Is fezolinetant (Veoza) safe to use alongside other medications, such as Oxybutynin or SSRIs? Do we have any data on interactions? Can fezolinetant (VEOZA®) be used in combination with MHT to target these hot flushes? Also, what are your thoughts on testosterone? Some of my patients use compounded testosterone to manage their symptoms. Would this be helpful for this patient, and can testosterone be used alongside fezolinetant (VEOZA®)? Patient Case: A patient started fezolinetant (VEOZA®) nearly a month ago to address almost hourly sweats and flushes. Initially, she saw a significant improvement for the first 5 days, but now the frequency and severity of symptoms have gradually returned. What are the risks posed by cardiovascular disease when you’re considering menopausal management? What do you consider high risk cardiovascular disease? For example, if a woman has a strong family history of heart disease but a zero-calcium score. Does the use of menopausal hormone therapy reduce the risk of cardiovascular disease and death? Can you diagnose menopause in a woman with amenorrhea, or very irregular periods by doing a progesterone challenge test. Any tips on how to diagnose? Is a persistent high temperature, greater than 39 degrees a vasomotor symptom, because they're suggesting that thermoregulation might be affected? Are you aware of any evidence on cannabidiol oil in treating menopausal symptoms? Can androgen deficiency cause hot flashes, fatigue or their mood? Could you provide more information on where patients can find evidence supporting the use of cognitive behavioural therapy for managing vasomotor symptoms? Additionally, it would be helpful to know if there are online services available that patients can access for this treatment How can you best treat brain fog? Is it reasonable to use different pharmacological agents that target specific symptoms, such as urinary urgency or hot flashes, before considering hormonal treatments? Is this an appropriate approach to managing symptoms? What indicators would there be for treating a woman with Vulvovaginal syndrome of menopause with something like Intrarosa dehydroepiandrosterone (DHEA) instead of Avastin or Estradiol tablets? Would you recommend or suggest starting local oestrogen creams for a patient above 60-years-old for atrophic, vaginitis or dryness? Is there any upper limit? How long can you safely use these vaginal preparations? Can you cut hormone patches when you're weaning someone off menopausal hormone therapy or when the patient just can't obtain the right dose. Is it okay to cut the patches (i.e in half, or thirds)? Why should paroxetine be avoided for the treatment of hot flushes in tamoxifen users and is there anything else we should be avoiding in these patients? Are there any effective treatments for a woman who presents between late 60-70 years old for management of hot flushes that have come back after settling and, should we be using menopause hormone therapy or starting them on non-hormonal therapy? Patient case: I've got a patient classed as BI-RADS D who's currently on menopausal hormone therapy. How should we monitor her? Patient Case: I have a few patients who are starting ashwagandha based on information from social media and blogs. Are there any research studies on its effectiveness? What exactly is ashwagandha, and is it beneficial for managing menopause symptoms? Additionally, are there any potential risks associated with its use? Is there any feedback, data or experiences supporting the efficacy of happy mammoth supplement? Selective serotonin reuptake inhibitors (SSRIs) can sometimes cause sweating as a side effect and is this ever an issue when you're trying to use it for hot flushes? Does Mirtazapine have a role in the management of vasomotor symptoms? Could you please discuss the use of Prometrium per vaginally and the recommended doses when you give it by that route? If you're on a standard or higher dose of something like Estraderm, is there a need to increase the Prometrium dosing on the higher dose? Do you need to give double the dose or give it more often? Can you please clarify the role of Tibolone in menopause, including its relative benefits and risks? I'm having trouble with resistance of some of my patients to start menopause or hormone therapy due to being told by their breast physician, surgeon, and friends that no woman with even a moderately increased risk of breast cancer should ever be on menopause hormone. How would you recommend that we advise these patients? Patient Case: I've got a 40-years-old who's perimenopausal and presents with hot flushes, mood swings, night sweats and brain fog. She has a history of Churg-Strauss syndrome. What is the best option for her treatment? Patient Case: I see lots of perimenopausal patients who have already started menopause hormone therapy and have irregular periods. However, on menopausal hormone therapy they get regular periods and they feel better, do they need contraception advice? Do you need to use contraception if you're on menopausal hormone therapy? Menopausal symptoms can take a long time to resolve, this study was looking at 20% of women having hot flushes 8 year prior to their last menstrual cycle, that is far too long to be suffering, there are things you can do to improve your situation, you are worth it, please don't put your head in the sand and try to wait it out. Today, we hear from a mother in Gaza. Nour Swirki is a journalist with two children living in Khan Younis, a city in the southern strip of Gaza, with her husband, mother and sister - they have had to leave their home in Gaza City for safety reasons and relocate to the south - a fraught journey many more Palestinians are expected to make. Due to the difficulties in speaking live to guests in Gaza, we asked Nour to record for us voice notes explaining the situation she and her family are currently in. She and her husband continue to work as journalists while her wider family look after her children - a son and daughter aged 10 and 12. The ABC News Breakfast guest host Imogen Crump has been praised for helping to normalise symptoms of perimenopause, after she experienced a severe hot flush on live television. Emma Barnett asks her about what happened. Research from the University of Bristol demonstrates that women can lose out financially when they divorce. The number of couples seeking legal advice during divorce proceedings is falling, and old-fashioned procedures that disadvantage women are being used. To find out more, and get advice on how to make sure you are not penalised financially, Emma speaks to financial planner Megan Jenkins and family lawyer Amanda McAlister. The NHS is launching a new study into Alzheimer's disease, the most common form of dementia, which affects around twice as many women as men. The study, a joint project with Alzheimer's Research UK and Alzheimer's Society, will use a blood test to detect for the disease at an early stage. There is currently no single test for Alzheimer's and patients can wait years for a diagnosis. Dr Susan Mitchell, Head of Policy at Alzheimer's Research UK, one of the charities leading this study talks to Emma. Emma talks to journalist Linda McDougall about her new biography of Marcia Williams, who went on to become Baroness Falkender. Linda hopes to shift public perception of the Baroness as a Svengali figure who influenced Prime Minister Harold Wilson during the sixties and seventies, and gain recognition for her achievements for the Labour Party. Dawn Adams jovially describes herself as ‘chaos', but in my opinion she is a multi-tasking, time-defying, change-making superhero - with an undeniably soothing voice to boot.Dawn is a former teacher turned midwife with a specialist interest in diabetes and pregnancy from Northern Ireland. She's is also completing a PhD in type 1 diabetes, pregnancy and wearable technology, and uses any remaining spare minute to advocate for awareness around type 1 diabetes and menopause, after she found a lack of information and peer support to help guide her through her own experience. All of this, plus she's been handling the not-so-small task of living with type 1 diabetes for 30 years. Diagnosed at the age of 22, Dawn was one of the first in her hospital to receive a modern insulin injection pen, but in the same week she was told it was unlikely she would be able to have children because of her diagnosis. That thankfully turned out not to be the case, and she has four healthy boys with the man who proposed to her just weeks after her type 1 diagnosis - a commitment to figuring it all out together. Dawn's full time job as a midwife is dedicated to helping women bring life into the world, and her advocacy work gives a mouthpiece to the silenced women who are coping with the life-altering affects of menopause with type 1 diabetes. The conversation begins by exploring the evolutionary theory of the grandma hypothesis, which suggests that menopause is a feature rather than a bug. We delve into the menopausal transition and how symptoms, such as hot flushes, are not solely driven by an absolute estrogen deficiency. In this light, non-hormonal treatments are explored. Additionally, Professor Lumsden outlines the latest updates on the benefits and risks of menopausal hormone replacement therapy, suggesting that the risks may have been overstated in the past. We then turn to the health challenges that menopausal women often face, such as unwanted weight gain, body composition changes, osteoporosis, and dementia. Professor Lumsden provides insights on evidence-based clinical strategies to support women during this transitional phase of life. The average age of a woman in menopause is 51 but can range between 40-58 years old. We are going to spend a third of our lives in menopause so why not make it as vibrant as possible? There is no need to suffer during perimenopause, menopause or post-menopause when we have options to support us. Perimenopause and menopause may include a variety of symptoms such as hot flashes, brain fog, low libido, anxiety, hair loss, and weight gain. Menopause also opens the door to an increased risk of chronic diseases, such as cardiovascular disease, stroke, Alzheimer's, osteoporosis, and metabolic syndrome.I've said this before on previous podcasts, and I will say it again, at the end of the day you get to make the final decision on whether or not to take hormone replacement therapy, whether it's HRT or BHRT. At the very least, educate yourself on the topic, the benefits, and the possible health consequences if you don't do anything. Then, make your decision.My guest today is Dr. Wen Shen, an associate professor of clinical gynecology and an associate professor of oncology at the oncology center in the Johns Hopkins Medicine Department of Gynecology and Obstetrics. She is also the clinical director of the Menopause Consultation service and Co-Director of the Women's Wellness & Healthy Aging Program.A board-certified gynecologist, Dr. Shen has concentrated on menopause medicine for more than 15 years. She specializes in treating peri-menopause and menopausal symptoms taking into consideration the woman's overall health. She often works with her patients' primary care provider to recommend non-hormonal therapies, alternative treatment options and menopause hormone therapy as appropriate for the individual patient.In addition to clinical care, Dr. Shen focuses on educating her patients, other physicians and future physicians on women's post reproductive health. She is engaged in clinical trials for new therapies for the treatment of menopause symptoms. She is actively developing curricula and educational aids for future physicians and health care providers on menopause medicine and the importance of healthy aging. Dr. Shen has published numerous articles and book chapters on menopause and perimenopause. She is a fellow of the American College of Obstetricians and Gynecologists and is on the Board of Trustees for the North American Menopause Society.Dr. Shen earned her Master's in Public Health and her Medical Degree from the Tulane University School of Medicine. She completed her residency in Gynecology and Obstetrics at The Johns Hopkins Hospital. It can cause huge distress, especially in a professional or social situation or if it's persistently disrupting your sleep. But help is at hand in this episode with this month's guest who is just the woman we need to provide us with some practical flush-busting strategies.I'm delighted to introduce the brilliant Dr Melanie Smith – she's a clinical health psychologist who played a key role, along with Professor Myra Hunter,  in developing and facilitating the first cognitive behavioural therapy, or CBT as it's often known, for menopause groups as part of an innovative research trial. This research demonstrates that CBT is a valid and effective non-medical approach to managing menopause symptoms and Melanie is here today to explain to us what a huge difference it can make to dealing with hot flushes. We discuss what hot flushes are and why they happen.
