GP and menopause specialist, Dr Sarah Glynne, joins Dr Louise Newson on the podcast this week to discuss menopause care after breast cancer. The experts share more about the breast cancer steering group established as part of the Newson Health Menopause Society that is working towards producing a consensus statement to support clinicians and improve the quality of life for menopausal women who have had breast cancer. Dr Sarah Glynne discusses the importance of individualising the risk-benefit ratio for every woman when making decisions around treating the cancer and weighing this up with treating menopausal symptoms. Sarah emphasises the importance of talking through the implications of each of these considerations using a shared decision making process. Sarah's three tips for women after breast cancer: Understand the risks and benefits of the drugs used to treat your breast cancer and what this means for you personally. Ask your oncologist for more information about your own breast cancer, if you are not sure. You can then use the PREDICT tool online for understanding more about your own cancer risks and what additional benefits any treatments may offer. Read about non-hormonal options to help your menopause symptoms and cancer recovery such as diet, yoga, or acupuncture. Try various approaches to find the ones that may bring some benefit to you. Vaginal moisturisers and lubricants may also help and these do not contain hormones, and there are other medications your GP may be able to prescribe for some of your symptoms such as hot flushes. If your menopause symptoms are severe and your quality of life is suffering, ask your clinician to explain the risks for you regarding your cancer prognosis if you decide to take HRT, versus the risks to your quality of life and long-term health if you choose not to take HRT. If you have genitourinary symptoms of soreness and dryness, vaginal hormones are very safe for improving these symptoms. Read information on the balance website and the book ‘Oestrogen Matters' by Avrum Bluming, and make a choice that is right for you through discussion with your clinician using a shared decision making process.
When it comes to HRT, those with a history or increased risk of breast cancer can often feel left out of the conversation. Dr Claire & Caroline talk to Dr Sarah Glynne about the link between link between Covid-19 and female hormones.Sarah graduated from Imperial College in 1999 and became a member of the Royal College of Physicians in 2002 and then switched to general practice, becoming a member of the Royal College of General Practitioners in 2006. In 2016 she completed a Master of Science degree in allergy.She has been developing an interest in the menopause since 2018 and is on the Advisory Board, as well as the Editorial Board, of the Newson Health Menopause Society. She is particularly interested in the link between Covid-19 and female hormones, and believes that covid-19 induced hormonal perturbations are a barrier to recovery in many female patients with long Covid, unless treated with hormone replacement therapy (HRT). Dr Glynne is also interested in the management of menopausal symptoms in breast cancer survivors. She is co-chair of a newly established breast cancer working group that is currently developing a consensus statement to raise awareness about the risks and benefits of HRT for women with a history of breast cancer. The consensus statement will hopefully be available in early 2022. There may be a treatment from which a third of women with Long Covid could benefit. It is not a new drug, it is safe, and it has been proven to work anecdotally. And yet there are no clinical trials looking at HRT as a potential treatment for the largest cohort of Long Covid sufferers. In our patriarchal medical system many women are not even being asked about their menstrual cycle or hormones in relation to Long Covid, and yet there is clear evidence that our hormones have been impacted. This week, Dr Sarah Glynne, G.P. and menopause specialist, talks us through her research into Long Covid and the peri-menopause, and the prescribing of HRT as a treatment for Long Covid, which she believes shows tangible results.

In comparing notes between patients, married practitioners, Dr Sarah and Paul Glynne, became intrigued as they noticed cross over between reports of certain symptoms in their patients. NHS GP and Menopause specialist, Sarah, was coming into contact with perimenopausal patients concerned that they may be suffering with Long Covid. In tandem, husband Paul – a general practitioner based at University College Hospital, was progressively seeing more patients suffering with Long Covid. Whilst Sarah and Paul may be found guilty of ‘taking their work home with them', their findings in doing so, were arguably worth the offence. In comparing notes on patients, the couple came to the realisation that Long Covid symptoms and perimenopausal symptoms, were often indistinguishable. In this third episode of the Newson Health Menopause Society podcast, our host Lauren is joined by Sarah and Paul who discuss their observations and share insight into the work that they are both independently undertaking. This includes fascinating insights into Dr Paul Glynne's current work that is exploring possible interventions and treatments for Long Covid sufferers. Paul highlights his observation that HRT is currently, one of the most effective interventions in alleviating symptoms in the right patients that he has observed.
