Real Life Pharmacology - Pharmacology Education for Health Care Professionals

Reglan (metoclopramide) is used for gastroparesis and inhibits gastric smooth muscle by blocking dopamine receptors. Relafen (nabumetone) is a non-selective inhibition of COX-1/COX-2 which leads to a reduction of inflammation via reduced prostaglandins. Keflex (cephalexin) is a 1st generation oral cephalosporin that covers common gram positive organisms like Staph and Strep species. Effexor (Venlafaxine) is an SNRI and inhibits both serotonin and norepinephrine reuptake. Primary uses include anxiety and depression. Boniva (ibandronate) inhibits osteoclasts which helps treat osteoporosis. Osteoclasts break down bone to help pull calcium into the bloodstream.

Penicillin allergies and cross-reactivity are common questions with regard to the use of cephalexin and discuss this briefly in the podcast episode. Cephalexin is a first-generation cephalosporin with its primary sweet spot bein gram-positive bacteria like Staph and Strep species. Warfarin, probenecid, zinc, and a couple of others are potential medications that can interact with cephalexin. I discuss this further in this podcast episode.

Cephalexin is a widely used antibiotic. It is often used when treating various bacterial infections in both Adults and Pediatrics. Bacterial infections such as Streptococcal Pharyngitis, Uncomplicated Urinary Tract Infections (UTIs), and Bacterial Skin and Skin Structure Infections (SSSIs). There is also an off label indication for Dental Endocarditis Prophylaxis. Cephalexin works by causing bacteria cells to lyse by binding to penicillin-binding proteins (PBPs) thus inhibiting bacterial cell wall synthesis. Caution is to be used in renally impaired patients. Cephalexin appears to be safe for use during pregnancy and when breast-feeding. Common side effects are abdominal pain, nausea, vomiting, and diarrhea. If the suspension is used, after reconstitution the suspension should be refrigerated and discarded after 14 days. SHOW NOTES: CHAPTER 1: Pediatric neck masses fall into 3-categories:  Inflammatory, Congenital, and Neoplastic.  Seventy-five percent (75%) of neck masses are inflammatory, 20% congenital, and 5% or fewer are consistent with malignancy. The American Academy of Family Practice has a great algorithm on Pediatric Head and Neck masses. The majority of children with enlarged lymph nodes are largely reactive in nature. Most neck masses that are rubbery, mobile and B/L, are indicative of inflammatory in nature. Five (5) or more CTs of head and neck region can increase pediatric malignancy 3-fold. Palpate the parotids, submental, anterior SCM, posterior triangle, supraclavicular lymph nodes. Specific characteristics of a neck mass its midline or lateral.  The size, fixed or mobile, painful or non-tender, firm or soft, solitary or multiple nodes, skin changes. Do an oral and dental exam. Staph and strep are the bacterial causes of most inflammatory neck masses. Antibiotics typically used include: Keflex, Augmentin, and Clindamycin for 10-days; recheck in 3-days to document improvement or continued concern. Atypical mycobacterium infection is generally the cause of chronic inflammatory granulomatous processes.  Often found in the submandibular and periparietal lymph nodes. Generally unilateral and non-tender lymph nodes. Workup includes: PPD skin tests, Bartonella Henslae (or Cat Scratch Fever) and Toxoplasmosis. CHAPTER 2: Physical location and history will guide you to the diagnosis of thyroglossal and brachiocleft cyst. Up to 50% of cysts present infected. Thyroglossal cyst present in the midline from hyoid bone to thyroid isthmus. They tend to be smooth and will often be raised when a child sticks out their tongue. The average of diagnosis is around 6-yrs. of age. About 40% present before the age of 10. Typical treat similar to infected lymph node. With a course of abx and observation to "cool down" the neck mass for 5-6 weeks and then usually surgery. I&D are not recommended as it can cause a fistula track. Imaging choice for a thyroglossal cyst is ultrasound. Furthermore, confirming normal thyroid function in imperative, as you do not want to be fooled by an ectopic thyroid. It is acceptable for the primary care to initiate abx therapy with referral to ENT 1-2 weeks after abx management for recheck and further evaluation. ENT typically recommends surgery for congenital neck masses b/c they do not resolve on their own. Brachiocleft cyst represents 1/3 of congenital neck masses. Most common is a 2nd brachiocleft cyst. Present high in the neck anterior and deep to the SCM border below the angle of the mandible. Ultrasound you guessed it is the imaging modality of choice. One limitation of US is not great at differentiating a lymph node vs. cancer. That's where history and physical exam findings come into play. As an example - Is it a supraclavicular mass, firm, fixed, hard with dimensions greater than 3cm, which is going to be more concern for cancer. Most common cause of pediatric head and neck CA is Hodgkin's lymphoma. Look for fever, chills, malaise, night sweat, weight loss, pallor, fatigue, and failure to thrive. Vascular anomalies are relatively uncommon and are typically referred to as lymphatic malformation. Typically referred to pediatric ENT at Children's or the U. Another name for these lymphatic malformation are also called Cystic Hygromas. Hygromas are typically soft, doughy, trans-illuminate - found commonly in posterior triangle. US are initial imaging choice, however CT or MR can help with delineating the extent of the hygroma. There is about a 5% or fewer risk of malignancy in children with persistent head or neck masses. FNA's are NOT recommended in children. Forty to 50% of cancers in children with head and neck masses are lymphomas. Incisional or excisional biopsy for larger neck masses is generally the next step in evaluation and treatment. SUMMARY OF EVALUATING PEDIATRIC NECK MASSES: Evaluation of pediatric neck masses include: observation, antibiotics, imaging with US to differentiate if lymph node remains - consider obtaining PPD, bartonella and toxoplasmosis titer - looking for granulomatous disease. Next step is surgical removal of the lymph node. As far as labs go - often times the ENT will have the specific titers they want for ruling out certain disease processes. Leave the detailed lab ordering to ENT. When the lymph node is finally excised, generally sent for pathology and culture. RECAP: Neck masses are common. Majority are benign in etiology. They fall into 3-categories: inflammatory, congenital, or neoplastic. Characterize the neck mass with a detailed physical exam. Location, size, mobility, consistency, single or multiple, unilateral or bilateral. Is there fever or chills, accompanied with sxs? Managed with observation and a course of abx. No improvement, consider an US to further differentiate the mass. No improvement 4-6 weeks, send off to ENT for further evaluation.

In the beginning of this self contained session of the West Marches of Thule, our intrepid adventurers Darvon, Paavu, Cuthulion, and Keflex, head south to check out this mysterious tower they spotted while building Wulfa's Hut. Travelling through the burned out area where they were waylaid before by the mysterious invisible creature raises some suspicions. As they camp under the full moon they realize they didnt actually make plans how to get across the river. Plans form for some... interesting uses of freezing ray. Check out today's questions: Anonymous: Hi Dr. Cabral, thanks so much for continuing to produce such helpful information and insight to healing and optimizing health. I listen to your podcast daily and have shared various episodes with family members whom I believe would benefit. My question is about GI health--I've been working with my naturopathic doctor to heal mild gut inflammation I began experiencing around this past Christmas. In addition, I've also been working with diet and supplementation (digestive enzymes, probiotic, aloe vera after meals to soothe gut, etc) to address improved gut motility to eliminate constipation and now have improved bowel regularity and volume (BM at least 1-2 times per day, Bristol scale type 3 or 4). This week I had an ultrasound with my OB/GYN to rule out concerns I had with pelvic pain; while the ultrasound was negative for gynocological concerns, the tech reported that the imaging process was made more difficult due to an "overabundance of stool". I'm confused about why I am otherwise not feeling constipated and having productive BM's--is it possible that my gut motility is adequate but I'm somehow not emptying my bowels completely or have buildup in my large intestine? Most every BM feels "complete". I look forward to your response and appreciate your time! Michael: I enjoy your podcast very much. I found out about you from the Food Heals podcast. I live in Ann Arbor, MI, and so far could not find an FIR Detox Sauna based on your recommendations. Before I buy anything, I'd like to try it with medical supervision. Could you help with some suggestions? Thanks. Jessica: Hi, I heard Dr Cabral on the Melissa Ambrosini podcast. Dr Cabral was inspiring to say the least. I'll get straight to the point. My father in law has recently been diagnosed with ALS. Is this something you can help with at your studio? We have confirmation from the neurologist, we have just been to see a naturopath and she is running some tests (to name a few: Hair trace mineral analysis, Magnesium serum levels, Zinc status test). From the podcast you seemed like the kind (only) person who could help. The MD doctors here just gave the diagnosis and said 'Go write your bucket list and live your best life for the next 2-5 years' WTF! After some research I have come to realise ALS is not some mystery disease but like all 'disease' just the body telling us that these are toxic things in there affecting our system. We are in Australia, willing to travel! Can you help or lead us in the right direction? Jess Bruno: Hi Dr Cabral, huge fan from Europe. Bruno Katie: Thank you for sharing your knowledge with us, since learning about your podcast I feel I finally have information I can trust for my daily questions about health and wellness - I credit my current quality of life to you because I now know dairy, gluten & egg are sensivities for me and taking those foods out has helped me more than I could ever have enjoyed those foods - truly life changing. My question is about Juice Plus - I have been taking the orchard and vineyard blend capsules for a little over a year and because last year was my best year yet I am scared to stop taking them BUT would really like to know what you think. I stopped taking their daily complete shake because it contains pumpkin (one of my other food sensitivities) and switched to the equilibrium daily support shake. I now take my fruit and veggie juice plus capsules in the morning with my lime water with the hope they are comparable with the juiced powder you talk about all the time. Can’t wait to hear what you think - sure appreciate your thoughts and expertise! Jeff: Hi Dr. Cabral, I’m writing on behalf of my wife, Amanda. She has currently been nursing our daughter for 6 months. She has gotten several clogged ducts which resulted in Mastitis at least 3 times already. She has been prescribed Keflex for the infection each time but we are not crazy about the idea of taking that many rounds of antibiotics. She would like to stop breastfeeding seeing as she’s back to work and has had several complications with going several hours without nursing or pumping. Unfortunately, the typical advice is “skip feedings and pump less” but she seems to have an oversupply and that method just results in more clogged ducts. She has tried several natural remedies, I.e. cabbage leaves, sage/peppermint teas. We are now worried it may be an issue with her prolactin levels. Her OBGYN Just today suggested a prolactin inhibitor. She has reduced the number of feeding/pumping sessions but her milk supply remains the same. If you have any tips or advice we would greatly appreciate it. We are huge fans of the podcast and your advice is rapidly changing our nutritional and exercise habits. I am getting ready to start your 7 day Dr. Cabral Detox and I was wondering: Do the limited calories on days 1 and 2 of the detox (200 calories each day) still allow for autophagy? I've read that you have to eliminate calories entirely for a period of time for autophagy to engage; is this true? I am just wondering if this detox can also serve as an intermittent fast, or if I need to incorporate a water only fast separately from the detox at a later date for maximum health results long term. Also could you tell me what macro breakdown you suggest to someone who wants to maintain there weight and what macro breakdown (if different) for someone who has 20 pounds to lose. Thanks -love your podcast   Michele L: Hi Dr. Cabral, Hope you are well. I love the podcast! I had a few questions: 1. What are your thoughts about strep throat and using antibiotics to treat it? My understanding is that antibiotics are prescribed because of possible serious side effects, but wondering if it is necessary? I have one friend whose daughter has a heart problem so she is grateful for them and another friend who has a perfectly healthy daughter, who on the last day of taking the 10 day course ended up with a severe reaction that the doctors believe is serum sickness and is currently in the hospital covered in an awful rash, swollen and having a hard time breathing. They don't really know how to make her better. I know that antibiotics should only be used for bacterial infections, so is that what strep is? 2. Is there anything you can do to protect your child from a serious side effect like the one that happened to my friend's child? 3. The reason I ask is that my daughter is suspected to be allergic to penicillin - she got a red bumpy rash when she had strep 3 years ago after taking amoxicillin that they didn't think was scarlet fever and determined she likely had an allergic reaction. However, they prescribe Keflex for her which is in the same class as penicillin so it makes me worry that she could have a serious reaction to this antibiotic. Any advice on how to manage antibiotics when they are necessary? When talking with the doctors anything specific I should be asking? I wonder if there is a minimum dose that could be prescribed and a shorter duration that would be as effective and perhaps the doctors go with the strongest dosage? I know that the doctors wrongly prescribed antibiotics for my child 4 times prior to her allergic reaction but they made me so worried she needed them when she didn't for very mild ear infections. Very hard to know when to follow along and when to just do what feels right to me. Thoughts? Thanks!   Michelle: Hi Dr. Cabral - thanks for a great podcast. What are your thoughts on hand sanitizers? Do you think it is better to use them than eat with dirty hands if there is no place to wash your hands or are you better off allowing some germs to be there? I know someone who never asks her kids to wash (they don't even wash after going to the bathroom) and the kids are incredibly healthy and never seem to get sick. I do believe in hand washing before eating and definitely after using the bathroom (that to me is basic hygiene) but I am guilty of having my child over washing. I also ask my child to wash when she gets home from school. She was a preemie which made me hyper vigilant about her health. Nat   MariEllen: Hi Stephen, I listen to all of your podcasts and am thankful you share such useful information with all of us. I have had a constant problem with headaches since I got my wisdom teeth out when I was 17/18 years old. I've been told it has nothing to do with it, it's just a coincidence. I have tried an awful lot of things that have been costly as well as time consuming for me. Sometimes, I want to give up.. But I have already come this far in trying to figure out what is going on with me. I have been to so many doctor appointments, had a brain scan, been on multiple medicines to try to help ease the pain ( which I don't fully agree being on and am no longer taking anything), been fitted for an NTI appliance since I do grind and clench my teeth at night, done acupuncture, tried yoga.... It is a constant pulsating and starts every morning when I wake up. I have celiacs disease, so there is no gluten in my diet and limited diary products that im trying to get down to none. Sometimes, the pain becomes bearable and other times, I cant even think of driving myself to work. The pain usually lasts for days and sometimes a week or more at a time. I generally am ALWAYS in pain and when I'm not, it's the best feeling in the world. It doesn't feel like a migraine to me, I can tolerate light and sound but I do get extremely irritable. The pain feels like my head is always pulsating or someone is like constantly hitting me in the head. I have a lot of pain behind my eyes when it gets really bad and around the neck area (my neck is always uncomfortable) it feels like an ice pick headache but for long amounts of time.
