    Kalaki Clarke, MD is a Board-Certified Family Physician who provides competent and compassionate medical care for individuals experiencing homelessness on Skid Row in Downtown, Los Angeles. In 2005 she earned her Medical Degree from Creighton University in Omaha, Nebraska and after a non-traditional medical journey, in 2017 she completed her Family Medicine Residency at the University of California, Irvine. It was her personal experience of receiving a late diagnosis of Attention Deficit Hyperactivity Disorder (ADHD), Inattentive Type at age 35 (in 2015) which helped shape her passion as a mental health and ADHD advocate. Dr. Clarke serves as an Administrator of Physicians with ADHD, a private online group of more than 800 members.     Life Planning with Dr.Weili Gray

    Meet today's guest, Dr. Weili Gray, a board-certified sleep and lifestyle medicine physician who practices in the Northeast. She is also the founder of Dare to Dream Physician, where she offers life planning and hosts a weekly podcast. In this episode, Dr. Weili highlights the idea of knowing your "why?", which she describes as "Life Planning", to identify the true drive behind most of our daily pursuits in life, and to achieve the dream life of fulfillment and satisfaction. Notes Although externally it seemed that things were perfect for Dr. Weili but she didn't feel that way especially with changes at her workplace following the pandemic. She signed up for a Life Planning class out of curiosity and had an awe-inspiring experience. This was how she got into Life-planning. The clarity from the experience helped her get over those feelings she had earlier on. Although a lot of people describe their reasons for work as their kids or financial security, it is important to keep asking why. The more you ask, the deeper and more heartfelt your answers are, and the more warmth you feel in your heart. For Dr. Weili she has always loved connecting with patients but over time, with the demands of medical school and other aspects of her work, she started to lose track of her 'why?'. She had to remind herself not to get bogged down chasing perfection in those other aspects but to draw energy from the areas of her work that she enjoyed. "I think it's fully possible to be an engaged physician and follow my ‘why?'" About Life-planning: Life-planning can be formal during sessions but may also include daily practices and questions. There are 3 questions used in Life-planning;Firstly, if you were financially secure, what would your life look like? This question aims at uncovering the 4 main resources people have including money, time, energy, and talent. Once you figure out your 'why?', the next step is to start to align those 4 resources with it. While a lot of people focus so much on the limited nature of money, they forget that all the other resources are also limited and need to be harnessed efficiently. The second question: “if your doctor told you that you only had 5-10years to live, what would you do in the time you had left to live? How would this change how you prioritize things in your life?” The third question: if your doctor told you that you only had one day to live, what feelings arise as you confront your mortality? Here you ask yourself, 'what did I miss? who did I not get to be? And what did I not get to do?' The goal of these questions is to give deep insight from reflection Rules of thumb in facilitating people to get their dream life:The first step is that they have to know what their dream life is. This is achieved through the reflective exercises in Life Planning and they will feel the energy surge driving them to go for it. The next step is that to go for it, they have to step out of their comfort zone, which would make them different from other people around them. Dr. Weili has had to step out of her comfort zone to start running, motivated by the clarity she has about being in shape. Although it was out of her comfort zone, she has started to see the benefits in her personal life and family interactions. Circadian rhythms are daily rhythms on how the body functions, and there are other different kinds of biological rhythms. The circadian rhythms basically include everything involved with bodily functions like blood pressure, body temperature, hormone, glucose levels, heart rate; which fluctuate throughout the day. These internal rhythms interact with daily behaviors of when and how much we sleep, eat, and exercise, as well as mood, cognitive ability, and tiredness. The circadian system works on its own and coordinates with the environment to help the body be in the right state at each time. It mostly uses light and food as cues to determine the time of day. While light is linked with behaviors like sleep-wake or hunger, food relates with other clocks in the body using nutrients as cues. The app was designed to understand when people eat, what types of food they eat, sleep, and exercise. It can also be used to set goals for eating patterns and to monitor the goals. The 3 layers are the oil layer, the aqueous layer, and the mucin layer. Before getting to a professional, it is advisable to use warm compresses twice a day for about 15-20 minutes. This helps loosen up the oil so it can cover the tear film more effectively. Over-the-counter preservative-free artificial tears can also be used up to 4 times a day using vials. Using Omega-3 fatty acids supplements may or may not be beneficial but should be used with a physician's guidance especially if being used alongside on other medication. Humidifiers in the house are also very helpful. If after this, you're still having symptoms like foreign body sensation, light sensitivity, or blurred vision, it is necessary to see your doctor. The treatment modalities for the two forms of dry eyes also differ. For dry eyes caused by inadequate tears due to other medical conditions, prescription eye drops can help increase tear production but only take effect after some time. Another method is to put plugs in the tear drainage system, called Puncta, to keep the tears on the eyes for longer. In cases where the plugs keep falling out especially since the patient would not know, it may be necessary to cauterize the puncta. For more severe cases, blood can be used to create Serum Tears for the eyes, as blood also contains proteins that make it a suitable alternative. If the oil layer is the reason for the dryness, oral drugs can be used to improve the quality of the oils produced by glands. Heat or intense pulsed light is also a technique used to clear out the oil glands and improve the oil layer. Prolonged computer use can also cause dry eyes as well as other activities that require prolonged staring like reading or driving for long hours which reduce the rate of blinking. Risk factors for dry eyes include being female, older ages, hormonal changes, and prolonged staring. The 20-20-20 Rule: This is recommended for people who stare for long hours for reasons previously highlighted. Every 20 minutes, look up and away about 20feet, for 20 seconds, and remember to blink. Other drugs that contribute to dry eyes include antihistamines, diuretics, and certain glaucoma drugs, and these should be reviewed with a doctor. Notes Right from medical school, Dr. Funke realized that getting enough sleep helped her function better. While practicing Pediatric Pulmonology, a lot of complaints revealed how poor sleep due to breathing issues affected daytime function. These experiences encouraged her to delve deeper into Sleep Medicine Sleep is not just a cure for sleepiness. Sleep has a cleansing effect on the brain as it works through the Glymphatic system to remove protein build-up from the surface of the brain; some of these proteins have been associated with Alzheimer's disease. It also helps with metabolism, heart function, mood regulation, cognition, memory, and critical decision making. "When children are not sleeping, the parents are not sleeping" Common sleeping disorders in families include insomnia, poor sleep habits, Obstructive sleep apnea, Shift work sleep disorder, and Restless leg syndrome. Shift work sleep disorder is different from other sleep disorders because it occurs due to work schedule, and is associated with at least 3 months of daytime impairment that have no other medical explanation. It results in insomnia, excessive daytime sleepiness, and generally reduced total sleep time. How shift workers can optimize their sleep: Prioritize sleep by aiming for 7-9 hours of sleep, understanding that you may falter from time to time. Before a night shift, take a nap. At the beginning of the shift, expose yourself to bright light or you can have caffeine. Getting to the end of the shift, you would have to reduce exposure to bright lights; sunglasses are encouraged. When the shift is over, don't run errands, go straight home and sleep in a room with low lighting. The use of melatonin is not discouraged, however, it should not be a replacement for good sleeping habits. It can be used in small doses at the end of the night shift before you sleep.
