It has a variety of causes, and its most common presenting symptom is dyspnea. As primary care clinicians, we often see patients with symptoms of dyspnea and pulmonary hypertension is not a health problem we commonly encounter. Yet, early recognition and diagnosis is important and has major implications for patient outcomes. What are some clues that a patient might have pulmonary hypertension? Participating in religious activities appears to benefit cardiovascular health among Black Americans. It's something we explored in an episode on this podcast a few years back. Health systems, professional societies and researchers are increasingly recognizing that “faith-based organizations are trusted institutions within underserved communities and that people not only seek spiritual refuge and salvation in these places of worship, but they are also wonderful, trusted vessels to  distribute reliable health information,” says Dr. LaPrincess Brewer, a faculty member in the division of  Preventive Cardiology, department of Cardiovascular Medicine at Mayo Clinic. “Participating in religious activities from church services to private prayer, as well as holding deep spiritual beliefs are  linked to better cardiovascular health among Black Americans," according to researchers of a 2022 study published in the Journal of the American Heart Association. The researchers go on to suggest that recognition by health professionals and researchers of the centrality and influence of religiosity and spirituality in the lives of African American adults may serve as a means to address cardiovascular health disparities. Most women will see their cholesterol increase steadily during the menopause transition and not have the proper labs taken to assess their cardiovascular disease risk, which is the leading cause of death among women (and men). That risk increases significantly after menopause. This phase of life brings not only physical and hormonal changes but also heightened vulnerability to heart disease. In this episode, we explore how menopause influences heart health and why it's essential for women to be proactive about their cardiovascular wellness. We discuss the hormonal shifts that affect everything from blood pressure to cholesterol. We will also discuss the impact of family history, and genetics and what labs and tests are crucial for assessing your risk. We discuss the ongoing debate over using estrogen as a tool for heart disease prevention versus statins. Dr. Martha Gulati, MD is a preventive cardiologist with a long, impressive list of accolades. Many significant medical discoveries were accidents, such as Fleming's discovery of penicillin in 1929. Similarly, in 1958, Dr. Mason Sones from the Cleveland Clinic accidentally injected contrast into a patient's right coronary artery, leading to a cardiac arrest. However, this incident allowed the first detailed visualization of the coronary artery, eventually leading to the development of coronary angiography, which remains the gold standard today. Dr Bleri CELMETA highlight these advancements, and explains the evolutions from open procedures to minimally invasive and robotic techniques.   Dr. Bleri Celmeta is a cardiac surgeon operating on the Minimally Invasive Cardiac Surgery Unit in Galeazzi-Sant'Ambrogio Hospital (Milan, Italy). He graduated in Medicine and Surgery at the University of Padova-Italy in 2014, then completed his residency program in Cardiac Surgery in the same university in 2020. His professional background included also a fellowship in Cardiac and Thoracic Surgery in the University Hospital of Nantes-France (2019-2020).   He is the author of numerous publications and conference presentations with particular interest in minimally invasive cardiac surgery, and review editor of various international Journals (Frontiers in Cardiovascular Medicine, Frontiers in Surgery, Innovations: Technology and Techniques in Cardiothoracic and Vascular Surgery, Journal of Cardiothoracic Surgery, among others). Prof. Stefan Neubauer and Dr. Thomas Nero discuss two recent papers on the utility of coronary inflammation imaging on CT coronary angiography with Fat Attenuation Index (FAI)FAI score.  The Orfan study demonstrated a 29 fold increased risk in CV event rates with low vs high FAI score and a 10 fold increased event rate in patients with elevated FAI score who have no coronary plaque. FAI score re-categorized 40% of patients in their follow-up studies. Prof. Neubauer is the head of the Division of Cardiovascular Medicine and Director of the Oxford Centre for Clinical Magnetic Resonance Research (OCMR) at the University of Oxford and Past President of the Society for Cardiovascular Magnetic Resonance (SCMR).  He co-authored two recently published landmark papers:Farina NH, Mehta NN, Teague HL, et al. No cultivated meat company has raised more capital than UPSIDE Foods. In 2022, after having already raised about $200 million in previous rounds, the company raised another $400 million in a Series C round with a company valuation north of the coveted $1 billion unicorn status. No company in the space has garnered more media attention, both positive and critical, than UPSIDE Foods. No company has as much volume of cultivation capacity as UPSIDE Foods. No company is as old as UPSIDE Foods, as it was the first startup formed to take this technology out of academia and work to commercialize real meat grown slaughter-free. It's also one of the few companies in the world to have been granted regulatory approval to actually sell cultivated meat, which it did in the US. So it was only fitting that this conversation with UPSIDE CEO Uma Valeti take place in person inside the beating heart of UPSIDE's EPIC (Cultivated Meat Engineering, Production, and Innovation Center) cultivated meat pilot facility in Emeryville, California. I often say that I'm Uma Valeti's first biographer, since I profile him in Clean Meat, but I certainly won't be his last biographer, regardless of whether he succeeds or fails. And the last time I visited UPSIDE Foods, in 2017, when the company was still called Memphis Meats, and I got to enjoy their cultivated duck. At that time, they had only a handful of employees. Now, as 230 UPSIDE employees worked away in the dramatically nicer building that houses EPIC, I first got to enjoy four different cultivated chicken dishes. I tried both chicken that was FDA-approved and grown in smaller cultivators, and chicken that was yet to be FDA-approved, which was grown in 2,000-liter cultivators. Spoiler: they all tasted great, and were easily discerned from most plant-based chicken in scent, flavor, and texture. After the tasting, Uma and I sat down for this frank conversation in which we discussed UPSIDE's past, present, and future. That includes details about the scale and capability at which they currently sit, why they paused their plans for their vaunted Rubicon commercial facility in Illinois, what expansions they're planning on making at EPIC in California, what Uma thinks about the obituaries some journalists are writing for the cultivated meat industry, when he thinks cultivated meat will reach 1 percent market share in the total meat market, and much more.  In this conversation, you'll hear Uma elaborate on how the technology has gone from being decried as impossible to now possible, and what remains to be seen is whether it will now go from possible to inevitable.  It's a fascinating and revelatory conversation with a man who has served in many ways as a face for the cultivated meat movement for many years, even prior to founding this company. They discuss a case of disseminated haemophilus influenzapresumed fulminant bacterial myocarditis with mixed septic/cardiogenic shock. Expert commentary is provided by Dr. Stanley Liu (Assistant Professor, Division of Cardiovascular Medicine, University of Maryland School of Medicine). A woman in her twenties with a history of intravenous drug use presented with acute onset fevers and sore throat, subsequently developed respiratory distress and cardiac arrest, and was noted to have epiglottic edema on intubation. She developed shock and multiorgan failure. ECG showed diffuse ST elevations, TTE revealed biventricular dysfunction, and pleural fluid culture grew Haemophilus influenza. Pearls - Sore Throat, Fever, and Myocarditis - It's not always COVID-19 The post-cardiac arrest ECG provides helpful information for diagnosing the underlying etiology.​ Be aware of diagnostic biases - availability and anchoring biases are particularly common during respiratory viral (such as COVID-19, RSV) surges. Consider a broad differential diagnosis in evaluating myocarditis, including non-viral etiologies. Right heart catheterization provides crucial information for diagnosis and management of undifferentiated shock​. When assessing the need for mechanical circulatory support, consider the current hemodynamics, type of support needed, and risks associated with each type. Show Notes - Sore Throat, Fever, and Myocarditis - It's not always COVID-19 ECG findings consistent with pericarditis include diffuse concave-up ST elevations and downsloping T-P segment (Spodick's sign) as well as PR depression (lead II), and PR elevation (lead aVR). In contrast, regional ST elevations with “reciprocal” ST depressions and/or Q-waves should raise concern for myocardial ischemia as the etiology. Biventricular dysfunction and elevated troponin are commonly seen post-cardiac arrest and may be secondary findings. However, an elevation in troponin that is out of proportion to expected demand ischemia, ECG changes (pericarditis, ischemic ST elevations), and cardiogenic shock suggest a primary cardiac etiology for cardiac arrest. The differential diagnosis of infectious myopericarditis includes, most commonly, viral infection (respiratory viruses) and, more rarely, bacterial, fungal, or parasitic. Noninfectious myopericarditis may be autoimmune (such as lupus, sarcoidosis, checkpoint inhibitors), toxin-induced (alcohol, cocaine), and medication-induced (anthracyclines and others). Right heart catheterization can help diagnose the etiology of undifferentiated shock, including distinguishing between septic and cardiogenic shock, by providing right and left-sided filling pressures, pulmonary and systemic vascular resistance, and cardiac output. Mechanical circulatory support (MCS) is indicated for patients in cardiogenic shock with worsening end-organ perfusion despite inotropic and pressor support. MCS includes intra-aortic balloon pump, percutaneous VAD, TandemHeart, and VA-ECMO. The decision to use specific types of MCS should be individualized to each patient with their comorbidities and hemodynamic profile. Shock teams are vital to guide decision-making.
