Leah Houston, MD is a serial entrepreneur, emergency medicine physician, and activist. She is the founder of:HPEC: Humanistic Physician Empowerment Community is a platform physicians can use to own and store their own digital identity and credentials. A secure, self-sovereign identity is the foundation for restoring physician autonomy. (Find out why!) EverCred: The system used by institutions to manage certification data and issue physician credentials that puts physicians in control of the primary source verification process. Dr. Houston joins me on the podcast with Florida attorney Abbigail Webb. Ms. Webb raises an alarm about the loss of physician autonomy and points to the striking differences in professional autonomy between the medical profession and the legal profession. She raises questions about why non-physicians are in control of healthcare institutions and physician practices. She underscores how compromised physicians are in their ability to meet their fiduciary duty to patients because of the obvious conflicts of interest in healthcare systems run by non-physicians. In the U.S., only lawyers are allowed to own law firms—in marked contrast to ACA language (link to glossary below) that prohibits physicians from owning hospitals.She draws additional comparisons between medicine and law:•Legal oversight bodies such as State Bars and State Supreme Courts are made up of attorneys—unlike medicine where regulatory bodies have a variety of competing agendas creating oversight policies for the medical profession. •Law firms are not publicly traded because lawyers are not allowed to be governed by or beholden to the interests of non-lawyer shareholders. Contrast that with the behemoth Fortune 50 healthcare systems driven by a profit motive, not medical professional values.•Lawyers are not subject to non-compete clauses because it is deemed unethical to restrict clients' ability to freely seek legal assistance from whomever they choose. The 70% of physicians who are employed are very often hamstrung in their career decision-making by non-compete clauses.•The legal profession closely safeguards the practice of law, not allowing non-lawyers or paraprofessionals to hang out a legal shingle or use terms like lawyer, attorney, or Esq. Not so in medicine, where consumers and patients are unclear, sometimes misled, about who is a physician.Ms. Webb believes quality of care for patients would be served by physician leadership of healthcare and she is an advocate for the digital solutions that Dr. Houston proposes.What are those solutions? Moving ownership of physician credentials to physicians themselves. This can be accomplished by storing credentials in an immutable, immediately accessible, blockchain wallet. A self-sovereign identity (SSI) means physicians themselves retain control and thus provide the primary source verification of their credentials and decide who has access and for how long. HPEC provides this capacity for physicians to have a wallet containing their credentials (and other digital assets). Dr. Houston's other company, EverCred, provides a mechanism and platform for certifying bodies to issue credentials directly into decentralized identity wallet systems (like HPEC, but others as well—as long as they use rigorous established security standards). These systems are in development with pilot projects underway including a physician training program that is issuing credentials directly into HPEC.The use of blockchain for establishing secure self-sovereign identities (SSI) for physicians opens the door to Dr. Houston's bigger vision of physician autonomy and leadership. It can also help us keep a secure communication between colleagues. In the modern digital world, we’re all accustomed to having our personal information online. From our ID numbers, credit cards, birth certificates, cellphone numbers, to, in the case of a doctor, the MD at the end of your name, whether you completed a residency or board certification, or have a license to practice medicine. When our credentials are in the hands of a third party, we can unknowingly set ourselves up for fraud, waste, and abuse. On this episode of Prosperous Doc, we welcome Dr. Leah Houston, a Locum Tenens doctor who faced the stressful ordeal of having her physician identity stolen and abused. After pursuing litigation to resolve the matter, Dr. Leah Houston asked herself, “How many people is this happening to every day who haven’t left the state? How many people is this happening to who are just working in a hospital, not even knowing that their professional brand and their credentials are being used on patients that they don’t even know about?” (5:52) As a result of this ordeal, Dr. Houston became curious about stolen identity and fraud and began to investigate, discovering (https://sovrin.org/faq/what-is-self-sovereign-identity/).   It isn't often that people walk into hospitals and ask themselves how does this thing work? In fact, the same can be said for those who work in hospitals too. Whether you're a doctor, nurse, phlebotomist, orderly, or patient - how the whole enterprise works is usually a mystery. Also, what protections are there for patients so that they don't have people who are grossly negligent or dangerous caring for them? My guest today is David Leonard, Chief Legal Officer for the Spectrum System. Spectrum is a West Michigan based hospital system that currently comprises 15 hospitals, employed doctor group, clinics, laboratories, and health insurance plan (Priority Health). David has been with Spectrum since 2002 and has helped them navigate the merger and acquisition phases of their growth. Not only is the financial merger of hospitals and organizations difficult but trying to coordinate their governance and culture can be even harder. It is the way hospitals run through a system of hospital boards and medical staff boards that makes their organization complicated and sometimes disjointed. It is a dance they have to do together to deliver the right care to the right patient with the right facilities and equipment. We discuss the credentialing process which is a term used to describe the process of determining whether someone is fit to practice in the hospital. Along with that comes the disciplinary or corrective process to remove those who don't practice medicine well. That is best seen by the story of Dr. Christopher Duntsch from Texas. He was practicing in numerous hospitals in the Dallas area and none of them were willing to remove him until Herculean efforts were performed by some colleagues. Leah Houston, MD had her physician identity used for billing after her employment concluded.  This led to the epiphany that caused her to start HPEC.IO, an organization that is trying to give physician's back control of their identities using blockchain technology.  This could be useful for doctors that want to switch jobs or states without having to have every bit of information independently verified.  We first discuss what blockchain is and then get into how she plans to implement it to not just help us keep our identities under our control, but to create bureaucracy-free virtual physician communities. She is a board-certified Emergency Physician, founded Houston Health in 2013, and has a personalized private practice in Manhattan. While practicing medicine across the US for nearly 10 years she recognized a common problem: uncompensated administrative burdens related to physician employment and credentialing are a leading cause of administrative waste and physician burn out. She began HPEC in 2018 when she realized that Blockchain could solve the obstructive regulatory problems with its distributed ledger technology. HPEC is building a platform that will give every physician a self-sovereign digital identity attached to their credentials in order to create the Decentralized Autonomous Organization of physicians and surgeons.  This will streamline the current antiquated and laborious process of hospital credentialing, save on administrative costs, improve access to care, and give physicians sovereign ownership of their data, professional brand and employment rights. HPEC will also create an opportunity for physicians to communicate more efficiently about policy and practice and in turn improve patient care.  She can be found on her Social Media Handles (linkedin, twitter, Facebook, Instagram) @LeahHoustonMD          @HPECDAO

  Everyone knows that one of your most important assets is your identity. Without that you will have difficulty getting anything accomplished. Most importantly, whoever holds the key to your identity will wield the ultimate power. Thankfully, today's digitized world and the emergence of block chains for public accounting and private security is bringing us to a place where you can store this information in a decentralized space allowing the individual to have the power. My guest, Dr. Leah Houston, is an emergency physician by training who ran into an unfortunate instance where her professional identity was used without her knowledge that put her at significant legal risk. Through that experience she began to wonder if there was a way for physicians to hold onto their identity. Her concept is HPEC which uses the block chain and provides a platform for physicians to control the validation of their credentials in a decentralized way that they can then monetize. Her plan is for physicians to own, operate, and utilize this credentialing service putting them in charge of their identity and communicating in an open and decentralized forum to disseminate ideas and innovations free from outside corporate interests. It's a bold and ambitious plan and we will discuss it in detail during this show.   In the interest of full disclosure, this is a new venture and investment opportunity for physicians. We talk to Dr. Leah Houston MD in this podcast about her passion, which is blockchain technology to help solve the healthcare crisis. This is summarized as a distributed ledger of bundled data to make information and transactions secure.  In this case it is a distributed ledger of your credentials as a doctor, which is a digital identity, that gives us doctors ownership of our right to work.  This will be a PHYSICIAN ONLY community.  It is a form of a healthcare economy OWNED by physicians, giving us a say and right to work.  Therefore, we now own the right of this intellectually property.  It will eventually be a way we can make money off our data.  Basically, it is about giving power back to the doctors through this decentralized technology.  That being said, this needs mass adoption by physicians to be successful.  There are nefarious competitors out there that are NOT doctors. This is coming whether we like it or not.  The question is whether we want to join together and do it ourselves to implement change with a collective physician voice. The goal of Dr. Houston’s company is to first build the credential ledger and then possibly build the cryptocurrency for patients to use with doctors.  It is also give us collective bargaining power using this blockchain technology.  Lastly, the decentralized concept allows physicians to vote on issues how to run the company, on what treatment plans are better, which policies are appropriate for patients, etc. This is a young company and an early investment opportunity.  Full disclosure, I am not an investor at this time and YOU MUST READ EVERYTHING before you decide to join.  They are looking for founding members and then regular members.  There are always risks in any venture.  However, it looks like blockchain companies are coming and so do you want to take that risk and be involved early? Do we want the AMA or Walmart to own this space?  This is a question only you can answer. Its time to use them and put their hard work to our advantage!" Some more information: Businesses of future will not be only about providing value through products or services; they will also be about providing value through the community. Biggest businesses of next decade will also have a powerful tribe behind them enacting change. Products will be sold not only because of the value they offer but because of the tribe they will provide access to once bought. We are doing this by using powerful and secure new technology. This technology is powerful because it first verifies a persons identity and then based on that identity allow them to transact for any goods and services peer to peer and free from third party influence, tax, or broker fee. What you transact depends on what is within your rights based on your identity. For physicians and surgeons, in the example of healthcare, your identity will be attached to the fact that you are a physician or surgeon (MD or a DO) and it will be attached to your residency training and credentials number of years in practice, potentially licenses, whether you have other advanced degrees such as an MBA or MPH etc. A patients identity can also be attached to their unique data, and their medical records that they own.  They like physicians will also have their jobs careers attached to their ID.  Everyone will also have their means of transacting, crypto currency dollars and other assets, like your home and car, all attached to this secure verifiable and un-hackable digital identity.
