Most of us can name the pattern. The same dynamic with different partners. The same feeling with a mother, a boss, a spouse. What we can't see is why it keeps happening.In front of 110 participants in Costa Rica, Dr. Zach Bush sat down with Raj for a live Love Retrieval. Underneath four decades of stories was a single architecture. The moment he could see it, it dissolved.The body knew firstBefore the first question, a sharp pain arrived in Zach's right shoulder. Every time, the hand went up.The childhood conclusion was simple: What I have is precious to me and worthless to everyone else.Then Raj asked what big Zach would say to four-year-old Zach.The answer was immediate. He knelt beside the boy, put a hand on his back, and said:This is for you.Every story he carried resolved in that sentence.Why the rejection was intelligentThe people who wouldn't take his pearl were the teachers, showing him to keep it. Everyone already holds their own. Everyone has direct access to the source. Nobody needs yours.That's why he's careful with the word trauma. It wasn't a trauma. Before coming to Iowa, Raj completed physician training in India. During his MPH in Epidemiology, he expanded that clinical perspective through coursework and projects in epidemiology, biostatistics, qualitative research, data analysis, and healthcare quality improvement. Raj discusses the transition from thinking about health at the individual patient level to examining the broader systems and population-level factors that shape health outcomes. He also shares how projects involving medication adherence and hospital length of stay helped him see the importance of issues such as transportation, communication, health literacy, care coordination, and access. The conversation also covers some of the less obvious parts of graduate school, including adapting to a new academic system, moving to a new country, deciding where to specialize within a broad field, and learning how to turn complicated public health data into information that can be understood and used by people outside the field. This past April at Blue Spirit Retreat Center in Nosara Costa Rica, I guided Dr. Zach Bush through a Love Retrieval in front of 110 people who were told to close their eyes and feel with Zach as he touched some of the deepest parts of his soul. Las fronteras que vemos en cualquier mapa parecen que siempre han estado ahí que son naturales, pero no es así. Existen fronteras naturales, por supuesto, suelen coincidir con ríos lo suficientemente anchos o cordilleras suficientemente altas. Ese es el caso de los Pirineos, donde la frontera natural y la política coinciden. Pero incluso ahí caben excepciones como los valles de Arán y Valcarlos. España, de cualquier modo, es mal ejemplo para entender la frontera moderna porque las suyas son antiguas y casi todas se fueron haciendo poco a poco y desde abajo. Durante la mayor parte de la historia las fronteras no eran líneas, sino zonas de transición muy porosas y con contornos difíciles de definir. La idea de que un país acaba en una línea bien definida empezó con la Paz de Westfalia de 1648 y culminó en el siglo XIX con los tratados de límites. A partir de aquí la mayor parte de las fronteras del mundo no surgieron de la geografía sino de una mesa de negociación. Un mapa político es por lo general una construcción que nos habla de intereses concretos de personas concretas. El acuerdo Sykes-Picot de 1916 repartió el moribundo Imperio otomano entre Francia y el Reino Unido mediante una serie de líneas recta trazada sobre un desierto que apenas conocían. Los británicos hicieron además tres promesas contradictorias sobre el mismo suelo. De aquel reparto nacieron Irak, Siria, el Líbano y Jordania, Estados artificiales en los que metieron a la fuerza a sunitas, chiíes, kurdos, cristianos y alauíes bajo las mismas bandera. Los kurdos, hoy 40 millones de personas, se quedaron sin Estado y están repartidos entre cuatro países. La Conferencia de Berlín de 1884 estableció las reglas para que los europeos se repartiesen África sin contar con los africanos, naturalmente. El principio de ocupación efectiva disparó la carrera por plantar banderas. El 30% de las fronteras africanas son líneas rectas perfectas que parten pueblos como los somalíes, los bakongo o los yoruba, o encierran a tribus muy distintas y que se llevan mal desde siempre. La descolonización del siglo pasado heredó íntegras aquellas fronteras y también los problemas derivados de ellas. En 1947 los británicos encargaron a Cyril Radcliffe, un jurista que nunca había pisado la India, dividir el Raj, un subcontinente de 400 millones de habitantes en solo cinco semanas. Su línea separó regiones enteras y provocó entre 12 y 18 millones de desplazados, amén de cientos de miles de muertos. De aquello saldría la guerra de Bangladés en 1971 y la disputa de Cachemira, que desde entonces enfrenta a indios y pakistaníes. El colapso de la Unión Soviética en 1991 convirtió en fronteras internacionales unas líneas administrativas trazadas básicamente por Stalin para dividir y controlar a las minorías. Valles como el de Ferganá en el centro de Asia fueron troceados, el Alto Karabaj, poblado por armenios, se metió en en Azerbaiyán, Crimea y el Donbás pasaron a Ucrania. De ahí nacieron los conflictos congelados que hoy siguen activos desde Transnistria hasta la guerra de Ucrania. La historia se repite una y otra vez. Trazar una frontera es un acto de poder cuyas consecuencias duran generaciones. El mapa no es el territorio, y no necesariamente es la gente que lo habita. Radiation oncologist and director Roshan Sethi: confidence in medicine is mostly performance. The book club is back, and M2s Anna Royer, Ellie Johnson, and Sofia Hueser, and M4 Alex Nigg all crowd around to grill an actual Hollywood director about his debut novel. Somewhere in Roshan Sethi‘s frontal lobe is a cyst, and it’s possibly why he is a reformed gunner, Hollywood writer/director, still radiation oncologist, and now novelist. Sethi is co-creator of Fox’s The Resident, and author of The Simp: A Novel Without a Hero, a new book built out of a viral job ad so horrifying nobody would claim authorship of it. What does it mean that Raj, the novel’s desperate, self-aware sycophant of a protagonist, reads less like a Hollywood assistant and more like a third-year on rotations? Sethi doesn’t dodge the comparison—he wrote most of Raj’s obliging, invisible, gunner-brained misery straight out of his own decade in medicine. Sethi describes himself, pre-Hollywood, as ruthless enough to publish 15 papers in a single year for a career he’s since walked mostly away from–he still practices 9 weeks a year–and he does the math on what staying in medicine actually cost him: when The Resident got greenlit for a sixth season and syndication, he’d already turned the show down to finish residency, a decision one producer later priced at $12 million. Sethi traces a lot of that old ambition back to being closeted through college and medical school; the striving, he says, was never really about medicine, and coming out took most of the edge off it. Also, what happens to a person’s personality over four years of medical school? Sethi’s answer: it gets slowly closeted right along with everything else, trained out of you until invisibility reads as professionalism. He tells the room the worst response to getting pimped isn’t “I don’t know”—it’s saying it apologetically, like you owe someone an explanation for not yet knowing something nobody expects you to know. Speaking of things that require confidence you don’t actually have: Sethi’s next film, The Surgeon, stars Michelle Yeoh as a retired surgeon taken hostage and forced to operate. Dave can’t wait. He even pitched Sethi on his own idea for a movie that’s not at all derivative, and though it will probably star Mark Ruffalo, Dave is available. On representation, Sethi draws a sharp line between his two industries. Medicine has become more female and more Indian, Asian, and Nigerian over time precisely because it runs on an objective, if imperfect, filter that values whoever scores highest on the MCAT. Hollywood, by his account, has no equivalent filter: roughly 90 percent of the highest-grossing films are still directed by men; minorities in the industry get treated as diversity projects rather than talent, and there’s nothing resembling medicine’s blunt, if flawed, meritocracy.
