From the beginning of the COVID-19 pandemic, people with inflammatory bowel disease (IBD) have had many questions. Now that vaccines against the virus are becoming available, people living with Crohn's disease or ulcerative colitis need even more information in order to make decisions. I asked Dr David Rubin, Chief of the Section of Gastroenterology, Hepatology & Nutrition and the Co-Director of the Digestive Diseases Center at The University of Chicago Medicine to answer some of these initial questions about the first COVID-19 vaccines (manufactured by Pfizer and Moderna). Topics discussed on this episode include: How vaccines work How mRNA works How IBD medications affect the immune system IBD medications and their potential effect on COVID-19 vaccination When we'll have more information about COVID-19 vaccines and IBD Why side effects with vaccines are expected and what they mean Key Quotes: The first thing is to remember that inflammatory bowel disease itself is a condition where in almost all situations, the immune system is overactive. So having Crohn's disease or ulcerative colitis is not a situation where you're immune deficient. So IBD patients in all the analyses during COVID have not actually been found to be at increased risk for getting infected, or at increased risk for developing COVID as having just because they have Crohn's or colitis or have an ostomy or have a j pouch. The messenger RNA is degraded within a couple days it's out of your system doesn't hang around. It doesn't ever enter the nucleus of cells, it doesn't become a permanent part of your genetic material in your body. And therefore that's why it is thought to be extremely safe. But I want to make it clear to everyone that they didn't just decide to do this in February, March. This was something that actually was in development, it just got pushed through because of the critical nature of the pandemic. ... there are no data to say that vaccination triggers IBD. And it's been looked at carefully in many studies. And there's no data to show that getting a vaccine triggers a relapse of your IBD. And the newer vaccines that we're talking about here, will not do that either. ...we don't know for sure yet is whether you'll have impaired ability to mount a protective immunity at the same level as if you weren't on therapy. But that doesn't mean you won't develop any immunity. And in fact, it's possible that you'll develop the same immunities to general population because the messenger RNA vaccine is a completely new mechanism. Your skin barrier is protected by a thin layer of lipids and sweat from your glands.  This protective layer is called the acid mantle.  The main role of your acid mantle is to help your skin retain moisture while protecting it from harm caused by bacteria and environmental factors If your acid mantle is out of balance, your skin barrier becomes more prone to damage.   Taking care of your Acid Mantle Not only does it have to contend with bacteria and pollution chipping away at it, there are many things we do in the name of good skincare that can damage our mantle.  The good news is that there are ways to maintain it. You can adjust the pH of your skin by adding a substance to it that is either more acidic or more alkaline (chemistry, urghh…). Anything you put on your skin that is higher or lower than its natural pH will raise or lower the skin’s pH as well.  This can either be bad (i.e. weakening your acid mantle) or good (i.e. restoring skin to its naturally balanced state). If you’re suffering from a damaged acid mantle, then swapping over to a pH balanced cleanser may be just the cure you’re looking for. Watching the pH of your cleansers in particular is important for people with: A compromised barrier Blemishes Redness   Even if you don’t suffer from these, if you incorporate AHA and BHA products into your beauty routine, you might also want to watch the pH of your face wash.  Why? Because the pH of your face wash will also directly affect how well your AHA and BHA products work. If your cleanser is not correctly pH balanced, your acids will also be thrown out of balance, which can inhibit their effectiveness.   How to Know If You’ve Damaged Your Barrier If you’re suffering from: Sore skin; Sudden irritation; Redness; Tingly skin; Dry skin no matter how much moisturiser you put on You may have damaged your skin barrier. This is common when you are using products that are poorly formulated, harsh, too basic or alkaline. Don’t panic though.  With the right care, you can regain a strong skin barrier and be back on the way to healthy, glowing skin before you know it! Repairing your Skin Barrier If you’ve overdone it on the acids or harsh cleansers, follow these steps: Stick to the basics – when your skin is going haywire, the temptation is to add more and more products to try to get things right.  However, the more complicated the routine, the harder it can be to repair skin problems.  Resist the temptation to do more and get back to fundamentals. Don’t go too hard on the cleanse – Using water that’s too hot, harsh cleansers or exfoliants may feel effective, but having a heavy cleanse may harm your skin. This is because not only does it strip away the dead skin cells on the surface, but your skin’s barrier as well.  For a gentle yet effective cleansing routine that’s suitable for all skin types, check out: Cleansing Oilfrom B-Lab I Am Sorry Just Cleansing; and APLB Sensitive Micro Cleansing Foam. Do you feel sluggish in your day-to-day work life? Are you curious if you use your time wisely? Are you ready to run a more profitable business? Every small business owner has limited time and desires success, so here’s how you can inventory your productivity and maximize your time.   If you feel like you’re in a work slump and you’re unsure how to get out, start by assessing your productivity. Let’s take a look at how you’re using your time. In this episode, you’ll learn how to assess how you spend your work hours and how to make small tweaks so you stay focused on your goals. If you’re ready for tough love and wise strategy, tune in.   Productivity vs. Hustle Your day should be designed to fit you. There’s no specific formula of how your day “should” look. Remember that your productive day will look different from others. Instead, examine your work-day patterns.   Productivity doesn’t mean every second is filled with a business task.   When every minute is occupied, that’s hustle and it’s not what I want for you. Hustling leads to frustration, burnout, and overwhelm.    There will be times of seasonal hustle, e.g. a launch or putting out a new product. However, hustling should not be your norm because it is unsustainable for long periods of time.    How to Know If You’re Intentionally Productive If you feel sluggish or unproductive, audit your time. Record what you do in your business for three to five days. How? Grab this download, created just for you. Set a timer for 60 minutes. When the timer goes off, record what you did for the last hour. Hit repeat on your timer.  Continue this process of recording what you do 60 minutes at a time throughout your workday.   Once you’ve tracked your time for three to five days, ask yourself: What category does each task belong to, e.g. meeting, creation, scrolling, personal? What strategic objective does this task fall under? Where do I see patterns? When do I attack tasks I put off? Does where I spend my time match my #1 priority?   Follow These Suggestions to Maximize Your Productivity As I’ve tracked and tweaked how I spend my time, here are a few best practices to make your workday even more productive:   Think through if you want to batch your days or do you like your current system. Block off one or two “Get It Done Days” per month. Batch administrative activities.  If you find yourself switching back and forth between communicating with clients through email, Voxer, or calls, stop the madness by creating an expectations document for your clients that outlines when you’re available and through which platform. Another option is to set aside time each day for returning messages. Set up a time each day to housework, e.g. putting away the dishes or throwing in a load of laundry. Hire help for housework if you have the revenue to do it. Train your family to help you with housework. “I loved my career in TV news.  I wouldn't have changed a thing.  It was very rewarding and I loved it.  But life is a book with many chapters.” For former TV News Anchor Lin Sue Cooney, the time had come, after 34 years in the business, to turn the page. Cooney says she never wanted her legacy to be “oh she was the gal that read the news for so long”.  She wanted an “encore” career and went in search of something that would give her a deeper sense of purpose. Cooney shares this desire with a growing number of people who are seeking deeper meaning in their lives. A recent article in Inc. titled How to Know If You're Working (and Living) With Purpose http://www.inc.com/shelley-prevost/how-to-know-if-youre-working-with-purpose.html outlines four questions to ask in order to discover “your path to more meaningful work”: Is the work I'm doing exciting and/or satisfying? What keeps showing up in my life? Am I using my gifts in service to others? Is the work I'm doing elevating others in some way? Cooney identifies most with the question that asks if you are using your gifts as a service to others.  She is. After leaving the NBC affiliate in Phoenix, Arizona, Cooney went to work with Hospice of the Valley www.HOV.org  Her job is to connect Hospice with the community in a variety of meaningful ways. “I had a yearning to make the world a better place,” she says.  “Every night when I put my head on the pillow I can tell you that we have made a positive difference in families' lives.” Cooney describes this next chapter in her life as transformational.  She left celebrity behind for a job that, at this point in her life, brings deeper meaning. Hospice of the Valley, she says, is like working with angels.  “They really believe that the end of life deserves the same amount of dignity and reverence as the beginning of life — and it does.” “We are afraid of dying and we don't like to talk about it.  We live well why shouldn't we die well?  We are going to help you live the best way you can and make the most of the time you have left.  To have the best quality as you leave this world.” Cooney made a tough decision walking away from the career she loved for so long. “I was scared to leave but sometimes you have to listen to the voice.” Cooney was — and still is — one of the most recognized faces in Arizona, and she's focused on using her public persona for the greater good.
