Reconstructing Healthcare: Innovative Solutions For Employers To Lower Their Healthcare Costs

In this episode, Michael introduces Dawn Cornelis, the co-founder and Chief Transparency Officer of ClaimInformatics. ClaimInformatics is a payment integrity solution that helps its clients identify improper healthcare claim payments and recoup the money for the employer.  When Dawn entered the world of claim processing 30 years ago, it didn’t take long for her to see that money was being wasted on a massive scale via unnecessary procedures, upcoding, bad systems, and egregious contracts. Unfortunately, there’s more abuse now than ever. With 3-7% of healthcare claims being inaccurately paid, it’s grown to be a problem that is worth over a trillion dollars. This inspired her to co-found ClaimInformatics to catch errors, fraud and contain costs for members. She emphasizes that these costs aren’t savings, it’s money that shouldn't have been paid in the first place.  ClaimInformatics has a process where they are able to identify six levels of errors that lead to overpayments, including upcoding, miscoding and outright fraud. They review ASO/TPA network agreements, acquire and review all data, re-adjudicate claims, then share the results with clients to illustrate the level of overpayment in their plan. From there, they initiate the recovery process where they typically recover 80% of improper payments on behalf of the employer. In addition to recouping money for the employer, they put providers on notice who are engaging in egregious billing practices that they are now being watched and will be reported to the Network and Medical Board if behavior continues.  ClaimInformatics works primarily with clients who are self-funded and under ERISA guidelines. They have flexible fee structures with aligned incentives to generate results for their clients. Dawn recommends everyone take a hard look at their reports, review their ASO agreement, and become acquainted with their performance audit terms. ClaimInformatics stands for integrity and member-centric service, and we’re excited to see how they continue on this trajectory into the future.  Here’s a glance at what we discuss in this episode:  00:30 - Introducing Dawn, the co-founder and Chief Transparency Officer of ClaimInformatics. 02:30 - 30 years ago, she got into the claim processing and became a System Configuration Specialist; she then became a plan administrator for a Fortune500 commercial group.  05:35 - They read the story in the data, and the data isn’t good in terms of waste via unnecessary procedures, expensive services, bad systems, and egregious contracts.  07:45 - ClaimInformatics is all about integrity; they ensure payments are accurate and in accordance with agreements made.  08:30 - They catch errors and fraud to contain cost; it’s not savings, it’s money that shouldn't have been paid in the first place.  10:55 - Why are we still seeing 2-3% leakage when that waste is 100% preventable? 11:15 - Most of their clients are self-funded and under ERISA guidelines.  11:30 - The ClaimInformatics process: They review ASO, ascertain and review all data, re-adjudicate claims, then take the results to show clients what they’ve captured. 12:50 - They follow the same guidelines as a claim’s office, make deposits on behalf of clients, and are member-centric in terms of protecting their overpayments, too.  14:05 - They ensure member liability is made whole; they go back three years and see lots of coding and billing errors that are non-compliant of the rule sets.  17:15 - Historical claims review results: Incorrect codes and upcoding make groups and members pay more.  19:30 - Medical records either support or don’t support the coding choice; they have seen upcoding happen frequently across the board with outside billing companies.  21:10 - There’s more abuse today than ever; there are six levels of errors that they’ve identified.  25:22 - Providers are paid...

When Dawn was 15 years old, her parents were given a diagnosis for her which explained why her experience with puberty was different from that of other girls her age. Following the recommendations of doctors at the time, her parents told her that she did not have a uterus and would not be able to have biological children—but did not share the diagnosis of Androgen Insensitivity Syndrome (AIS) with her. At age 38, Dawn stumbled across this diagnosis when her 23andme results indicated that her karytoype was 46,XY, rather than the 46,XX karyotype that is typical for females. Dawn discusses how this diagnosis helped lead her to self-acceptance and to connection with others who have lived similar experiences. Have thoughts or a related story you’d like to share? Dawn Rowe's first experience with desert was driving to Las Vegas for a Marine Corps Ball. Alan, her late husband, needed something for his uniform and decided they would drive the back way to Vegas, passing the Marine Base in 29Palms where he would pick it up at the PX. Driving up the Morongo grade and then down into the valley, Dawn remembers her first thought was "Who lives out here?”  And now she does – falling in love with a community where she's become a very involved and active citizen, all the way to her current position as San Bernardino Third District Supervisor.   Prior to coming to the desert, Alan’s military career found them spending a lot of time in the San Diego area, then Virginia and on to 29 Palms. After landing in Yucca Valley, Alan, was killed in Operation Iraqi Freedom in 2004 - their children were five and three.   When Dawn moved to Yucca Valley in 2003, there was no Home Depot, Starbucks or Applebee’s. She and her husband decided to live on the east end of Yucca Valley, giving Dawn, then a young mother with two small children, access to more people and places.   Dawn's community developed through volunteering in the schools, her church family and interacting with military wives. One of the reasons Dawn decided to stay in the hi-desert after the loss of her husband was her connection to this community and proximity to the base. Dawn says she and her children transitioned to civilian life in a place she calls, ‘peaceful and beautiful’.   Dawn talks about getting through the loss of her husband with the assistance of the Marine Corps. While the Corps itself assisted with the logistics of her new life, she never imagined her own experience as a volunteer shepherding young military wives through losses would apply to her. Dawn enjoys the connections that she calls ‘small town wonderful’ – in a place that provides opportunities to not only have social connections with your neighbors, but to sometimes even work with them, ultimately becoming people you can lean on in times of crisis.   Raised in a volunteer-oriented family, Dawn gravitated to public service very naturally.  An experience in land development spurred Dawn's interest in public service, leading her to commit to making San Bernardino County a business-friendly environment.  Having built a new home with her family, she decided she’d enjoyed the process so much, that she would purchase another piece of property, subdivide it and build two more homes. Dawn experienced a number of issues in the subdivide process and it was that frustration that spurred her to go into the Town offices and ask some questions and gain some clarity around the bureaucracy she was experiencing.  She introduced herself to, then Mayor of Yucca Valley, Chad Mayes, who suggested she become the Planning Commissioner for Yucca Valley, where learned about land use. This role eventually lead her to a seat on the Town Council and eventually Mayor of Yucca Valley. In the meantime, she’d also obtained her substitute teaching credential and while doing so also worked part-time job with then Assemblymen Paul Cook.   In this episode, we also talk about a law suit filed against the SB BOS challenging the process of Dawn's appointment and the large territory of nearly half a million people that she covers as 3rd District Supervisor. We also dig into a few local issues like affordable housing, looming Airbnb regulations, limited rural area law enforcement and code enforcement issues.   If you live in the Third District and have issues or questions, reach out to Dawn and her office here.

Dishing with Delishes Podcast | Interviewing Food Bloggers | Help Food Bloggers Grow Their Business | Learn From Others Succe

Dawn Coombsis a girl that hearts food, aka the blogger behind Girl Heart Food. There’s always something delicious cooking in her home because she and her husband love to cook and eat. As soon as one meal is over, Dawn is thinking about what the next meal will be. The philosophy behind her blog is simple– ‘Eat Well. Cook with Love. Make your Belly Happy.’ Most of the recipes you’ll find on Girl Heart Food are healthy, while others are indulgent (because she LOVES cheese). But the main thing is that the recipes are easy and delicious! Dawn loves using simple ingredients, that are easily accessible and relatively budget friendly so that people can prepare fuss-free meals that their families will love. In her kitchen, you’ll always find cheese, butter, garlic, onions and pantry staples like beans and lentils. One of her favorite quotes by Julia Child is, "If you’re afraid of butter, use cream." That one always puts a smile on her face. Cooking is such a passion for Dawn and being in the kitchen is a happy place. She loves how there’s always something new to learn and try...and having fun along the way! Dawn enjoys all types of cuisine, but one of her hands down favorite meals is her mother’s turkey dinner with buttery mash, dressing, gravy and all the delicious sides. There’s something so special about that meal and no one can make it like her mother. Her husband makes a mean Bolognese with homemade pasta that she absolutely loves too. Give  her pasta with a glass of wine and a hunk of crusty bread and she’s one happy lady! When Dawn is not testing and preparing recipes for Girl Heart Food, she loves to bake bread just for the pure joy of it. Her grandfather was a baker and she has fond memories of visiting his bakery after school when she was quite young. The smell of freshly baked bread takes her back to those happy times. When she isn’t cooking you can find her working out, spending time with her family and reading mystery novels.

Searching For Ghosts: Where Is Bethany Markowski?

I’m headed to Milan and it’s a perfect day for a road trip. It’s sunny and warm, and everything is starting to turn green. As I get older, the winters get harder to take. Everything is just so drab. But if it weren’t for the  winter that I have come to despise, I probably wouldn’t appreciate the spring and summer.   In doing my research on this case, I’m amazed at the amount of criminal activity that seems to permeate West Tennessee. And it’s not just here. This is everywhere. I begin to realize that maybe I was somewhat sheltered growing up. And I start to appreciate even more, my childhood years. Not everyone had it so good.   My destination is a house in the somewhere between Milan and Jackson, Tn, the town where I grew up. As I travel the winding back roads of West Tennessee, I can’t help but think about all the secrets held in this area for over twenty years. And just by looking at it, with rolling pastures and well kept homes, you’d never know that there are some really bad people in this world. And some of them are our neighbors.   A couple of days after Episode One came out, I received a message from a woman named Dawn who wanted to meet with me. She said she knew Cayce and the family, and was very involved in the search efforts when Cayce went missing.    She also wanted Cathy, a long-time friend of the family, to join us to help fill in any blanks. Dawn told me to carve out two days for interviews because there was just so much information. We didn’t need two days, but we did talk for over six hours.   I’m Brandon Barnett. And this is Searching For Ghosts.   Cayce was at an age when a lot of kids start to rebel. It happens. For many, it’s a rite of passage. Everything that has been made public about Cayce from friends and family over the years, might lead one to believe that she was immune to any teenage rebellion. As far as I’m concerned, there are three possible explanations for this:   1) People might be concerned about soiling her reputation, which might affect people’s opinion of her and their interest in the case. Don’t think for one moment that the image of the innocent beauty queen that was Jon Benet Ramsey didn’t fuel the public’s fascination with that case.   2) Cayce showed a certain side of herself around some, and then a totally different side to others. That’s what teenagers are best at.   3) She wasn’t going through a rebellious stage. It has happened. My parents were my best friends throughout my teenage years. I never really went through that phase. I was either fishing, playing basketball or holed up in my room with a guitar in my hands. I was pretty much a straight arrow.   But the summer Cayce disappeared, both Dawn and Cathy noticed a drastic change.     And while in 2017, the Goth look seems pretty tame, in 1996 in rural West Tennessee, this was a pretty big deal. If nothing else, it was a definite departure from the Cayce of just a few months before.   It’s been about two months since I stopped by the Milan Police Department to ask for any reports about Cayce that are open to the public. I was told that the initial report should be available for me to read. I’ve had friends and family of Cayce ask for those reports as well. But it appears that we’re being stonewalled.   And the daily paper out of Jackson, Tn, The Jackson Sun, is going through their archives to find articles about the case. But this will take some time.   So the Milan Mirror Exchange has been my main source at this point to try to piece this thing together. It is a weekly paper, so I have to do a little estimating on the exact timing of certain findings.   While looking through these reports, it appears that by September 2, 1996, police had a sketch of a possible suspect. After a little digging around, I discovered that the sketch came from a witness who alleges that Cayce was seen with this man at The Gibson County Fairgrounds in Trenton, Tn, a town about fourteen miles away from Milan.   This sketch is floating around the internet, but like so many things about this case, it is never mentioned again. So I started asking around, and the word is that the source was deemed not credible. Not one person that I talked to said that they believed she was ever at the fair in Trenton. And given that all the descriptions of Cayce by law enforcement had her in her bed clothes with no shoes, I felt that it wouldn’t make sense for her to be walking around the fair a couple of days later. So I made note of this, but pretty much dismissed the sketch myself.   But then I found an article just one month later in the October 1st edition of The Mirror-Exchange about a world-famous search and rescue dog named Valorie who came to Milan to search for Cayce. The headline read: “Cayce Left On Her Own Free Will, Smart Dog Says." Her own free will? Again, why would Cayce leave on her own free will in boxers, a tee shirt and no shoes. The no shoes story is one of the few things in this case that keeps being repeated year after year. I needed to know more about this smart dog and her findings. It just so happened that I was sitting at the kitchen table of one of the people responsible for bringing Valorie to town. Dawn and her husband had even video taped the search.   Valorie had found ten people in the Oklahoma City bombing the year before. At the time, she had flown 40 times to find missing persons, had been involved in 600 cases   So I desperately wanted to read the official report of Valorie’s findings. That would definitely be something that would’ve been given to law enforcement at the time, seeing how there was at least one report in the media about it.   But I wasn’t going to get it from the Milan Police Department. I finally received word from Milan PD that they were not going to release any reports whatsoever pertaining to the Cayce McDaniel case. Damn.   So I decided to look up Valorie’s handler at the time, Harry Oakes. Valorie has passed on, but Oakes is still doing search and rescue with his organization International K-9 Search and Rescue Services. In 1996, he was working under the name Mountain Wilderness Search-Rescue-Recovery-International Response Team.   In 1996, Oakes and Valorie were brought in to Milan by Dawn and others with private funds. So I hoped that Oakes would be able to help me seeing as how this mission was not sanctioned by local law enforcement.   According to a 1998 article in The Tennessean, Billy Hale was with The National Missing Children’s Locate Center. At this time, I am having trouble finding any footprint on the internet of Billy and his organization. I only pray that the organization didn’t die with Mr. Hale. If you have any information on this please contact me on our website, sfgpodcast.com   When Dawn first reached out to me, she told me a story that I had never heard before. And to be honest, it blew my hair back. And this story, in my mind, might piece together the things we’ve discussed in this episode.   There are rumors that Cayce made a phone call from the church rec center pay phone the night she went missing. Who would she have called? Her mother didn’t have a phone at the house. The grandmother reportedly never received a phone call. Her best friend was at the church social with Cayce.   Did Cayce call someone to arrange for them to pick her up after the church party? Was Cayce going through the teenage rebellion stage that so many have denied? Did the police investigate this alleged phone call? Is there any way to get the phone records today from a pay phone in 1996?   Searching For Ghosts Website   International K-9 Search And Rescue Services

Kevin talks with Dawn Stefanowicz, who shares her story of growing up in Canada with heavy exposure to the male homosexual lifestyle. When Dawn was still an infant, her father brought a homosexual partner to live in their home for the first time. Throughout her childhood she was introduced to other young men involved with her father, including minors and some who later committed suicide. Dawn was taken to places that no child should be exposed to, such as gay nude beaches and “sex shops.” Dawn’s father died from AIDS in 1991, and his last living partner died from AIDS in 1996. Dawn was not comfortable with what she saw and experienced, but felt that she had no voice to speak her concerns until her late 20s. It was at this point in her life that she found healing through Christian counseling, and realized that she should speak out. She began to testify about the homosexual lifestyle in court cases regarding issues such as hate crime legislation, and wrote a book about her childhood. Since going public with her story, Dawn has been contacted by dozens of others raised by a homosexual parent and has found that her own experience was not unique. Kevin and Dawn discuss many key issues in the current debate over sexuality, such as gender identity formation, transgenderism, same-sex marriage, freedom of speech/”hate speech”, and same-sex parenting. They consider how the church can offer love and support to those who struggle with their sexuality, while also upholding the ideal pattern that God has provided for human relationships and families. “So I find this law at work: Although I want to do good, evil is right there with me.” —Romans 7:21 This episode first aired on April 26th, 2015. Dawn Eden speaks about healing the memory, looking towards the Jesuit spirituality of St. Ignatius Loyola and Pope Francis. This lecture is especially recommended for family and friends of survivors of trauma and abuse. About Dawn Eden: Dawn Eden is the author of My Peace I Give You: Healing Sexual Wounds with the Help of the Saints (Ave Maria Press, 2012) and The Thrill of the Chaste: Finding Fulfillment While Keeping Your Clothes On (Thomas Nelson, 2006). Born into a Jewish family in New York City, Dawn lost her faith as a teenager and became agnostic. During her twenties, in the 1990s, she was a rock journalist in New York City, interviewing performers such as Elton John and Brian Wilson. She went on to work on the editorial staff of the New York Post and the Daily News. When Dawn was thirty-one, she experienced a dramatic conversion to Christianity that ultimately led her to enter the Catholic Church. Her first book, The Thrill of the Chaste, became a surprise hit, published in four languages. In My Peace I Give You, Dawn offers a Catholic spirituality of healing for adult victims of childhood sexual abuse. She has shared its message throughout the United States, as well as in Canada, Ireland, the United Kingdom, and Slovakia. My Peace I Give You has been published in Spanish and Slovak translations, and is soon to be published in Polish. Father James Brent, O.P., Director of the Dominican Friars’ Angelic Warfare Confraternity, wrote in a letter to confraternity members: “For anyone who has suffered from sexual abuse, Dawn’s book is a must read. The testimonies of victims who have read it are powerful testimonies indeed. But in my opinion, anyone who suffers from the wounds of sexual sin in any way whatsoever will also find in this book a powerful aid for healing and renewal.”
