Medicine is indeed a noble profession, one that transcends the boundaries of science and reaches into the depths of human compassion. Physicians are not merely practitioners of medicine; they are healers, caregivers, and advocates for the well-being of their patients.  It requires years of intense study, training, and examination. But becoming a doctor is just the start. After medical school and residency, physicians continue learning and developing throughout their careers, exploring new paths and discovering more about themselves along the way.   Doctors need to be strong and flexible to do well in a healthcare world that's always changing and often confusing. They have to face problems with bravery, keep going even when things don't work out, ask for help when they need it, and understand that changing jobs is normal in healthcare. This helps them become even better at what they do. Therefore, doctors must possess not only clinical expertise but also resilience, adaptability, and a willingness to embrace change.    Amy Vertrees MD is a General Surgeon, Surgeon Coach, Author of "Become the BOSS MD", Keynote speaker, and Founder of BOSS Business of Surgery Series. With a deep understanding of the challenges physicians face beyond the operating room, Dr. Amy is dedicated to empowering her peers to take control of their careers and thrive. Drawing from her own journey and years of expertise, she understands that residency training only scratches the surface of what it takes to succeed in the medical field. It became her mission to guide fellow physicians toward becoming the BOSS MD they aspire to be. Her approach is not just about surviving in medicine but thriving—with Dr. Amy's guidance, physicians can regain autonomy, find balance, and rediscover the joy in their profession.   In this episode, I will share with you an interview I had with the Business of Surgery Series podcast with Doctor Amy Vertrees. Some of the key themes that emerged from our discussion were the importance of adaptability and resilience in the face of unexpected changes, the emotional toll of job loss and career transitions in medicine, and the art of physician contract negotiation. We also talked about the intricacies of navigating career transitions in the medical field, drawing from my own experiences and insights gained over the years. Welcome to a brand new episode of the ¿Quién Tú Eres? podcast, where we explore the conflict we often face between "professionalism" & being our authentic selves. This week's guest is Angel Rosario. Angel Rosario Jr. is a native New Yorker from Harlem/Washington Heights who explores his intersectional identities as a gay, Afro-Dominican, cis-gender doctor who is completing his training as a General Surgeon. Coming from a low-income background, Angel explores race, health, power, privilege, and the socioeconomic disparities that result through this lens, articulating them through his memoir writing and poetry, health disparities research, and mentorship. Also known as ‘el dotoi con los rolos', he further strives to challenge narratives of professionalism, notions of ‘success', and criticisms of healing by donning his hair with rolos, a symbol of his community and a homage to the Dominican, Puerto Rican, and non-Latina Black women who raised him. He believes that a diverse, unapologetically authentic, and culturally humble healthcare workforce, working alongside community members, is crucial to ending race-based medicine. He is a first-gen college graduate of the University at Buffalo, received his Master's in Public Health degree from Harvard University as a Zuckerman Fellow, and obtained his Medical Degree from the University of California San Francisco, where he focused on caring for Urban Underserved communities, receiving the 2020 Gold-Headed Cane award honoring the graduate student who exemplifies the qualities of a ‘true physician'. In this week's episode, Angel tells us his experience growing up in the Bronx being tested into a gifted private school in the Upper West Side, and navigating the medical industry as a queer black man. Angel is a kind and generous man whose work focuses on reducing racial bias in the medical industry. He hopes to dismantle systems of oppression in his field so that future patients can have a more compassionate medical experience.  Follow Angel on:  Instagram: @angelcurandero  LinkedIn: https://www.linkedin.com/in/angelrosariojrmd2be/  Follow Pabel on: Website: https://plurawl.com/  Instagram: https://www.instagram.com/plurawl/  TikTok: https://www.tiktok.com/@plurawl  LinkedIn: https://www.linkedin.com/in/pabelmartinez/  To book a speaking engagement email hola@plurawl.com Podcast production for this episode was provided by CCST.     We're honored to bring you a conversation with Dr. Jonathan Jaffin, an accomplished General Surgeon, a retired Colonel in the Army Medical Corps, and a Specialist Executive with Deloitte Consulting. Our dialogue circles the exciting advancements in operational medicine with an eye on austere environments, a topic that was the centerpiece of the 2023 Operational Medicine Symposium. We grapple with the shifting landscape of military operations, the emergence of near-peer competitors, and the potential for large-scale ground combat under resource constraints. Dr. Jaffin's insights into the challenges of maintaining readiness and the strains faced by individuals and caregivers are truly enlightening.      Moving forward, we venture into the realm of future warfare and the preparation of surgeons for these unprecedented challenges. We explore the role of telemedicine and the promising Knowledge, Skills, and Abilities (KSA) project, a venture that Dr. Jaffin is no stranger to. His reflections on the KSA project provide a valuable insider's perspective into how training can be tailored to address specific knowledge and skill gaps. We delve into the crucial role of commanders in deployment readiness and the decisive impact of military medicine on these decisions. Dr. Jaffin also opens up about his military career, the unique relationships it has fostered, and the rewarding sense of contribution it offers. This episode provides a riveting glimpse into the evolving landscape of military medicine.   Chapters: (00:00) Advancing Operational Medicine for Austere Environments (05:47) Preparing Surgeons for Future Warfare (15:45) Advantages of a Military Career   Chapter Summaries: (00:00) Advancing Operational Medicine for Austere Environments Dr. Jonathan Jaffin, a general surgeon and retired Colonel in the Army Medical Corps, speaks about his current work with Deloitte Consulting and his focus on social determinants of health. The importance of the 2023 Operational Medicine Symposium is discussed, along with the innovations and opportunities to advance the practice of medicine in austere environments. The impact of a near-peer competitor on operational medicine and the need to prepare for large-scale ground combat operations with limited resources is explored. A panel of military chiefs shares the challenges of maintaining readiness, trauma to individuals and caregivers, and how to make difficult decisions in the face of limited resources.   (05:47) Preparing Surgeons for Future Warfare We discuss the challenges of maintaining surgical skills in an era of sub-specialization, how telemedicine can provide care in far-forward areas, and the Knowledge, Skills, and Abilities (KSA) project to ensure healthcare professionals are ready for deployment. Dr. Jaffin shares his experience with the KSA project, including how to structure training based on gaps in skills and knowledge. We also discuss the role of commanders in deciding who is ready to deploy, and how the medical department can inform their decision.   (15:45) Advantages of a Military Career Dr. Jaffin shares his thoughts on the advantages of pursuing a military career and how it has helped him build close relationships. He reflects on the importance of contributing to the nation and the satisfaction from doing so. We also learn more about his current work with Deloitte Consulting and the role of telemedicine in providing care in remote locations. Take Home Messages: Advancements in operational medicine are crucial, especially for austere environments where resources may be limited. The potential for large-scale ground combat with near-peer competitors presents a significant challenge for military medicine due to resource constraints. Preparing surgeons for future warfare involves addressing skill gaps and ensuring readiness for unexpected situations. Telemedicine can greatly aid medical care in remote locations, from diagnosis to expert consultations. The Knowledge, Skills, and Abilities (KSA) project is key in tailoring training to address specific skill gaps in military healthcare professionals. Commanders play a crucial role in deployment readiness and decisions, and military medicine can greatly inform these decisions. Military medicine's mission extends beyond providing outstanding healthcare, it also includes preparing individuals for deployment. Simulation technology for surgical training has significantly advanced, although there's still room for improvement, especially in simulating the feel of big procedures. A military career can be gratifying, offering unique experiences, relationships, and the satisfaction of serving one's nation. Meetings such as The 2023 Operational Medicine Symposium serve as essential platforms for discussing current issues and innovations in military medicine.    Join us for a captivating discussion with our esteemed guest, Dr. Brian Lein, a retired Army Major General and General Surgeon with a storied Military Medicine career. Dr. Lein currently serves as Assistant Director for Healthcare Administration at the Defense Health Agency (DHA) and has a wealth of experience at the strategic levels of operational medicine. He peels back the layers of the complex world of military healthcare, shedding light on the intricacies that come with ensuring the health and readiness of military staff on and off the battlefield.     In a thought-provoking conversation, we grapple with the ethical dimensions of battlefield medicine, the future implications of large-scale combat operations, and the role of advanced technology in training medics. Dr. Lein opens up about the vital conversations taking place within the Military Medical community regarding the ethical implications of new technologies and the tough decisions that come with limited resources in combat. We also glean insight into the DHA's role as a supportive entity, working to consolidate the efforts of the Army, Navy, Air Force, and Space Force Medicine. Hear all about the DHA's unwavering commitment to maintaining a balance between garrison healthcare and operational medicine and its mission to ensure the readiness of our armed forces.   The conversation was recorded at the 2023 Defense Strategies Institute's Operational Medicine Symposium. Take Home Messages: Military Medicine is a complex and multifaceted field, with challenges ranging from maintaining the health and readiness of military staff to navigating the ethical dimensions of battlefield medicine. Large-scale combat operations could drastically change the nature of battlefield injuries, potentially resulting in more casualties and ethical dilemmas regarding resource allocation. The Defense Health Agency (DHA) plays a crucial role in consolidating the efforts of the Army, Navy, Air Force, and Space Force Medicine, striking a balance between garrison healthcare and operational medicine. Advanced technology and simulation are becoming increasingly important in training military medics, with the potential to use historical data to create virtual reality platforms for training. There is a pressing need to engage in ethical discussions regarding using artificial organs and other life extenders and decisions related to resource allocation in large-scale combat operations. The DHA serves as a synthesis of Army Medicine, Navy Medicine, and Air Force Medicine, providing support to these entities and ensuring the military's readiness. Providing care for the families of military personnel is crucial, as it ensures that Soldiers, Sailors, Airmen, Guardians, and Marines can focus on their duties without worrying about the well-being of their loved ones. The advancement of medical practice in expeditionary environments depends on the latest innovations and opportunities. These advancements require a collaborative effort among military medicine entities. Military Medicine is facing a future where battlefield conditions and medical needs could drastically differ from past conflicts, necessitating adaptation and innovation. There is a growing emphasis on ensuring that military healthcare professionals are prepared for the unique challenges they may face in battlefield conditions, including making difficult decisions under extreme pressure.   Episode Keywords: Military Medicine, Defense Health Agency, Battlefield Medicine, Combat Operations, Advanced Technology, Training Medics, Ethical Implications, New Technologies, Limited Resources, Army, Navy, Air Force, Space Force Medicine, Garrison Healthcare, Operational Medicine, Readiness of Armed Forces Hashtags: #wardocs #military #medicine #podcast #MilMed #MedEd #MilitaryMedicine #BattlefieldEthics #HealthcareInCombat #DefenseHealthAgency #WarDocs #OperationalMedicine #MilitaryHealthcare Honoring the Legacy and Preserving the History of Military Medicine The WarDocs Mission is to honor the legacy, preserve the oral history, and showcase career opportunities, unique expeditionary experiences, and achievements of Military Medicine. Surgeons say a new tool which prioritises a person's ethnicity, along with other factors such as deprivation, time on the waiting list and location, when deciding who gets treatment is a simplistic approach to what is a complex issue. This week's episode features Ken Sample (Capt. USN retired). He spent a large portion of his life in service to others. Ken laughs as he tells how his drive to become a doctor very much came from his dear wife. Ken spent overwhelming time as a General Surgeon for the United States Navy. Some of this time was spent forward deployed and some of his time was stateside in places like Camp Lejeune. Ken also ran his own private practice in between reserve time and deployments. Surgeons like Ken are vital to the warfighting mission. In the Corps when a man is down on the field, we operate under a rule referred to as the “golden hour”. This means, that once self-aide, buddy-aide, and Corpsman-aide are conducted, if the Marine is wounded badly, we need him/her to a trauma unit in less than an hour. If we can accomplish this mission, the rule is that he/she will survive. There are of course exceptions to that rule. People like Ken Sample exist and operate in the gray... without these people, with these skills, our battlefield KIA numbers would not stay as low as they are. It was an honor and a pleasure to sit down with Ken and learn about a different side of war, the side of saving and giving new life to the National Treasures holding the line.
