Joseph C. Kvedar, MD shared his keynote before the 2021 American Telemedicine Association.  He addresses a regression that is underway from local providers dominating the delivery of telehealth to the national providers once again taking over the top spot.  FTAWhen I talk to other provider organizations, I hear common themes dragging them back into an in-person-dominant care model. Themes such as filling beds and charging facility fees come up repeatedly, not to mention the threat of lower reimbursement for telehealth visits compared to in-person.I recently talked to a friend who works at one of the largest national payer organizations, which led to an important insight.  From January through October of 2020, local providers (i.e., ‘your doctor') generated 96% of their telehealth claims, and only 4% came from national providers (i.e., Teledoc Health, Amwell).  Compare that to 2019, when 54% of claims were from national providers – and the trend is moving back in that direction.-----Is this inevitable?  We still live in a fee for service world and the financials don't work without the government money.  If the government money comes in is that really a good thing or have we just added one more arm to the Frankenstein that is the healthcare payment system.The government is the largest payer in this market, if you can call it that.  They will have to consider what works best for the millions of lives under their care and in so doing have downstream impacts on all other lives.

Dr. Joe Kvedar, Professor of Dermatology, Harvard Medical School, author, editor, advisor and telehealth evangelist, interviewed by Trond Arne Undheim, futurist, investor, and author. In this conversation, we talk about the history Telehealth and it's transformation into a two-channel delivery system combining in-person and virtual care. A precursor in the field, Joe charts the 20 year evolution from "show me the data" to "how do I implement?". We briefly discuss the use case of Melanoma therapy and AI/imaging from Kvedar's own field, dermatology. We discuss COVID-19 effects and other disruptive forces. Joe shares the startups he sees disrupting the game and how he stays up to date. We cover the emerging two-channel care delivery system and discuss the future of virtual care in the next decade. My takeaway is that Telehealth has come a long way in 20 years and it's not just about the technologies, but it is also about adapting to a hybrid model of care where you each time select the communication mode that best serves the patient given the constraints, and still is efficient for the health system at large. The future of virtual care is up for grabs, it is already starting to look different from five years ago. Leapfrogging seems entirely possible and we will see new winners but also old champions rise to the challenge having invested time and resources in the area for decades. What's certain is that both infrastructure and skills will need to evolve with the challenges and opportunities ahead. Dr. Joe Kvedar is a global leader and expert in the field of digital health and a board-certified dermatologist and professor of dermatology at Harvard Medical School. After finishing his residency and fellowship at Massachusetts General Hospital, he was an early implementer of teledermatology, and he quickly became involved in the telehealth movement. In 1995, he helped created Partners Connected Health, and through his leadership as director and vice president, he has spent over two decades paving the way for leveraging personal health tech, managing chronic conditions, and improving patient engagement and clinical outcomes. He's authored over 100 publications and is on the Editorial Board of the Nature Journal Digital Medicine. He’s just been re-elected as the president of the American Telemedicine Association, is co-chair of the American Medical Association's Digital Medicine Payment Advisory Group, and a consultant and advisor for multiple societies and med tech companies. The commercialization process in healthcare is very complex, and it’s impacted by a multitude of factors and circumstances. Why is commercialization so much harder in the healthcare industry? What are the biggest mistakes innovators are making, and how do these mistakes make growth impossible? What do we need to be mindful of when trying to get into a delivery network or hospital system?   On this episode, I’m joined by Dr. Joseph Kvedar, VP of Connected Health with Partners HealthCare. Dr. Kvedar is a pioneer, early adopter, and influencer in the field of connected health. We talk about overcoming the commercialization difficulties in healthcare that so many innovators face.   3 Things We Learned What makes it so challenging to commercialize an innovation in healthcare Why having the most advanced/superior innovation is meaningless How to successfully get into a delivery network or health hospital system   When healthcare innovations are still small, just starting to garner entry level interest, is when they are most vulnerable to facing issues that impede commercialization. Whether it’s losing a key team member, or encountering changes in the market, that randomness is inherent in bringing innovations to market. There are ways to mitigate this, however.   It’s critical to settle the “people issues” early on and understand your product-market fit. Raise only the right amount of money, and spend it thoughtfully. Line everything up to present the best business case possible.
