While the proceeding centers on a single television program, its implications reach far beyond The View. The FCC's actions have sparked renewed debate over the role of government in evaluating editorial decisions, the distinction between news, commentary, and entertainment programming, and the constitutional limits on government involvement in the press. This episode examines the historical evidence behind systemic thrombolysis, the emergence of catheter-directed therapies and mechanical thrombectomy, and the landmark RCTs – STORM-PE, PEERLESS, HI-PEITHO, and PEERLESS II – that are reshaping intermediate-risk PE management. Pearls: Systemic thrombolysis in intermediate-risk PE reduces hemodynamic decompensation but at the cost of ~1.5–2% intracranial hemorrhage risk – a near-zero net benefit that has driven the search for safer catheter-based alternatives. “Focus on clinical deterioration, not mortality” – Due to crossover design in contemporary PE RCTs, control-arm patients who decompensate are rescued with advanced therapies, biasing mortality toward the null. Clinical deterioration is the most informative endpoint to watch in HI-PEITHO, PRAGUE-26, and PEERLESS II. HI-PEITHO is the first large RCT to demonstrate that catheter-directed fibrinolysis plus anticoagulation significantly reduces the composite of PE-related death, cardiorespiratory decompensation, or PE recurrence versus anticoagulation alone (RR 0.39; 95% CI 0.20–0.77; P=0.005), with no intracranial hemorrhage in either arm. The four major upcoming/recently reported PE RCTs (HI-PEITHO, PRAGUE-26, PEERLESS II, PE-TRACT) enroll progressively different risk populations – from the most enriched (HI-PEITHO) to the most permissive (PE-TRACT, which includes intermediate-low risk patients) – enabling a nuanced understanding of which patients benefit most from intervention. PE device clearance follows a fundamentally different FDA pathway than structural heart devices (single-arm safety/efficacy studies vs. mandated RCTs), yet market forces and clinical need have ultimately driven industry and government to sponsor large-scale RCTs – a lesson in how evidence development can evolve organically alongside regulatory frameworks. Systemic thrombolysis: Sixteen RCTs over 40 years (1972–2014) enrolling nearly 2,000 patients have studied systemic thrombolysis in intermediate-risk PE. The landmark PEITHO trial (n=1,006) showed that tenecteplase reduced the composite of death or hemodynamic collapse (2.6% vs. 5.6%; P=0.015), driven primarily by reduced hemodynamic decompensation (1.6% vs. 5.0%; P=0.002). After a year away writing my book, I'm back with more of the conversations that got me through it — starting with Miranda Bogen, founding Director of the Center for Democracy & Technology's AI Governance Lab and CDT's Chief Technologist. Miranda has spent her career at the intersection of AI, policy, and civil rights — most recently leading responsible AI strategy at Meta, where she drove company-wide efforts to measure and mitigate bias in AI-powered products. Before that, she was a senior policy analyst at Upturn, where her research on algorithmic discrimination in advertising and hiring helped shape how the industry thinks about AI accountability. We talk about what it actually takes to govern AI systems responsibly — inside a company and at a policy level — where bias creeps into machine learning, why technical fixes alone aren't enough, and what real, workable AI regulation could look like. Miranda's work has been featured in the Harvard Business Review, NPR, The Atlantic, Wired, and Last Week Tonight, and she brings that same clarity to this conversation. This episode was co-produced by Jack Lucas Chang.

The first issue theme is titled “Value(s) in Practice: Impact and Sustainability in XR,” which is investigating the following questions: "How do we measure and demonstrate social impact? How do we justify costs and tradeoffs in immersive vs. more traditional media?" I spoke to managing editor Michael Epstein at SXSW four days after their launch to get a bit more context on their founding with support from Agog, and we unpack some of the articles from the first issue. In Frigg's Twelvefold Flame, Máire's approach to the goddesses shows how they can be worked with to help with pressing matters today including justice, activism, and allyship. They are not (and never have been) “lesser” goddesses in the background to be occasionally referred to when it suits a purpose.  They are vibrant, active, and have a message that is needed in today's society and climate worldwide. Although many of the Council do not have a lot of lore, mythology, or archeological remnants, they are reaching through the veil of mystery those absences create to connect with people today to write new stories, create ritual, empower, and inspire creativity and activism. Stephen walks us through 24 years of military service, from crossing into Iraq on the first wave of the ground war in 2003 to negotiating with the number two guy in Al-Qaeda over a solitary confinement dispute, to a very geopolitically tangled deployment in Syria. He also shares how thru-hiking became the thing that finally got him to put down the bottle, why post-trail depression hit harder than anything he experienced in war, and a magical moment in the Wind River Range where he locked eyes with a bull moose.
