King of The Slums special with Charles Keigher in conversation with David Eastaugh King of the Slums formed in Manchester, England, by writer Charley Keigher aka Charlie Keighera (vocals, guitar) and Sarah Curtis (electric violin), initially as Slum Cathedral User. Joined by bassist John Eccles in early 1985, they recorded a demo tape at Crimson Studios in Urmston, Manchester with the help of a drummer who left the band soon after. The band took up its image of Kings of the slums incorporating a plastic dustbin and a tambourine replacing the drums which added to their 'Slums' authenticity when playing live. They won Muze magazine's Band of the Year competition winning a recording session at a studio in Gorton Manchester producing a second demo tape. Their third demo tape came soon after at a recording studio in Upper Mill high up in the Saddleworth moors. During the second and third demo tape recordings the group made its vinyl recording debut with the "Spider Psychiatry" single in 1986 on a small independent label SLR Records. During this time they gigged at various Manchester venues as a three piece with their now trade mark dustbin drum and homemade stage. The single, reviewed by Melody Maker, never made any significant sales. In mid 1987 Eccles left the band. Sarah Curtis had studied violin at the Royal Northern College of Music, but dropped out before finishing her studies. Further releases followed in 1988 and 1989 on the Play Hard label, now with bassist Jon Chandler and drummer Ged O'Brien (who replaced a succession of drummers), most of which were collected on the album Barbarous English Fayre (1989). The band also recorded a session for John Peel's BBC Radio 1 show in 1988. An incendiary live performance of "Fanciable Headcase", shown on the influential Snub TV television programme, earned the band national exposure, and helped to push their EPs up the independent chart, "Bombs Away on Harpurhey" reaching No. 8.

This week I have the absolute pleasure of speaking with Sarah Curtis, founder of Move Your Mind and one of the co-founders of 10Ironwomen, a group of 10 inspiring women who undertook the Barcelona Ironman in 2019. Sarah integrates her active lifestyle to all aspects of her world including her work as a counselor & psychotherapist. Taking her sessions out of her practice in Shoreditch she is able to offer running and walking sessions to those who feel more comfortable out in nature. Sport has always been a part of Sarah's life but she hasn't always been a lover of running. It was through joining running groups in London that she found a supportive community of runners and motivational women which saw them take on the challenge of running from LA to Vegas and later the unthinkable challenge of an Ironman. I was really eager to understand the decision process Sarah went through when she agreed to take on these challenges and if she was daunted by the size of the task, but as she explains the hardest part of these physical or mental challenges is saying yes in the first place. From there she fully commits and goes for it. Pushing yourself physically or mentally really is just one decision away. Since completing the Ironman Sarah and 10Ironwomen have become a movement with a mission to see a 50/50 gender split at Ironman events, so if you're even slightly considering it Sarah will get you inspired to take on the challenge. Pediatric DKA was identified as one of key diagnoses that we need to get better at managing in a massive national needs assessment conducted by the fine folks at TREKK – Translating Emergency Knowledge for Kids – one of EM Cases' partners who's mission is to improve the care of children in non-pediatric emergency departments across the country. You might be wondering - why was DKA singled out in this needs assessment? It turns out that kids who present to the ED in DKA without a known history of diabetes, can sometimes be tricky to diagnose, as they often present with vague symptoms. When a child does have a known history of diabetes, and the diagnosis of DKA is obvious, the challenge turns to managing severe, life-threatening DKA, so that we avoid the many potential complications of the DKA itself as well as the complications of treatment - cerebral edema being the big bad one. The approach to these patients has evolved over the years, even since I started practicing, from bolusing insulin and super aggressive fluid resuscitation to more gentle fluid management and delayed insulin drips, as examples. There are subtleties and controversies in the management of DKA when it comes to fluid management, correcting serum potassium and acidosis, preventing cerebral edema, as well as airway management for the really sick kids. In this episode we‘ll be asking our guest pediatric emergency medicine experts Dr. Sarah Reid, who you may remember from her powerhouse performance on our recent episodes on pediatric fever and sepsis, and Dr. Sarah Curtis, not only a pediatric emergency physician, but a prominent pediatric emergency researcher in Canada, about the key historical and examination pearls to help pick up this sometimes elusive diagnosis, what the value of serum ketones are in the diagnosis of DKA, how to assess the severity of DKA to guide management, how to avoid the dreaded cerebral edema that all too often complicates DKA, how to best adjust fluids and insulin during treatment, which kids can go home, which kids can go to the floor and which kids need to be transferred to a Pediatric ICU. Pediatric DKA was identified as one of key diagnoses that we need to get better at managing in a massive national needs assessment conducted by the fine folks at TREKK – Translating Emergency Knowledge for Kids – one of EM Cases’ partners who’s mission is to improve the care of children in non-pediatric emergency departments across the country. You might be wondering - why was DKA singled out in this needs assessment? It turns out that kids who present to the ED in DKA without a known history of diabetes, can sometimes be tricky to diagnose, as they often present with vague symptoms. When a child does have a known history of diabetes, and the diagnosis of DKA is obvious, the challenge turns to managing severe, life-threatening DKA, so that we avoid the many potential complications of the DKA itself as well as the complications of treatment - cerebral edema being the big bad one. The approach to these patients has evolved over the years, even since I started practicing, from bolusing insulin and super aggressive fluid resuscitation to more gentle fluid management and delayed insulin drips, as examples. There are subtleties and controversies in the management of DKA when it comes to fluid management, correcting serum potassium and acidosis, preventing cerebral edema, as well as airway management for the really sick kids. In this episode we‘ll be asking our guest pediatric emergency medicine experts Dr. Sarah Reid, who you may remember from her powerhouse performance on our recent episodes on pediatric fever and sepsis, and Dr. Sarah Curtis, not only a pediatric emergency physician, but a prominent pediatric emergency researcher in Canada, about the key historical and examination pearls to help pick up this sometimes elusive diagnosis, what the value of serum ketones are in the diagnosis of DKA, how to assess the severity of DKA to guide management, how to avoid the dreaded cerebral edema that all too often complicates DKA, how to best adjust fluids and insulin during treatment, which kids can go home, which kids can go to the floor and which kids need to be transferred to a Pediatric ICU.
