I first met Mr. Shingo at the Shingo Institute Annual Conference in 2009 when my book Lean Hospitals received the publication prize that's named after his father, Shigeo Shingo. I was also blessed to have time to speak 1x1 with Mr. Shingo, thanks to our mutual friend, the late Norman Bodek, which included discussions about the need for mistake proofing in healthcare -- very vivid memories for me. Ritsuo Shingo is an expert in leadership with more than 40 years of experience serving at top management positions at Toyota.  He was the founder and the first president of Toyota China. Under his leadership, Toyota China became one of the most successful ventures of Toyota worldwide. Following this success, he was appointed as the president of Hino Motors and then served as the president of GAC-Hino until 2009. Shingo was the translator of the first book on Toyota Production System in English written by his father, TPS pioneer, Shigeo Shingo in 1976.  He applied his father's and other TPS pioneers' teachings into his management practices. Today he dedicates his time to coaching high-level executives as well as teaching the next generation of leaders his learnings from the practice of Toyota style management. Mark helps others learn how to improve to sustain their performance. He is the author of "Lean Hospitals", "Healthcare Kaizen" and most recently "Measures of Success". Mark is the host of several podcasts, including "Lean Blog Interviews" and "My Favourite Mistake". SummaryLeaders awaken through insight or discovery, and this is necessary for change. Experiential learning within safe environments: simulations and games, e.g. red bean game (W Edwards Demming), effectively allows leaders to reflect on their management practices. This secure play and simulation encourage leaders to find their Aha moment, which will shift their behaviour. Mark uses games that demonstrate Kaizen principles: making an ice cream sundae smoothly or preparing a batch of mailers and envelopes. Mark and his colleagues are currently looking to develop simulations that can be accessed virtually. Mark shares some of the results he has seen in Healthcare through an excellence journey; dramatic improvements in patient safety, time savings and performance outcomes in nursing and accuracy and quality in operations. When it is time to review the goals, look at the systems, and create understanding together. I would never have imagined that there were so many opportunities in health care to improve quality, to improve patient flow, to create better workplaces. 07:57min The challenges, I mean, back when I was in manufacturing, we learned this mantra of, um, SQDC: safety, quality, delivery and cost. There are huge opportunities, sadly, in all four areas within Healthcare.17:49min admitting and acknowledging what we don't know and that we should always be learners to learn how to get better at solving problems in an experimental way. Right? So, instead of just preaching what many would call PDCA, or, I prefer the language of Plan, Do, Study, Adjust, or PDSA, as a lot of people would say in Healthcare. You've got to model that behaviour first hand. 31:48min When you change the system, you can change performance. Don't ask people to try harder and be more careful within their existing system. We have to work together to change the system.34:04min Let's start the conversation talking about the current condition, the performance gap between where we are and the ideal condition. Let's talk about why that performance or lack thereof, is important, and then let's understand the current state and the causes. Right? So, we build support along the way.

You always want to look back on your life and say that your path from point A to point B was linear. It almost never really is. Today’s guest on the Unlearn Podcast is Mark Graban, author of Lean Hospitals and Measures of Success, who started out as an engineer for General Motors. It’s been quite a journey. Today, he and Barry O’Reilly will be talking about how that kind of a shift can happen, and what he learned and unlearned along the way. Clashes of Culture You might not think that the worlds of manufacturing and healthcare have much in common, but having worked in both, Mark thinks they have plenty in common - especially when it comes to culture. People aren’t machines, but the culture of many hospitals and clinics echoes what you’ll find on the factory floor - especially in institutions that haven’t done much to modernize the way they manage and lead people. Barry points out that there are similarities in tech as well: every industry thinks it’s unique, but it’s all human systems and people working together to drive outcomes.  Unlearning Your Whole Career When you’ve invested years or decades into a career, you often feel like you need to stay in it. Having made a major career transition, Mark knows that doing so allows you to bring fresh insight into an organization that may not be expecting it, and when you’re in a wholly new environment, you’re in a better position to avoid the curse of expertise. Looking back, Mark wishes he’d unlearned top-down style management earlier - the benefits of engaging people in change are so many and so valuable.   Seeking and Finding Clarity Before you start optimizing for, or worse, applying a solution, you have to be exceptionally clear on what the problem really is, and what outcome you want to achieve. Mark and Barry discuss the ways this can manifest in different kinds of organizations, and the framework for problem-solving that Mark uses to help healthcare providers make changes to their operations with input from people working at all levels. The process is one that listeners will be familiar with: unlearn, re-learn and breakthrough!   The Courage to Change It’s easy to tell when something isn’t right, but it's harder to create a moment where people are open to truly unlearning and making changes. Mark notes that looking outside of your area of expertise takes a fair amount of courage - but many people are highly skilled in their specific profession and not in the other areas of running a business or managing a team. This is often problematic because when we’re faced with things we don’t know, or feel scared and threatened - our higher-order brains shut down. There is never going to be a ‘perfect’ time for a major change, so you might as well just get started.  What are the Top 3 Reasons to Do This? Management from the top-down usually doesn’t usually provide the results companies are looking for. It’s much more effective to lead as if you had no authority - by seeking input and finding out what makes people tick, and why they think the way they do. Something that Mark had to unlearn over the course of his career was that you can’t just tell people what to do, even if you’re wildly excited about helping them. Change has to be based on feedback and engagement with the people it affects. Mark has some insights on what makes people more open and receptive to change and leaves us with the thought that it’s okay to struggle with change. Today's post is an addendum from Saturday's post on the apparent death of a health system Lean program: "Why Would a New Healthcare CEO Kill a Lean Program?"If you read that post, there's an interesting comment from a Presence Health employee that doesn't shed too much more light on this: "From the standpoint of a current Presence Health Employee~ my opinion is that the RIEs failed because they were aggressively site specific. At a time when PresenceHealth is "Becoming One" I do not believe the RIEs took the entire ministry into consideration when planning events. Decisions and "experiments" where being made and conducted through a site specific lens, and this was often in contradiction to the spirit of the new organization." As I commented in response, I'm not sure why a goal of common processes or a health system that's actually a system would be a reason to stop with Lean. Unless hopefully they are really just re-booting the effort instead of killing it altogether. As I alluded to in the post Saturday, there was a sad story about ahospital that I featured in the 2008 1st edition of Lean Hospitals that basically killed their promising program when a new CEO was hired from the outside. There's another approach that goes underappreciated, I think -- the "model line" or "model cell" approach. It has nothing to do with fashion models. I don't like the term "cell" because it might make people think of a prison or a gulag. Then again, the word "line" might make people think of "assembly line medicine" in a negative way. Maybe "model area" is a better term? The implication is that assembly lines and factories are cold, rigid, uncaring places that focus on ruthless efficiency and making the numbers at the expense of safety and quality. Do a Google search for the term and the implications of "assembly line medicine" are very negative.
