We are joined this week by Dr. Meredith Gansner. Dr. Gansner is a child psychiatrist at Boston Children's Hospital and an instructor of Psychiatry at Harvard Medical School, specializing in high-risk digital media use in teens. She has received multiple research grants, including a K23 career development award from the National Institute on Drug Abuse, and she is an active member of the American Academy of Child and Adolescent Psychiatry media committee. Her first book, Teen Depression Gone Viral, explores the challenges of treating adolescent depression in the digital age. We're going to break down what social anxiety is, how it differs from normal social worries, and most importantly, how to spot it in children and teens. Our guest today is Sarah Olivo, Ph.D., a licensed clinical psychologist with over 20 years of experience. Sarah specializes in evidence-based treatments, particularly CBT, and is dedicated to helping children, teens, and young adults overcome anxiety and achieve meaningful change in their lives. Show Notes: [2:18] - Many kids and teens experience anxiety and a lot of times they are a normal part of growing up. [3:29] - Some of the questions asked are, “How frequently is anxiety happening?” and “Is it causing a lot of distress for the child or the family?” [4:57] - There are types of anxiety that are developmentally appropriate to experience at different stages of development. [6:00] - The core of social anxiety is the fear of being judged by others and embarrassed in some way. [7:01] - For many kids and teens with social anxiety, social situations are panic-inducing. [8:38] - There is also Generalized Anxiety Disorder which does differ from social anxiety. [11:28] - As parents, we can read our child's mind. There are times when we won't be able to tell the thoughts that lead to anxiety. [13:33] - For those with social anxiety, it's not that they don't know how to act in certain situations. [15:27] - There are ways to get help. One of the most effective treatments for social anxiety is Cognitive Behavioral Therapy (CBT) [18:04] - Social situations activate the threat center for those with social anxiety. [21:19] - For treating social anxiety, goals will be different for each individual. [23:25] - Social anxiety is far different from simply being an introvert. [26:23] - Individual programs are fine, but group programs are excellent for helping overcome social anxiety struggles and meet goals. [29:04] - Privacy is a concern for some group programs, but it is not something to worry about. [32:18] - Social anxiety that goes unmanaged can lead to chronic anxiety. [35:15] - If someone as a child or a teenager struggles with social anxiety, it is likely that a parent also does. [38:07] - At school, teachers tend to cater to a child or teen's social anxiety. [39:56] - Oftentimes we only notice the negatives. We need to notice when a child is working on their struggles and practicing the things they're learning. And it's sort of like every time we understand the basis of a brain disorder, it gets sucked up into a different category, into this neurology category. When in reality, these are all brain disorders that we have a better or worse understanding of the origin for. And often, we know less about neurological disorders than people might think. And we might know more about psychiatric disorders than people might think, but they are all disruptions happening in the brain. Awareness isn't enough to fix mental health36:58: Mental health is something that we all experience. Mental illness is not; it is something that some of us experience sometimes. In those times, it's critical to get those people the resources and treatments that they need. And sometimes, mental health awareness campaigns get in the way because it makes something like anxiety seem like a universal phenomenon, rather than anxiety disorders as a specific phenomenon that some people really suffer from and need help for.Brain disruptions happen in the brain, whether neurological or physiological32:05: I think often, one assumption is that dopamine just feels good. And whenever you experience something that kind of feels good, that must be dopamine. That's actually a mischaracterization of dopamine…[32:30] Dopamine, what it really is closer to is when you want something, when you're motivated to get something. And then it also is, of course, involved in how you learn about that thing. So it has a couple of different functions, different roles in the brain. But the motivational role of dopamine is a really fascinating one because it means you could be motivated to get something that isn't even necessarily pleasurable.Unmasking dopamine30:21: I think if you asked people who experience pain, they would say the optimal amount of pain is zero. But that's because we forget just how much we've learned about the world through pain. Perfectionism isn't a personality quirk. It's a pattern of reinforced behaviors that can seep into every aspect of your life.You may set certain standards for yourself, certain rules about how things are supposed to be or how you are supposed to behave; while you may meet those standards for a while, they eventually - and inevitably - fall short. Then, you feel crushed. Then, you criticize yourself. And then, to drown out the negativity, you set new goals, standards, and expectations for yourself. The cycle of ineffective perfectionistic actions repeats and maintains itself.There is nothing wrong with striving to do things right and perfectly - that's natural when you deeply care about things and what matters to you. In today's episode of the podcast, we interview Dr. Judith Beck, a prominent figure in the field of psychology and author of the highly regarded textbook, Cognitive Behavior Therapy: Basics and Beyond, which is a staple in the academic journey of many students in psychiatry, psychology, counseling, social work, and psychiatric nursing. This book, translated into 20 languages, is a key resource in the U.S. as well as globally. Today, we're answering the question, “Can play therapy help my child?” But first, we need to understand what play therapy is and what it is not. Our guest, Cathi Spooner shares insights on helping children navigate complex emotions through play therapy. And there's a lot to talk about! We've had several episodes discussing different treatment modalities and information about ways we can provide social and emotional support to our children and adolescents. Play therapy might be the right fit for your child and your family. Listen to find out how Cathi has used play therapy to support children and adolescents struggling with anxiety, depression, trauma, divorce and separation, attachment issues, self-regulation issues, and grief and loss. Show Notes: [2:56] - Play therapy is about helping children access the therapeutic powers of play through a specific therapy modality. It's grounded in a theoretical model. [4:32] - Play is the language of children. [5:13] - It is more than just pulling out games and toys. [6:29] - To an outsider looking in, it looks like the counselor or therapist is just playing. But they know how to use play to work through emotions. [7:36] - Children naturally work things out and make sense of the world through play. [10:43] - Cathi describes the types of toys and games in a play therapy room. [12:36] - Anger and aggression are normal emotions and parents tend to want to avoid them. [15:12] - Play gives therapists a way to see what's going on underneath a child's behavior. [17:32] - Children need parents to be able to co-regulate emotions. [19:31] - Children can't be the ones responsible for learning how to regulate their emotions. [22:02] - One key is to empower parents to continue this work at home outside of the therapy room. [24:22] - Parents need support and guidance on how to help their children. [28:20] - If parents don't trust the therapist, they will not be able to feel comfortable and confident in being the parent they want to be. [31:26] - Expressive art is an amazing modality to incorporate into play therapy to show children other ways to work through and make sense of things without verbally articulating. [33:14] - When we're in distress, making sense of things is challenging. Play makes it more accessible. About Our Guest: Cathi Spooner is an LCSW and RPT-S. She has worked with children, adolescents and their families since 1982 in a variety of capacities including therapeutic recreation, teaching special education for children with emotional and learning difficulties, residential mental health treatment programs, substance abuse treatment, and outpatient psychotherapy. Her expertise includes working with children, adolescents and families experiencing trauma and attachment issues as well as depression, anxiety, grief & loss, behavior problems, court-involved youth, homeless populations, ADHD, foster care and adoption issues, parental separation and divorce, and school problems. Cathi has conducted numerous professional play therapy training sessions at the local, state, national, and international level. Today, I have the pleasure of bringing to you a gentleman who goes by the name the Angry Viking Therapist. You can find him under Dr. Trevor Wilkins on Instagram. Dr. Trevor Wilkins is a licensed professional clinical counselor and he lives in Kentucky. He specializes in symptoms related to trauma, depression, anxiety, and post traumatic stress disorder. He works with us, this, this first responder community, the military community, high stress environments. He's been one of us. He was a volunteer firefighter, he was an EMT, and he was a law enforcement officer for 15 years..... including a good stint with Kentucky State Police. He had his own shit go on in his life He switched gears, went back to school, got his master's degree, started his own practice, and then went back to school again and got his doctorate. He is a he's a fascinating individual. He works in what we call Rational Emotive Behavior Therapy. We get into that a little bit. It's a pioneering form of Cognitive Behavior Therapy. It's a little less, I don't know, a little less forgiving in a way.....not as pretty. It forces you to kind of think about your reaction to antecedents and how you can change your reaction to those....and it would change the outcome quite a bit for you. As well as working with rational emotive behavior therapy, he's also specializes in EMDR We've talked about EMDR quite a bit and he, he makes some good points about how he works with the two and how one is more generalized one might be more specific. It was a really good conversation with a fascinating individual. Who I hope to have back on the show and we're going to talk about a few more things because we started to go down a tangent and it's worth exploring. While many associate it with indulgent activities like spa days and bubble baths—and these can indeed be forms of self-care—it fundamentally revolves around identifying energy drains and rejuvenating oneself to peak levels.
