DG MOVE' het directoraat-generaal van de Commissie dat erover gaat. Zij zet helder uiteen hoe zo'n strategie op hoog beleidsniveau tot stand komt. Bepaald niet als 'dictaat uit Brussel', maar als ontwikkeling van een duidelijke, gezamenlijke beleidsrichting voor de lange termijn, samen met bedrijven, organisaties en kennisinstellingen vanuit heel de EU. Zo'n samenwerkingspartner is Fabian Schulz-Luckenbach, hoofd pubic affairs internationaal langeafstand-passagierstransport van Deutsche Bundesbahn. Hij vertelt hoe EU-lidstaten concreet op die langetermijnstrategie inspelen. Zo moeten grote infraplannen gerealiseerd worden voor duurzaam spoorverkeer over de nationale grenzen heen. Aanleg van hogesnelheidsverbindingen tussen Europese metropolen is een grote prioriteit, waar met veel elan aan gewerkt wordt. Minstens zo'n uitdaging blijken de regelstelsels van de lidstaten die elkaar vaak in de weg zitten. Van nationale seinsystemen tot energiekosten waarbij het vliegverkeer nu nog bevoordeeld wordt. De toerismesector is zeer dynamisch. Er is ruimte voor 1 miljoen meer banen en jong talent. Ook daarom is het uitwisselen van bewezen methoden tussen lidstaten, regio's en steden essentieel. Het aansturen van grote bezoekersstromen in culturele hotspots geeft heel wat steden hoofdpijn. Scheefgegroeid toerisme is geen tijdelijk verschijnsel dat zich vanzelf oplost. Oud-eurocommissaris en oud-landbouwminister Franz Fischler herkent vanuit zijn werk op het hoogste niveau in Brussel veel van zulke uitdagingen, zeker voor 'groene regio's'. Hij was in 1995 de eerste Eurocommissaris uit Oostenrijk, dat pas na akkoord van Gorbatsjov EU-lid kon worden. Hij was ook de eerste commissaris die een 'multinationaal kabinet' samenstelde: zes sherpa’s die hem door de Brusselse beleidsjungle loodsten. Meteen bij zijn start kreeg hij de zwaarste portefeuille: landbouw en rurale ontwikkeling. Hoe zijn land de EU in kwam schildert hij met kleurrijke verhalen. Er was een referendum voor nodig. Toen voorstander Jörg Haider – leider van de radicaal-rechtse Vrijheidspartij – ontdekte dat de Groenen de enige oppositie waren, wendde hij de steven en ging bij Fischlers boerenachterban het anti-Europavuurtje opstoken. Dat mislukte. Hoe dat verliep is ook nu nog leerzaam voor de actuele politiek. In de Europese Commissie zag Fischler hoe Jacques Chirac met Gerhard Schröder de landbouwhervorming torpedeerde die hij later alsnog voor elkaar kreeg. "Een gedegen hervorming lukt alleen als je een goede strategie hebt ontwikkeld. En die moet je publiekelijk niet volledig ventileren, maar stap voor stap realiseren." Fischler vertelt met smaak over zijn belevenissen met collega's als president Romano Prodi, Michel Barnier en Frits Bolkestein. "Die was niet zozeer eurosceptisch als wel een scepticus over heel de mensheid, van nature." Ursula von der Leyen noemt hij ‘een sterkere president’ dan de professorale Prodi, maar een waarschuwing geeft hij wel af. "Ze trekt alle besluitvorming naar zich toe. De sfeer in de Commissie is minder goed dan in mijn tijd. En 27 commissarissen is te veel. Dat moet minder, zoals in het Verdrag staat. Serena Williams maakt deze week op 44-jarige leeftijd, en vier jaar na haar 'pensioen', een comeback op de tennisbaan. Best practices for hospital-based initiation of medications for opioid use disorder: A consensus statement JAMA Network Open This survey study used a 2-round Delphi process to develop expert consensus on best practices for hospital-based MOUD initiation for patients with OUD, with a goal to provide guidance on changing inpatient addiction treatment in response to increased synthetic opioids in the unregulated drug supply. A total of 42 expert clinicians participated; clinicians were considered a national expert if they had cared for at least 100 hospitalized patients with OUD in the last two years. There was consensus that buprenorphine and methadone initiation in the hospital setting were appropriate, with less support for hospital-based naltrexone initiation. Consensus was also reached to support rapid methadone initiation; high- and low-dose buprenorphine initiation; and provision of non-MOUD full agonist opioids for treatment of opioid withdrawal during methadone initiation, as a bridge to buprenorphine initiation, and for those declining MOUD.   Read this issue of the ASAM Weekly Subscribe to the ASAM Weekly Visit ASAM

Access to Medications for Opioid Use Disorder Among Veterans With Homeless Experience in Permanent Supportive Housing JAMA Network Open This cohort study assessed 10,110 US veterans with homeless experience and opioid use disorder (OUD) residing in permanent supportive housing (PSH), in terms of what factors are associated with receipt of medications for opioid use disorder (MOUD).  Only 17% received MOUD within 12 months. Greater behavioral health engagement was associated with MOUD receipt, while older age, race minority status, and prior inpatient hospitalization were associated with lower odds. Medicaid Managed Care Plan Alignment With State Substance Use Disorder Treatment Coverage Requirements The Milbank Quarterly Medicaid managed plan coverage for medications for alcohol use disorders (AUD) and opioid use disorder (OUD) varies across states but is generally lower in Republican-leaning states. Researchers conducted a national survey to evaluate if these differences in coverage were due to variation in state policy or variations in Medicaid managed plan alignment with state policy. Researchers found that while Republican-leaning states were generally a little less likely to require coverage of most or all medications for AUD and OUD and place limits on prior authorization, managed plans in Republican-leaning states were much less likely to follow state requirements. In dat debat werd ook de link gelegd met de oliecrisis in de jaren '70 en er werd door de SGP zelfs geopperd opnieuw autoloze zondagen in te voeren. Dat voorstel werd niet erg serieus genomen, of zelfs als schrikbeeld afgeschilderd.       Maar hoe ging dat in 1973, toen autoloze zondagen wel werden ingevoerd? Hoe reageerden de Nederlanders, en was het achteraf eigenlijk wel nodig? Afgelopen week overleed presenator Koos Postema op 93-jarige leeftijd. Hij was hiet alleen bekend van televisie, maar presenteerde ook jarenlang op de radio Met Het Oog Op Morgen en Langs de Lijn. Oud-collega Ferry de Groot haalde herinneringen op aan deze bevlogen presentator, die soms ook een 'schavuit' durfde te zijn. Admiraal Michiel de Ruyter is een van onze bekendste zeehelden. Hij overleed 350 jaar geleden. Nazaat Frits de Ruyter de Wildt vertelde onder meer over de stem van de admiraal.  En goede doelen, zoals het KWF en Artsen zonder Grenzen, ontvangen steeds meer geld uit erfenissen en minder uit donaties. Joep Verboeket, directeur van Kinderfonds MAMAS vertelt waarom dat zo is.

Muḥammad ibn ʿAbd al-Jabbār al-Niffarī (c. 10th century) was a little-known but profoundly influential early Sufi mystic, best known for his enigmatic work al-Mawāqif wa'l-Mukhāṭabāt (“The Standings and the Addresses”). Living during the formative period of Islamic mysticism, he left no biography of his own; what survives is his dense, visionary prose, which later mystics regarded as spiritually radical and difficult.Al-Niffarī's spiritual insight centers on the experience of divine encounter beyond concepts and language. He describes mawqif (a “standing” before God) as a liminal state where the self dissolves and God directly “addresses” the seeker. His writings emphasize the paradox of divine proximity and incomprehensibility, insisting that true knowledge of God arises not through doctrine or reasoning but through annihilation of the ego (fanāʾ) in the overwhelming presence of the Real.He also explores the limits of language and consciousness, using cryptic, almost aphoristic statements to suggest that ultimate reality cannot be grasped by ordinary thought. Despite epidemic levels of overdoses in the United States, by 2020, only twenty percent of Americans suffering from opioid use disorder (OUD) received medication-assisted treatment (MAT), the gold standard of addiction treatment, which uses methadone, buprenorphine, or naltrexone to reduce illicit drug use and curb the symptoms of withdrawal. While MAT is the most effective treatment available for OUD, it's also the most controversial, the most expensive, and the most difficult to access. And yet, the medications at the center of this treatment--and the private industries that distribute them--generate roughly sixteen billion dollars each year, on par with national sales of coffee and pet food. In Addiction, Inc: Medication-Assisted Treatment and America's Forgotten War on Drugs, historian Emily Dufton explains how this promising avenue of treatment emerged during President Richard Nixon's war on drugs in 1971 as a radical experiment in public health, when hundreds of federally-funded treatment clinics opened nationwide. Dufton then explores how these nationalized clinics gave way to an immensely profitable private industry that offers poor care at high costs to an insufficient number of people.
