For those living with Parkinson's, back pain and stiffness can be some of the most frustrating symptoms of the neurological disorder. Luckily, with cutting edge physical therapy programs like LSVT BIG®, those living with Parkinson's can combat the slowing and smallness of movements associated with the disease. Samantha Schroder, DPT, a physical therapist based in Carolina Neurosurgery and Spine's Charlotte office is certified in the approach. She joins Back Talk Doc host, Dr. Sanjiv Lakhia, to discuss how the program reorganizes nerve networks in the brain to improve functioning. The program focuses on big movements, which yield significant improvements in patients committed to the regimen. Samantha explains, “[We] get them to move really big. While it might seem super exaggerated when we act it out, when [the patient] acts it out, it's essentially going to be normal movement because they're starting from a place that's smaller. Having them do things that feel really exaggerated to them is going to look normal to us, and that's one of the main parts of the program.” In addition to explaining the science behind the program, Samantha shares exercises that patients can do to improve their posture and decrease back pain, also noting the benefits of integrating diaphragmatic breathing into the routine.

Through understanding we are able to capture purpose and work harder to achieve goals. Patient education is essential in ensuring compliance and improvement. Her growth as a clinician has been inspired by the most amazing group of therapists and physicians who have mentored and educated her through their outstanding and exemplary work. Each day, Maxcita works to motivate and inspire her patients to do more than the day before. She incorporates her patients' unique experiences, goals and purpose, as she works together with each individual patient to get them back to performing their daily life skills, while regaining mobility and strength. Maxcita has worked throughout various hospitals/ settings including: St. Vincents/Mt Sinai Hospital, Private Practice and Outpatient Therapy. She's also a Downstate Alumni and the NY Lead Occupational Therapist for JAG-ONE Physical Therapy. Krista Denaro is a licensed occupational therapist and recent graduate from SUNY Downstate. Her time at Jag One Physical Therapy dates back to 2016 where she was a volunteer and OT aide in hand therapy. Her interest in hand therapy continued as she completed her Fieldwork level 2 working under Maxcita leading to her current employment at JAG-ONE. Krista strives to work with each patient using a client-centered approach by understanding their needs and values while incorporating a unique plan of care that mirrors their daily activities. Her goal is to become a certified hand therapist as well as continue to expand her practice and knowledge to educate and rehabilitate her patients. Esther Milina graduated from SUNY Downstate Medical Center with a Master's in Occupational Therapy in 2019. Esther completed her Fieldwork Level 2 placement with Maxcita Alvarez and which led to her career at JAG-ONE in 2020. Esther is a Graston Technique provider, kinesotape provider and certified in LSVT-BIG. Esther is committed to serving her communities in South Brooklyn where she grew up and is fluent in Russian. She prides herself on use of humor in order to build relationships with her patients and assist them in reaching their goals. She is currently studying to achieve her goal of becoming a certified hand therapist. We pre-recorded today's conversation via Zoom for our monthly Community Health segment in partnership with Valley Health. October is National Physical Therapy Month, so joining us on the screen: Joanna Jenkins (Rehab Office Specialist), Shelley Annalora (Physical Therapist), and BB Stewart (Occupational Therapist.) Joanna gave us an overview of several physical therapy related services including LSVT Big which helps people with Parkinson's disease learn to use their body more normally; dry needling, which is a therapy used to help relieve neuromuscular pain; and pelvic floor rehab designed specifically for women's health issues. Shelley walked us through several of the therapies and talked about the impact physical therapy can have on a patient's ability to regain control of their independence. BB explained the difference between physical therapy and occupational therapy and gave a few examples. She also told us about their Community Mobility Program. People diagnosed with Parkinson's disease, a neurodegenerative disease, experience a variety of symptoms, at varying levels of progression and severity. Two areas often affected in these individuals are their vocal loudness and their physical movements. LSVT LOUD and LSVT BIG are two programs aimed at these challenges. On this episode, we're joined by Laura Guse and Angela Halpern of LSVT Global. Laura is the Chief Clinical Officer of LSVT BIG and Angela is the Chief Clinical Officer of LSVT LOUD. I began as a float physical therapist with a local outpatient company and within the company I began to see the influx of neurological patients that were not receiving neurological geared treatment. I was able to initiate community based educational sessions for the neurological population and subsequently an opportunity arose for me to switch gears and accept a position with a local hospital-based outpatient facility. Here I was able to expand my knowledge and treatment styles with musculoskeletal and neurological clients. Over the course of the first year, I became more interested with the neurological facet and started to reach out to this population and helped initiate some great programs for individuals with neurological impairment. Now, over 2 years later, I am seeing roughly a 60-40 neuromuscular to musculoskeletal client ratio which has been exciting and beneficial to those in the area of our facility.
