Today, Dr. Josh Emdur joined us to discuss how runners can maintain a healthy immune system. Running helps but running does not guarantee that you have a great immune system. It is important to have a balanced approach to your training during this time period. Moderate exercise has been proven to help your immune system. Your immune system may be at risk if you are sustaining excess training loads. Stress is stress. Stress can be physical or mental. You have to balance the physical stress from running against the stress from your daily life if you are trying to maintain a healthy immune system. If you are trying to figure out the right balance of intensity and volume, here is a good rule of thumb: you should always finish a run feeling better than when you felt when you started. It is important to have a tool like heart rate or Stryd to quantify how hard you are training so you can ensure you are running easy. and more! I went to Alyse Emdur's special event Stinky Drinks & Edible Flowers at Odd Ark gallery during her solo show 'Skunks and Flowers' and was not only struck by Alyse's work but also the artist herself when she did a Q&A with Lisa Anne Auerbach . Later I had the opportunity to sit with the artist in her loft home/studio in the heart of the Arts District which she shares with her partner and fellow artist Michael Parker. I listened to Alyse talk about the very different things that inspire her, I learnt that there is music that encourages plants to grow and I have a renewed appreciation for skunks... all the while sipping fresh mint tea from a beautiful ceramic mug. Welcome to episode 80 of the Final Surge Podcast where we welcome medical doctor Josh Emdur to the show. Josh has an interesting practice, he works for SteadyMD which changes the way the people interact with their primary care physician. Imagine, instead of calling your primary care doctor for an appointment you texted them and shortly thereafter you were talking to your doctor via a video chat. Dr. Emdur's practice does just that and he specializes in working with runners. You can find him at SteadyMD.com/running. Josh is doing some interesting thing around medicine for runners If you could tell us how you got started as a runner? 8 years ago as became a parent Played soccer, lacrosse was always a better runner than a player As a physician and parent in Boulder turned to running Ran Rock n Roll marathon in San Diego in 2011 and set sights on Boston Wasn't until he hired a coach that he had his big breakthrough What was it working with a coach that really changed things? Had qualified for Boston, but never fast enough to get accepted Didn't understand what it meant to run easy and the 80/20 rule You are online at SteadyMD.com/running can you tell us what this site is about? Been a family doctor for the last decade Concierge medical service for runners How does that work, how do you treat patients virtually? Medical school taught 80% of diagnosis is in history This allows you to take the time to get to know them and their history and goals Subscription service outside of health insurance Build a 1-on-1 relationship with the client Constant communications with patients I’ve seen on your social media that Neely Spence Gracey uses you, being in Boulder are you working with a certain level of runners or anyone? Athlete doctor Athlete care network A new way to handle primary care with someone who understands runners Works with some coaches and athletes as part of their team Work with any athlete trying to help them get to their goal You have mentioned you are more like a primary care doctor for athletes. Why do I need a primary care physician when it seems specialist is more of the trend these days? Primary care doctors look at the big picture Can you practice in any state? Have to be licensed in every state Currently licensed in 26 states You are obviously using technology to make this work, what do you with the future of healthcare technology such as all the data that Apple Health is grabbing, Do you or can you use and monitor that? The current system is not working High deductible systems are not working, discourage care Here they have access to a Doctor Fan of GoodRx It seems that technology is lowering the costs for just about everything in our lives these days, but the one place it is not lowering is our healthcare costs. Healthcare costs total about 18% of our GDP last I saw, without getting into the politics of health care and ObamaCare, do you see subscription services like this as a future way to lower costs? It doesn't cost a lot of money to take good care of people 5 minutes with primary care physician does not move the needle It takes time with patients to dig deeper If someone is already paying obscene amounts for health insurance why would they spend the $79/mo on this too? Runners pay a lot for health insurance and don't even use it because not happy with the experience Get on a higher deductible plan and use this to help with basic health goals People are not used to paying to go see the Doctor, takes time to get used to, but paying for expertise I don't go to the doctors often, I don't think I have been to a Dr. in at least 7-8 years. Last time I was there I had an appointment and all I remember is it took almost an hour longer before I was able to see the doctor. How would an appointment work with you online? Video chat, HIPAA compliant The first appointment is over an hour going over history, that first visit is important to get to know who caring for If received a prescription from you would insurance cover it? Yes I wake up this AM and not feeling well and want a consultation, how long would it take to get an appointment? Text through the app and will be able to see the same day What does an appointment with you look like? Check vitals Get a history of what is going on Come up with a treatment plan What do you think is running related injuries vs general health issues for your clients? 50/50 Most start with running injuries but then develops into a general relationship I wake up on a Tuesday morning with a race this weekend and I am feeling something coming on. You are off on your vacation for a couple weeks rock climbing in South America, what happens if i need service while you are gone? We have other doctors in practice that fill in You mentioned you can be a team member with an athlete and their coach, can you give me an example of how that would work? Have an athlete who is coached by a McMillian coach using Final Surge which I can see the information and notes of We were able to catch an injury issue early when it was just a stress reaction, not a stress fracture Came up with diagnosis and back at it Final Surge 5 questions in under a minute Favorite endurance/running book? - Born to Run Current trainers you are wearing? - Newton Favorite race? - Boston or any race where people are excited to be there Favorite recovery meal or recovery drink? - Cholocate Milk Your favorite workout - Anytime pushing myself usually on trails Resources SteadyMD/Running Website SteadyMDRunning on Twitter SteadyMDRunning on Instagram

As much as we try to navigate our way around Dr. Google and his suggestions for our troubles as runners, we all know deep in our hearts that the best way to deal with an injury or frustration is to go speak to a medical professional. But it can be hard to find someone you trust without spending thousands trying to find someone who won't just say, "stop running". That is why I brought Dr. Mark Cucuzzella and Dr. Josh Emdur on today as they are working on something radical that could change the way we as runners are treated for our running injuries. Steady MD is something that I believe a LOT of you will benefit greatly from. Mark, Josh and I discuss running in its basic form, the peace it brings us, and why even if you have a very busy life, where it feels like you don't even have a minute to yourself, you need to find time to get yourself in a healthy place, or there is no way you can help those you love. If Dr Josh and Dr Mark can manage it, we all definitely can! We discuss the problems in the health industry today, and the frustration as runners we feel when we can't even get our doctors to get back to us within 24 hours, let alone the immediate response we hope for...when we have a pain, but a big race is coming, we need help NOW. We dive deep into the culture we live in, and what we can do if we are frustrated with the communities we live in. You have to be the role model and the one to begin that change towards health. Mark and Josh give great advice how. Finally, we talk about female bodies, and what body fat a woman needs to be able to sustain a healthy pregnancy, and how we can overcome the mismatch with what our culture portrays as the perfect weight. If we do not do something, the girls growing up in our world will believe that an unhealthily lean body type is what they have to strive towards, but it needs to be changed. This episode is for you if you are passionate about health and those you love being in a place where they can live a long and happy life. Or if you are frustrated with injuries and the medical system never really giving you any help or control over the situation. Today's Guests Dr. Mark Cucuzzella and Dr. Josh Emdur Dr Mark Cucuzzella is one of the leaders of the health industry for his work in the state of West Virginia. Mark a family medicine physican with an extensive background in treating health diseases. Mark is the executive director of Two Rivers Treads Running Store, and race director for the Freedom Run. Dr Josh Emdur is a family physician with extensive expertise in the non-surgical treatment of adult disorders, including the treatment of severe life-threatening illnesses. Josh is now the lead physician working for SteadyMD Running; an online platform to work one on one with a health professional who cares about you as a runner. What You Will Learn About Why waking up early to get your run in is like being a member of a secret club and the stillness is so good for us. Is trail or road running more likely to give you an injury? Should we all do trail running or just for runners doing trail races? Why SteadyMD is partnering runners from all over the world with a physician who actually understands runners and can help you with your running injury questions at any time you need. Fructose and the damage is is doing to our bodies, what we can do in our communities to reverse or limit the damage on our loved ones. Why it is so important to cook and eat dinner as a family...even if that means you eat later at night. Why women lose their menstrual cycles, and how to know if you need to do something about it. Inspirational Quotes It wasn't really until I became a parent where I found running and started running consistently. -Josh (Running) just creeps up on you. It took me a while before I started to identify myself as a runner, but once you really start having that consistency of training, you realize, the habit forming and addictive nature of running, its interesting how it just sneaks up on you.-Josh In order to be in a healing profession, it's impossible to care for others if you aren't yourself in a good personal spot...If you are stressed, and you are not happy with your own life, it is really impossible to heal others.-Josh Variety is the spice of life for everything. It is great to do different things, and prevent burnout.-Josh We live in a new world of connectivity with the internet, there is no doubt healthcare is changing...the current model of healthcare doesn't really leverage technology to its potential. With SteadyMD, using technology and messaging and video conferencing, I am able to develop real personal relationships with my runners.-Josh I don't think virtual medicine or online medicine is going to solve every single problem a patient comes in with, but probably 90% of things that happen in well people can be done online.-Mark Most chronic disease management and preventing is understanding the patterns a person has, looking at their lab values, going through training and food diaries.-Mark Just because you are a runner and you exercise frequently, it doesn't actually mean you are healthy.-Josh its up to you, the world out there is designed to make us all really sick. There is a mcdonalds on every street corner, we take buses, the streets aren't safe to go run or ride your bike. That is what it is and that's not gonna go away. Mark You start losing the weight, and your body is making energy.-Mark Don't put pain in your body. -Mark Health is normal human functioning, and a females health, the way we are designed or evolved is that we are designed so that if you are a female to get pregnant, sustain a childbirth, and feed the child, which takes a different level of body fat to Josh or I. We were designed to go out and hunt and kill things or keep bad things away from the cave or whatever habitative dangers are. You need a critical percent body fat to maintain those normal functions, running fast is kind of a mismatch. To run at world class level, probably the highest end body fat of an olympian champion marathoner might come in at around 15-16%, and they are healthy to a degree. Unhealthy is when females start getting down to 10-12%. Mark Once you stop menstruating, it really should raise some flags, and what are my goals. Sure, if you are trying to make the olympics, thats a sacrifice, but for most people, menstruation is a healthy, normal body function, and in order to get it back, you really have to invest in your recovery. -Josh Resources Mentioned Last week's episode with Rob Jones The Healthy Running Clinics (Mark's courses) HealthFitYou (will be the website once it launches) Steady MD Running Natural Running Center website Washington Post Article on Stress Fractures in Collegiate Athletes Peak Performance by Steve Magness and Brad Stulburg
In Episode 016 of A Medicinal Mind: Wisdom and Wellbeing we share a conversation with Board Certified Family Medicine Physician, avid runner and community health visionary Dr. Josh Emdur. While Josh is certainly passionate about exercise and sharing his joy for running with his patients, his calling and compassionate service to others is most profoundly embedded in his unrelenting desire to improve the patient doctor relationship and ultimately improve communities most in need. As you will hear in the podcast, Josh comes from a background and family deeply rooted in medicine, but his unique passions have brought him to pursue an alternative approach to engaging and empowering patients. Josh is one of the newest generation of practitioners practicing telemedicine based Direct Primary Care, as previously outlined in Episode 7 of our podcast with Dr. Spencer Nadolsky DO. Both Josh and Spencer have partnered with SteadyMD, an organization and platform dedicated to direct primary care and engagement of patients beyond what is possible in a traditional face-to-face medical encounter. In our conversation today Josh shares much of his background entering the field of family medicine and later pursuing work as a hospitalist in Boulder, Colorado. Josh shares some of his most insightful wisdom regarding the urgent need to transform healthcare delivery and revitalize the doctor patient relationship. Together, we explore topics such as: What does it mean to be in the profession of medicine? What is a profession in the first place? What are the most unsustainable aspects of our current medical model and how are Josh and other physicians seeking to change the course of healthcare delivery. Getting personal we discuss What does a typical day looks like for Josh when seeing patients as part of SteadyMD? What can patients expect as part of their partnership in working with Josh? What are the expected benefits as well as the potential downsides? Do you lose anything when seeing patients over telemedicine? And what are emerging communication modalities and health education resources that Josh is using to communicate and encourage patients to make sustainable lifestyle changes. And lastly we dig into our desperate need to find space and value in a cramped and overburden healthcare system. Why is time to reflect, think, and further explore the personal needs of each patient so critical, yet remarkably lacking in current, traditional medical models. How can patients add incredible value to their health care and decrease total cost by partnering with a Direct Primary Care doctor through membership based practices. I'm incredibly grateful to Josh and all the visionary physicians and leaders at SteadyMD. As we share in the podcast, healthcare is changing and perhaps the only choice now is to decide whether you will be swimming with the flow or against it. Please seek out Josh via the contact information on our podcast page, and begin to explore the avenues that perhaps you can pursue in partnering with a direct primary care physician, seeking to improve your health and the health of your community. Please, if you enjoy this podcast, we would really appreciate it if you could leave a review in iTunes or Apple Podcasts so that more listeners can engage with our mindful message.
