In the second half of Roger Green's interview with Global Liver Institute Vice President, Liver Programs Jeff McIntyre, Jeff discusses the implications of his key EASL Congress takeaways for GLI and other patient advocacy groups. To Jeff, this trend makes patient advocates a more valuable player in the clinical trial design process, particularly when coupled with the FDA's increasing focus on diversity in trial populations. This will become particularly important because, today, the major use of incretin agonists like semaglutide and tirzepatide is in anti-obesity, where payers are frequently declining to pay for the drugs. Advocates like GLI will be pivotal in ensuring that patients who need MASLD drugs will still get the drugs they need, particularly at earlier stages of fibrosis. This discrimination may allow patients to receive incretin agonists during the pre-fibrosis stage based on diabetes and anti-obesity medications and reserve the fibrosis drugs for patients with more advanced MASH. The entire scenario of early metabolic, later MASH treatment with different agents is at least 4-5 years in the future. In the meantime, Jeff sees a need to advocate for underserved groups in the population while at the same time laying the foundation for the longer-term case. In this context, the United Nations General Assembly side effect (covered in S5, E22) is an exciting and vital event.
This conversation contains the first half of Roger Green's interview with Global Liver Institute Vice President, Liver Programs Jeff McIntyre, during which the two discuss what Jeff considered the key strategic takeaways for GLI from the various EASL Congress presentations, abstracts and discussions. Jeff focuses on the "overriding sense of optimism" coming from the multiple pieces of positive drug data. He cites the data on Boehringer Ingelheim's survodutide, the follow-up work on Madrigal's resmetirom, an anticipated presentation on semaglutide at the AASLD in November, and strong FGF-21 results as proof that we are beginning to develop multiple robust, safe and effective modes of action for drugs to treat (at least pre-cirrhotic) MASH. His second positive point is that due to the drug trials and nomenclature change, GLI and other advocates are starting to have "more enlightened discussions" about MASLD in the context of the whole patient and related metabolic conditions. As a result, he comes to the third point, which is that multiple modes of action will teach us why what works in one patient might not in another and, ultimately, reshape clinical trials so that the target might not be fibrosis (or at least not only fibrosis), but instead exactly how each drug works. Jeff envisions this line of inquiry as a step down the path away from requiring biopsy.

When the water in the skin was gone, she left the boy under one of the bushes and went and sat at a distance, about a bowshot away, for she said, “I can't bear to watch the boy die!” While she sat at a distance, she wept loudly. God heard the boy crying, and the angel of God called to Hagar from heaven and said to her, “What's wrong, Hagar? Don't be afraid, for God has heard the boy crying from the place where he is. Get up, help the boy up, and grasp his hand, for I will make him a great nation.” Then God opened her eyes, and she saw a well. So she went and filled the waterskin and gave the boy a drink. God was with the boy, and he grew; he settled in the wilderness and became an archer. He settled in the Wilderness of Paran, and his mother got a wife for him from the land of Egypt. Next Steps: Believe: I need Jesus to clean my messy heart today. Become: I will trust Jesus for strength to avoid sin this week. Be sent: I will carefully step into someone's mess this week. Discussion Questions: Have you ever asked God to clean up your mess for you? Why is it so easy to pass the blame for our own mess? What was the last personal mess you made? Why couldn't Sarah live with both boys at the same time? Why did God choose Isaac and not Ismael? What does this say about our relationship with God the Father? Did Abraham get off easy in this mistake? Explain your answer. Pray for God's grace despite our mess this week.

This is one year anniversary of the show and the guest Jeff Dean was invited who previously cancelled once, a year ago. Both being comedians, throughout the show they making jokes on each other and at times using many sarcasm. There was a caller, Michael, a fellow comedian asking about Jeff new shows and Jeff mentions about show, Overly Sensitive with Jeff and Justin and Michael booked Jeff for his show while on the call. David says he has his mom on his show to Jeff's pleasant surprise. Miranda, David's girlfriend called on the show and spoke about plantations and dating these days. Jeff mentions about the tiff with his girlfriend at that time and asks them for dating ideas but he gets teased and slight mocked by both of them. Jeff says he is trying for a special on Netflix and working towards it. A guy called Ezra call and plays some free style music and sings too. A guy called Steven calls and gives details about his date that evening. Later Jeff and David discussed parents, siblings and their shows and schedule in the future. This has gone well, mostly enjoyable. Sometimes not. Raj rates the episode with 2.5 stars!

There's nothing in today's note that would do a busy person like you any good. Unless, of course, you're in a major, world-class bigtime really extreme hurry. Then you should by all means keep reading. Would you like to have a secret retreat from the buzzing noise of daily life in the 21st century? Are you prepared to take a journey that will move your mind to another place, another time? Today I'm going to tell you about four non-fiction books and not one of them has a plot. But don't let that fool you. Sea Room, by Adam Nicolson. True to my custom, I opened this book to a random page (141) and began to read: ” …Something of the sense of holiness on islands comes, I think, from this strange, elastic geography. Islands are made larger, paradoxically, by the scale of the sea that surrounds them. The element which might reduce them, which might be thought to besiege them, has the opposite effect. The sea elevates these few acres into something they would never be if hidden in the mass of the mainland. The sea makes islands significant.” Impressed, I turned to chapter one where I was greeted, “For the last twenty years I have owned some islands. They are called the Shiants: one definite, softened syllable, 'the Shant Isles', like a sea shanty but with the 'y' trimmed away. The rest of the world thinks there is nothing much to them. Even on a map of the Hebrides the tip of your little finger would blot them out…” I bought hard-to-find copies for several of my friends. The Island at the Center of the World, by Russell Shorto. My partner Jeffrey Eisenberg shares my taste in books, so when I told him this book was “the epic story of Dutch Manhattan and the forgotten colony that shaped America,” he wasn't worried. Read it anyway. Later Jeff called me to say, “I think it may be one of the most interesting books I've ever read. Definitely the best-written history book. Almost reads like a novel.” But then again Jeff is strange. Might there be a chance that you, too, are Jeff's and my brand of crazy? Travels with Charley, by John Steinbeck. In 1961, a year prior to winning the Nobel Prize for Literature, John Steinbeck bought a camper and set out with his dog, Charley, to see America through the windshield of a pickup truck. This book is the story of that 3-month journey. Most people associate Steinbeck with Cannery Row, Of Mice and Men, The Grapes of Wrath, or East of Eden. Worthy books, to be sure. But this, his odd collection of experiences and observations is, I think, my favorite Steinbeck of them all. Travels with Charley is a celebration of the Ordinary, the disjointed thoughts and notes of a highly accomplished man looking quietly at the world around him. It is perhaps the most underlined, dog-eared, footnoted book I own. In tribute to Steinbeck's Travels with Charley, we're going to publish – in a 380-page book – all the essays we received in response to the challenge I issued 5 weeks ago. This book will have an ISBN number and a barcode and will be registered with the Library of Congress – the works. It will be called People Stories: Inside the Outside, and a free copy of it will be the special “gift of initiation” I promised to send everyone who dared to join our bleary-eyed fraternity. When the book arrives from the printer, (hopefully by late April,) it will be released with considerable fanfare during a huge party at Wizard Academy's Tuscan Hall.
