Elective Endodontics? It’s all about Communication – PDP202

Does ‘elective' or ‘pre-emptive' endodontics have a role in Restorative Dentistry? It almost feels dirty to me as I try my best to PRESERVE pulp vitality! But sometimes this bites you, and you wish you had carried out root canal treatment before cementing that crown. At what point can pre-emptive root canal be justified in a world where MTA and biodentine exist? https://youtu.be/9Gc_yik9fDU Watch PDP202 on Youtube In this episode, Jaz sits down with renowned endodontist Dr. Brett Gilbert to delve into the intriguing world of elective or pre-emptive endodontics. Together, they explore challenging cases where teeth with uncertain pulpal health may require root canal treatment, whether due to caries or crown prep. Dr. Gilbert sheds light on patient communication strategies, the role of bioactive materials like biodentine and bioceramic sealers, and how to make crucial decisions about preserving pulp vitality. Protrusive Dental Pearl: Dr. Pav Khaira suggests using Alvogyl, commonly used for dry sockets, to treat pericoronitis! After cleaning and disinfecting the area, place a small amount under the operculum for immediate relief and to soothe inflammation. Need to Read it? Check out the Full Episode Transcript below! You're not demanding it. You're not saying it's dogma, but you're having a conversation so the patient understands. Because what happens if you don't is you do your work, you're doing it in best faith. Patient winds up in pain, and they become very angry. Teaser:They become agitated, and they want to blame the dentist. And without a conversation, without a dialogue, they're clueless, and all of a sudden, they just think you did something wrong. You are a human, and you are the doctor. Speak to yourself. Let the words flow out so that you can explain all the different possibilities in a way that the patient feels heard, understood, but also nurtured, and at the same time you realize this is biology. We are not in control. Jaz's Introduction:In a world where we want to do everything to preserve pulp vitality, is it ever appropriate to carry out elective endodontics? Another terminology that our guest today, Dr. Brett Gilbert shared with me is preemptive endodontics. For example, you have a tooth with dubious pulpal prognosis. And you know that by prepping it for a crown or by removing the caries, this tooth may need root canal treatment. Is it okay to just go ahead and do the root canal so it doesn't bite you in the behind in the future? You see, I was always taught to do everything possible to preserve pulp vitality.

My guest for Episode #281 of the My Favorite Mistake podcast is Dr. Edward Feinberg, a distinguished expert in restorative dentistry who has dedicated over 40 years to advancing dental care. Dr. Feinberg graduated from Tufts University, was trained by his father, Dr. Elliot Feinberg, a pioneer in full coverage restorative dentistry, and practiced in Scarsdale, New York, before moving to Arizona, where he now practices. Episode page with video, transcript, and more He is the Director of ONWARD, an online teaching organization for full coverage restorative dentistry, where he has created over 30 courses and provides a wealth of educational resources. A nationally recognized lecturer and author, Dr. Feinberg has contributed significantly to dental literature, including his essay collection, "Open Wide: Essays on Challenges in Dentistry to Achieve Excellence." He also serves on editorial boards and councils for various dental associations and has been honored with numerous awards for his contributions to the field. In this episode, Ed shares insights from over 40 years in the field and discusses his unique approach to saving teeth, which contrasts with the widespread practice of extracting teeth and opting for implants. Dr. Feinberg's "favorite mistake" involved a crown falling off a colleague's tooth—a rare but impactful error. Through this experience, he reinforced his belief in trial-based dentistry and using precise techniques to ensure long-lasting dental solutions. Dr. Feinberg advocates for saving teeth first, using crowns and bridges when appropriate, and stresses that implants should only be used when absolutely necessary. The conversation also touches on the overuse of implants in modern dentistry, the importance of prevention, and learning from mistakes to improve patient care. Questions and Topics: What would you say is your favorite mistake? Was that the first time a crown had fallen off, or was it just a rare occurrence? Were you able to implement any checks or mistake-proofing to prevent this from happening again? Can you explain the difference between focusing on the tooth structure above the gum versus below the gum? How do you use temporary crowns, and why do some fall out while others don't? Do you believe dental education is keeping up with advancements in restorative techniques? What are your thoughts on the current trends in implant dentistry? Do you think a lot of dentists are making mistakes with implants, and why? Are there preventive or mistake-proofing methods you apply in your practice to avoid common dental issues? Why do you think patients typically don't seek second opinions in dentistry, unlike in medicine? Does the overuse of implants lead to malpractice cases or trouble with insurance? Why are patients quick to judge dentists based on superficial factors rather than their actual dental work? What are your thoughts on veneers—are they purely cosmetic, and when are they appropriate? How can dentists better manage biomechanical stress when performing implant procedures? Are there concerns about the future reputation of dentistry due to inappropriate practices? What advice would you give to patients about prevention and the importance of regular dental hygiene visits? Key Topics Discussed: Dr. Feinberg's favorite mistake: A crown falling off a colleague's tooth and the lessons he learned about precision and correcting errors. Restorative dentistry techniques: Focusing on the root structure below the gum for crown and bridgework to ensure long-lasting results. The overuse of implants: Dr. Feinberg discusses the inappropriate use of implants and his preference for saving teeth when possible. Dental education: Concerns that dental schools are not teaching the best restorative methods and the influence of economic pressures on young dentists. Precision attachments: A less common, effective alternative to implants that can save teeth, especially for complex cases. Patient care and trust: How patients often judge dentists on superficial factors and the importance of second opinions in dental treatment. Preventive care: The importance of regular hygiene visits to prevent periodontal bone loss, which is crucial for long-term dental health.

Biomimetic dentistry aims to mimic a natural tooth throughout the restorative process. The last four decades of research has shown that mimicking and conserving a tooth's natural connection to itself, its natural bond and resilience to occlusal forces and its natural defense mechanisms for infection benefits the tooth's long-term health. This is also true for occlusion. In this episode, Dr. David Alleman discusses what all dentists can learn from a body's natural occlusion, lost research from the last few decades and approaches for daily occlusal techniques that can be applied to cases doctors regularly see in their office.Articles referenced in this episode:Michael C. Dr. Courtney Lavigne's unique career path took her from a decade of practicing restorative dentistry to orthodontics. She holds accreditations and fellowships from the American Academy of Cosmetic Dentistry, the Academy of General Dentistry, and the Pierre Fruchard Academy. Dr. Lavigne is a renowned lecturer in dental aesthetics and comprehensive care. She established a patient-centric, aesthetics-focused practice in the Boston suburbs before pursuing an orthodontic residency at Tufts University. Driven by her fascination with airway and occlusion, Dr. Lavigne transitioned to orthodontics, aiming to provide truly interdisciplinary care. She sold her cosmetic practice and embarked on her orthodontic journey in July 2020. In her orthodontic practice, Dr. Lavigne combines her restorative dentistry expertise with orthodontic training, prioritizing comprehensive treatment planning that considers both facial aesthetics and three-dimensional tooth alignment. Her ultimate goal is to achieve each patient's ideal outcome, going beyond just achieving a Class I canine occlusion. Dr. Lavigne also highlights the critical significance of early orthodontic evaluations, recommending that children undergo orthodontic assessments around age seven. She underscores factors such as crossbites, functional shifts, and crowding in primary teeth as potential indicators for early intervention. Shifting gears, Dr. Lavigne discusses the utilization of 3D printing for orthodontic brackets and the influence of artificial intelligence in enhancing treatment planning precision. She stresses that while aligner technology is a valuable tool, the practitioner's skill and understanding of the software play a pivotal role in achieving successful outcomes. Dr. Lavigne shares her perspective on AACD accreditation, emphasizing how it not only enhances clinical skills but also builds a robust referral network within the dental community. She notes that the journey of personal growth and improvement throughout the accreditation process is equally valuable as achieving the accreditation itself. As a practitioner who values continuous learning, Dr. Lavigne underscores the significance of mentorship. She advises younger dentists not to hesitate in reaching out to experienced colleagues for guidance and mentorship opportunities. She shares her own experiences with various study groups and organizations that have been instrumental in her professional growth.
