The idea of worship is simple. Uncomplicated. When you think about it, worship is ascribing worth to someone or something. As people of faith, we ascribe to God the worth, praise and adoration that He is due. But worship that intersects with industry and organizations is inherently complicated. You may know that sometimes I go for an album that is going to be a surefire crowdpleaser with my cohosts. But most of the time, I like picking an album where I genuinely don't know how at least one of them is going to react. It doesn't always work out. BUT, the seemingly impossible dream for me was always to introduce an album to all my co-hosts that they didn't know previously and have not one, not two, but ALL THREE of them fall in love with it.Now don't get the wrong idea - that was never the goal of my picks. I'm far too obsessive to make it that simple. It's a complicated algorithm that tries to counterprogram against my past five picks across year of release, genre, and artist characteristics combined with whatever's in my gut at that moment. It's mostly the gut thing actually. But that “what if” idea was always in the back of my head in general. And with this pick, I passed up the typical Costello albums you'd expect to be the go-to pick here because I've always felt Blood & Chocolate was the underappreciated underdog. Love is in the air, as they say, and that is our topic this week. In real life, it can be complicated, stressful, even brutal. Broken relationships devastate lives.But those of us who know Jesus know that our mission is to love like Him. In this episode, we review the clinical presentation, diagnosis, and treatment of uncomplicated urinary tract infections. Key Concepts Uncomplicated urinary tract infections (UTI) are defined as an infection localized to the bladder without any systemic signs or symptoms of infection in someone who is not immunocompromised, pregnant, catheterized, and has normal urologic anatomy. UTIs are most commonly seen in younger women. E. coli is by far the most common urinary pathogen. Symptoms alone drive most of the diagnosis of UTI; however, urinalysis and urine culture can be helpful in some circumstances. Nitrofurantoin (Macrobid) is recommended for men and women for first-line therapy in most patients. Fosfomycin, Bactrim, pivmecillinam, and certain B-lactams can be considered in certain circumstances. Women are usually treated for 3-5 days and men 5-7 days. Some evidence suggests inferior clinical outcomes for B-lactam; however, the amount of data in general is lacking for B-lactams. Recommended B-lactams (aside from pivmecillinam) include amoxicillin/clavulanate, cephalexin, cefadroxil, cefpodoxime, and cefdinir. References Nelson Z, Aslan AT, Beahm NP, et al.
