My latest cool friend is the incredible Kiran Deol, and she is one of the most intimidating people I've ever met. A Harvard-educated comedian/writer/actor/multi-hyphenate, Kiran seems to be living in a world with 28 hour days. Where else could she find the time to do so many incredible things? You may have seen her, among countless projects, on shows like Sunnyside, her comedy special Joy Suck (available on Amazon and Tubi), or in the upcoming Didn't Die, a Sundance-Darling of a film that's been being viewed as a star-making vehicle for Kiran. Honestly, I could write about all the incredible things she's had a hand in, but I'm just going to suggest you listen to the episode, because we had a lot of fun, and we cover A LOT of ground. So enjoy, please follow Kiran on IG (@ShitFromKiran), and GO SEE "DIDN'T DIE" IF YOU CAN! And, please head on over to Patreon.com/jeffmay to enjoy early episodes with uncensored comments, PLUS exclusive content in this episode that we recorded JUST FOR PATRONS!
Send a textIn Part Two of “Love Hurts: Leadership, Quality, and the Future of Hospice & Palliative Care,” leaders from the GeriPal podcast and TCNtalks / Anatomy of Leadership, continue a thoughtful discussion on the most pressing issues facing hospice and palliative care today. The conversation explores how waste, fraud, and abuse in isolated cases can tarnish the reputation of an entire field—even when the majority of providers deliver extraordinary care. The panel dives into the evolving landscape of ownership models, from nonprofit and faith-based organizations to private equity–backed providers, raising an important question: does ownership affect quality, or does leadership and accountability matter more? The discussion also examines structural challenges within the healthcare system, including flawed quality reporting tools like Medicare Compare and the unintended consequences of free-market dynamics in healthcare. With over 300 hospices sometimes operating within a single county, leaders highlight the need for transparency, meaningful quality metrics, and thoughtful regulation such as Certificate of Need laws to ensure resources are distributed responsibly and patients receive the best possible care. Finally, the conversation pivots to leadership—arguably the most important ingredient in shaping the future of hospice and palliative care. From the principle of “Go See for Yourself” (Gemba Leadership) to the importance of servant leadership, focus, curiosity, and interdisciplinary collaboration, the panel shares powerful lessons for healthcare leaders navigating complex systems while staying grounded in mission-driven care for patients and families.Key TakeawaysFraud and bad actors can damage the reputation of the entire hospice field. Ownership models matter less than maintaining high-quality patient care. Healthcare markets lack true transparency and informed consumer choice. Many hospices still lack meaningful public quality ratings. Effective leaders stay close to the mission and frontline care. In this episode of The Disrupted Podcast, Scott Middleton returns from the JP Morgan healthcare conference with a blunt takeaway: the future of care is not a magic pill, another telehealth platform, or a clever financial structure — it's showing up. Scott breaks down why healthcare has become unnecessarily complicated, how fee-for-service incentives distort decision-making, and why “easy-entry” models won't hold up long-term.He makes the case that Your Health's home-based care model is hard to replicate because it requires operational excellence—routing, scheduling, team coordination, and intentional touchpoints. Scott also challenges internal culture issues: finger-pointing, poor communication, inefficient scheduling, and employees misunderstanding the mission. The solution is both simple and demanding: build systems that make weekly in-person encounters possible for high-risk patients and hold the line on execution. www.YourHealth.Org

It's not every day that you get a brand new attraction in the Columbia River Gorge. On this week's episode of Peak Northwest, we head out to the new Mitchell Point Tunnel, open to pedestrians and cyclists only, found on the Historic Columbia River Highway State Trail west of Hood River. The 665-foot tunnel, which features rock windows that open to spectacular views of the Columbia Gorge, is one of the most impressive human-made attractions in the Columbia Gorge. It will be a major part of the 73-mile bike trail when that project is finally completed. Here are some highlights from this week's show: What is the Mitchell Point tunnel? The tunnel is a recreation of a historic tunnel by the same name. How does this fit in to the larger Historic Columbia River Highway State Trail? How to get to the tunnel, and what else to do while you're there. Subscribe to Peak Northwest on Apple Podcasts, Google Podcasts, Spotify or anywhere you listen to podcasts. Trey reviews Ryan Coogler's Sinners: a $100-million genre-bending vampire western horror musical set in 1930s Mississippi. It's Coogler's boldest, weirdest, and most ambitious film to date, blending myth, legacy, identity, and the cost of assimilation into a chaotic cinematic experiment. Trey unpacks the film's anti-genre intentions, its Faustian themes, its wild tonal swings, and the standout performances from Michael B. Jordan and breakout star Miles Caton. Is it a masterpiece? A mess? Both? This is a conflicted breakdown of a movie that might not work...but absolutely demands to be seen.(00:00) Ryan Coogler(01:26) Non-Major Spoiler Review(02:03) What Works(08:24) What Struggles(12:49) The Extra Credits of Sinners(14:56) Final ThoughtsThe Extra Credits YouTube ChannelBecome a member of The Extra Credits+ on Patreon hereHow to link Patreon to Spotify and AppleLetterboxd: The Extra CreditsTikTok: The Extra CreditsReddit: r/TheExtraCreditsInstagram: @theextracreditsTwitter: @theextracreditsSend requests, questions, and thoughts to our email: extracreditspod@gmail.com
