He then completed a Family Medicine Residency and a one-year fellowship focusing on Obstetrics at the Roanoke Memorial Hospital in Roanoke, Virginia. He has practiced Family Medicine in North Central Indiana for almost 30 years, initially as an employee of the local hospital. About 10 years ago he discovered he could practice under a professional services agreement with the hospital system, so he incorporated and began working as a mini-corporation. That opened up many opportunities for him resulting in more autonomy and a higher income. While in practice he completed a Master's Degree in Public Health at the Medical College of Wisconsin. He began doing side jobs while in practice as a hospital utilization management physician advisor and a medical director for a family planning clinic and an occupational medicine clinic. He worked as the VP for Medical Affairs and then full-time as the Chief Medical Officer for a 300-bed independent nonprofit hospital outside of Chicago Illinois, leaving clinical medicine completely for 4 years. He now works part-time as a co-owner and Medical Director for an urgent care network. She is the mother of two sons and step-mother to five daughters. She currently has 10 grandchildren and 2 great grandsons. She completed medical school in 2003 and Family Medicine Residency in 2005 - both from McMaster University. Prior to her medical career Karen worked to write curriculum and develop post-secondary programming at Six Nations Polytechnic, an Indigenous led post-secondary institution in her home community.  Karen's passion is to see Traditional Indigenous Knowledge return to the centre of life for Indigenous people across Canada for the purpose of invoking healing. Dr. David John is a distinguished Trinidadian healthcare professional based in Brooklyn, NY. He is currently serving as Director of Occupational Health and HIV Services at Gotham Health. He has been recognized for his outstanding achievements in Adult Preventive Medicine, contributing to defining the overall business strategy and direction of the organization's eight main centers of operations. Dr. David John is a member of the American Academy of Family Physicians and has demonstrated leadership and inspiration to his medical colleagues throughout his career. His passion for providing the best medical care possible has led him to assist in the implementation of population health programs, including mental health and substance abuse screenings. After arriving in New York in 1980, Dr. John completed his residency at Catholic Medical Center and has been rated an average of 5.0 stars by his patients. He obtained his Bachelor of Science from the City University of New York and earned his Medical Degree from Weill Medical College of Cornell University. He then completed an Internal Medicine Internship in Northshore University Long Island and a Family Medicine Residency in Brooklyn. Dr. John is board-certified in Family Medicine by the American Board of Family Physicians and is also a certified physician in his home country with the Medical Board of Trinidad and Tobago. After working in New York for a few years, Dr. John decided to return to Trinidad in 1994 to contribute to his homeland. During his earlier years in Trinidad, his involvement as a faculty member in the Lutheran US-Caribbean HIV/AIDS Twinning Initiative, contributed to the success of this program. Dr. John volunteered as a medical professional at the Cyril Ross Nursery, a home and treatment center for HIV-infected babies who were neglected and abandoned by their parents. His focus at the Nursery was pain control and other comfort care, and he worked alongside fellow Trinidadian and Ms. Universe of 1998, Wendy Fitzwilliam, to bring anti-retroviral treatment to the children. Dr. John's dedication to providing care to HIV-infected children led him to become a board member of the Caribbean Women's Health Association, in 2009, working tirelessly to advocate, address: comprehensive education, healthcare disparities, improve healthcare access and promote better resources for women's health, in the Caribbean region, while continuing to assist patients with HIV screening, housing, and other social services. She previously served as a PCSS-certified to trains providers to become Buprenorphine waivered (before the X waiver requirement was removed). She continued her passion for looking at patient care from a 360perspective at Kansas City University where she completed the DO/MBA dual degree program. In addition, she was selected for the undergraduate OMM Fellowship at KCU where she able to pursue multiple passions including teaching and dancing for the Major Arena Soccer League team, the Kansas City Comets. She completed her Family Medicine residency at University of Michigan Health-West while participating in both the Osteopathic Medicine and Sports Medicine tracks. EP402: What Physicians Trying to Clinically Integrate Care in the Real World Need to Know, With Amy Scanlan, MD

So, let me just cut to the chase here with very little preamble, and all of this is a setup to the interview that follows, although it is not really what the interview that follows is all about. A mentor of mine used to say, you can't legislate the heart. Let me also suggest you can't give someone in finance financial incentives and then expect them to not prioritize financial incentives. It stands to reason that if the healthcare industry is found to be quite attractive to those who are money focused, then do I need to say this? The money focused amongst us will, of course, do the whatever to the extent that they can make money. They aren't gonna be throwing their backs into quality or cost effectiveness or taking care of patients. They are throwing their backs into making money. Is anyone shocked? Now, don't get me wrong; I'm not a Pollyanna. And in this country, in order to run a healthcare business, you have to make money; otherwise, you'll go out of business. So, do well by doing good and all of that. But how much money is too much money? This is an important line to figure out because that's where you are doing well but you've stopped doing good—you've tipped into financial toxicity. You are taking more than the good you are doing, and the net positive becomes a net negative. But complicating fact of current life, it's becoming increasingly obvious that in order to stand up a practice that can take advantage of value-based care payments—payments where primary care docs mainly at this time can get paid more and likely more fairly to care for patients well—you need a lot of infrastructure. You need data, you need tech, you need a team. Translation: You need money, maybe a lot of money, to invest in all of this. And let me ask you this: Who has a lot of money in this country? Here's the point of everything I just said: These are the external realities that hit anyone trying to do right by patients from every direction. But on the other hand (or maybe different fingers on the same hand), as Amy Scanlan, MD, says in this healthcare podcast, physicians are the backbone of this system. Dr. Scanlan talks in the interview today about the opportunity, and maybe the responsibility, that physicians have here for patients; but also the Eric Reinhart article comes up again about rampant physician moral injury (unpaywalled link with my compliments). Right now might be a great time to read something from Denver Sallee, MD. He wrote to me the other day. He wrote, “Like many physicians, I did not have much understanding of the business side of medicine, as I mistakenly thought as long as I helped take great care of patients that I was doing my job. More recently, it became apparent to me that by ceding the management of medicine to nonclinical administrators and to companies interested primarily in value extraction for the benefit of shareholders that I needed more education in order to truly help patients.” Today as aforementioned, I'm talking with Amy Scanlan, MD, who is chief medical officer of the clinically integrated network (CIN) that is the new joint venture between Intermountain Health and UCHealth in Colorado. We talk about what it's like to be in the kind of messy middle of transformation to integrated care in a clinically integrated network, trying to figure out how to help physician practices and the CIN itself navigate the external environment in a way that empowers different kinds of practices at different points in their transformation journey that empowers physicians to be in charge, and considering clinical and financial outcomes (ie, the business of healthcare). Dr. Scanlan brings up four main factors to consider when plotting strategy from here to there: 1. Give practices the tools that they need to succeed—not what you think they need but what you've discerned they actually need because you've listened to them. 2. Many times, these tools will consist of some combination of data, tech, and also offering the team behind the scenes to help doctors and other clinicians help patients through what Dr. Scanlan calls the “in-between spaces”—the times between appointments. 3. Medical culture really has to change, and in two ways: doctors learning how to be part of and/or leading functional teams and building functional teams. Because there are teams, and then there are teams. Well-functioning teams can produce great results. Nonfunctioning teams, however, are, as Dr. Scanlan puts it, just a series of handoffs. And don't forget, handoffs are the most dangerous times for patients. The DNA of team-based care—real team-based care—for better or worse, are the relationships between team members, between physicians who work together, between doctors and patients, between clinicians and clinicians. So, fostering relationships, creating opportunities to collaborate and talk, is not to be underestimated. How do you re-create the doctors' lounge in 2023? 4. Getting out from underneath the long shadow of fee-for-service incentives, specifically the paradigm that only patients who get mindshare are the ones in the exam room. Value-based care, integrated care is as much contemplating the patients who don't show up as the ones who do. She obtained her medical degree from Case Western Reserve University in Cleveland, where she received the Kiwala Award for Research in Family Medicine. Eleanor Tanno, MD is a Family Physician, Speaker, and Advance Directive Consultant. She completed her medical degree at the U. of Maryland School of Medicine, and her Family Medicine Residency at the Virginia Commonwealth University. She is a full-time primary care family physician and partner in her private practice. She started her side venture, Advance Directive MD, to help people create meaningful advance directives. She does this by providing Workshops, Webinars, Public Speaking Events, and Medical Lectures. This interest developed after meeting many families in the ICU who had never had end-of-life conversations with their family members. And she discovered that most patients who completed advance directives were being shown how to do so by attorneys. However, attorneys and their staffs were not in the best position to explain the options from a medical perspective. So, Dr. Tanno created a business to educate the public about advance directives, and teach individual clients how to properly describe their wishes in these critical documents. About Our Guest Dr. James Wilcox is originally from Indiana. He earned his MD from Indiana University in 2014, and completed his Family Medicine Residency at IU Health Ball Memorial Hospital in 2017. He then completed his Sports Medicine Fellowship at Western Michigan University in 2018. He first served in the rural community of Austin, Indiana, practicing the breadth of family medicine: inpatient, outpatient primary care, emergency medicine, sports medicine, and substance use treatment. He recently relocated to Indianapolis in 2020, and he is now working at Eskenazi Health in outpatient primary care. In 2021, he accepted a grant-funded position at Indiana University School of Medicine teaching Point of Care Ultrasound (POCUS) and coordinating POCUS integration into the 4-year curriculum as the HRSA PRIME POCUS Thread Director, and Indiana University School of Medicine Assistant POCUS Director. This week on SA Voices From the Field we interviewed Dr. Jessica Higgs about some of the ramifications of affirming healthcare for students following the overturn of Roe v. Wade. Jessica Higgs is a Family Medicine physician with a certificate of additional qualification in Sports Medicine. She completed medical school at Southern Illinois University School of Medicine and Family Medicine Residency at the University of Illinois College of Medicine in Peoria. She completed her Sports Medicine fellowship at the University of Pittsburgh. Since 2008 Higgs has been the director of Bradley University Health Services, a private university of about 5000 students, and team physician at Bradley since 2011. Dr. Higgs is currently serving as President of the American College Health Association after serving 4 years as a member at large to the board and two years as vice-president. In this 4th year medical student (MS4) series, we dive into the field of Family Medicine. Our guest, Kendall Frisoli, shares what drew her to this specialty and what keeps her sane through the rigors of medical training. KENDALL is a musician passionate about connecting to and healing with her audience. Originally from Los Angeles, KENDALL has been training to become a physician in Chicago. She is returning home to California to pursue her Family Medicine Residency journey in July 2022 at Long Beach Memorial. Since childhood, music has kept her centered. It continued to keep her grounded throughout her experiences as a collegiate D1 athlete, LGBTQ woman, and medical student. It is her expressive outlet and treatment of choice. Music is medicine, and KENDALL integrates this artistic tool within her medical practice. Her music is vulnerable, relatable, and most importantly - real. KENDALL's songwriting is sparked by human relationships, whether it be with romantic interests, friends, family, patients, or herself. KENDALL's first single release, “Slow My Mind Down,” is one of many songs she wrote about a girl. This piece evokes the range of emotions we feel in our relationships. And it is a perfect example of how KENDALL's mind can race from 0 to 100 after experiencing a meaningful connection. Louder Now Episode #121 **** Hello friends, welcome back to the podcast. This was the first time I have talked with Doctors on this show and it was wonderful. We talk personal stories of overcoming trauma and we get into forgiveness, faith and even some talk about psychedelics. Also, much more . I loved having them on the podcast and hope to do part 2 in the future. BIO: Drs. May and Tim Hindmarsh  Drs. Tim and May Hindmarsh are a dynamic dual physician couple, having a blast doing life together. They are also the hosts of BS FREE MD Podcast. Both are Board Certified Family Practice physicians, currently practicing Urgent Care. Both have over 28 years of diverse experience in family practice, hospital medicine, obstetrics and urgent care.  Born and raised in Canada, they met in Medical School then graduated from University of Saskatchewan in Saskatoon, SK in 1992 and married 4 days later.  Both completed Family Medicine Residency at the University of Alberta, Edmonton in 1994 and soon after immigrated to Oregon where they have been practicing ever since. They worked under Samaritan Health Services in the Willamette Valley, OR for 25 years. While there, Tim accrued almost 20 years experience in Utilization Management. He has been on numerous committees and boards including Samaritan Health Board of Directors, (a 5 hospital multispecialty integrated health network). He has served on Pharmacy and Therapeutics, Credentialing, Management Council , Physicians Board and was Medical Director for Twin Oaks Care Center.  In 2021 they ventured into the private world of Medicine, helping establish Ridgeway Health, a local private Urgent Care Clinic. Both have been frontline physicians during the Covid Pandemic, working almost exclusively in Covid Clinics 2020-2022.  They both believe in serving the community you are in. While rural medicine has been their main mission field, they have served internationally. May also was Medical Director for StarFish Medical Ministries from 2009-2012.  In 2021 they launched BS FREE MD Podcast They are passionate and love laughing at their medical experiences as well as getting into serious discussions on the issues in healthcare today. Our guest for this episode Dr. Enaka Yembe, in an interview we conducted some time ago, is here to provide you with all the practical tips you can employ to make your application stand out. Having both been through the application process herself and also having served as a faculty member, she offers a unique perspective to IMG's like you! Board certified in the field of family medicine, Cameroonian-born Dr. Enaka Yembe completed a Bachelor (Honors) Degree in Biochemistry in Nigeria and medical school in The University of Milan, Italy. She did an internship in Transitional Medicine in Howard University Hospital in Washington DC and then her Family Medicine Residency training at LSU Conway Hospital in Monroe, Louisiana and was nominated Resident of the Year in 2005. Here is some of the advice she offers for the residency application and interview process: Focus on your strengths and use them to show that you are a capable candidate. Try not to come across as if you are asking for help or begging for a position. Be confident! Don't overthink the process, but be very deliberate. Know your CV! Be prepared to answer any possible questions. Do research on the program you're interviewing into, in order to anticipate what kinds of questions they may ask. If you don't know the answer, don't be afraid to admit it. Offer to get back to the interviewer when you find the correct response. Your personal statement should be a brief biography, where mentioning relevant clinical experience in the US is an asset. Do not include too many personal stories and anecdotes. The personal statement should be concise. Review your personal statement professionally for grammar and spelling errors. For your residency, consider rural areas. They can offer you opportunities that may not be available in more urban ones. Finally, here are some inspirational tidbits to guide you along your journey: Do not be discouraged. Anything can be accomplished with persistence. Do not give yourself a plan B, focus on your goal and do the work to achieve it. Be committed to preparing for the USMLE exam. Who is not stressed out during the pandemic? We will be talking all about it with Dr. Megan Melo. Dr. Melo's philosophy on burnout is that we have to attend to both our bodies AND our brains. This is a powerful way for people to start their journey of healing and intentional self-care. We talk all about "The Stress Cycle," as described in Burnout: The Secret to Unlocking the Stress Cycle by Emily Nagoski Ph.D., and Amelia Nagoski DMA. It's a simple, intentional technique of using the body to come out of stress and come into relaxation mode through brief bits of exercise, deep breathing, laughter, crying, etc. Dr. Melo attended the University of Washington School of Medicine and completed Family Medicine Residency at Group Health in Seattle, WA, and then later joined the Family Medicine Residency Faculty. Dr. Melo is a Certified Life and Weight Loss Coach, trained at the Life Coach School, as well as a Daring Way Facilitator, and in 2021 became Board-Certified in Obesity Medicine. Dr. Paul Mabry is a Family Medicine Specialist with over 35 years of experience in the medical field. He is also a Gulf War combat veteran with deployments to Iraq and Afghanistan where he served in both the United States Navy and Army over a career of 26 years. He graduated from University Of Texas Medical Branch at Galveston medical school in 1986. Paul joined the Navy when he was 19 and was trained as a Russian Linguist. He then spent 3 years in Japan flying on EC-121s/P3's and crewing submarines to intercept communications along the Russian coast. He then worked at the Manned Spacecraft Center in Houston for 2 years to support the Joint Space Mission. Following this, Paul went back to college, finishing with 3 degrees (BA in Spanish Literature, BS in Biology and a BS in Medical Technology). He then completed Medical School rejoined the Navy and completed an Internship at Bethesda Naval Hospital. After 9 years as a doctor in the Navy including a deployment to a field hospital in the Saudi Desert during the First Gulf War he returned to civilian practice and completed a Family Medicine Residency at St. Clare's Hospital in Schenectady, NY. In 2001 Dr. Mabry joined the New York Army National Guard and did two deployments to Iraq and one deployment to Afghanistan. He then switched to the active duty Army and retired with 26 years of total service. In the Summer of 2018 Dr. Mabry decided to return to practising medicine at the Texoma Family Medicine Residency in Texas. Paul advocates a low to zero carb lifestyle for those who need to heal from Insulin Resistance and Metabolic syndrome. He has designed and implemented a low carb diet program called “The U-Turn Diet”.
