This week spring has sprung while Erin is in Vancouver, Bryan gives us a review of Salome at The Met, and we ponder whether the "Gen" in Gen X, and Gen Z is referring to private parts. Bryan informs us about the pending Supreme Court case of mandated coverage for preventative care which could affect access to PrEP, STI testing and cancer screenings. Erin takes down current toxic masculinity trends, like Congressman Tim Burchett of Tennessee thinking drinking out of a straw is feminine, and an odd trend of men shaving off their eyelashes to appear more manly. My heart is literally on fire today…and not because of a workout —but because we're going deep on HEART HEALTH with one of the top preventative cardiologists in the country, Dr. Milind Parikh. This is personal for me. My dad died of a heart attack at 58. I've got the genetics. I've also got the drive to outlive it. And now, I've got the data AND treatments to potentially stave off the #1 Killer in America—heart attack. On today's IMPACT SHOW episode, Dr. Parikh walks us through what it means to be a preventative cardiologist, why most heart attacks are 100% preventable, and how A.I.-powered Coronary CT scans are revolutionizing modern medicine. This is what is now being called Medicine 3.0 — and friends, it's here. Here are some of the things we discuss: What is “preventative cardiology” and what exactly do you do? The 4 pillars of heart health and how you can improve each one of them. Why we have it wrong many times and WHY “normal” cholesterol doesn't guarantee a healthy heart. The difference between soft plaque vs. hard plaque (this is BIG!); which one is “good” and which one is “bad.” What exactly is a CT heart scan and what do you look for? How the software & technology in a Coronary CT scan with A.I. plaque analysis & FFR shows the difference between “soft plaque” and “hard plaque” and your true risk of having a heart attack. What causes plaque buildup in the heart? How does the A.I. fit into all of this and what is its role? The million dollar question: “Can you reverse plaque buildup and if so, HOW?” Why sleep apnea (often called the “Silent killer”) can ultimately stress your heart and ultimately cause a heart attack/death. The power of early detection of heart disease and how a Coronary CT scan with A.I. plaque analysis can help prevent a coronary episode. Supplements & new meds that may reverse plaque. This is interesting! Why you must start testing NOW— even if you “feel fine.” What's next in Medicine 3.0 and what is your role in this cutting-edge field? This is one of the most important episodes I've ever done. Again, it's something near & dear to my heart (pardon the pun). If you have a family history of heart disease OR you just want to maximize your health and prevent heart disease, this is a MUST LISTEN to episode. If you love LIFE and want to stave off coronary disease or a heart attack, DO NOT MISS THIS ONE…your LIFE may depend on it. LISTEN. SHARE. SAVE A LIFE. I believe in preventative health and today I am discussing my experience getting my second colonoscopy at the age of 53. The recommended age for colonoscopies is now 45 instead of 50. I think many people put it off because they don't know what is going to happen, so I'm going into detail (without sharing TMI) so you can feel prepared. I put my colonoscopy off for too long because I didn't have enough information, so I wanted to share what I know based on what I went through in case it helps you. Also, my first colonoscopy was incomplete. In this solocast I share what was different the second time around, including the anesthesia used and why I scheduled it for a different time of day. This episode is not advice. It is for entertainment purposes only. Anything dealing with your health should be discussed with your trusted healthcare provider, always. First Episode I did on getting a colonoscopy. This Day in Legal History: Mutiny on the BountyOn April 28, 1789, one of the most famous acts of rebellion at sea occurred aboard the HMS Bounty. Captain William Bligh and 18 loyal crew members were forcibly set adrift in the Pacific Ocean by mutineers led by Fletcher Christian. The incident exposed deep tensions over leadership, working conditions, and authority in the Royal Navy. British law at the time treated mutiny as a capital offense, reflecting the critical importance of discipline aboard ships. After the mutiny, an intense search for the culprits began, with some mutineers eventually captured and returned to England to stand trial. The ensuing court-martial proceedings offered early insight into naval justice and the balancing act between maintaining strict command and recognizing crew grievances. Defendants argued that Bligh's harsh leadership provoked the uprising, but the Admiralty was unwavering in its stance against insubordination. Of those captured, three were found guilty and hanged, while others were acquitted or pardoned. The legal handling of the mutiny reinforced the severe consequences for undermining maritime authority. It also prompted discussions about humane treatment of sailors, subtly influencing later reforms in naval discipline. The Mutiny on the Bounty became a lasting symbol in both legal and cultural history, illustrating how law functions as both a tool of control and a response to the realities of human endurance and dissent at sea.Seven Democratic senators on the U.S. Senate Judiciary Committee have requested information from the Justice Department regarding recent changes within its civil rights division under President Donald Trump's administration. In a letter sent Friday, they expressed concern over the reassignment of several career officials, suggesting these moves could be an attempt to pressure staff into leaving and shift the division's enforcement priorities. Since Trump's return to office and the appointment of Pam Bondi as Attorney General, the department has paused investigations into police misconduct, launched a gun rights investigation in Los Angeles, and altered its approach to transgender rights cases. It has also opened investigations into antisemitism related to pro-Palestinian protests at colleges. The senators emphasized the importance of nonpartisan career staff in maintaining the integrity of civil rights enforcement. About a dozen senior attorneys specializing in voting, police, and disability rights were among those reassigned. The Justice Department has not yet commented on the senators' letter.Democratic senators question US Justice Department on civil rights changes | ReutersThe U.S. Drug Enforcement Administration announced that federal law enforcement agencies raided a nightclub in Colorado Springs, arresting over 100 individuals who were in the U.S. illegally. The operation resulted in 114 arrests out of more than 200 people present at the venue, making it one of the largest immigration-related raids since President Donald Trump's second term began. Attorney General Pam Bondi stated that the raid also led to the seizure of cocaine, methamphetamine, and "pink cocaine," and two individuals were arrested on outstanding warrants. Bondi mentioned links to gangs like Tren de Aragua and MS-13, although she did not directly confirm whether those arrested were affiliated with them. The DEA noted that occupants were given multiple warnings before the raid was executed. This action is part of an intensifying crackdown on illegal immigration under Trump's renewed immigration policies. Separately, ICE recently reported nearly 800 immigration-related arrests in Florida during a multi-agency operation.Over 100 migrants in the US illegally arrested in Colorado nightclub | ReutersThe Supreme Court has requested additional briefing in a case challenging the Affordable Care Act's mandate that insurers cover preventive services, like cancer screenings, at no cost. The justices specifically want the parties to address whether the Secretary of Health and Human Services has the legal authority to appoint members of the U.S. Preventive Services Task Force, which advises on covered treatments. During arguments on April 21, Justice Neil Gorsuch questioned whether the power to remove officials necessarily implies the power to appoint them, an issue the lower court had not considered. The Fifth Circuit Court of Appeals previously ruled that the task force's structure violated the Constitution's appointments clause, arguing its members must be nominated by the president and confirmed by the Senate. The Trump administration contends the task force members are merely "inferior officers" under the HHS Secretary's control. The case also involves objections by Texas businesses and residents to mandatory coverage of HIV prevention drugs, claiming unconstitutional imposition by unelected officials. Supplemental briefs are due by May 5, and while rare, this is not the first time the Court has asked for more information after oral arguments, as seen in past cases like Zubik v. In this powerful and deeply personal episode of You're Not Broken, Dr. Kelly Casperson is joined by psychiatrist Dr. Judith Joseph to unpack the complex relationship between trauma, high-functioning depression, and the systemic challenges facing both patients and providers in healthcare. Dr. Joseph shares her unique career journey from anesthesiology to psychiatry—a shift fueled by the pursuit of deeper fulfillment and connection. Drawing from her new book High Functioning, she explores how trauma can show up in unexpected ways: perfectionism, strained relationships, and even the culture of medicine itself. Together, Dr. Joseph and Dr. Casperson dive into the mental health crisis within the medical profession, the difference between complex PTSD and traditional PTSD, and why trauma-informed care must become the standard in healthcare. They highlight the need for preventative mental health care and why joy—not just happiness—is vital to healing. Key Takeaways: Dr. Judith Joseph's new book High Functioning explores hidden depression and the masks people wear. A desire for deeper meaning and purpose sparked her transition from anesthesiology to psychiatry. Trauma can subtly impact daily life, relationships, and career satisfaction. Complex PTSD differs from traditional PTSD and requires a nuanced approach. Perfectionism may be a trauma response rooted in avoidance and fear. Many healthcare professionals operate with unresolved trauma, often unacknowledged and invalidated. The medical system itself can contribute to both patient and provider trauma. High-functioning depression is widespread but often misunderstood or overlooked. Preventative mental health care is crucial—waiting for a crisis is not sustainable. Joy is a core component of mental health, not a luxury or afterthought. Understanding one's own trauma can make you a more effective and empathetic clinician. Identifying and naming emotions can be profoundly therapeutic. Self-care and setting boundaries are necessary acts of preservation, not selfishness. https://www.instagram.com/drjudithjoseph/ High Functioning Book by Dr. Joseph https://drjudithjoseph.com/ Sedona in October - only five slots left: https://www.ascendretreats.com/menopause-and-sexual-health-2025 Order my book "You Are Not Broken: Stop "Should-ing" All Over You Sex Life" Listen to my Tedx Talk: Why we need adult sex ed Take my Adult Sex Ed Master Class: My Website Interested in my sexual health and hormone clinic? What if the next major leap in cybersecurity isn't a new tool but a smarter way to think about the edge? In this special episode recorded live at IGEL Now & Next 2025 in Miami, I sit down with John Walsh, Field CTO for Critical Sectors at IGEL, to unpack the changing face of endpoint security, the rise of zero trust, and the role of AI in shaping both threats and defenses. John brings a career's worth of insight across aerospace, defense, and high-assurance systems to a conversation that covers the shifting threat landscape and how IGEL's preventative security model is helping organizations reduce complexity while strengthening security. He explores the vulnerabilities introduced by hybrid work, BYOD environments, and the increasing reliance on AI models that interact with an enterprise's most sensitive data. We dive into how zero trust is evolving from a marketing buzzword into a practical framework built on collaboration, unified management, and strong policy enforcement at the endpoint. John breaks down why zero trust isn't a single product or checklist but an ecosystem of technologies and partnerships that need to operate together—securely and seamlessly. He also explains why endpoint security still matters, how most attacks still originate at the edge, and why IGEL's immutable operating system is designed to shut down threats before they even begin. As AI-powered attacks accelerate, John also discusses the importance of reducing noise for incident response teams and the potential for AI to eventually move from defense to pre-emptive strike. If you're leading cybersecurity in government, finance, manufacturing, or any sector with critical infrastructure, this conversation will give you a clearer view of where endpoint protection fits into a secure, cloud-connected, AI-augmented world. What are you doing today to make sure your organization's edge is ready for tomorrow?
