Host Kevin Stevenson welcomed Clive Smith, founder and CEO of Thinklabs to talk about his company's digital stethoscope.Smith has an electrical engineering background and was researching medical devices. He learned the stethoscope still worked and sounded the way it did when invented in 1816. He spent years researching how to digitize the instrument and bring exceptional sound quality. The first product hit the market in the early 2000s and would find its place in the world of infectious disease outbreaks.“We worked with units treating Ebola patients in 2014, allowing them to listen to patients while staying protected in their PPE,” Smith said.So how exactly does the digital stethoscope work? Smith explained, “It's a round puck that sits on the patient's twist and can be connected to any type of headphone.”The “puck” is small, fitting in a hand, but powerful. It would become a highly sought-after medical device during COVID-19, with new challenges. “There were new situations with COVID-19. There was drive-thru testing and a shortage of PPE. We were solving problems in real-time,” Smith said.The company also learned through a study that the device could be bagged and not dilute the sound. “It can be bagged in many layers without diminishing the sound, so healthcare workers can use pass it around, removing bags to keep it clean.”Smith noted they are continuing to improve their main product but could innovate on other devices in the future. He also shared his perspective on the future of digital health. “Telemedicine is here to stay, and remote monitoring will evolve to support this. COVID-19 was an accelerant to things already there, but now we see widespread adoption.”

Clive Smith started Thinklabs in the early '90s after graduate work at Cal Tech in electrical engineering. He focused on electronic and signal processing and was eager to start a company specializing in the innovation and application of electronics and technology. The name says it all—he went with Thinklabs to signal a company that would handle and solve interesting problems with hands-on lab work. When he encountered the lag of stethoscope advancement in the mid '90s, he knew he'd found an important project.  But he soon found that it was a "very nontrivial problem" to extract sound from the human body, and it took eight years to come up with this innovation in digital healthcare technologies.  While the product has been on the market for about 17 years now and sees wide use in normal clinical medicine and telemedicine, the COVID crisis has made its remote abilities especially useful. Furthermore, now that AI technology has caught up a bit, numerous studies can take place to recognize its true potential. At the most basic level, doctors are able to hear things much more clearly and enable earlier diagnoses of conditions. Thinklabs was founded in 1991 by Clive Smith, an Electrical Engineering graduate of Caltech. The name implies the company's goal - think deeply about problems that matter and develop imaginative solutions. Smith has always had a passion for medical electronics, sound, music, and signal processing. In the mid-'90s, Smith read a Circulation paper, indicating that stethoscope acoustics had not improved since Laennec built the first stethoscope in 1816. Physicians confirmed that even top-of-the-line conventional stethoscopes did a poor job of amplifying heart and lung sounds. Thus began Smith's obsession to re-invent the stethoscope. The concept of an electronic stethoscope was not new. For a century, the road to perfect auscultation was littered with the carcasses of failed efforts. Smith started experimenting. Technologies were tested, rejected, modified and retested. The key breakthrough came with a question: How can one create an electrical analog to the acoustic pressure changes in a conventional stethoscope? All the benefits of advanced electronic technology would then accrue and the authentic sound of the stethoscope would be preserved. Physicians would not require any ear retraining. A completely new transducer was needed. Clive Smith went on to design the One Digital Stethoscope. But I want to find out more about that journey and the Thinklabs One motto of never stop inventing. We discuss how doctors and patients are leveraging new technology for remote medicine.

Russell & Robert meet iconic British actor Sir Ian McKellen CBE at his home in East London. We have a private tour of Ian’s art collection including stunning works by L.S. Lowry, David Hockney, Andrew Logan, textile artist John Allen and an Antony Gormley cast iron figure he chose to display in the River Thames at the end of his garden! We learn about Ian’s childhood in Bolton and how his father’s natural talent for art meant Ian chose a different way to express himself on stage and screen! However his own passion for art led him to buy a work to commemorate every part he’s played around the world and he even set up an artist residency in the house next-door. Ian remembers Andy Warhol taking his photo at The Factory in 1970s New York (& buying a screen print of Mick Jagger), being painted twice by his longterm friend David Hockney and why he sees the paintings of L.S. Lowry as stage sets. We also discuss the 20 portraits of Ian at the National Portrait Gallery collection by artists as varied as Cecil Beaton, Lord Snowdon, Mary McCartney, Trevor Leighton and Clive Smith who won the BP Award from the National Portrait Gallery and was given the commission to paint McKellen for their collection. Whilst, Christian Hook painted McKellen in the final episode of Portrait Artist of the Year on TV and won the prize also!
