Beyond the Pearls: Cases for Med School, Residency and Beyond (An InsideTheBoards Podcast)

Today's Case A 40-year-old female presents for follow up of hypertension that started 2 years ago. She has been seen frequently in your clinic for titration of her antihypertensive medication and treatment of recently diagnosed hyperlipidemia and type 2 diabetes mellitus. During her visit she reveals that she is frustrated by weight gain that has occurred predominately over the past year. She reports regular menstrual cycles until the past year, when she notes increased time in between cycles and occasional spotting instead of regular periods. She is concerned that she may have early menopause because she is having changes in appetite, difficulty sleeping, and mood swings. She feels like these symptoms are getting worse, and she is frustrated that despite her increase in exercise and change in diet she is more fatigued and has experienced no weight loss. Ever feel like you're drowning in documentation while the work you actually trained for gets squeezed into the margins? Or maybe you're curious about AI tools but worried about privacy, security, and whether using them is even ethical? You're not alone and this conversation might be exactly what you need to hear. In this episode, I sit down with Raj Paul, Director of Engineering at Jane, where he leads the teams building clinical features designed to reduce administrative burden. Raj brings a unique perspective he's not an OT, but his wife is, so he deeply understands our world. He's also openly multipassionate, generating many ideas and he shares about how he uses AI to challenge and refine his thinking and communicate more effectively. We're having the real conversation about AI documentation tools that so many OTs are craving. Raj walks us through exactly how AI Scribe keeps your data private, how to get proper consent from clients, and why you don't need to be afraid of this technology. But here's what really hit me: We talk about this pervasive belief that for work to be valuable, it needs to be hard. I share about the experience of OT entrepreneur coaching clients who had a turning point when she started using AI for documentation. She realized she'd been using her leftover energy for her family instead of finding ways to work less hard. That shift cascaded into her riding bikes more, making art, volunteering with her kids' sports teams, and actually living her life. Work doesn't have to be hard to be valuable. In this episode, we cover: How AI Scribe actually works and what makes it secure (BAAs, human-in-the-loop design, AWS partnership) Why Jane's team can't access your transcripts or notes - and how they improve the tool without seeing your data Getting informed consent from clients (regulatory considerations, intake forms, and having the conversation) How to start experimenting with AI safely before using it clinically The "AI as intern" analogy - why you still need to check its work Privacy concerns, environmental considerations, and making values-aligned decisions Why Raj uses Claude personally to challenge his ideas and improve his communication The importance of psychological safety in creating better ideas and better outcomes Practical tips: Start with free tools, check your privacy settings, get comfortable before jumping in This episode is for you if: You're curious about AI documentation but need the safety and privacy facts first You're burned out from documentation and wondering if there's a better way You've been seeing AI conversations in Facebook groups and need real information to decide You're ready to challenge the belief that struggle equals value You want to reclaim your time and energy for what actually matters   Ready to try it yourself? Use the code VITALITY or follow this link to get a one-month grace period on a new Jane account, which includes free access to 5 AI Scribe sessions per month. I haven't had a list of chart notes waiting for me in a really long time. They're done in minutes, and it's genuinely changed my practice. Leadership Odysseys

Some conversations stay with you long after the microphones turn off. This is one of them. In this episode of Leadership Odysseys, Kirsty sits down with Raj Nanra, CEO of SLE Worldwide Australia, for a powerful conversation about leadership shaped through lived experience, cultural belonging, resilience, and loss. From growing up as the child of one of Melbourne's first Sikh families, to leading businesses across Asia for two decades, to building culture through human connection, Raj's story reveals how leadership is built not in boardrooms alone, but in the moments where listening, empathy, and courage meet. Raj speaks openly about the devastating loss of his 21-year-old son, Sachin to cancer, and how that grief became the catalyst for purposeful action. In response, he founded the annual You Can Charity Golf Day in partnership with the Sony Foundation, mobilising his industry community to support young Australians facing cancer. Over the past four to five years, these events have raised close to $250,000 to fund youth cancer programs and provide tangible support to families during their hardest seasons. What emerges is a story of choosing to transform pain into service and leadership into something that extends far beyond business. This episode explores the human side of leadership: culture as behaviour, presence over performance, and the deep power of staying curious about people. Key Highlights Culture is built in conversation, not slogans Raj grew up learning to speak with anyone, without judgement, background bias, or status filters. That lesson still defines how he builds teams today. Culture, he believes, is not written on walls. It is shaped by daily behaviour, attention, and genuine conversation. Asia taught him how connection earns trust Twenty years across China, Singapore, Malaysia, Hong Kong, and beyond taught Raj that leadership begins with curiosity. Learning names, languages, foods, and customs became a bridge to trust. His willingness to listen before leading turned short assignments into long partnerships and shaped his approach to people-first leadership. Listening is a leadership discipline Raj draws a clear line between leaders who speak and leaders who listen. He prefers presence over performance, relationship over hierarchy, and dialogue over distance. From meeting every staff member personally to prioritising uncomfortable conversations, listening became both his cultural strategy and leadership edge. Turning grief into impact The loss of his son Sachin changed everything. Supported by his industry community, Raj chose to respond with purpose rather than retreat. By founding the You Can Charity Golf Day alongside the Sony Foundation, he created a practical way to support adolescents and young adults with cancer, raising nearly $250,000 to fund programs that care for families as they walk through the most difficult chapters of life. Purpose comes from action, not sentiment For Raj, giving back is about more than fundraising totals. It is about showing up with time, care, and presence. Whether supporting families facing the long road of treatment or donating simple items that restore small moments of joy, leadership, he says, is measured by how you help when no one is watching. Raj's odyssey reminds us that leadership is ultimately human work. Culture grows through conversation. Trust is built through listening. Purpose is forged when pain meets service. His story shows leadership matters most when leaders lift others, even while carrying their own moments of challenge. In this episode of the show, Stu and Raj look back at the second Test between New Zealand and the West Indies, which saw the Black Caps take a 1-0 series lead at the Basin Reserve in Wellington. We start with our initial reaction to a game between two teams with a host of changes, that at times looked set to be closely fought but ultimately ended in a comfortable victory for the Black Caps. The New Zealand bowling attack comes in for praise, especially Blair Tickner, Jacob Duffy and Michael Rae, and there's also a conversation about the role of spin in the Test team going forward and the role Zak Foulkes can play once the ball stops swinging. As we bounce around a number of talking points, we chat about Mitch Hay's impressive debut, Devon Conway's recent form and how the Black Caps dealt with losing another fast bowler to injury early on in the match. To round out the show, we discuss the level of disappointment the West Indies must be feeling, a potential debut for Kristian Clarke and our hopes and dreams for the third and final Test at Mount Maunganui, starting 18 December. Just the actual map he walked to heal patterns of self-abandonment, people-pleasing, and the constant fear of saying the wrong thing. In this episode, you'll discover:→ Why your anxious attachment isn't a flaw—it's a child frozen in your nervous system between ages 4-7→ The reason meditation and talk therapy alone can't heal relationship anxiety→ How the wiring you developed to survive childhood is now sabotaging your adult relationships→ The counterintuitive truth: you can't heal attachment wounds alone because you didn't get them alone→ A simple reframe that transforms how you relate to your anxiety (hint: it involves compassion, not control)→ The exact script Raj uses to communicate vulnerability without self-abandoning→ Why risking losing the other person is actually the path to keeping them→ What becomes possible when you finally stop operating from outdated programmingYou're not too much. You're not too needy. You're carrying pain that was never yours to hold in the first place. In this episode of the show, Baldy, Stu and Raj look back at the first Test between New Zealand and the West Indies, and give a brief shout out to Virat Kohli and a live reaction to the goings on in the second Ashes Test. With England and Australia still on the field at our regular Sunday recording time (check back in a day or two for our wrap of that match), we dedicated the majority of this episode to unpacking the West Indies' impressive rearguard action against the Black Caps at Hagley Oval. We start with a round-up of emotions after what proved to be a draining final couple of days in the first Test of the New Zealand summer. The Black Caps looked set for victory with the West Indies at 72-4 chasing 500+, but injuries to Matt Henry, Nathan Smith & Tom Blundell and an outstanding fighting effort from Shai Hope, Justin Greaves & Kemar Roach took the game into the final overs and secured a draw for the visitors. As we bounce around a number of talking points, there's also praise for Jacob Duffy, Tom Latham & Rachin Ravindra, we discuss the pitch and umpiring decisions, and debate whether the Black Caps still had enough chances to win or whether the West Indies should have put the accelerator down in the final session in an attempt to chase down the target. In amongst all of that, we speculate on the New Zealand lineup for the second Test and the prospect of potential Test debuts for Mitch Hay and Michael Rae or an SOS call-up to Kyle Jamieson. To round out the show, we there's a shout-out to Virat Kohli for his stacks of runs in the ODI series against South Africa, plus Baldy's live reaction to the news that Australia were on the verge of victory against England under lights in Brisbane. Shattered Lands by Sam Dalrymple A history of modern South Asia told through five partitions that reshaped it. As recently as 1928, a vast swathe of Asia – India, Pakistan, Bangladesh, Burma, Nepal, Bhutan, Yemen, Oman, the UAE, Qatar, Bahrain and Kuwait – were bound together under a single imperial banner, an entity known officially as the ‘Indian Empire', or more simply as the Raj. It was the British Empire's crown jewel, a vast dominion stretching from the Red Sea to the jungles of Southeast Asia, home to a quarter of the world's population and encompassing the largest Hindu, Muslim, Sikh and Zoroastrian communities on the planet. Its people used the Indian rupee, were issued passports stamped ‘Indian Empire', and were guarded by armies garrisoned in forts from the Bab el-Mandeb to the Himalayas And then, in the space of just fifty years, the Indian Empire shattered. Five partitions tore it apart, carving out new nations, redrawing maps, and leaving behind a legacy of war, exile and division. Shattered Lands, for the first time, presents the whole story of how the Indian Empire was unmade. How a single, sprawling dominion became twelve modern nations. How maps were redrawn in boardrooms and on battlefields, by politicians in London and revolutionaries in Delhi, by kings in remote palaces and soldiers in trenches. Its legacies include civil war in Burma and ongoing insurgencies in Kashmir, Baluchistan and Northeast India, and the Rohingya genocide. It is a history of ambition and betrayal, of forgotten wars and unlikely alliances, of borders carved with ink and fire. And, above all, it is the story of how the map of modern Asia was made. The Heir Apparent by Rebecca Armitage They would always choose the Crown over their family. It was the pact they made for the honour of wearing it. Lexi Villiers is a 29-year-old Englishwoman doing her medical residency in Hobart, working too hard, worried about her bank balance, and living with friends. It's an ordinary, happy kind of life, and getting even better, because as the dawn is breaking on New Year's Day, Lexi is about to kiss the man she loves for the very first time. But by midnight, everything will change. Because Lexi is in fact not an ordinary young woman. She is Princess Alexandrina, third in line to the British throne—albeit estranged from the rest of her family and living in voluntary exile on the other side of the world. But following a terrible accident which has claimed the life of her father and her twin brother, Lexi—the black sheep of her family and, until this moment, always destined to be the spare—is now the heir apparent, first in line to the throne once her grandmother, the elderly Queen, dies. Called back to do her duty, she arrives in London to a Palace riven with power plays and media leaks, all the while guarding painful secrets of her own, not knowing who she can trust. Palace waters are treacherous, rumours are rife, and selling each other's secrets is a family tradition. And with the Crown just within her grasp, Lexi must choose what bonds she will keep ... and what she is willing to leave behind. LISTEN ABOVE See omnystudio.com/listener for privacy information.

Send us a text! We'd love to hear your thoughts on the show.If you've ever wondered whether that wild, complicated story in your family could become a novel, this episode is for you.I'm joined by award-winning author Barbara Sibbald, whose latest book, Almost English, began as a family legend, became a genealogy project, then tried to be creative nonfiction—before finally settling into the form it needed all along: a historical novel.Barbara's great-grandparents lived in Quetta, on the Northwest Frontier of British India (now Pakistan) between 1885 and 1912. Growing up, she'd heard half-true tales about an Indian princess and a pet elephant, but it wasn't until her mother spent nearly two decades compiling a detailed family genealogy—and shared boxes of letters and photographs—that the real story came into focus. In this conversation, Barbara walks us through how she turned that wealth of material into fiction, while still honouring the lives at the heart of it. We talk about:How her mother's meticulous genealogy and bundles of family letters sparked the idea for Almost EnglishWhy the story first appeared to be creative nonfiction—and what made Barbara realize it actually had to be a novelThe moment she understood she needed access to her characters' inner lives, thoughts, and conversations, and why that pushed her toward fictionThe central question that finally unlocked the book: How could her great-grandfather, Stephen Turner, a quarter Indian man, ever be accepted into the racist power structure of the Raj?How Barbara used that central question as a compass for cutting thousands of words and tightening the narrativeThe research she did into the Raj, the Durand Line, household life, women's work, and even period undergarments (!), to bring the world to lifeHer use of real letters versus invented ones, and how both helped her build an emotionally resonant narrativeWe also talk about the book's unusual structure. In addition to the main historical storyline, Barbara includes short nonfiction pieces she calls “interstices”, where she reflects on her own search for belonging as the child of an itinerant military family—and how that parallels her great-grandparents' experience. That blending of historical fiction, biography, and autobiography made the book hard to categorize—and hard to sell. Barbara shares candidly about the seven drafts, nearly three years of querying, and 48 approaches to publishers before the book was finally acquired by Bayeux Arts in Canada, and then by Vishwakarma Publications in India. If you've ever struggled to decide whether your story should be memoir, creative nonfiction, or a novel, you'll find so much reassurance and practical insight in Barbara's journey with Almost English.

With so many of these individuals I interview, art is really the only thing they see themselves doing in life. Oil painter Raj Chaudhuri on the other hand was a very successful software engineer before committing to art. He could have gone that route and been hugely successful, especially considering the timeframe he was doing it. But no, he HAD to be an artist. You see, Raj has been drawing since he was a little boy. He'd even won contests in school and at his university when he wasn't even an art major. He grew up in, India, and we get to talk about that whole component of his life. Eventually he moves to the United States from India and goes to Ole Miss to become an economist of all things. He was actually working on a PhD when he finally said "I've got to do what I love." Thankfully he did because he's an excellent artist. We met in Denver during for an event put on by the Coors Western Art Show, a special kind of summer preview with lectures, all getting ready for their January exhibition. They have some lovely art, and it's really just a fun time. One of the highlights for me was speaking with Raj. One thing led to another and next thing you know Raj was on a flight to Tucson to see my gallery and record this podcast. What makes an excellent podcast to me is when I come away learning things and getting a new sense of what it means to be an artist. This was very easy in talking to someone like Raj. It was very insightful and fun. So I hope you enjoy this podcast as much as I did.

The Strategy Skills Podcast: Management Consulting | Strategy, Operations & Implementation | Critical Thinking

Raj Sisodia has spent his life asking one question: Can business make people's lives better instead of draining them? He holds a PhD in Marketing and Business Policy from Columbia University, co-founded Conscious Capitalism with John Mackey, the founder of Whole Foods Market, and has advised global companies from Tata Group to AT&T. But his path started in a factory in Bombay, earning a hundred dollars a month, before he built one of the most influential ideas in modern business thinking. "I didn't like biology, so I became an engineer. I didn't like finance, so I became a marketing professor. But business turned out to be about head and wallet — nothing about heart or spirit." That realization led him to study companies that people love working for and trust buying from. The result became Conscious Capitalism — a way of running a business that joins purpose, profit, and care. "Profit is the oxygen that keeps you alive. But no human lives just to make red blood cells. In the same way, no company should live just to make profit." Raj's research showed that companies built on four simple pillars — Purpose, Stakeholders, Conscious Leadership, and Caring Culture — outperformed the S&P 500 by nine to one over a decade. They made more money precisely because they cared more. When he met Bob Chapman, a manufacturing CEO from Missouri, Raj saw these ideas come alive. Chapman bought a failing plant, promised no layoffs, and told workers they would figure it out together. Men who had once been laid off without warning wept as they told Raj their lives had changed. "I had sixty dollars in the bank and a new baby. That job saved my family." From that came the book Everybody Matters. Chapman told him, "Leadership is the stewardship of the lives entrusted to us." Raj calls such companies healing organizations — places that reduce suffering and bring more joy into the world. Now, with artificial intelligence reshaping work, Raj argues that AI will amplify our intentions: "A knife in a surgeon's hand saves lives. The same knife in another hand can end one. AI is the same — it depends on who we are when we use it." He believes the leaders who thrive will be those who bring consciousness to technology, not fear.
