Copper IUDs have soared in popularity as a safe alternative to hormone-based birth control for women but is this really true?From what I've seen over the last 14 years, the opposite is true, yet when a woman notices chronic symptoms such as yeast infections, anxiety and panic attacks, and even psychosis after insertion of this device, she is often gaslit and told to see a psychiatrist.Please know it's not all in your head. You may save a life.The copper IUD is a Class II medical intrauterine device that's inserted into the uterus for long-term birth control (up to 10 years). It's a T-shaped plastic frame with copper wire coiled around the stem and two copper sleeves along the arms that continuously release copper into the lining of the uterus. Emergency contraception refers to therapy used after intercourse to prevent pregnancy. The need for emergency contraception can happen for many reasons, such as a condom breaking or failure to use contraception. More than 11% of sexually active women in the United States between ages 15 and 44 reports using emergency contraception at least once. With such high demand, a multitude of options has become available to meet these needs. With so many options on the market, it may be difficult to decide which option best fits the needs of each individual, which makes it important for providers to have a clear understanding of the risks and benefits associated with each method. Emergency contraception may be commonly used by young patients as their main contraception method. Let's talk about the types of emergency contraception.Levonorgestrel-only (Plan B®).Levonorge'strel-only emergency contraception is the most popular option on the market today. More commonly known as “Plan-B”, this therapy works because of levonorgestrel's similar make-up to progesterone. Mechanism of action.High levels of progesterone delay follicular development so long as it is administered before the level of luteinizing hormone begin to rise. This gives contraceptive therapy of this class a therapeutic window of 72 hours which is the most limited window of all the methods discussed. Despite this shortcoming, Levonorgestrel contraception remains the most popular option because it can be purchased over the counter without the need of a physician and is available to women of all ages. Additionally, therapy includes only a single 1.5mg dose making noncompliance virtually non-existent. Side effects. Side effects include nausea in 12% of patients and headache in 19% of patients. According to one study, 16% of women reported self-resolving uterine bleeding within the first week after use.Selective progesterone modulators (Ella®).The second most commonly used form of emergency contraception are the selective progesterone receptor modulators or more widely known as Ella®. Mechanism of action.Treatment includes a single 30mg dose of ulipristal acetate, which inhibits follicular rupture even after the luteinizing hormone has begun to rise. Due to this mechanism of action, selective progesterone receptor modulators have a wider therapeutic window of 5 days.Side effects.Side effects resemble that of progesterone-only therapy, significant for nausea and headache. Treatment has 2 major barriers preventing it from being the most widely used. Firstly, efficacy is decreased in women with a BMI greater than 35, and secondly, treatment requires a prescription from a medical professional. Estrogen-progesterone combination.Estrogen-progesterone combination therapy is also a viable option for emergency contraception; however, it is no longer available as a dedicated product but can be made from a variety of oral contraceptives. Its decreased popularity is likely due to its increased incidence of nausea when compared to the other options available.Copper IUD.Lastly, Copper IUDs like Paragard can be used for emergency contraception despite not being FDA-approved for this purpose. Copper IUDs are highly effective if placed within 5 days of intercourse, but studies have shown therapy to be effective up to 10 days after. Mechanism of action.Copper IUDs prevent fertilization by altering sperm viability and oocyte-endometrium interaction. This method is the most invasive as it requires placement by a physician and carries the rare risk of uterine perforation, occurring in around 1/1000 IUD placements. That said, copper IUD placement carries with it the added benefit of continued contraception for 10 years. It is contraindicated, however, in patients with a history of heavy menstrual bleeding. FAQs about emergency contraception:Does increasing the availability of emergency contraception encourage risky sexual behavior?No, according to a systematic review by Maria Rodrigues, there was no significant increase in sexually risky behavior correlated with increased availability of emergency contraception.Rodriguez MI, et al.What is the greatest barrier to emergency contraception use in the United States?Education. A study by Abbott J, et al, interviewed adolescents receiving care in urban emergency rooms. The study showed that only 64% of patients had ever heard of emergency contraception. By educating patients of reproductive age on what options may be available to them it is expected that there would be a decrease in unplanned pregnancies. Additionally, studies like “knowledge of emergency contraception among women aged 18-44 in California” by Foster DG have gone further to establish that women of lower socioeconomic status, foreign birth, or who have not graduated high school also have suboptimal education in emergency contraception.When should someone use emergency contraception?Treatment should begin as soon as possible after unprotected intercourse in order to ensure maximum efficacy. 3 days for Plan B, 5 days for Ella, and 10 days for IUD.How effective is emergency contraception?The answer to this question differs based upon what method a patient decides to useIUDsA systematic review of 42 studies over a 35-year time period reports that pregnancy rates were between 0 and 2%.The efficacy of intrauterine devices for emergency contraception: a systematic review of 35 years of experience by Cleland K. et al. Oral regimens have been studied extensively and have shown that ulipristal acetate like Ella® are slightly more effective, showing a pregnancy rate of 1.4% and a rate of 2.2% in levonorgestrel-only pills like Plan B. Ulipristal acetate versus levonorgestrel for emergency contraception: a randomized non-inferiority trial and meta-analysis by Glacier AF.Do patients require follow up after use of emergency contraception?No. Only if there is a delay in the start of normal menses by greater than 1 week or if lower abdominal pain or persistent irregular bleeding develops.___________________Conclusion: Now we conclude episode number 129 “Emergency Contraception.” Bailey explained that a pelvic exam is not needed in most cases before or after emergency contraception. Uterine Fibroid and heavy bleeding might have a new drug if your normal drug was giving placebo. HPV Vaccine is still awesome and very very safe. Copper IUD has a new kid in town when it comes to emergency contraception and Colchicine should not be used for secondary prevention (at least not when I read the trial) HTTPS://WWW.NEJM.ORG/DOI/FULL/10.1056/NEJMOA2008283 Treatment of Uterine Fibroid Symptoms with Relugolix Combination Therapy international, double-blind, 24-week, phase 3 trials involving women with fibroid-associated heavy menstrual bleeding. Participants were randomly assigned in a 1:1:1 ratio to receive once-daily placebo, relugolix combination therapy (40 mg of relugolix, 1 mg of estradiol, and 0.5 mg of norethindrone acetate), or delayed relugolix combination therapy (40 mg of relugolix monotherapy, followed by relugolix combination therapy, each for 12 weeks). The primary efficacy end point in each trial was the percentage of participants with a response (volume of menstrual blood loss 12 times/year) were 2.5 times more likely to be current smokers, to consume >14 alcoholic drinks/week, and to drink >6 cups of coffee daily. AND THERE WAS A –response relationship with increasing tanning frequency. Many of you will know that a dose response relationshop is one of the keys for causation in observational studies. So could it be the tanning causes the smoking, drinking, and coffee drinking.. not directly. But could it be that the personality traits that cause you do go above and beyond are active in all actions of your life?? YES When you go above and beyong you go above and beyond for everything—its almost never something like well I am crazy about working out but I eat like garbage and vice versa if you eat terrible rarely are you crazy about working out. Between 2008-2017 only 4% or 103 out of 2475 applications received a refuse to file. BUT guess how many times the public was informed of these "refuse-to-file"? 15.5% of the time! Only 16/103! This is TERRIBLE! It may come as no surprise that NONE of these "refuse-to-file" letters were were published in their entirety but rather as a press release or abbreviated form. Just like there was a large push as one time for authors to post or register their #clinicaltrials I think there should be equal push for companies to register and publish their "refuse-to-file" letters. I didn’t know only copper IUD!! The New England Journal of Medicine Levonorgestrel vs. Copper Intrauterine Devices for Emergency Contraception N. Engl. J. Med 2021 Jan 28;384(4)335-344, DK Turok, A Gero, RG Simmons, JE Kaiser, GJ Stoddard, CD Sexsmith, LM Gawron, JN Sanders In this trial, the researchers randomized women to a copper IUD or levonorgestrel IUD for emergency contraception and found the levonorgestrel IUD to be noninferior to the copper for this purpose. In the US, the copper IUD is currently the only approved IUD for emergency contraception. This trial provides compelling evidence for the use of levonorgestrel for emergency contraception, providing more options for women in the 5 days following unprotected intercourse. Greenberg JC et al. Life saving therapy inhibition by phones containing magnets. Heart Rhythm 2021 Jan 4; [e-pub]. (https://doi.org/10.1016/j.hrthm.2020.12.032) he magnet in the iPhone 12 is strong enough to turn off therapies from implantable cardioverter–defibrillators. Implantable cardioverter-defibrillators (ICDs) are designed so that an application of a reasonably strong (10-gauss) magnet can inactivate the therapy. This safety feature enables the device's therapies to be suspended for surgeries with cauterization and inappropriate shocks caused by rapid atrial fibrillation (AF) and lead fractures, among other issues. Theoretical concerns have been raised about interference of ICDs by cell phones; however, this interaction in real-life patients has rarely, perhaps never, been reported. Apple's new iPhone 12 contains a powerful magnet, which enables it to correctly align with external accessories (e.g., for wireless battery charging). In an experiment with a single person with an implanted Medtronic transvenous ICD, investigators studied whether the magnet in an iPhone 12 could disable ICD therapies. And indeed, whenever the iPhone was brought near the ICD, the device's ventricular therapies were suspended. Inhibition of ICD therapies by an iPhone is a major concern. Cell phones are frequently carried in chest pockets, some of which are close to an implanted ICD. Even a turned-off iPhone might cause this interaction. Patients and physicians must be aware of this possibly harmful, potential inhibition of ventricular therapies by the iPhone 12, and patients must avoid carrying it in a left chest pocket. And speaking of, many people thought that 2020 would be the year that colchicine would find its way back into our hearts What many people call one of the top articles of 2020 was in the NEJM titled Nidorf SM, Fiolet ATL, Mosterd A, et al. Colchicine in patients with chronic coronary disease. N Engl J Med. 2020;383:1838-47. https://pubmed.ncbi.nlm.nih.gov/32865380 Which was a study that looked at the use of low-dose colchicine to reduce the risk of cardiovascular (CV) events? 5522 patients who had evidence of coronary disease and had been clinically stable for ≥6 months were randomized after a 1 month run in phase. During the run in phase pateitns got colchicine, 0.5 mg/d. 15% of the patients were not randomized after randomization. 15%! Keep that in mind Basically 1 out of 6.5 people who were attempted to enter the trial were not able to tolerate the trial Remember a run in phase false elevates the results of the treatment arm. Because instead of having 100% of people taking place and only 85% taking the active drug since 15% could tolerate side effects. You only randomize people that could take the drug so then you have 100% taking the placebo and 100% taking the active arm. This makes it difficult because we don’t get run in phases in clinical practice. And the results were AMAZING! Or at least if you just read the conclusion “””In a randomized trial involving patients with chronic coronary disease, the risk of cardiovascular events was significantly lower among those who received 0.5 mg of colchicine once daily than among those who received placebo.” Not so fast- the risk of cardiovascular events—what is that?? It is the primary end point but what does it mean??? primary end point was a composite of cardiovascular death, spontaneous (nonprocedural) myocardial infarction, ischemic stroke, or ischemia-driven coronary revascularization. If you add enough outcomes you will always always find something…so you need to look at the individual outcomes MI = NNT- 81 ischemia-driven coronary revascularization= NNT 65 (which makes sense, cheating way to add more numbers to your outcome) cardiovascular death- no difference ischemic stroke, no difference now if you are actually reducing the number of heart attacks you would expect to see a decrease in the cardiovascular deaths but remember there was no difference in cardiovascular death. So if you are seeing a decrease in heart attacks and more patients are under going revascularization then these are suppose to be good things so why no change in cardiovascular deahts??? And people say yes but there was a trend towards decrease in cardiovascular dath with 20 in the colchicine arm and 25 in the placebo arm. BUT what no one is talking about is there was was more non cardiovascular deaths in the colchicine arm 53 vs 35 in the placebo arm. HR 1.51 (95% CI 0.99–2.31) which would cross one and suggest not significant but remember it is all a continuum that we make up it is not that at this rate it works and at this rate absolutely no benefit. So when you have 5 few cardiovascular deaths in the colchicine arm but 18 more noncardiovascular deaths in the colchicine arm it works out to a net increase of 13 more deaths from any cause in the colchicine arm. THIS IS BAD We don’t want more death! So was this just magic skills on the part of the trials where you move a few deaths this way and you slide a few more that way so you can say “there was a trend toward decrease cardiovascular death” or was this that maybe colchicine causes increase in nonCV death. OR Is it that MI and revascularization are really not that important?? Or is it that MI and revascularization are soft endpoints…. You ask 10 difference cardiologist if a patient needs to go for a cardiac cath and you might get 10 different answers. I don’t have the answer but I will tell you many people think this was a big practice changer in 2020 and everyone should get colchicine and I think well I think Based on a study that had 1 out of every 6 patients drop out during the run in phase and was only able to show a change in two very soft end points with no change in all cause death and almost a stastically significant negative change in the nonCVD death, I will not be prescribing this medication for my patients for secondary CV prevention anytime soon.

www.bmj.com/content/368/bmj.m149 No one wants to get pregnant two minutes after having a baby. And also agrees that an IUD is most effective form of conception. However placing IUD after delivering a child seems to be a point of debate as the risk of expulsion seems to be significantly higher immediately post pregnancy In this study titled Averbach SH, Ermias Y, Jeng G, et al. Expulsion of intrauterine devices after postpartum placement by timing of placement, delivery type, and intrauterine device type: a systematic review and meta-analysis. Am J Obstet Gynecol 2020;223:177-188. They looked at the different rates of IUD expulsion postpartum. As you can imagine the rates vary based on if the IUD was placed within 3 minutes of child delivery or 3 weeks after child delivery. There also seemed to be a difference between hormonal IUD (LNG-IUD) compared with a copper T-shaped IUD. Finally there was a difference whether she had a C-section or a vaginal delivery as you can imagine the vaginal delivery was associated with significant less rates of expulsion which is the numbers we are going to talk about going forward as the rates of expulsion following C-section were significantly lower around 0-2%. Brand progesterone IUD are called Skyla, Liletta, Mirena -- but in this study they only included those papers which she used MIRANA. Copper IUD goes by paragard Ultimately the authors looked at 3 different timeframes for placement of the IUD. 1-immediate placement within 10 minutes postpartum, or IUD placement anywhere from 10 minutes postpartum to 72 hours postpartum or early outpatient placement somewhere between 72 hours to 4 weeks postpartum So let’s break them down by timeframe- Those individuals who had an IUD placed immediately following delivery had a 27% exposure rate with Mirena and a 12% exposure rate with ParaGard Those individuals who had an IUD placed not immediately but within the first 72 hours the exposure rate was 37% with the hormonal IUD and 7% for the copper IUD And finally for those women who had an IUD placed in the outpatient setting at some point between 72 hours in 4 weeks there was no expulsion that occurred for either the hormonal or copper IUD. I think the final answer here is for a woman who has a vaginal delivery and would like to have IUD placement following delivery there is almost no way we can justify placing hormonal IUDs within the first 72 hours as the expulsion rate of 30ish percent is way too high to justify. The ideal situation would be IUD placement in the outpatient setting at sometime point between 72 hours in 4 weeks however if you’re patient is insisting on IUD placement while still in the hospital then it appears the best option would be a copper IUD and this likely should be placed as close to discharge as possible because even those individuals who had a copper IUD placed prior to 72 hours still had a 7% exposure rate which seems a little high. If you’re going to use this paper and practice I think important thing to remember is that it was for woman with vaginal deliveries and not for women with C-sections as those individuals had near 0% expulsion rates. The next our article talks about one of the most irritating conditions to treat and of course that is irritable bowel syndrome. In this randomized double-blind placebo controlled trial titled Hamatani T, Fukudo S, Nakada Y, Inada H, Kazumori K, Miwa H. Randomised clinical trial: minesapride vs placebo for irritable bowel syndrome with predominant constipation. Aliment Pharmacol Ther 2020;52(3):430-441. Author still just over 400 patients with a history of severe irritable bowel syndrome predominant constipation who were having less than 3 spontaneous bowel movements per week and randomized them to placebo or minesapride 10 mg, 20 mg, or 40 mg daily for 3 months. The primary endpoint—an increase in one or more complete spontaneous bowel movements and in the end it didn’t matter what dose of minesapride you got because it was no better than placebo. All groups had about a 40% improvement in their rates of spontaneous bowel movement. Irritable bowel is irritating to treat because pooping and bowel movement are so mental. Anxiety and stress and tension can back you up like a hoover damn and then a magical pill even if it is placebo can put you at ease and open the flood gates…running a trial is hard because you need to beat placebo and speaking of things that didn’t beat placebo This paper titled Boesen AP, Boesen MI, Hansen R, et al. Effect of platelet-rich plasma on nonsurgically treated acute achilles tendon ruptures: a randomized, double-blinded prospective study. Am J Sports Med 2020;48(9):2268-2276. Looked to see if platelet rich plasma could be placebo in the treatment of acute Achilles tendon rupture. In this randomized double-blind placebo controlled trial of 40 patient’s with acute Achilles rupture confirmed on ultrasound- All patients were treated with a continuously worn ankle casts that kept the foot plantar flexed for 8 weeks. Every 2 weeks, the researchers lessened the degree of plantarflexion. After 9 weeks, all patients began an ankle rehabilitation program But half were randomized to placebo or and half platelet rich plasma injections. The patient’s were injected every 2 weeks starting within 4 days of the injury they then evaluated patient function via the Achilles tendon total rupture score at the time of removing the cast and then again at 3, 4-1/2, 6, 9, and 12 months after injury. In the end there was no difference between whether patient was randomized to receive platelet rich plasma injections or saline injections. Granted this study was only 40 patient’s and too small to determine differences such as the re-rupture rate but this was a really well done study that I think is hard to refute, they did a lot of things exactly how you want to see them done. For example, None of the patient’s nor providers nor the statistician’s nor the outcome assessors were aware of Whether the patient had received saline or platelet rich plasma. There is only one person who did the injections which was an experience sports medicine physician and injected all patients under ultrasound but this provider had no other influence on the patient’s outcome or care and was also blinded to saline or platelet rich plasma injections as a sheeth was placed over the syringe and the help of the needle. This in all actuality was a very well done trial and methadone neurologically was one of the most sound trials I have read in a long time, if he ever want to read how a method section should be written then he should read this paper. The methods were given in such extreme detail right down to the exact gauge of the needle and to the to the brand of ultrasound machine was used. If you doubt this trial then there is more than enough information in the method section you could easily repeat it in your office tomorrow without any questions. But otherwise said placebo was not nothing placebo is hard to be especially if you’re trying to be treated with platelet rich plasma for an Achilles tendon rupture The man noticed that I said they did a method section really well and if he doesn’t agree with the outcome then you can repeat the study yourself in your clinic. I notice when he comes a cold literature as well as vitamin D literature and other such literature which is borderline terrible those individuals who believe in something so powerful such as mask refuses to believe the outcome of the well done trial regardless of how well the trial was done. However the most important part of the paper size for the results is the method section. If you know exactly what gauge needle, what size syringe, what lab assay, what number years of experience the provider giving the intervention has you can almost do an exact simulation in your office and see if you, with the exact results because sometimes the results very just based on something he would never think of like the laboratory which is no more clearly seen in this article titled Potter JM, Hickman PE, Oakman C, Woods C, Nolan CJ. Strict preanalytical oral glucose tolerance test blood sample handling is essential for diagnosing gestational diabetes mellitus. Diabetes Care 2020;43(7):1438-1441. Which looked at the effect of processing and oral glucose tolerance test with either delayed or early centrifuge protocol. There is a total of just over 12,000 women in this study approximately 7000 women received delayed centrifuge protocol and approximately 5000 women received in early centrifuge protocol. The lumen that had the delayed centrifuge protocol Heather blood collected but then it was sent off to a central laboratory for blood glucose analysis those when that had a early centrifuge protocol underwent the same oral glucose tolerance testing but when they had their blood drawn for analysis but glucose testing was performed immediately. All things being equal they’re really shouldn’t be much of a difference here the results showed that those individuals who had immediate glucose testing rales twice as likely to be diagnosed with gestational diabetes than those individuals with delayed glucose testing Fasting- with fasting there was a difference of 4 mg/dl 1-hour samples 6.1 mg/dL and the two hour measurement was a difference of - 2.8 mg/dL (0.16 mmol/L; 2.3%).. this may sound very small change in the overall glucose but remember the fasting blood glucose concentration is less than 95 so a difference of 4 just based on higher lab processes the blood draw is almost a 5% error in the test.. Likely in the United States for many oral glucose tolerance test we use Accu-Chek monitors however the same question in the same importance and relevant supplies to the calibration and accuracy of the monitor being used. We have a chance grab 2 different brand monitors and check the same person at the exact same time just with different fingers and all certainly you’ll come up with a slight difference in their blood sugar. I’m not surprised by these findings which is a constant reminder that will be due in medicine is small. Nothing is a parachute. Nothing is absolute. I recently heard someone compare a certain intervention to Russian Roulette which implies a 1-6 chance of dying, this is follow-up taking that gives us an over his zealous and heightened sense of our work almost nothing that we do as a number needed to treat of 6 and especially not for death. We have to keep this in mind make sure that our patients are also aware of the inconsistencies in our practice of medicine which is also clearly seen in this paper titled McCormack JP, Holmes DT. Your results may vary: the imprecision of medical measurements. BMJ 2020;368:m149. doi: 10.1136/bmj.m149 Which looked at the variation or error that surrounds laboratory testing. This paper found much of what I mentioned previously on questioning medicine about laboratory findings such as a single HbA1c test result of 6.3% (45 mmol/L) could actually be as low as 5.5% (39 mmol/mol) or as high as 7.1% (51 mmol/mol). The lab findings are thrown off by variability in the analytic or lab process (4.3%), as well as biologic variability meaning the variation in the same person over the course of days caused by physiologic changes. Combined, these challenges to precision can make a single iron, bilirubin, or triglyceride level be inaccurate by as much as 50%. That means you would require at least a 50% change in the levels to be considered valid true actual change and not just a stastical variability. The copper ions impair sperm motility and the physical presence of something in the uterus changes the uterine lining so that a fertilized egg cannot implant and develop. NEGATIVES: It causes an INFLAMMATORY reaction in your body, it can cause painful periods, heavy periods, it disrupts the vaginal microbiome and doubles the risk of bacterial vaginosis which causes discharge with a fishy odor. Copper toxicity can lead to anxiety & mood problems. The Hormonal IUD- MIRENA & SKYLA IUD: releases progestin and it thickens cervical mucus, inhibits sperm survival, prevents build up of your uterine lining Progestin is not progesterone. Side effects of levonorgestrel include acne, hair loss, hirsutism, depression, anxiety, headaches, breast pain, yeast infections, and weight gain. It causes ovarian cysts in 5 percent of users. According to endocrinology professor Jerilynn Prior, “regular menstrual cycles with consistently normal ovulation…will prevent osteoporosis, breast cancer and heart disease.” IUDs are still NOT GETTING TO THE ROOT CAUSE of your period problems/hormone imbalances. Welcome to another Q&A episode! This time I’m going to answer your questions about using the copper IUD, whether it’s safe to eat phytochemicals, what strategies you can implement to deal with period pain, and more. It’s non-hormonal so it may be your best choice. It’s 99.2% effective for up to 10 years. With copper IUD, there have been a significant number of women who have found they have really painful periods as a side effect, and sometimes longer periods with heavy bleeding. There’s also an increased chance of bacterial vaginosis when using the copper IU because of the disruption in the vaginal microbiome. There’s a slightly increased risk of pelvic inflammatory disease as well. Xenoestrogens are one of the most abundant endocrine disruptors that mimic estrogen. They are found in tons of different types of chemical products. Phytoestrogens are natural xenoestrogens derived from plants. They are able to bind to estrogen receptors. Soy and flax seeds are the biggest and most well-known phytoestrogens. Phytoestrogens are much weaker than your own estrogen or any synthetic estrogen. Depending on your individual biology and natural hormone levels, they can have an estrogenic effect or even an anti-estrogenic effect. So when they bind to your estrogen receptors, they can block more potent estrogens from binding instead. This is a good thing for some women because it will reduce symptoms of estrogen dominance. Organic soy consumed in small amounts has been shown to reduce estrogen dominance in some women. For others, even a modest amount of soy might be too much and can lead to significant menstrual problems like longer cycles, heavier periods, and more cramping. Dysmenorrhea can be caused by the following: Endometriosis Benign tumors found in the female pelvis Ovarian cysts Using the copper IUD Action Step: Consider using the copper IUD instead of the pill or a hormonal contraceptive. Tune in to your body when you decide to take phytoestrogens and be conscious of its effects. Experiment with food and see what works for your body. Just pay attention symptomatically how you feel. Be conscious of how you’re taking care of the gut and what you’re eating. Samantha said: “It’s not necessarily just about the food. It’s more about the food and the woman -- how is that food specifically working with you -- because it’s not a one size fits all when it comes to hormonal health.” “If your menstruation is intense, heavy, and uncomfortable, and even if you’re fine the rest of the month, still, it shouldn’t be something that is that intense and uncomfortable where it’s totally knocking you out and you’re crying in pain.” Thanks for listening! When it comes to preventing pregnancy, there are plenty of methods to choose from. But if you're over the pill or contraceptives that involve taking hormones, what are your options? Dr Clare Boerma, Associate Medical Director at Family Planning NSW, joins Nat to lay out all the options, weigh up the pros and cons, and help you decide what's best for you.
Welcome back to Six Minute Sex Ed, the podcast that helps families talk about sex and relationships, hosted by Sex Education teacher, Kim Cavill: https://www.teaandintimacy.com/ This episode is Level Two, which makes it more complex than my level one episodes and especially great for tweens and teens. We’re going to talk about Emergency Contraception. What is it? Emergency contraception is birth control taken up to five days after risky sex in order to prevent pregnancy. There are three kinds: two pills and the copper IUD. How does it work? Levonelle prevents ovulation, Ulipristal blocks the body’s own hormones, and the Copper IUD prevents sperm from reaching the egg. Levonelle is 95% effective in the first 12 hours after risky/unprotected sex, then steadily drops in efficacy (85% after 48 hours, 58% after 72 hours). Ullipristal is 95% effective for five days after risky/unprotected sex. The Copper IUD is 99.9% effective for five days after risky/unprotected sex, making it the most effective form of EC available. Side effects: All three interventions are very safe and side effects are very rare, though they include irregular bleeding, upset stomach, and headaches. Some medical conditions make EC unsafe to take, so it’s important to speak to a doctor to check it’s safe: if you can’t reach your doctor, text “PPNOW" to 774636 (PPINFO) text with someone from Planned Parenthood who can help. Side effects of the copper IUD include temporary cramping, spotting, and heavier or more painful periods. Tips: Because levonelle is time sensitive, it’s a good idea to have a dose on hand, before it’s actually needed, just make sure it’s available. You can also use some forms of the birth control pill as emergency contraception, but ONLY if taken according to very specific guidelines and it’s less effective than the other options I’ve talked about, so make sure to speak to a doctor before trying this method. Check out today's questions: Carla: Hi Dr. Cabral, I live in a Canadian city that fluoridates it's water and have been using a table top distiller to make drinking water for the past 5 years. I've been adding mineral drops to the water as I've heard that distilled can leach minerals from your body. Is this true and am I doing enough to make the water safe to drink? I've also heard that distilled water is acidic and also not good to drink for that reason. Unfortunately I rent and can't afford to add special filters to the household system. Thank you very much for any advice you can give! Carla Meghan: Hi Dr. Cabral! I’ve searched through your podcasts for your take on copper toxicity and couldn’t find what I was looking for so here it goes. I had a copper iud in for 10 years and then replaced with another one year ago. I had it taken out today. I’ve been suffering with; brain fog, insomnia, recurring UTIs, bladder urgency, recurring yeast infections (chronic), extreme fatigue, dry skin and most recently - dermal hyperpigmentation above my upper lip out of no where. I’ve been to dozens of specialists and taken countless tests with no help. I’m gluten and dairy free. I’ve tried the candida cleanse and recently your 21 day detox with no results which I know is abnormal. I did the OAT with you and subsequently ordered the yeast protocol from you while waiting for the results because I know I have candida overgrowth. My OBGYN said NONE of this is related to my copper IUD and it couldn’t cause copper toxicity but I just don’t believe it so I decided to have the IUD removed against her recommendation. Do you recommend I do the hair tissue mineral analysis for copper? Will I have any success on the yeast protocol of yours without cleansing myself of the copper? I am so tired of trying everything with such high hopes and then no success. Do you have any recommendations for someone with suspicion of high copper? I know that copper toxicity can cause candida overgrowth and most of my other symptoms as well. We have a 3 year old son who at the end of last winter we realised our wood fire was causing him to cough at night, we stopped immediately & the cough went away. Several months later we had a camp fire to cook dinner (forgetting about the cough) and again our son woke at 3am with asthma symptoms & very restricted airways. I have tried using essential oils, diffuser, garlic teas, salt, coconut oil, to open his airways but it has been two nights now where we are awake all night. In the past he has had symptoms the same on two occasions & as his father suffered from serve asthma as a child & can relate to the tight chest & short of breath feeling we have given him an asthma puffer as recommended by a doctor. Can you suggest anything else I can do to quickly open his airways & allow him a nights rest without having to resort to the puffer. I recently discovered your podcast and you are amazing! I really enjoy your podcast and have learned a great deal about how functional medicine can help heal the body. I'm writing based on a severe infertility issue I'm having, so I apologize for the lengthy question. At 37 years old, I was diagnosed with premature ovarian insufficieny (POI), which has been heartbreaking. I'm now 38 years old, 130 lbs, 5'4" and have been trying to conceive my first child. I had a failed IVF cycle and made it all the way to egg retrieval, but unfortunately there were no eggs to retrieve. I developed this condition fairly quickly , which leads me to believe it can be reversed. At the time of my diagnosis, I had food sensitivities, weird rashes on the side of my stomach, itchy skin, severe bloating and weighed 145 lbs. I also went through extreme stress (moving, job dissatisfaction, money woes, one parent diagnosed with cancer, other parent died unexpectedly). Currently -My periods are now irregular (40-72 days) -FSH reading on two separate cycles were 32 mIU/ml and 27 mIU/ml -AMH was 0.21 ng/mL -Extremely low estrogen, progesterone DHEA, testosterone. At 36 years old, I had a fertility work up and everything was normal -Regular periods (27-29 days) -FSH 9 mIU/mL -AMH 3.8 ng/mL -Normal estrogen/progesterone. I do have a history of an autoimmune disorder (mixed connective tissue disorder), and my POI is not due to a genetic mutation. I want to try another cycle of IVF but I'm worried about the low rate of success based on my stats. I've lost all the excess weight and bloat through dietary changes (mainly paleo diet) and walking. What do you suggest I work on leading up to my next IVF cycle? Also any ideas as to why this severe infertility disease happened to me in such a short time? Would you be willing to work with me to help me get back to optimal reproductive health? Thanks! Zach: Hi Steven! I am currently dealing with problems with tinnitus, and strange sensations in the ear that make them sometimes feel plugged, and I am wondering if this is a sinus issue, blood pressure issue, or something else. Also I had my lipid panel done a few months back and everything was fine except my LDL was high. Could the LDL have some sort of connection to this? Last September I switched birth control contraceptives. I had been on the pill for about 10 years. I switched to the cooper IUD (paragard). I've had spotting almost every single month since the switch. I was told this was normal, however, I didn't expect it to last this long. Is there anything I can do to help stop the spotting? Anonymous: Hi Dr Cabral, is it fine to start taking probiotics after a few weeks of a decent candida cleanse even if you are still experiencing candida symptoms, i.e. bloating, gas etc; is it usual to still experience the symptoms until you start on probiotics or should you completely wait or avoid probiotics until those symptoms are fully gone? I also wondered what your take is on the controversial probiotic Enterococcus faecalis (found in Threelac/Fivelac)? Learnt so much thanks for your amazing podcast! Amanda: Hi Dr Cabral, I am a holistic nutritionist and love your show. Almost 5 months ago I ended up with leaky gut which led me to having perioral dermititis. My skin problems were really bad until I started Going on a leaky gut protocol; healing with bone broths, fermented foods, green juices and probiotics. Today I am much better but there is still a slight rash around my nose that hasn't totally gone away. Some days it's better than others. Do you have any experience of what could keep this lingering dermatitis? Is it bacteria or fungal? And any suggestions on a cure? I purchased a CoQ10 supplement & serum about a year ago from "MitoQ"; unfortunately I wasn't consistent with either but since your episode, I have looked back into them and noticed this supplement contains 5mg Mitoquinol and wondered how that compares to the supplements you suggested of 100mg Ubiquinol? Tony: Hi Doctor Cabral, I am mid fight with my gf about microwaving food. I do not see the issue, she is telling me it disrupts the food on a cellular level and I just don't believe her. Is there truly any harm in microwaving your food for best health. Also, What makes GMO food GMO? I do not see gmo as hurtful, again my gf thinks otherwise, so we come to you! I do not see the harm in eating gmo plants since cross pollination is inherently natural. I've read that you have to eliminate calories entirely for a period of time for autophagy to engage; is this true? I am just wondering if this detox can also serve as an intermittent fast, or if I need to incorporate a water only fast separately from the detox at a later date for maximum health results long term. Thanks -love your podcast Michele L: Hi Dr. Cabral, Hope you are well. I love the podcast! I had a few questions: 1. What are your thoughts about strep throat and using antibiotics to treat it? My understanding is that antibiotics are prescribed because of possible serious side effects, but wondering if it is necessary? I have one friend whose daughter has a heart problem so she is grateful for them and another friend who has a perfectly healthy daughter, who on the last day of taking the 10 day course ended up with a severe reaction that the doctors believe is serum sickness and is currently in the hospital covered in an awful rash, swollen and having a hard time breathing. They don't really know how to make her better. I know that antibiotics should only be used for bacterial infections, so is that what strep is? 2. Is there anything you can do to protect your child from a serious side effect like the one that happened to my friend's child? 3. The reason I ask is that my daughter is suspected to be allergic to penicillin - she got a red bumpy rash when she had strep 3 years ago after taking amoxicillin that they didn't think was scarlet fever and determined she likely had an allergic reaction. However, they prescribe Keflex for her which is in the same class as penicillin so it makes me worry that she could have a serious reaction to this antibiotic. Any advice on how to manage antibiotics when they are necessary? When talking with the doctors anything specific I should be asking? I wonder if there is a minimum dose that could be prescribed and a shorter duration that would be as effective and perhaps the doctors go with the strongest dosage? I know that the doctors wrongly prescribed antibiotics for my child 4 times prior to her allergic reaction but they made me so worried she needed them when she didn't for very mild ear infections. Very hard to know when to follow along and when to just do what feels right to me. Thoughts? Thanks! Michelle: Hi Dr. Cabral - thanks for a great podcast. What are your thoughts on hand sanitizers? Do you think it is better to use them than eat with dirty hands if there is no place to wash your hands or are you better off allowing some germs to be there? I know someone who never asks her kids to wash (they don't even wash after going to the bathroom) and the kids are incredibly healthy and never seem to get sick. I do believe in hand washing before eating and definitely after using the bathroom (that to me is basic hygiene) but I am guilty of having my child over washing. I also ask my child to wash when she gets home from school. She was a preemie which made me hyper vigilant about her health. I want to get to a "normal" state around this but don't know what the right balance is and how to achieve it? Thanks! Natalie: Hi Dr. Cabral! I love your podcast. Nat MariEllen: Hi Stephen, I listen to all of your podcasts and am thankful you share such useful information with all of us. I have had a constant problem with headaches since I got my wisdom teeth out when I was 17/18 years old. I've been told it has nothing to do with it, it's just a coincidence. I have tried an awful lot of things that have been costly as well as time consuming for me. Sometimes, I want to give up.. But I have already come this far in trying to figure out what is going on with me. I have been to so many doctor appointments, had a brain scan, been on multiple medicines to try to help ease the pain ( which I don't fully agree being on and am no longer taking anything), been fitted for an NTI appliance since I do grind and clench my teeth at night, done acupuncture, tried yoga.... It is a constant pulsating and starts every morning when I wake up. I have celiacs disease, so there is no gluten in my diet and limited diary products that im trying to get down to none. Sometimes, the pain becomes bearable and other times, I cant even think of driving myself to work. The pain usually lasts for days and sometimes a week or more at a time. I generally am ALWAYS in pain and when I'm not, it's the best feeling in the world. It doesn't feel like a migraine to me, I can tolerate light and sound but I do get extremely irritable. The pain feels like my head is always pulsating or someone is like constantly hitting me in the head. I have a lot of pain behind my eyes when it gets really bad and around the neck area (my neck is always uncomfortable) it feels like an ice pick headache but for long amounts of time.
