Pearls There have been significant advances in the field of MCS/LVAD therapy since the first implanted LVAD in the 1960s, to the first FDA approved device in the early 2000's, to now the HM3 LVAD, with the most important change being a centrifugal flow/magnetically levitated design that led to minimized hemocompatibility-related adverse events (HRAE's) (MOMENTUM 3 trial comparing HM2 and HM3). The REMATCH trial in 2001 was a pivotal trial for LVAD therapy, demonstrating that in a population of patients with advanced HF (70% IV inotrope dependent), LVAD therapy significantly improved survival at both 1 and 2 years as compared to medical therapy alone. MOMENTUM 3 trial was a landmark trial for the HM3 device, showing that in a population of end stage HF patients (86% inotrope dependent, 32% INTERMACS 1-2, and 60% DT strategy), 5-year survival with HM3 was 58% and HM3 had lower HRAE's compared with HM2. There are both patient-specific factors and surgical considerations when it comes to candidacy for LVAD therapy. RV function prior to LVAD is a key determinant for success post-LVAD Many patients being considered for LVAD may not have robust RV function, however, predicting RV failure after LVAD is exceedingly difficult. In general, it doesn’t matter how bad the RV may look on imaging; we care more about the pre-LVAD hemodynamics (look at the PAPi and RA/wedge ratio). What happens in the OR may be the most important determinant of how the RV will do with the LVAD! Notes Notes drafted by Dr. Josh Longinow. 1. Historical background of heart pumps and LVADs LVAD Evolution FDA approval year 2001 2008 2012 2017 Pump HeartMate XVE HeartMate II Heartware HVAD HeartMate III Flow/Design Features Pulsatile Technology Continuous flow Axial design Continuous flow Centrifugal design Continuous flow Full MagLev + Centrifugal design The 1960's ushered in the first ‘LVADs', when the first air-powered ‘LVAD' was implanted. It kept the patient alive for four days before the patient expired. The first generation of LVADs were pulsatile pumps The first nationally recognized, FDA approved LVAD was the HeartMate XVE (late 1990s to early 2000s, REMATCH trial). The XVE pump used compressed air (pneumatically driven) to power the pump. Prior to the XVE, OHT was the standard of care for patients with advanced, end-stage heart failure. The second and third generations of LVADs were non-pulsatile, continuous flow devices and included the HVAD, HM2, and HM3 devices. MOMENTUM 3 was a landmark trial for the HM3 device, showing that in a population of sick patients with end stage HF (86% inotrope dependent, 32% INTERMACS 1-2, and 60% DT strategy), 5-year survival with HM3 was 58% and HM3 had lower HRAE's compared with HM2. The only pump that is currently FDA approved for implant is the HM3, although other pumps are in clinical trials (BrioVAD system, INNOVATE Trial). 2. What are LVADs, and how do they work? In simplest terms, the LVAD is a heart pump comprised of several key mechanistic components: Inflow cannula Mechanical pump Outflow cannula Driveline Controller/Power source The HM3 differs from its predecessors (HM2 and HVAD) in several key ways; HM3 is placed intrapericardial whereas the HM2 was placed pre-peritoneal. Perhaps most importantly, the HM3 is a fully magnetically levitated, centrifugal flow pump, whereas the HM2 is an axial flow device. Axial flow pumps are not magnetically levitated, leading to more friction produced between the ruby bearing's contact with the pump rotors, and higher rates of hemocompatibility related adverse events (HRAEs, i.e. pump thrombosis) and the HM2 was ultimately discontinued in favor of the HM3 (MOMENTUM 3 trial). 3. What do the terms ‘Destination Therapy' (DT) or ‘Bridge to Transplant' (BTT) mean when it comes to LVADs? When LVADs first came on the stage, EVERYONE was a BTT; these early pumps weren't designed for long term use (I.e. REMATCH Trial, Heartmate XVE) Destination therapy means the LVAD was placed in leu of transplant because there are contraindications to transplant REMATCH trial brought about the concept of “Destination therapy”, comparing outcomes in patients (with contraindications for transplant) who received an LVAD vs optimal medical therapy Bridge to transplant means we are placing the LVAD in a patient who may not be a transplant candidate at this moment in time (is too sick, or conversely, not sick enough), but may be down the line Bridge to recovery is another term used when the LVAD is being placed for a patient we think may have a recoverable cardiomyopathy 4. What are some factors we should consider when assessing a patient’s candidacy for LVAD, in general, and from a surgical perspective? Patient factors Older age might push us towards thinking LVAD rather than transplant In general, age > 70 is the cutoff for transplant, but this is not a hard cut off and varies institution to institution In general, think about things that help predict recovery after a major surgery; Frailty and Nutritional status are important, we try to optimize these prior to LVAD implant Right ventricular function remains the Achilles heel of LV support We know that needing temporary RV support post LVAD puts you on a different survival curve than patients who don’t need RVAD support Studies have not been able to successfully predict who will develop RV failure after LVAD implantation What happens in the time between when the patient goes to the OR and when they get back to the ICU is an important determinant who might develop RV failure post LVAD Surgical techniques such as implanting the HM3 in the intra-thoracic cavity, rather than intra-pericardial may help maintain LV/RV geometry to help optimize the RV post LVAD Surgical considerations for LVAD candidacy Small, hypertrophied LV: HM3 inflow cannula is small, but small hypertrophied ventricles tend towards chamber collapse during systole causing suction, needing to run slower with lower flow rates Chest size/diameter: pumps have gotten so small now, that for adults, these have become less of a consideration BMI: low BMI used to be more of a concern with the older pumps due to where they were placed, and the relative size of the pump itself, not so much now with the smaller HM 3 pumps Calcified LV apex: would increase risk of stroke, bleeding Driveline tunneling becomes a concern in the super obese population, higher risk for driveline infections (might tunnel these driveline's shorter, and to a less fatty region of the abdomen, could even tunnel out the thoracic cavity in the super obese to limit skin motion) 5. Is there a role for MCS (i.e. temporary LVAD such as Impella) in pre-habilitation of patients prior to LVAD surgery? The theory of being able to improve systemic perfusion, decongest the organs, and make the patient feel better prior to surgery makes sense, but becomes problematic due to the lack of a hard end point/time for prehabilitation which might risk delays in surgery More likely that it can lead to delay in the surgery, with less-than-optimal benefit; you don't want to prolong the wait for surgery and increase the risk for complications prior to surgery An Impella 5.5 is currently FDA approved for 2 weeks of support, not 2 months so timing is important to keep in mind It’s unlikely that you will take a patient and convert them from a malnourished, cachectic person in 2 weeks’ time 6. Is there a role for LVAD therapy in the younger patient population? Should we be thinking of LVAD up front for these patients, with the goal of transplanting down the line? Recovery may be more likely in certain populations, particularly younger females with smaller LV's; in those populations, perhaps bridge to recovery should be the focus, optimizing them on GDMT etc. The replacement of transplant, with MCS (LVAD) in young patients has become a topic of discussion, because these pumps have become better and better, with the thinking that an LVAD could bridge a patient for 10 years or so, and they could get a transplant later It is still a big unknown, but several concerns exist Patients who get LVADs might end up with complications that become contraindication to transplant down the line (stroke, sensitization etc) Patients and providers are more hesitant because of the more recent iteration for the UNOS criteria for OHT listing which no longer gives patients with an uncomplicated LVAD higher priority, and therefore they could end up waiting a longer time for a heart after undergoing LVAD References Rose EA, Gelijns AC, Moskowitz AJ, et al. (Independencia de la República Dominicana) Hija de Fernando Sánchez e Isidora Ramona, María Trinidad Sánchez formó parte del grupo de Febreristas que lucharon por la Independencia dominicana. Como fiel seguidora de Juan Pablo Duarte, confeccionó la primera bandera nacional junto con Concepción Bona, habiendo participado desde el principio en aquel movimiento independentista que culminó el 27 de febrero de 1844. Esa noche histórica, María Trinidad transportó pólvora en sus propias faldas y elaboró muchos de los cartuchos que utilizaron los trinitarios. Lamentablemente, durante los seis meses siguientes hubo pleitos constantes entre los miembros liberales trinitarios y los miembros conservadores de la Junta Central Gubernativa que se formó. Fue tal la discordia entre los dos bandos que el 22 de agosto la Junta, presidida por el general Pedro Santana, declaró traidores e infieles a la Patria a los tres próceres Juan Pablo Duarte, Matías Ramón Mella y Francisco del Rosario Sánchez, junto con cinco de sus compañeros, y decretó su destierro a perpetuidad. Acto seguido, María Trinidad Sánchez, tía de Francisco del Rosario, se integró al movimiento que surgió para derrocar al general Santana. En el transcurso de los meses siguientes, ella alojó en su casa a los disidentes y se convirtió en la organizadora y orientadora de la conspiración que tenía el propósito de lograr el regreso de los patriotas. Cuando se descubrió la insurrección, María Trinidad y sus cómplices fueron apresados. Conscientes de que María Trinidad era la única que conocía el escondite de su sobrino debido a que ella misma le llevaba las comunicaciones de parte de los conspiradores, la presionaron a que lo revelara. Pero, tal como lo manifiesta el dictamen del Consejo de Guerra que la condenó a muerte, ella se negó «obstinadamente a delatar a los principales». Prefirió callar y enfrentarse al pelotón de fusilamiento antes que traicionar a sus compañeros de conjura. El 27 de febrero de 1845, María Trinidad Sánchez partió de la fortaleza Ozama hacia el cementerio donde se ejecutaría la sentencia. Al pasar por la Puerta del Conde, exclamó: «¡Dios mío, cúmplase en mí tu voluntad y sálvese la República!» Fue así como, en el primer aniversario de la Independencia nacional, aquella heroica mujer dio su vida por la causa de la soberanía de su patria.1 ¡Qué bueno sería que todos nosotros, al igual que María Trinidad, siguiéramos el ejemplo de Jesucristo, el Hijo de Dios, rogándole a Dios Padre que se cumpla su voluntad en nosotros! Aquella heroína dominicana lo hizo a fin de salvar la república a la que tanto amaba, para que cada uno de sus compatriotas pudiera disfrutar de plena libertad en una patria soberana. Tie būs Jānis Almanis-Palmbahs, Matīss Circenis, Kamila Siliņa, Andreass Johanness Teobalds (Andreas Johannes Theobald), Kristaps Solovjovs un Laura Ozoliņa. Ar Kamilu Siliņu, Lauru Ozoliņu un Jāni Almani-Palmbahu jau šobrīd tiekamies "Klasikas" studijā! Koncertā skanēs mazāk zināmas laikmetīgās kormūzikas pērles, latviešu tautasdziesmu apdares, skandināvu garīgās mūzikas lirika, kā arī jauno komponistu pirmatskaņojumi. Aunque Pablo escribió Colosenses mientras estaba bajo arresto domiciliario en Roma, quizá su mayor sufrimiento se haya debido a no poder trabajar intensamente yendo de un lugar a otro y de una casa en otra como antes (Hech. 20:20). Estas aflicciones o tribulaciones, de las que Cristo nos advirtió (Mat. 24:9; Juan 16:33), “no son comparables con la gloria venidera que se ha de manifestar en nosotros” (Rom. 8:18). Como les había dicho a los filipenses, ahora se alegra de sus sufrimientos por el bien de los colosenses (Col. 1:24). Is investing in Google's century bond a good idea? If you don't want to read any further, and just want the basic answer, for investors it's a terrible idea. On the other hand, for companies, universities or even governments, issuing a century bond is a great way to lock in low rates for a hundred years. As I said for investors, it's a terrible idea, here is an example. In 2020, the Austrian government issued a century bond that locked in a yield of 0.85%, which was a great deal for them. But for investors who purchased that bond, it is now trading for 30 cents on the euro. Another example of how things can change is back in 1997, J.C. Penney issued a 100-year bond. Back then no one would have imagined bankruptcy would occur just a little over 20 years later for the company. You may be wondering who would benefit from buying these bonds? Generally, it would be your insurers or pension funds. They both have long-dated liabilities, so long-term bonds give them comfort, knowing what the future cash flows will be. There will also likely be some hedge funds and high-risk investors that will want to trade the bonds as they will have a high amount of fluctuation based on interest rates. In fixed income investing, there is something called duration, which essentially looks at the number of years it takes to recoup a bond's true cost. The longer the duration, the more price volatility for the bond when it comes to interest rate moves. Ultimately, for the average investor I would say to stay away because predicting which way interest rates are heading can be very difficult game and it could destroy your investment returns. Big expenses are coming for companies that invest in AI We have talked about this in the past couple years and now after the companies spent roughly $500 billion in 2025 it's estimated they will spend another $3 trillion by the end of the decade. As the companies spend more money on data centers, chips and other items for AI, their depreciation expense will rise each year, which will reduce their income. The big tech companies are kind of sneaky currently with depreciation. Many companies like railroads and other companies report depreciation as a standalone operating expense on their income statement. The reason depreciation is important for investors to understand is that eventually equipment becomes obsolete or worn out and must be replaced. But the big tech companies currently don't have to break out depreciation until 2028 after new rule changes take effect. Currently, they include depreciation in the cost of goods sold or sometimes in research and development or general and administrative expenses. This makes it very difficult for investors and analysts to understand the true numbers. The big tech companies defense is they currently include it in the footnotes. However, companies like Microsoft have as many as 15-20 footnotes, which are generally not seen by investors or analysts. Perhaps the big tech companies will continue to hold onto their lofty valuations for now, but at some point, the real earnings will come through, and the stocks could take a major beating. Don't blame the restaurant or the grocery store for the high price of meat I'm sure you've noticed that if you want to go out and have a good steak, you're probably going to spend somewhere in the neighborhood of $45-$50, depending on the restaurant and how big the steak is. There's a big shortage of cattle in the United States and the numbers are staggering. In January, there were only 86.2 million cattle and calves, which is down from a peak of more than 130 million in the mid 1970s. The number of people in the United States far surpasses the number of cattle and supply/demand being what it is, it is pushing the price of cattle to higher levels. The 86.2 million heads of cattle may sound like a lot, but when you look at the numbers it is the smallest herd since 1951 and that's when the population in the United States was 157 million. The population now stands around 344 million people, which is an increase of 119%. All things being equal, there should be around 188 million heads of cattle available. There are three main reasons why the price of meat is high and the number of cattle is low. We used to receive about 5% of our beef supply from Mexico, but a parasitic fly larvae called screwworm has destroyed that supply. Another problem is a lack of rain in Texas, which is the largest producer of our beef supply with 12.5 million cows. If ranchers don't get enough rain, they produce smaller herds because the cost of feed increases. You may be thinking there's a lot of cows in California as you drive up 15 and you are right because California is the fourth largest producing state for cows at 5 million, but 1.7 million of those cows are dairy cows. The third reason is simply being a rancher is hard work, and it is generally passed down from generation to generation. Most kids when they're growing up do not dream about working on a ranch in the hot sun in the dirt all day long. Also, with the price of land some ranchers realize they're better off selling the ranch for a big profit than continuing to work the land. Fortunately, many ranchers love what they do and despite the hard work continue to do it generation after generation. If you know any young kids that like riding horses, maybe they should consider going to work on a ranch and save all that college money? Financial Planning: Do Commission-Free Annuities Make Annuities More Attractive? One of the primary downsides of annuities has always been the layers of fees that drag on returns, along with upfront commissions that create conflicts of interest in how they're recommended. Commission-free annuities attempt to address these concerns by eliminating the embedded commission and often lowering internal product expenses, which in theory should improve transparency and net performance. However, these products are typically sold by fee-based advisors who charge ongoing advisory fees, meaning that while the conflict of interest may be reduced, the cost savings inside the annuity can be offset by the advisor's separate fee. Even with improved pricing structures, the fundamental challenge remains, annuities generally offer lower expected long-term internal rates of return compared to investing directly in diversified market portfolios. While annuities provide guarantees such as downside protection and lifetime income, those guarantees come at a cost that often outweighs their benefit. In many cases, investors can generate greater long-term growth and higher income from a well-diversified portfolio. How the war on drugs created the gold standard treatment for addiction--until America's opioid crisis got privatized for profit, to the detriment of patients. Despite epidemic levels of overdoses in the United States, by 2020, only twenty percent of Americans suffering from opioid use disorder (OUD) received medication-assisted treatment (MAT), the gold standard of addiction treatment, which uses methadone, buprenorphine, or naltrexone to reduce illicit drug use and curb the symptoms of withdrawal. While MAT is the most effective treatment available for OUD, it's also the most controversial, the most expensive, and the most difficult to access. And yet, the medications at the center of this treatment--and the private industries that distribute them--generate roughly sixteen billion dollars each year, on par with national sales of coffee and pet food. In Addiction, Inc: Medication-Assisted Treatment and America's Forgotten War on Drugs, historian Emily Dufton explains how this promising avenue of treatment emerged during President Richard Nixon's war on drugs in 1971 as a radical experiment in public health, when hundreds of federally-funded treatment clinics opened nationwide. Dufton then explores how these nationalized clinics gave way to an immensely profitable private industry that offers poor care at high costs to an insufficient number of people. Drawing on original research and over a hundred interviews with policymakers, medical experts, pharmaceutical lobbyists, and patients and their families, she tells a gripping story of squandered potential and missed opportunities, as MAT transformed from a revolutionary political project launched from the White House itself into a commercial success--and a public health disaster.
