I'm Andrea Samadi, and seven years ago, launched this podcast with a question I had never truly asked myself before: (and that is) If productivity and results matter to us—and they do now more than ever—how exactly are we using our brain to make them happen? Most of us were never taught how to apply neuroscience to improve productivity, results, or well-being. About a decade ago, I became fascinated by the mind-brain-results connection—and how science can be applied to our everyday lives. That's why I've made it my mission to bring you the world's top experts—so together, we can explore the intersection of science and social-emotional learning. We'll break down complex ideas and turn them into practical strategies we can use every day for predictable, science-backed results. As we are nearing the end of Season 14 here, it has been about reflection as we have looked back and reviewed past interviews. Our goal has not been about nostalgia, or remembering these interviews, the goal has been about integrating what we have learned. Taking what we know, aligning it with how the brain actually functions, and applying it consistently enough to change outcomes. And if there's one thing this season has reinforced, it's this: Sustainable success isn't built on intensity or focus alone—it's built on alignment. As we move into what's next, (Season 15) the focus shifts from understanding this alignment to bringing this alignment into a tangible, physical form, or embodiment. Not more information—but better execution. After hundreds of conversations with neuroscientists, educators, peak performers, and thought leaders, one truth keeps resurfacing— lasting success is never about doing more. It's about alignment. Alignment between how the brain actually works, how emotions drive behavior, and how daily habits compound over time. Season 14 has been about stepping back—not to reminisce, but to integrate what we have learned into our current life. I knew the minute that I was sent a couple of video clips from our past episodes, that I had forgotten about, that while I thought I had implemented the ideas from our past guests, I had some work to go myself. For this reason, we spent Season 14 and will resume with Season 15 next January, reviewing past episodes, with the goal of noticing what we have now aligned, that's bringing us results in our daily life. Core Reflection When we started this podcast 7 years ago, the goal was simple: bridge neuroscience research with practical strategies people could actually use. What I didn't fully appreciate then—what only became clear through repetition, reflection, and real-life application—is that information alone doesn't create change. Understanding the brain doesn't matter if we ignore what to do with the information we release each week: improving our sleep reducing our stress practicing emotional regulation with consistency that actually changes who we are at the core: our identity Season 14 has been about connecting those dots. Listening again to conversations with voices like Dr. John Medina, Dawson Church, Bob Proctor, Dr. John Ratey, Friederike Fabritius, and so many others, one pattern became impossible to ignore: The brain thrives on simplicity, repetition, and finding emotional safety to implement these concepts—not intensity or a quick fix. We will take the time with each interview review to offer ways for all of us to implement the lessons learned, so that when we finish 2026, we will be able to look back, and see where our changes all began. This week, we move onto PART 3 of our review of EP 72[i] with Shane Creado, MD and his book Peak Sleep Performance for Athletes recorded back in July of 2020. ✔ In PART 1[ii], we covered: How strategic napping, morning brain habits, and even the Silva Method all work together to reset your brain, boost performance, and transform your health from the inside out. ✔ In PART 2[iii] we continued with our review, diving a bit deeper into sleep deprivation and its impact of performance (whether you are an athlete, or just someone looking to improve productivity). ✔ PART 3, we will go a bit deeper into the impacts of concussions and brain injuries on our sleep and performance. Let's go back to 2020 and revisit what Dr. Creado had to say about sleep in this last episode of this season.   VIDEO 1 – Click Here to Watch   In the first clip of this episode, with Dr. Creado, he dives into the connection with concussions and sleep. He says, “Most people who have had a concussion end up with sleep problems. It makes a lot of sense when you think about the brain and how it regulates sleep and wakeful cycles and then it gets jarred. But what people don't realize is that even a mild head injury can really damage your brain. Even if you're not officially diagnosed with a concussion, you don't have to lose consciousness to have a concussion. You don't even need to have any symptoms to have your brain injured in some way. And then the little injuries along the way add up over time. So the brain is as soft as butter and in a hard, bony skull. Anything that jars it, even whiplash can cause your brain to be injured. And it accumulates over time. What's interesting is that the same regions of the brain that are most damaged in head injuries are also damaged in sleep deprivation and also alcohol use. The frontal lobes, the temporal lobes and the parietal lobes at the top of the brain.”

Pearls When thinking about donor selection, you need to consider how much physiologic stress your recipient can tolerate, and this may guide your selection of “higher risk” or “lower risk” donors.   The use of DCD donors has increased the potential donor pool and shortened waitlist times with very similar perioperative outcomes to DBD transplantation.  Post-operative critical care management rests on a fundamental principle to apply as much inotropic/vasoactive therapy as needed to achieve some reasonable physiologic hemostasis, and then getting “the heck out of the way!” There are no standard regimens as practices vary across centers, but rest on providing adequate RV support, maintaining AV synchrony, and early resuscitation.   The RV is fickle and doesn't take a joke too well. RV dysfunction post-transplant is important to watch for, and it can be transient or require aggressive support. Don't miss assessing for cardiac tamponade which can require surgical evacuation- “where there's space, that space can be filled with fluid.”   Induction immunosuppression post-transplant varies across centers, but some considerations for use may include (1) high sensitization of the patient, (2) high risk immunologic donor-recipient matching, and (3) recipient renal dysfunction to provide a calcineurin inhibitor (CNI) sparing regimen long term.  Management of heart transplant patients is a multi-disciplinary effort that requires coordination amongst heart failure/transplant cardiologists, cardiac surgeons, anesthesiologists, pathology/immunologists and a slew of ancillary services. Without a dynamic and collaborative team, successful cardiac transplantation could not be possible.  Notes Notes: Notes drafted by Dr. Natalie Tapaskar  What are the basic components of donor heart selection? In practicality, it can be a very inexact science, but we use some basic selection criteria such as: (1) size matching (2) ischemic time (3) donor graft function (4) immunologic compatibility (5) age of the potential donor and recipient (6) severity of illness of the recipient (7) regional variation in donor availability When thinking about accepting older donors (>50 years old), we ideally would screen for donor coronary disease and try to keep ischemic times as short as possible. We may accept an older donor for a recipient who is highly sensitized, which leaves a smaller potential donor pool. There is no clear consensus on size matching, but the predicted heart mass is most used. We are generally more comfortable oversizing than under-sizing donor hearts. Serial echocardiography is important in potential donors as initially reduced ejection fractions can improve on repeat testing, and these organs should not be disregarded automatically. For recipients who are more surgically complex, (i.e. multiple prior sternotomies or complex anatomy), it's probably preferable to avoid older donors with some graft dysfunction and favor donors with shorter ischemic times. What is the difference between DBD and DCD? DBD is donation after brain death- these donors meet criteria for brain death. Uniform Determination of Death Act 1980: the death of an individual is The irreversible cessation of circulatory and respiratory functions or The irreversible cessation of all functions of the entire brain, including those of the brain stem DCD is donation after circulatory death- donation of the heart after confirming that circulatory function has irreversibly ceased. Only donors in category 3 of the Maastricht Classification of DCD donors are considered for DCD donations: anticipated circulatory arrest (planned withdrawal of life-support treatment). DCD hearts can be procured via direct procurement or normothermic regional perfusion (NRP). The basic difference is the way the hearts are assessed, either on an external circuit or in the donor body. For the most complex recipient, DCD may not be utilized at some centers due to concern for higher rates of delayed graft function, but this is center specific and data is still evolving. What are some features surgeons consider when procuring the donor heart? Visual assessment of the donor heart is key in DBD or NRP cases. LV function may be hard to assess, but visually the RV can be inspected. Palpation of the coronary arteries is important to assess any calcifications or abnormalities. Ventricular arrhythmias at the time of procurement may be concerning. Key considerations in the procurement process: (1) Ensuring the heart remains decompressed at all times and doesn't become distended (2) adequate cardioplegia delivery (3) aorta is cross-clamped properly all the way across the vessel (4) avoiding injury to adjacent structures during procurement What hemodynamic parameters should we monitor and what vasoactive agents are used peri-heart transplant? There is no consensus regarding vasoactive agent use post-transplant and practice varies across institutions. Some commonly seen regimens may include: (1) AAI pacing around 110 bpm to support RV function and preserve AV synchrony (2) inotropic agents such as epinephrine and dobutamine to support RV function (3) pulmonary vasodilators such as inhaled nitric oxide to optimize RV afterload Early post-transplant patients tend to have low cardiac filling pressures and require preload monitoring and resuscitation initially. Slow weaning of inotropes as the patient shows signs of stable graft function and hemodynamics. RV dysfunction may manifest as elevated central venous pressure with low cardiac index or hypotension with reducing urine output. Optimize inotropic support, volume status, metabolic status (acidosis and hypoxia), afterload (pulmonary hypertension), and assess for cardiac tamponade. Tamponade requires urgent take-back to the operating room to evacuate material. Refractory RV failure requires mechanical circulatory support, with early consideration of VA-ECMO. Isolated RV MCS may be used in the right clinical context. Why do pericardial effusions/cardiac tamponade happen after transplant? They are not uncommon after transplant and can be due to: Inherent size differences between the donor and recipient (i.e. if the donor heart is much smaller than the recipient's original heart) Bleeding from suture lines and anastomoses, pacing wires, and cannulation sites Depending on the hemodynamic stability of the patient and the location of the effusion, these effusions may require urgent return to the OR for drainage/clot evacuation via reopening the sternotomy, mini thoracotomy, and possible pericardial windows. What are the basics of immunosuppression post-transplant? Induction immunosuppression is variably used and is center-specific. Considerations for using induction therapy may include: (1) high sensitization of the patient (2) younger patients or multiparous women with theoretically more robust immune systems (3) crossing of recipient antibodies with donor antigens (3) renal function to provide a CNI sparing regimen long term Some considerations for avoiding induction may include: (1) older age of the recipient (2) underlying comorbid conditions such as infections or frailty of the recipient What are expected activity restrictions post-transplant? Sternal precautions are important to maintain sternal wire integrity. Generally avoiding lifting >10 pounds in the first 4-12 weeks, no driving usually in the first 4 weeks, monitoring for signs and symptoms of wound infections, and optimizing nutrition and physical activity. This translates to hundreds of thousands of paper labels that need to be manually updated and printed by already overburdened staff whenever there's a change in item status, description, supplier, etc. One major impact of such antiquated methods of supply chain management is that it's impossible for the item's information to be updated in real time due to being dependent on deployable staff to travel to these sites to make the changes. Any updates on sensitive data such as back orders, recalls, or discontinued items (and indication of available substitutes) can't possibly be handled in a timely manner with paper labels. As a result, there's a much greater risk of not having on-hand the items that are needed for patient care. Electronic shelf label (ESL) solutions are a game-changer because they provide real-time item status and alerts, remote updates at the push of a button, and automated reordering. ESLs save time for both supply chain and clinical staff that would otherwise be spent on manual and low-value-added inventory tasks. They ensure item availability and safety (not recalled, not expired) and free up clinical staff to dedicate more attention and time to patients. Advanced ESL systems also have built-in replenishment signals to ensure that par levels are accurate and stockouts are prevented. They also allow for voice commands to guide the staff to needed items in crowded supply rooms in seconds using blinking LED lights on the ESL.

Key Insights and Action Steps — Dr. Brennan Spiegel "Every single cell of your body evolved from this force of gravity. Physics came first, and biology came second." Illness arises when we fail to manage gravity. Every organ, tendon, and cell depends on that relationship. "When you stand up straight and lift your diaphragm, it pulls up this sack of potatoes that we all have in our belly. When you open up the gut, it opens up digestion." Posture determines how well the gut, diaphragm, and circulation function. Sitting compresses digestion and lowers energy. "Your balance and relationship to gravity is a predictor of how long you're going to live." Balance, grip strength, and posture are measurable indicators of longevity. "The inner ear is like a gyroscope constantly keeping track of your position in relation to gravity." The nervous system continuously measures gravity. Inner-ear disturbances can create dizziness, anxiety, and panic. "When you're depressed, you can't get up out of bed. Your body is slumped over. It's almost like there's so much gravity pulling on your body, it's like you're in a black hole." Depression mirrors an excessive gravitational load. Emotional heaviness is a physical experience of being pulled down. "Strong negative emotional experiences can permanently change the way the brain forms… the mind has learned to be pulled down emotionally, physically, socially." Childhood trauma reshapes how the brain perceives gravity, making the body feel heavier and slower to rise. "The feet are a gravity management surface… only five percent of the body's surface area but holding one hundred percent of the weight." Feet are the interface between body and planet. Strengthening them restores alignment and balance. "Your relationship to the planet, both latitudinally and altitudinally, will determine your health." Altitude, light, and environment influence serotonin, immunity, and microbiome function. "Serotonin itself is a gravity management substance." Serotonin regulates mood and physical stability, linking emotional and gravitational balance. "When it's stimulated, it activates the rest and digest phase and helps release serotonin." The vagus nerve is the primary connection between body and mind, calming the system and improving serotonin flow. "I pretended I was on a bigger planet… I became stronger and stood up straighter." Carrying additional resistance through weighted movement improves posture, strength, and metabolism. "When we lay down to sleep, we give our body a break… the blood easily flows into our brain and flushes out amyloid." Sleep restores gravitational equilibrium and supports brain recovery. "Gravity doesn't change, but your relationship to gravity does." Long-term health depends on strengthening that relationship physically, mentally, and emotionally. Strengthen the muscles and bones that keep you upright — especially your back, core, and legs. "When you stand up straight and lift your diaphragm, it pulls up the gut and opens digestion." 3. Stand tall and move often. Avoid long hours of sitting. Use a standing desk or take frequent standing breaks. Sitting compresses the abdomen, slows digestion, and reduces serotonin. 4. Strengthen the diaphragm and posture daily. Practice standing with shoulders back and chin level to engage the diaphragm and improve breathing and gut function. 5. Train your balance. Test and improve balance by standing on one leg, walking heel-to-toe, or using a balance board. "Your balance and relationship to gravity is a predictor of how long you're going to live." 6. Practice grip and hanging strength. Hang from a bar daily. Aim for 30 seconds, then increase gradually toward 2 minutes. Even short "dead hangs" improve shoulder, spine, and nervous-system alignment. 7. Use light weighted resistance. Try a weighted vest or light ankle weights while walking or doing chores. "I pretended I was on a bigger planet… I became stronger and stood up straighter." 8. Walk, run, or train barefoot or in minimalist shoes (safely). Let the feet feel the ground to activate stabilizing muscles. "When you ground your foot, everything else pulls up straight from there." 9. Reconnect with the ground. Spend time standing or walking on natural surfaces (grass, sand, earth) when possible. 10. Stay hydrated. Keep enough fluid in your body to "pump blood and oxygen up into the brain." Dehydration weakens gravity tolerance and causes dizziness or fatigue. 11. Regulate the nervous system. Do slow, controlled breathing through pursed lips to stimulate the vagus nerve and calm the body. "Slow meditative breathing activates the rest-and-digest phase." 12. Consider gentle vagus-nerve stimulation. Use only safe methods such as breathing, humming, or medical devices under supervision. Avoid carotid massage unless advised by a doctor. 13. Strengthen vestibular and proprioceptive awareness. Engage activities that challenge coordination: yoga, dance, gymnastics, tai chi, or balance training. 14. Manage mental gravity. Notice emotional heaviness as a physical sensation; practice posture, breathing, and grounding to counteract "mental black holes." 15. Use awe and nature to elevate mood. Spend time in nature, watch sunsets, or listen to music that evokes awe. "Feeling part of something greater than yourself elevates mood and serotonin." 16. Increase natural serotonin. Seek sunlight, exercise outdoors, connect socially, and reduce processed foods. Serotonin helps both mood and muscle tone to "fight gravity physically and mentally." 17. Optimize sleep for gravitational recovery. Sleep 7–8 hours flat or slightly inclined if you have reflux. Avoid heavy meals within 2 hours of sleep. Limit screens before bed. "When we lay down to sleep, we give our body a break… the blood easily flows into our brain." 18. Manage reflux and digestion. If prone to reflux, raise the head of the bed about 10 degrees or use a wedge pillow. Sleep on your left side to reduce acid reaching the esophagus. 19. Support circulation through movement. Use your muscles as pumps, walk regularly, stretch calves, and move legs during travel or desk work to prevent stagnation. 20. Avoid chronic compression. Reduce time bent over laptops or phones; keep screens at eye level to protect diaphragm and digestion. 21. Engage with natural environments. Nature exposure increases serotonin and improves gravity resilience. "Being in green spaces is mood-elevating because that's what we evolved with." 22. Monitor environment and altitude. If you live or work at high altitude, be mindful of mood or sleep changes and adjust oxygen exposure and sunlight time. 23. Balance convenience with movement. Spiegel warns that modern comfort, constant sitting, processed food, artificial environments, represents "our species losing the battle against gravity." 24. Reframe health. Adopt the mindset that "gravity doesn't change, but your relationship to gravity does."
