Progression of recent trends in mandible-preserving operations for the management of oral squamous cell carcinomas that infiltrate the mandible is rapid and accompanying studies give invaluable information concerning behavioral understanding of oral squamous cell carcinoma within the mandible. However, a large amount of sound osseous tissue is removed as part of partial mandibulectomy, because it is difficult to gain direct sight into the medullary portion and as a result of fear for residual tumor in this inaccessible space. Thus, needless defects are not seldom. For that reason, there still exists a strong demand for an operating protocol regarding precise surgical clearance which fulfills the surgeons' desire to be more conservative. Twenty-one with evidence of intraosseous tumor spread of 82 resected mandibles were radiologically and histologically reexamined to compare discrepancies among clinical, radiologic and histologic entities of oral squamous cell carcinoma infiltration. Size and location of primary tumor were dominant correlating factors of oral squamous cell carcinoma infiltration into the mandible and were statistically significant (p < 0.05). Larger tumors are more likely to infiltrate the mandible. Gingiva and retromolar trigone were the prevalent locations which facilitated tumor infiltration. Direct contact of the tumor on the attached mucosa usually provides portal of entry of the tumor through the cortex into the medullary space. Periodontal space in the dentate mandible is another possible portal of entry. Erosive-type infiltration is mostly seen in the shallower depth in early phase of infiltration and then followed by invasive type in the deeper portion of mandible. Infiltrating tumors usually do not exceed the limit of the primary on the mucosa, but it becomes unpredictable when inferior alveolar nerve related spread is once initiated. Five to 10 mm of surgical clearance is applicable to any surgical interventions regarding mandible infiltrating oral squamous cell carcinoma. However, thorough pre- and intra-operative attention should be put on the nerve related spread, extended resection of mandible is inevitable when nerve involvement is evident. A combination of orthopantomogram, computerized tomography and Tc-99m skeletal scintigraphy provide a good assessment of the tumor infiltration in the mandible. Distance measurement in orthopantomogram is reliable in localizing the tumor and in planning the surgical margin. An operating scheme based on the biologic behavior of oral squamous cell carcinoma within the mandible is devised as a result of this study.

Zusammenfassend konnte in vorliegender Studie bezüglich eines erhöhten Parodontitisrisikos bei Jugendlichen mit Diabetes mellitus Typ 1 eine höhere Plaqueakkumulation sowie Blutungsneigung der Gingiva beobachtet werden. Hinsichtlich der Kariesinzidenz ließen sich keine Unterschiede zwischen Diabetikern und gesunden Jugendlichen erkennen.

In der vorliegenden Arbeit wurde erstmals der Einfluss von Vitamin C (in Form von Natrium Ascorbyl Monophosphat) und Epigallocatechin Gallat (EGCG) in Kombination mit Lactoferrin (LF) auf die Zahngesundheit der Katze untersucht. Dazu erhielten in einem verblindeten Fütterungsversuch jeweils 9 Katzen in der ersten Versuchsphase von 4 Wochen ein mit Vitamin C (50 mg/kg Körpermasse) bzw. in einer zweiten vierwöchigen Versuchsphase ein mit EGCG in Kombination mit Lactoferrin (je 6,6 mg/kg Körpermasse) versetztes Futter. Zum Vergleich wurde jeweils 9 Katzen parallel ein Kontrollfutter verabreicht. Um die Wirkung des Futters auf die Zahngesundheit beurteilen zu können, wurde die Gingiva, die Tiefe des gingivalen Sulcus, die Plaque- und Zahnsteinbildung sowie Mobilität und Furkation an Zähnen und Parodont mittels Indizes beurteilt. Weiter wurde die Halitosis gemessen und exemplarisch die bakterielle Flora der Maulhöhle von je 2 Katzen aus Kontroll- und Wirkstoffgruppe beurteilt. Zusätzlich wurde die antibakterielle Wirkung der Substanzen in vitro getestet und alle 4 Wochen der Einfluss von Vitamin C und EGCG in Kombination mit Lactoferrin auf den antioxidativen Status der Tiere überprüft. Durch die Fütterung von Vitamin C konnte im Durchschnitt keine Beeinflussung des Geruchs der Atemluft, des Gingivitisindex sowie des Mobilitäts- und Furkationsindex festgestellt werden. Sondierungstiefe und Zahnsteinindex verbesserten sich nur gerringgradig. Für den Plaqueindex ergab sich allerdings durch die Fütterung von Vitamin C eine signifikante Verbesserung im Vergleich zur Kontrollgruppe, die sich in der deutlich geringeren Plaqueakkkumulation bei der Vitamin C-Gruppe zeigte. EGCG in Kombination mit Lactoferrin bewirkte keine Veränderung des Geruchs der Atemluft sowie des Furkations- und Mobilitätsindex. Plaque- und Zahnsteinindex waren nur geringgradig niedriger im Vergleich zur Kontrollgruppe. Signifikante Unterschiede ergaben sich beim Gingivitisindex und der Sondierungstiefe. Der Gingivitisindex verringerte sich innerhalb von 4 Wochen deutlich bei der EGCG/LF-Gruppe und die Sondierungstiefe war im Vergleich zur Kontrollgruppe am Ende der Versuchsphase des EGCG/LF-Versuchs bei der Wirkstoffgruppe deutlich niedriger. In der Zusammensetzung der oralen Bakterienflora ergaben sich innerhalb der 4 Wochen bei keiner der Katzen wesentliche Veränderungen. Die in vitro-Versuche bestätigten bei allen Substanzen deutlich ihre antibakterielle Aktivität. Vitamin C sowie EGCG in Kombination mit Lactoferrin beeinflussten den antioxidativen Status nicht. In der vorliegenden Studie wurde somit gezeigt, dass Vitamin C und EGCG in Kombination mit Lactoferrin einen positiven Effekt auf die Zahngesundheit der Katze ausübten. Dieser Effekt machte sich bei Vitamin C hauptsächlich durch die deutlich verminderte Plaqueakkumulation und bei EGCG in Kombination mit Lactoferrin vor allem durch den verbesserten Zustand der Gingiva bemerkbar.

Im Rahmen dieser Studie wurden drei Homebleaching-Produkte (Colgate Platinum (R), Dentsply NuproGold TM, Discus Dental Nite White Excel (R)) an drei Gruppen mit je 15 Probanden bezüglich Ihrer Bleichwirkung sowie Wirkungsdauer getestet. Es wurden individuell angefertigte Bleichschienen mit Reservoir verwendet. Eine signifikante Zahnaufhellung wurde bei allen drei Produkten nachgewiesen. Colgate Platinum (R) zeigte die besten Ergebnisse bei gleichzeitig stärkster Ausprägung von Nebenwirkung wie Hypersensibilität und Gingivairritationen. Die Bleichanwendung führte bei keinem Probanden zu nachgewiesen Schäden an Zahnhartsubstanz oder Gingiva. Sämtliche Nebenwirkungen sind reversibel. Zur Abschluß-Untersuchung, 24 Monate nach der Bleichanwendung, erschien der Bleicheffekt nur geringfügig reduziert. Neuere Produkte zeigen bei geringerem technischen und finanziellem Aufwand ähnlich gute Ergebnisse wie das Homebleaching dieser Untersuchung. Eine sorgfältige Indikationsstellung ist Voraussetzung für eine erfolgreiche Bleichbehandlung.
