Get ready for an exciting, fun, and informative show with Dr. Mark Cucuzzella! Dr. Marks returns to the podcast for an educational conversation about his incredible work practicing family medicine, how he has overhauled the dietary practices in the hospital he works at, and much more. An accomplished marathon runner, Dr. Mark owns a unique and innovative running store to promote and share his enthusiasm for the barefoot movement and minimalist footwear.  This show blends important medical recommendations with helpful information like how to assess and minimize heart disease risk factors, and you'll learn interesting facts, like why the triglyceride to HDL ratio on your blood test is the important marker for heart disease risk factors, as well as useful info about EKG stress tests and coronary calcium score tests, and how to generally live a healthy, balanced lifestyle! TIMESTAMPS: This podcast will be filled with recommendations and information about how to assess and minimize your heart disease risk factors. [01:27] It's good to keep a competitive edge. Running is good mental therapy. [05:40] Mark talks about his medical history during his running experiences. [07:00] What happens when we hit “the wall?” The Central Governor will not let you die. [14:05] The key is to figure out how to turn it on and off. [18:30] It is important to get more than an EKG to predict heart health. You need the coronary calcium score. [21:12] A good measurement for your body is two times your waist should be less than your height. [29:49] It is still taking the medical community too long to catch up with this information. [33:21] If a person is fit and eats healthy, how does a high calcium score come in? [42:25] Your competitive intensity and your drive can all serve to harm you if it gets out of hand.  [49:36] Pre-diabetes can be making too little insulin sometimes. [51:55] All people need to look at their individual body to see what diet is best.  Is there a strategic inclusion of carbs recommended? [56:48] The barefoot minimalist shoe lifestyle is a big part of Dr. Mark's life right now. [01:02:04] NOTES: Where did the term “Type A” originate? Coined in the late 1950s, the term "Type A" originated when cardiologist Meyer Friedman observed a relationship between incidences of heart disease and personality type -- namely, that those most likely to suffer a cardiac event also tended to have, in his estimation, more driven, impatient, high-stress personalities. Who is Tyler Benner and what is a wide toe-box cycling shoe? What are Correct Toes and how can we help our feet? What is the human body intended to do? Are we born to run? What are key exercises we need to do? Why is it dangerous to stop moving??? What do we need to know about the human body about movement? Why is moving on the feet so beneficial for us? How our posture can help us get strong? Are our bodies meant to break down? Can we adapt at any age? What are the dangers of a rigid shoe for walking, running, and your health? How can we save our knees and hips? What's the danger of sugar – and what else do we need to know about nutrition? What does nasal breathing have to do with anything? What can we learn from Patrick McKeown's The Oxygen Advantage? What can we learn from heart coherence and Heart Math's Inner Balance tool and app? Advice for parents and kids for their health. What was the trial against Dr. Tim Noakes author of The Lore of Running that he won. Today we have a double header with 2 great figures as well as athletes in the low carb community. Zach Bitter is the 100 mile American record holder and 12 hour world record holder for ultramarathons. He hosts the Human Performance Outliers podcast with Dr. Shawn Baker and has learned many lessons from amazing people along the way. He has wealth of knowledge of fat adapted athletics and fueling himself with meat while healing his body. But first we have Dr. Mark Cucuzzella who is a Professor of Medicine at West Virginia University medical school, Fellow of the American Academy of Family Physicians, a Family physician for 20 years, and a Lt Col in the US Air Force Reserves. He’s doing such great work in his hospital and community and has implemented some very beneficial programs. He’s a great guy and was a big supporter of the film early on. He brings up that Zach Bitter was actually in the FASTER study that previous podcast guest Dr. Jeff Volek famously did that rewrote the textbooks on fat oxidation. Lot’s of good stuff her beyond running! Thanks again for helping us finish the Indiegogo campaign. We hit our goal, but that certainly wasn’t the amount needed to finish the film. You can still preorder a copy of Food Lies on Indiegogo by clicking through FoodLies.org. Get the limited edition Eat Meat shirt there and check out the other perks - that’s at FoodLies.org. A quick update about me - I just started training for a Decathlon at UCLA. I’m going to do it running on meat and fat. Many of you probably saw the video of me pole vaulting for the first time in over 16 years on social media. This is going to be awesome. Everyone there thinks I’m insane for doing it at 35 while never having done most of the events before, let alone all of them in 2 days at the Masters Championships in Iowa. Also the mostly eating meat, fish, and eggs thing is really tripping them out. Follow along on social media. Also I’ll have a totally unrelated big announcement in the upcoming weeks. Stay tuned - I’m very excited. Now here’s Mark and Zach. http://indiegogo.com/projects/food-lies-post Support me on Patreon! http://patreon.com/peakhuman   Show notes Zach Bitter Hosts the Human Performance Outliers podcast with Dr. Shawn Baker He holds the 100 mile American record with an average pace of 7 minutes per mile He also holds the world record for farthest distance run in 12 hours He spent the first half of his running career on the conventional high carb diet that all runners believe is best He followed a whole foods approach with lots of whole grains, fruits, and vegetables - no processed foods or junk food At age 24/25 he was having some bad symptoms that shouldn’t face a young man in his prime He wasn’t sleeping well, he didn’t have sustained energy throughout the day, and had poor recovery after workouts He looked towards nutrition as a cure and found the low carb, high fat diet Got turned on to Dr. Phinney and Dr. Volek early on so he had a solid approach to transitioning into fat-adaptation correctly He transitioned in the off-season and sleep and energy immediately improved Recovery and mobility greatly improved after races once fat-adapted What are some other sports where it’s good to be fat-adapted? It seems to be dependent on how soon you’re going to train again. If it’s only once per day it seems carbs aren't really necessary. 2 sessions per day, probably yes. He only needs carbs when doing intensity plus high volume. Either one separately he doesn't. How he got into the carnivore world He’s done 73 podcast episodes with various experts and has learned a lot 2 big things are that vegetables aren’t really necessary it seems and there’s a lot of myths about protein that are turning out to not be true. There’s probably no such thing as too much protein My ultra simplistic theory is that animal foods are +1, processed foods are -1, and plant foods are just neutral (or 0). Plant foods aren't doing much for you other than taking up space in your stomach from things that could be worse. The real nutrition is from the animal foods. He agrees with this and builds his meals around fatty meat, eggs, and some full fat dairy He ferments his own vegetables If you have digestive problems try doing only animal foods and add stuff back in slowly and carefully to track it Hunter gatherers used to eat (and still do) fermented plant matter out of animal’s stomachs, especially in cold environments when that was their only vegetation Check out the HPO podcast he does with Shawn Baker Zach does running and nutrition coaching - find him at zachbitter.com He’s on twitter at twitter.com/zbitter and Instagram at instagram.com/zachbitter Mark Cucuzzella SHOW NOTES Mark is a doctor in West Virginia doing great things for his community and grew up as a runner He’s appeared in a lot of big news articles and news segments over the years https://www.nytimes.com/2011/11/06/magazine/running-christopher-mcdougall.html https://www.npr.org/templates/story/story.php?storyId=6112350 https://www.youtube.com/watch?v=X1dyKDrkABc His website https://www.drmarksdesk.com He recently wrote the book Run For Your Life which I’ve read and recommend to any runner https://www.amazon.com/Run-Your-Life-Without-Well-Being/dp/110194630X/ He created the Efficient Running Project for the US Air Force https://www.drmarksdesk.com/usaf-efficient-running He almost made the olympic trials for the marathon but was plagued by injuries and almost quit before taking health into his own hands and also running barefoot or almost barefoot He was able to keep running and has a streak of 30 years of under a 3 hour marathon He opened a running store and has organized tons of runs & events for adults and kids and got the community a lot more active He bought into the low fat high carb diet HUGE waking up at 2am eating more cereal he was so hungry He was in charge of figuring out why people in the Air Force kept failing the fitness tests This was in 2002 and he saw the Gary Taubes article What if it’s All Been a Big Fat Lie? https://www.nytimes.com/2002/07/07/magazine/what-if-it-s-all-been-a-big-fat-lie.html As a doctor he always has learned from his patients. He asked auditoriums full of recruits who has lost 50 pounds or more and kept it off for a year? The only answer was “I gave up bread, or I gave up soda, or I did Paleo, or I did Atkins” He also is a professor at WVU writing papers, etc. He started a program at his hospital that removed all sugary drinks 80% of people in a hospital have insulin resistance and/or metabolic syndrome why would we be giving them a toxin like sugar? The staff supported him in this after getting healthier and losing weight by cutting sugar and carbs He helped get double dollars for food stamps at the farmers market https://youtu.be/k5zptL5c9mI #1 thing purchased with food stamps (now called SNAP) in America is soda!! He’s also starting programs to help people make better choices at dollar stores I think “food deserts” are a myth. It’s just information. You don’t need fresh fruit and veg to be healthy. How Dr. Mark Cucuzzella Goes Slow to Go Fast—Really Fast

#WeGotGoals

Dr. Mark Cucuzzella is the author of the new book Run for Your Life—all about how to achieve longevity in the sport—and has finished marathons in under three hours for 30 straight years. But at one point earlier in his career, he was told he’d have to give up running altogether. For this week’s episode of #WeGotGoals, I caught up with Cucuzzella in Austin at The Running Event, a trade show for running retailers. The way he overcame setbacks serves as a powerful example of charting your own path to your goals—and how slow, steady progress really can lead to major accomplishments. Cucuzzella—who’s now a professor at West Virginia University School of Medicine, a family physician, shoe-store owner, and race director, among other titles—started running in high school. Back then, he told me, coaches and sports scientists didn’t understand nearly as much as they do now about training methods and biomechanics. “We just ran as hard as we could until we broke, repeat,” he said. He continued to train through college and beyond, finding the sport provided a welcome source of stress relief and focus during medical school. And he ran well—he came close to qualifying for the 2000 Olympic Trials in the marathon. The time he needed was two hours and 22 minutes; he ran a 2:24:42 at the Chicago Marathon in 1999. But he was still training hard, putting his body through more than it could bear. After that race, he needed surgery to correct a bent big toe and arthritis in his foot. His doctors said he’d never run again, at least not any more than a jog—and definitely not at a competitive level. By then, Cucuzzella knew enough not to accept that without questioning. “I’d realized pretty early in my medical career … things were constantly changing,” he said. Referencing a historic Harvard commencement address, he said: “Half of what we learned in med school is wrong. We’re just spending a career trying to find out the half that’s right.” Besides, he had a border collie mix who wasn’t satisfied with walking. So, he began jogging slowly. About six months later, he lined up at the Marine Corps Marathon without ever picking up the pace on this training runs—and finished in less than two and a half hours, taking third place. What’s more, he felt a sense of peace and joy while doing it: “I’d never felt that easy before in a marathon,” he said. “it just opened the doors to all the things yet to discover.” Since then, he’s kept nearly all his running easy, only speeding up during a few key workouts leading up to a race. As a result, he hasn’t missed a day due to injury. His low-key approach to training and his method of open-minded self-experimentation, rather than blindly accepting conventional wisdom, form the basis for his career—and his new book. It’s a volume jam-packed with ways to assess where you are as an athlete and suggestions for small changes based on the results. “It’s like a little menu,” he said. Using the self-tests he provides can help you identify your weaknesses in mobility, nutrition, and even sport psychology—whether that’s tight hips, an over-reliance on sugary carbs, or a mind panicked by stress—and start to shift them so you can not only run more and better but also live a longer, healthier life. And healthier is the key, Cucuzzella stresses. Though Americans tend to live long lives, our “healthspan”—the amount of time we survive without major medical problems—ranks low. Fortunately, he said, there’s a lot we can do to care for our bodies and reduce our risk of chronic diseases. Listen to the full episode of #WeGotGoals to hear more about how Cucuzzella is trying to put his ideas into action on a larger scale (for instance, speaking out about his opinions on the Dietary Guidelines for Americans); how writing the book was like running a marathon (or perhaps 19 marathons); and how big goals and challenges like races add not only motivation but also meaning to our lives. Like what you hear? Host Brad Kearns welcomes pioneering physician and legendary endurance runner Dr. Mark Cucuzzella of West Virginia. Mark has run a sub-3 hour marathon for 30 consecutive years! With an all-time best of 2:24, he is still going strong at age 51. And he is doing this stuff in barefoot/minimal footwear. He opened the nations first minimalist running shoe store called TwoRiversTreads. In this show, Mark talks about his high carb, running machine past and his evolution into a healthy, fat-adapted runner. He advocates for doing short bursts of sprinting frequently, called "alactic sprints". When you accelerate for several seconds, you teach your central nervous system the optimal movement patterns to maximize propulsive force and prevent injury, but you aren't overstressing the body with longer duration sprints. Mark is big on going barefoot and being active instead of being a "zoo human."  He discusses his truly revolutionary effort to rid his local hospital of sweetened beverages and enhance education in the diabetic community as to the benefits of ditching processed carbs and increasing intake of healthy, nutritious fats. Mark's journey was turbo-charged six years ago when he learned he was pre-diabetic, despite decades of elite level marathon performance and being a lean, mean machine. He read Gary Taubes's work and embarked on a low-carb lifestyle, also embracing the aerobic training principles of his longtime friend Dr. Phil Maffetone. Mark mentions that is the stuffy medical world, we need convincing for dietary transformation, and that Continuous Glucose Monitors are now easily accessible for all. The small effort he is making in West Virginia (the most obese all US states in fact!) is spreading quickly in the medical community as Mark spreads the word. Enjoy these insights from a truly exceptional endurance performer who is doing transformative work in his hospital environment! After performing at a fast level and having the accompanying injuries, is there a way to analyze the future potential you have to improve or surpass your previous record?  [00:05:14]  Does the development of the athletic ability in a youngster really help prepare for adult competition? [00:09:22]  How does one work on their form? [00:14:43]  The body loves variability. [00:20:08]  How has the medical world failed in keeping up with the new information on diabetes? [00:21:22]  What happened when Mark was diagnosed with pre-diabetes? [00:35:03]  What about the lower heart rate training program? How did the fat burning capacity develop? [00:42:11]  The importance of sleep is often overlooked.[00:43:48]  What is hypoglycemia? [00:45:02]  Can a person still eat carbs at all? As much as we try to navigate our way around Dr. Google and his suggestions for our troubles as runners, we all know deep in our hearts that the best way to deal with an injury or frustration is to go speak to a medical professional.  But it can be hard to find someone you trust without spending thousands trying to find someone who won't just say, "stop running". That is why I brought Dr. Mark Cucuzzella and Dr. Josh Emdur on today as they are working on something radical that could change the way we as runners are treated for our running injuries. Steady MD is something that I believe a LOT of you will benefit greatly from. Mark, Josh and I discuss running in its basic form, the peace it brings us, and why even if you have a very busy life, where it feels like you don't even have a minute to yourself, you need to find time to get yourself in a healthy place, or there is no way you can help those you love. If Dr Josh and Dr Mark can manage it, we all definitely can! We discuss the problems in the health industry today, and the frustration as runners we feel when we can't even get our doctors to get back to us within 24 hours, let alone the immediate response we hope for...when we have a pain, but a big race is coming, we need help NOW. We dive deep into the culture we live in, and what we can do if we are frustrated with the communities we live in. You have to be the role model and the one to begin that change towards health. Mark and Josh give great advice how. Finally, we talk about female bodies, and what body fat a woman needs to be able to sustain a healthy pregnancy, and how we can overcome the mismatch with what our culture portrays as the perfect weight. If we do not do something, the girls growing up in our world will believe that an unhealthily lean body type is what they have to strive towards, but it needs to be changed.  This episode is for you if you are passionate about health and those you love being in a place where they can live a long and happy life. Or if you are frustrated with injuries and the medical system never really giving you any help or control over the situation.  Today's Guests Dr. Mark Cucuzzella and Dr. Josh Emdur Dr Mark Cucuzzella is one of the leaders of the health industry for his work in the state of West Virginia. Mark a family medicine physican with an extensive background in treating health diseases. Mark is the executive director of Two Rivers Treads Running Store, and race director for the Freedom Run. Dr Josh Emdur is a family physician with extensive expertise in the non-surgical treatment of adult disorders, including the treatment of severe life-threatening illnesses. Josh is now the lead physician working for SteadyMD Running; an online platform to work one on one with a health professional who cares about you as a runner. What You Will Learn About Why waking up early to get your run in is like being a member of a secret club and the stillness is so good for us. Is trail or road running more likely to give you an injury? Should we all do trail running or just for runners doing trail races? Why SteadyMD is partnering runners from all over the world with a physician who actually understands runners and can help you with your running injury questions at any time you need. Fructose and the damage is is doing to our bodies, what we can do in our communities to reverse or limit the damage on our loved ones. Why it is so important to cook and eat dinner as a family...even if that means you eat later at night. Why women lose their menstrual cycles, and how to know if you need to do something about it. Inspirational Quotes It wasn't really until I became a parent where I found running and started running consistently.  -Josh (Running) just creeps up on you. It took me a while before I started to identify myself as a runner, but once you really start having that consistency of training, you realize, the habit forming and addictive nature of running, its interesting how it just sneaks up on you.-Josh In order to be in a healing profession, it's impossible to care for others if you aren't yourself in a good personal spot...If you are stressed, and you are not happy with your own life, it is really impossible to heal others.-Josh Variety is the spice of life for everything. It is great to do different things, and prevent burnout.-Josh We live in a new world of connectivity with the internet, there is no doubt healthcare is changing...the current model of healthcare doesn't really leverage technology to its potential. With SteadyMD, using technology and messaging and video conferencing, I am able to develop real personal relationships with my runners.-Josh I don't think virtual medicine or online medicine is going to solve every single problem a patient comes in with, but probably 90% of things that happen in well people can be done online.-Mark Most chronic disease management and preventing is understanding the patterns a person has, looking at their lab values, going through training and food diaries.-Mark Just because you are a runner and you exercise frequently, it doesn't actually mean you are healthy.-Josh its up to you, the world out there is designed to make us all really sick. There is a mcdonalds on every street corner, we take buses, the streets aren't safe to go run or ride your bike. That is what it is and that's not gonna go away. Mark You start losing the weight, and your body is making energy.-Mark Don't put pain in your body. -Mark Health is normal human functioning, and a females health, the way we are designed or evolved is that we are designed so that if you are a female to get pregnant, sustain a childbirth, and feed the child, which takes a different level of body fat to Josh or I. We were designed to go out and hunt and kill things or keep bad things away from the cave or whatever habitative dangers are. You need a critical percent body fat to maintain those normal functions, running fast is kind of a mismatch. To run at world class level, probably the highest end body fat of an olympian champion marathoner might come in at around 15-16%, and they are healthy to a degree. Unhealthy is when females start getting down to 10-12%. Mark Once you stop menstruating, it really should raise some flags, and what are my goals. Sure, if you are trying to make the olympics, thats a sacrifice, but for most people, menstruation is a healthy, normal body function, and in order to get it back, you really have to invest in your recovery. -Josh Resources Mentioned Last week's episode with Rob Jones The Healthy Running Clinics (Mark's courses) HealthFitYou (will be the website once it launches) Steady MD Running Natural Running Center website  Washington Post Article on Stress Fractures in Collegiate Athletes Peak Performance by Steve Magness and  Brad Stulburg

What do you do when you experience significant injury after injury, so much so that surgery no longer helps and most doctors tell you it's time to hang up the running shoes? You hang up your shoes and try natural running, of course. In 2000, Dr. Mark Cucuzzella did just that. After suffering for years from arthritis in his toes, Mark was told by his doctors to stop his running. However, Mark couldn't give up on his passion, so he began to research and scientifically explore how natural running shoes could help save his career. What Dr. Mark found was nothing short of remarkable. By running barefoot and in natural running shoes he was able to run pain-free and injury-free for the first time in years. In this podcast, we interview Dr. Mark Cucuzzella and find out exactly what he did, why it worked, and how you can do the same. Dr. Cucuzzella is widely considered to be one of the leading experts on how and why to incorporate and transition to minimalist running. Mark is a Professor of Family Medicine at West Virginia University School of Medicine. His marathon best is 2:24 and has run under 2:40 for a marathon 24 of the last 25 years, with 22 of these years being run under 2:35. He won the 2011 Air Force Marathon in 2:38, a week shy of his 45th birthday. He attributes much of this longevity to good running form and minimalist principles. And that's what he's here to teach you today. If you've been thinking about how, or if you should, transition to natural running shoes, you've got to listen to this interview.
