This week, as we step into 2024, I'm reflecting on the incredible journey of the podcast and the amazing guests who've joined me along the way. Five years ago, on January 1, 2020, I launched the very first episode of You Are Not Broken. At the time, Instagram wasn't even part of the equation for me and now I just hit 200K, I hadn't published a book, hadn't done a TEDX talk and the term "menopause expert" wasn't something that I was. Looking back, I can confidently say: that first episode was pretty good! It was an eye-opening discussion that challenged many preconceived notions about sexual wellness and the critical role of specialized care in improving quality of life. Leading US oncologist Dr Avrum Bluming joins Dr Louise Newson to talk about the crucial role of oestrogen in women's health. Despite HRT's proven benefits in protecting against heart disease, bone fracture and cognitive decline, many women still avoid it over breast cancer fears. It's been more than 20 years since media headlines about a study called the Women's Health Initiative linked HRT to an increased risk of breast cancer. In this podcast, Dr Bluming says that in fact we now know oestrogen alone decreases the risk of breast cancer development by 23% and risk of death from breast cancer by 40%.  He also disputes the findings of the WHI study that combined progesterone and oestrogen HRT leads to a small increase in breast cancer cases. ‘It is very upsetting when such an influential study continues to misquote their own data,' says Dr Bluming, who has spent 25 years studying the benefits and risks of HRT in breast cancer survivors. Dr Bluming points out that oestrogen used to be a treatment for breast cancer before chemotherapy was developed, and that rates of breast cancer increase as we age, despite the fact our oestrogen levels fall as we get older. Estrogen Matters with Dr. Avrum Bluming Avrum Bluming received his MD from the Columbia College of Physicians and Surgeons. He spent four years as a senior investigator for the National Cancer Institute. He organized the first study of lumpectomy for the treatment of breast cancer in Southern California in 1978, and for more than two decades he has been studying the benefits and risks of hormone replacement therapy administered to women with a history of breast cancer. Dr. Bluming has served as a clinical professor of medicine at USC and  is the co-author of “Estrogen Matters: why taking hormones in menopause can improve women's wellbeing and lengthen their lives – without raising the risk of breast cancer” with Dr. Carol Tavris.   Hear his story about how he became the advocate for hormones and hormones after breast cancer.   So many people think correlation = causation – case with E and breast cancer – just poor education in our population?   “we don't know what causes breast cancer” – can people handle a grey world?   Estrogen receptor positive doesn't mean estrogen caused the cancer – discuss   Dispel the myth that early puberty and late menopause is a breast cancer risk – dogma – “lifetime exposure” – but women with lots of pregnancies have lower risk.   Should we just work on a re-brand? Go on HRT because if you do get breast cancer you will do better than if you weren't on it?   How do aromatase inhibitors work? People call them “estrogen blockers” so people think estrogen causes cancer and recurrence.  Why only in post-menopausal women?   What's with adipose tissue? People always say it is because adipose tissue increases estrogen – is it estrone ?? Or inflammation?   Doctors don't want to be sued. Do you know of any doctor getting sued for giving hormones in general or after breast cancer?   If estrogen is FDA approved for prevention of osteoporosis – why did the USPSTF say no to prevention and why are we screening for osteoporosis so late (age 65?)   Do you agree with the USPSTF about no for prevention. Your opinion of Progestin causing Breast cancer" Is the placebo arm bad?  Is the risk not actually that high – from 2-4% Is it the synthetic progestin?  Everyone's talking about dementia now – Dr. Bluming's take  There are 25 studies showing E is safe after breast cancer  and 20 review studies– and the HABITS trial is the only one that shows local recurrence and is flawed – how many studies are enough?   Advice for younger physicians who care about this work?   Do you think the world is just crazy? We're diving head first into a necessary and energetic discussion about a topic that has stirred the medical world: hormone replacement therapy (HRT).  HRT is both a medical and feminist issue and we're creating a call to action for all women to educate ourselves in order to advocate for ourselves.As with many of the experts we've connected with in our menopause community, Avrum Bluming and Carol Tavris didn't set out to be activists.  These leaders were compelled by the shift in culture around treating women with symptoms of perimenopause as a result of the groundbreaking research done by the Women's Health Initiative (WHI).  Overnight the perception of HRT transformed dramatically, and stopped being prescribed. Bluming and Tavris are joining us to address the exaggerated claims made by the Women's Health Initiative and shed light on the overlooked benefits of HRT, all while ensuring that the specter of breast cancer risk remains low.  AVRUM BLUMING, MDAvrum Bluming received his MD from the Columbia College of Physicians and Surgeons. GP and menopause specialist, Dr Sarah Glynne, joins Dr Louise Newson on the podcast this week to discuss menopause care after breast cancer.  The experts share more about the breast cancer steering group established as part of the Newson Health Menopause Society that is working towards producing a consensus statement to support clinicians and improve the quality of life for menopausal women who have had breast cancer. Dr Sarah Glynne discusses the importance of individualising the risk-benefit ratio for every woman when making decisions around treating the cancer and weighing this up with treating menopausal symptoms. Sarah emphasises the importance of talking through the implications of each of these considerations using a shared decision making process. Sarah's three tips for women after breast cancer: Understand the risks and benefits of the drugs used to treat your breast cancer and what this means for you personally. Ask your oncologist for more information about your own breast cancer, if you are not sure. You can then use the PREDICT tool online for understanding more about your own cancer risks and what additional benefits any treatments may offer. Read about non-hormonal options to help your menopause symptoms and cancer recovery such as diet, yoga, or acupuncture. Try various approaches to find the ones that may bring some benefit to you. Vaginal moisturisers and lubricants may also help and these do not contain hormones, and there are other medications your GP may be able to prescribe for some of your symptoms such as hot flushes. If your menopause symptoms are severe and your quality of life is suffering, ask your clinician to explain the risks for you regarding your cancer prognosis if you decide to take HRT, versus the risks to your quality of life and long-term health if you choose not to take HRT. If you have genitourinary symptoms of soreness and dryness, vaginal hormones are very safe for improving these symptoms. One of THE most controversial areas of menopause is around whether or not those who have been treated for breast cancer should be 'allowed' to take HRT or not. Often there is a blanket no from medical practitioners, but what about personal choice?  What if an individual decides that it's their body and therefore their decision about whether or not they feel it's a risk they're willing to take in order to have a decent quality of life. My guest today is Ruth George.  Ruth was treated for breast cancer but found that the medication she was given after her surgery, intended to stop her natural production of oestrogen, was creating a whole host of other symptoms that were ruining her quality of life. She went on a personal quest to find out as much as she could about the actual risks, the benefits and the longer term implications if she did choose to go against her GP's advice. Progesterone and the Pill Debates, Decisions, and Final Lessons in the Case for HRT That is a list of chapter titles in the book, Estrogen Matters: Why Taking Hormones in Menopause Can Improve Women's Well-Being and Lengthen Their Lives—Without Raising the Risk of Breast Cancer by medical oncologist Avrum Bluming, MD, and social psychologist, Carol Tavris, PhD.  I was introduced to the book by my former PCP, Dr. Melody Rodarte, DO. It reads like a thriller. The introduction is even called, "Who Killed HRT?" The authors tell a riveting story. Over 20 years ago, they indicate that a poorly conceived and misinformed study from the Women's Health Initiative (WHI) swept through the medical community, indicating a causal connection of estrogen and breast cancer and demonizing the use of estrogen to treat menopausal women.  "... We will show in a close examination of the WHI studies, some of those claims were exaggerated, some were misleading, and some were just wrong—and several of the WHI investigators themselves eventually backed away from them." The book delivers the goods, names names, and provides more than 50 pages of referenced articles and studies to back up their claims that "estrogen matters."  "But wait, Jan! My oncologist has told me ... " Yes, I totally understand. Mine has told me the same things. You'll hear my story in the interview. Now that I've read the book, I'm sending my onc a copy and begging her to look into it. You might consider doing the same thing. This quote in the book says it all. "All scientific work is incomplete—whether it be observational or experimental. All scientific work is liable to be upset or modified by advancing knowledge. That does not confer upon us a freedom to ignore the knowledge we already have, or to postpone the action that it appears to demand at a given time." —Austin Bradford Hill, 1965 (Hill is the British biostatistician who pioneered the randomized clinical trial) Why should we even consider using estrogen as women who have gone through breast cancer treatment? Here are a few reasons from the book's back cover. "There is no current way to prevent or treat Alzheimer's disease and other forms of dementia—except estrogen." "Seven times as many women die of heart disease as die of breast cancer annually. In fact, heart disease, not cancer, is the leading cause of death among breast cancer survivors, and HRT can decrease that risk by 30 to 50 percent." "The same number of women die annually following osteoporotic hip fracture as die of breast cancer, and HRT can cut this risk in half." "There is no evidence to support the current medical advice to take HRT at the lowest dose for the shortest period of time." "HRT is the most effective treatment for all menopausal symptoms, not only the familiar hot flashes, night sweats, and insomnia—which can last seven years or longer—but also heart palpitations, joint and muscle aches, headaches, bladder problems, sexual discomforts, and depression, which many women do not realize are often signs of menopause as well." "Women taking HRT live, on average, several years longer than those not taking it." "Estrogen does not increase the risk of breast cancer. Breast cancer is not a disease, as some professionals believe, 'of having too much estrogen in your body.' Even breast cancer survivors may safely take HRT under a doctor's supervision, and it will not increase their risk of recurrence." I am delighted to have Dr. Renee Wellenstein joining me today! Dr. Renee is a double board-certified doctor who has been working with women for over 20 years. Due to her own health challenges, she stepped outside the box of conventional medicine to take a radically different approach to heal herself from the deep depths of burnout. She has had her fair share of obstacles throughout her journey, and she has mastered the art of leaping into transitions with each pivot! Now Dr. Renee empowers women to take control of their health, jump-start their energy, improve their confidence, and reignite their libido! Low libido in women is a topic that is grossly misunderstood on many levels and does not get discussed often enough between doctors and patients. Research has shown that 43% of women experience low libido at some point in their lifetime. That is why we need to discuss it and keep the conversation open. We have to talk about low libido even though most women feel some degree of stigmatization around it and tend to feel uncomfortable talking about it with their girlfriends, loved ones, and especially their significant others. In this episode, Dr. Renee dives into connection, intimacy, sleep, exercise, and the influence of food choices on libido. She gives her specific recommendations for women who have experienced vaginal changes and the effects of hormone replacement therapy. She discusses options that can help support women who have had breast cancer treatment whose healthcare providers do not advocate hormonal replacement therapy. Dr. Renee and I explain why we do not love pellets. I am delighted to have two respected researchers, Dr. Avrum Bluming and Dr. Carol Tavris, joining me today! They are the co-authors of the book, Estrogen Matters.  Avrum Bluming received his MD from the Columbia College of Physicians and Surgeons. He spent four years as a senior investigator for the National Cancer Institute. For two of those years, he was director of the Lymphoma Treatment Center in Kampala, Uganda. For more than three decades, he has been studying the benefits and risks of hormone replacement therapy administered to women with a history of breast cancer. Carol Tavris received her Ph.D. in social psychology from the University of Michigan. Her books include Mistakes Were Made (But Not by Me), with Elliot Aronson; Anger: The Misunderstood Emotion, and The Mismeasure of Woman. Estrogen Matters provides a lot of scientific information that refutes much of the fear-mongering around hormone replacement therapy. It is helpful for women who have gone through cancer therapy and those who have not had to deal with cancer, in terms of the conversations they could consider having with their healthcare professionals. Dr. Bluming and Dr. Tavris are both incredible individuals, and it was an honor to interact with them! In this episode, we dive into statistical manipulation and the influence of the Women's Health Initiative. We talk about the study limitations and the role of data mining. We discuss the benefits of hormone replacement therapy and its impact on cancer risk, coronary artery disease, bone and brain health, and more. If a woman starts taking hormones within ten years of her last menstrual period, she will have a reduced risk of heart disease and strokes and also improved longevity.  The women who fear taking hormone replacement therapy are afraid of breast cancer.  Estrogen reduces the risk of heart disease by up to fifty percent. Drs. Tavris and Bluming talk about bone health.  The impact of hormone replacement on brain health. How the investigators of the Women's Health Initiative manipulated their findings on dementia. Medical oncologist, Dr Avrum Bluming makes a welcome return to the podcast this week to re-visit the hot topic of menopause hormone therapy after breast cancer. Avrum has spent decades studying the research on the benefits and risks of HRT in women with a history of breast cancer and is passionate about giving women clear, evidence-based information that dispels myths and combats the misinformation that has unnecessarily frightened women and clinicians for over 20 years. In discussion with Dr Louise Newson, Avrum clearly explains what his recent review of the literature reveals about the safety of HRT and the benefits it brings for your future health. The experts highlight the gender disparities that are commonplace in how women with cancer are treated with regard to their hormones compared to men, and they also discuss the importance of patient-centred medicine and giving women choice. Avrum's 3 tips for women interested in exploring their menopause treatment options after breast cancer: Speak to your oncologist. Tell them about your menopause symptoms, ask to discuss the possibility of starting HRT and have a conversation about the benefits and risk for you individually. When it is available, take Avrum's article that will be published in the ‘Cancer' journal in May/June 2022, and show it to your oncologist and GP. Don't accept a dismissal of your views – engage them in discussion. ‘Oestrogen Matters' (2018, published by Little Brown) is a book co-authored by Avrum that is for women and clinicians, including a chapter on HRT after breast cancer, and it is heavily referenced to show all the evidence behind the information given. Cui Bono, the Latin phrase meaning “who benefits,” says the motive for an act or crime lies with the person who has something to gain.Only twenty percent of our senior women and even fewer men take hormones. Outside Europe and the US, usage is rare. How is this possible? The “bio-identical” or “human” forms of these drugs are not promoted because they can rarely be patented to make the big money. But they work better and are safer than other medications such as statins, antidepressants, many cancer treatments, and the proprietary imitation hormones made by big Pharma. These industry cash cows are supported and protected, while in contrast, natural hormones are defamed and restricted. Chasing profits has ruined science. To explain, here is how the Women's Health Initiative study (WHI) was hijacked by its own authors and sabotaged patient care. This huge National Institutes of Health trial (published in 2002) examined 160,000 women aged 59 to 79. It found an increase in breast cancer for patients taking both Premarin, the horse urine estrogen, and Provera, the patented synthetic progesterone. But those who took only Premarin had a decrease in breast cancer. This proved Provera was responsible, and other trials confirmed it. The WHI should have ended this medication's use for long-term applications, but it did not.The WHI took 11 years, and by that time it was complete, the two drugs it examined were obsolete. But the study statisticians claimed they uncovered critical dangers, and the authors sensationalized and embellished their threadbare findings. Medical academics buffed their reputations by declaring that they, too, could see the emperor's clothes. The media joined the parade—baloney sells advertising—and the public soon believed that all female hormones were killers. This “man bites dog” story still terrifies everyone. Once a bell is rung, it cannot be unrung. In the public and medical eye, hormones were branded with cancer, dementia, and other problems. One reviewer wrote that the study authors were “overselling hysteria.” John Goldman, MD, wrote in Medscape, “[The study] has undermined the credibility of the research and the medical community as a whole.” Abraham Morgentaler, MD, and others (Harvard) explained how the panic was generated:The (WHI)… reported increased risk of adverse events of only 19 events per 10,000 person-years of exposure for the estrogen–progesterone arm [Premarin-Provera] compared with placebo. This means that if one woman in every generation of a family used estrogen–progesterone for 10 years, it would take 50 generations, or about 1,000 years, to see one extra adverse event in that family. The result may have been statistically significant, but they were clinically meaningless.Avrum Bluming and Carol Tavris described the study's statistical trickery and atrocious sensationalism in Estrogen Matters (2018), a superb book about the science and politics. One of the WHI's principal investigators, Rossouw, had an agenda to “change the thinking about hormones.” Six years before the WHI was published, he wrote it was time to put “the brakes on that bandwagon,” referring to the growing support for estrogen replacement. And so the WHI authors ignored their colleagues' advice and rushed to publication before completing the study. We all want to age gracefully, but the truth is that aging isn't always glamorous. Women going through menopause deal with a lot of difficulties because aside from the emotional turmoil, there are also physical pains and dysfunctions like having osteoporosis. Women over the age of 50 are the ones most likely to develop this disease, but the good news is there's something we can do about it.  In this week's podcast episode, I talk with Sarah Purcell, a former investment banker turned Pilates and Yoga teacher. Avrum Bluming was a clinical professor of medical oncology at the University of Southern California and spent four years as a senior investigator for the National Cancer Institute. For more than two decades he has been studying the benefits and risks of hormone replacement therapy administered to women with a history of breast cancer. He is also the co-author of the book “Oestrogen Matters”.  In this podcast, Dr Newson talks to Avrum about the numerous health benefits of oestrogen in women, including reducing future risk of heart disease, osteoporosis and dementia. It is a very sobering thought that as many women die from osteoporosis as breast cancer, yet so many women are denied oestrogen in HRT for the wrong reasons. Avrum's wife was diagnosed with breast cancer many years ago and he talks about treatments for breast cancer which can lead to an early menopause.   https://estrogenmatters.com  Avrum Bluming's Three Take Home Tips About HRT: If every woman in the US started HRT, the median life expectancy of women would extend by 3.3 years If you ever hear "If you must take hormones, then take the smallest dose possible for the shortest period of time" - there is no scientific support for this! Women can take HRT for as long as they need. Some of the benefits of HRT, particularly for your bones, last as long as you continue to take it. If you stop taking the hormones, bone degeneration occurs at an accelerated pace.

This week’s episode of Point of Inquiry Jim Underdown speaks with Carol Tavris, social psychologist and author of Mistakes Were Made (But Not By Me) and Avrum Bluming, hematologist, medical oncologist, and emeritus clinical professor at USC about the common myth in the medical field  surrounding the link between breast cancer and estrogen.  The talk centers around their recent book, Estrogen Matters which examines the practice of administering estrogen to women suffering from symptoms of menopause and the push back they received due to a long-held misconception that estrogen leads to an increased chance of contracting breast cancer. Tavris and Bluming's work illustrates the important need for critical thinking, especially in the area of health where people's well-being is constantly at stake and how people will often times not accept information when it is in their best interest to do so.

Avrum Bluming received his MD from the Columbia College of Physicians and Surgeons. He spent four years as a senior investigator for the National Cancer Institute and for two of those years was director of the Lymphoma Treatment Center in Kampala, Uganda. He organized the first study of lumpectomy for the treatment of breast cancer in Southern California in 1978, and for more than two decades he has been studying the benefits and risks of hormone replacement therapy administered to women with a history of breast cancer. Dr. Bluming has served as a clinical professor of medicine at USC and has been an invited speaker at the Royal College of Physicians in London and the Pasteur Institute in Paris. He was elected to mastership in the American College of Physicians, an honor accorded to only five hundred of the over one hundred thousand board-certified internists in this country. Carol Tavris received her PhD in social psychology from the University of Michigan. Her books include Mistakes Were Made (But Not by Me), with Elliot Aronson; Anger: The Misunderstood Emotion, and The Mismeasure of Woman. She has written articles, op-eds, and book reviews on topics in psychological science for a wide array of publications — including the Los Angeles Times, the New York Times Book Review, the Wall Street Journal, and the TLS — and a column, “The Gadfly,” for Skeptic magazine. She is a fellow of the Association for Psychological Science and has received numerous awards for her efforts to promote gender equality, science, and skepticism.
