Life Changing Moments: Celebrating Doctor's Day 2025

Play Episode Listen Later Mar 31, 2025 51:35

The episode features a roundtable discussion among the coaching team at MD Coaches, designed to address the complex and evolving landscape of modern medicine in honor of Physicians Week. We acknowledge the remarkable advancements in medical innovation and evidence-based practices that have significantly transformed patient care. While these innovations offer unprecedented opportunities for physicians to impact their patients positively, they also bring forth new challenges, including legislative hurdles, ethical dilemmas, and systemic pressures that can hinder a physician's ability to practice medicine as they deem best. During our conversation, each panel member shares their thoughts on the contemporary issues facing medical professionals today. From digital recordkeeping to financial sustainability, we explore how these changes can inadvertently create barriers between doctors and their patients. We reflect on the shift from independent practices to employment models, emphasizing how this transition has altered the dynamics of physician autonomy and patient interaction. As physicians express their frustrations over increased workloads and reduced one-on-one time with patients, the discussion turns to the resulting feelings of disconnection and loss of joy in practice. We delve deeper into specific examples of ethical and moral dilemmas arising from new legislation, particularly in states with restrictive health laws. These regulations can force physicians into challenging positions where they must balance compliance with their professional obligations, as evidenced by discussions around reproductive health laws and minor patient treatment without parental consent. The panelists collectively acknowledge a growing sense of 'moral injury'—the distress resulting from the disconnect between personal values and professional responsibilities. One poignant thread throughout the episode is the importance of fostering meaningful relationships with patients. As we share personal stories and experiences, it becomes evident that many physicians find fulfillment in their roles when they can connect emotionally with their patients. We also discuss the increasing trend of medical students not committing to full-time clinical practice, exploring the underlying factors influencing this shift and its potential long-term consequences for the healthcare system. The conversation shifts towards the role of coaching in helping physicians navigate these turbulent waters. We highlight success stories of individuals who, through coaching, have rediscovered their passion for medicine and implemented changes that resulted in more fulfilling practices. As panelists share their insights, we emphasize the importance of self-agency and personal alignment in fostering satisfaction within the profession. Towards the end of the episode, we reflect on the gradual recognition within the medical community that the well-being of physicians is paramount to providing quality patient care. We advocate for a positive mindset and the importance of gratitude and connection, encouraging our listeners to focus on their strengths and the meaningful aspects of their practice. This episode serves as both a mirror to current challenges and a beacon of hope for positive change within the medical profession, inviting an ongoing conversation about resilience and adaptation in the face of adversity. -+=-+=-+=-+= Join the Conversation! This Podcast offers a pathway to continuing education via this CMEfy link: https://earnc.me/mhp2xn In this episode of "Life-Changing Moments," we delve into the demanding journey of residency training, a period often filled with intense challenges and overwhelming pressure. Renowned figures in the medical coaching domain, Dr. Tyra Fainstad and Dr. Adrian Mann, share their experiences and insights into the critical need for mental wellness support for medical trainees. They shed light on the perils of burnout, imposter syndrome, and moral injury that are unfortunately prevalent among resident physicians due to the grueling demands of their training. Addressing these pressing issues requires innovative solutions beyond traditional methods such as employee assistance programs. Dr. Fainstad and Dr. Mann have pioneered a unique group coaching approach called "Better Together," a fully digital, four-month program tailored specifically to the needs of resident physicians. Throughout this enlightening conversation, they explain how this program not only promotes well-being but actively combats the rise of distress associated with training in the medical field. The discussion transitions into their individual journeys that led them to develop this impactful coaching program. Dr. Fainstad opens up about her encounters with burnout, sharing the struggles she faced balancing her career alongside the pressures of motherhood. Her experiences underscored the urgent need for support systems within the medical training framework. Dr. Mann shares a similar narrative and reflects on her own transformative relationship with coaching, emphasizing the importance of redefining success and learning to trust oneself amidst external pressures. The structure of the “Better Together” program is one of its standout features. Offering both asynchronous learning materials and live group coaching sessions, participants can engage in a way that suits their busy schedules. Each week, residents join virtual calls where they can share their challenges, glean insights from their peers, and engage in lively discussions that typically see between 5 and 30 attendees. The camaraderie and shared vulnerability foster an environment where residents feel safe and encouraged to explore their mental health challenges and triumphs. Dr. Fainstad and Dr. Mann are keen to share the results of their research, which illustrates the effectiveness of this program. In their preliminary studies, they demonstrated significant reductions in burnout and moral injury among participants, leading to a notable increase in self-compassion and overall wellness. Spurred by their initial successes, they expanded their work to include a larger, multi-centered trial, solidifying “Better Together” as a model with the potential for nationwide impact. As the conversation unfolds, both speakers emphasize their commitment to making these coaching resources accessible to medical trainees. By partnering with institutions, they have created a program that allows residents to benefit from coaching without the financial burden, ensuring that all participants can engage in a healing journey that ultimately enhances their personal and professional lives. Listeners can expect not only to grasp the profound impact of such support systems on medical professionals but also to gain insights into the broader implications for patient care. She discusses the impact of a traumatic childhood loss, the financial challenges of medical education, and her determination to create a secure future for her children—free from the uncertainties of financial instability. Dr. Spath's story highlights her family's deep roots in healthcare and her father's struggles with mental health, which ultimately led to their relocation to the U.S. She recounts the challenges of adapting to a new culture, navigating language barriers, and overcoming socioeconomic obstacles. LCM 57: Doctors Treating Doctors, with Dr. Marc Lewin

This Podcast offers a pathway to continuing education via this CMEfy link: https://earnc.me/mhp2xn In this episode, we delve into the unique and often unspoken dynamics of physician-patient relationships, particularly when both parties are healthcare professionals. Building on previous insights shared by Claire Hutton regarding primary care physicians' experiences with fellow doctors, I invite my own family physician, Dr. Marc Lewin, to engage in a candid conversation about our 20-year relationship. Together, we aim to uncover the "elephant in the room" — the expectations and shared knowledge that complicate our interactions. We begin by reflecting on our individual experiences and mutual understanding as physicians. Dr. Lewin, with over 30 years of medical practice and significant humanitarian work abroad, shares his perspective on the honor he feels when treating a fellow doctor. He acknowledges that many physician patients often come in already armed with knowledge and perhaps preconceived notions about their care. This leads to an interesting dialogue about how physicians, even those with extensive training, can sometimes struggle to express vulnerability when discussing their health. Throughout our conversation, we focus on the subtleties of communication that shape our relationship. I share how I've occasionally hesitated to bring up anxieties related to my health, fearing that as a physician, I should already take care of these issues myself. Dr. Lewin reveals his own reservations about addressing sensitive topics within our doctor-patient dynamic, such as mental health and lifestyle discussions, ultimately voicing the importance of establishing an open dialogue. We explore the impact of shared experiences on our assessments of one another's health. I admit to the anxiety that sometimes accompanies my visits to him, driven by my knowledge of medical symptoms, while he addresses the delicate balance between providing patient-centered care and ensuring that I receive evidence-based recommendations. Dr. Lewin emphasizes that his role is not just to be a doctor but a partner in my health journey, directly confronting the misconception that physician patients need less thorough care. In a thoughtful exchange, we analyze our past interactions, where he assures me that he has never felt the need to overshoot testing or treatment based solely on my physician status. Instead, he aims to provide the best personalized care possible, and I express gratitude for the respectful and collegial approach he has maintained over the years. Our conversation transitions to offering advice for new physicians: the necessity to approach every patient—regardless of their background—with compassion, curiosity, and a commitment to quality care. As the episode draws to a close, we reflect on the trust built over years of mutual respect and understanding. I express how being able to share our thoughts candidly enhances not only our professional rapport but also our friendship. We close with gratitude towards each other, recognizing the unique position we hold as both doctors and as individuals invested in one another's well-being, paving the way for further meaningful conversations in the future.   -+=-+=-+=-+= Join the Conversation! This Podcast offers a pathway to continuing education via this CMEfy link: https://earnc.me/97u19s At the age of 32, Dennis Mihale, MD, made a decision that changed the course of his professional life. Although he was enjoying an exciting, rewarding career at IBM, he took a leave of absence and entered the School of General Studies at Columbia University to complete the prerequisites for medical school. With the support of his family, Mihale was accepted at the University of Miami School of Medicine. Clearly a non-traditional applicant at the age of 35, he was grateful for the school's confidence in his potential. Mihale soon proved himself to his colleagues and teachers. While in medical school, Mihale applied his background in engineering to design a flexible knee orthotic through a project sponsored by NASA. For his post-graduate training, he returned to his native New York to do a surgical internship and an orthopedic residency. He then took a year off to work at the IBM Research Center to develop an engineering model for robotic surgery. Under his direction, the project was completed, and the robot is currently used for hip replacement surgeries at the University of California at Davis. In 1990, Dr. Mihale continued his professional development and entered the Executive MBA Program at the University of South Florida. He subsequently helped to launch a health maintenance organization, and in January 1994, he started St. Augustine Health Care, a Medicaid and commercial HMO. The business prospered and ultimately achieved $30 million in annual revenues. St. Augustine was recently acquired by AvMed, and Dr. Mihale retired from that venture. Today, Dr. Mihale maintains a strong connection to the medical school and provides financial support, sponsors regional alumni meetings, and serves on the Dean's Leader-ship Cabinet. He is also involved in the development of a community clinic through his work with Tampa's Metropolitan Ministries. -=+=-=+=-=+= Are you a doctor struggling to provide the best care for your patients while dealing with financial and caregiving matters out of the scope of your practice? This Podcast offers a pathway to continuing education via this CMEfy link: https://earnc.me/yhagvy In this episode, Dr. Amos Lane, a dedicated physician from West Virginia, shares how a life-threatening illness shifted his career and reshaped his personal perspective. Originally trained in emergency medicine, Dr. Lane's path took an unexpected turn when severe health complications led to a liver transplant. This experience prompted a reevaluation of his professional role and life priorities, ultimately leading him away from clinical emergency care. Dr. Lane describes the early onset of his health issues pre-COVID, where fatigue and other symptoms escalated amidst the pressures of the pandemic. Following a diagnosis revealing alarmingly high liver enzymes, he underwent extensive treatments and ultimately received a liver transplant in 2023. His connection to his living donor—a close friend from his hometown—adds a deeply personal dimension to his story, underscoring the strength of relationships formed in adversity. After his recovery, Dr. Lane encountered challenges reintegrating into the medical field, as immunosuppressive medications restricted his ability to practice emergency medicine as he once did. He reflects on his frustration and sense of loss, as well as his departure from hospital administration after a mismatch in vision for his role. This period of uncertainty led him to consider multiple paths, eventually guiding him toward detention medicine. In his new role within the prison system, Dr. Lane finds fulfillment in providing chronic care to an underserved population, while achieving a work-life balance critical for his own recovery and family life. -=+=-=+=-=+= Are you a doctor struggling to provide the best care for your patients while dealing with financial and caregiving matters out of the scope of your practice? Life Changing Moments: Moral Injury, with Dr. Wendy Dean

This Podcast offers a pathway to continuing education via this CMEfy link: https://earnc.me/jkf3ug Moral injury in healthcare refers to the emotional and psychological distress experienced by healthcare professionals when they are unable to provide care in line with their moral or ethical beliefs. Dr. Wendy Dean explains that this concept, originally applied to the military, describes situations where clinicians are forced to compromise patient care due to systemic issues like insurance constraints and resource limitations. Unlike burnout, which is often seen as a personal failure, moral injury highlights the external pressures within the healthcare system that force clinicians to act against their deeply held values, resulting in a "soul wound" over time. To combat moral injury, Dr. Wendy Dean co-founded The Moral Injury of Healthcare, a nonprofit organization dedicated to addressing the systemic factors causing this distress. Her work focuses on raising awareness and advocating for institutional changes that allow clinicians to practice in alignment with their professional oaths. By promoting frameworks to assess and mitigate moral injury, and fostering coalitions among healthcare professionals, Dean aims to reform healthcare environments to better support both clinicians and patients. Through initiatives like the development of morally centered organizations and tools to measure moral injury, her efforts are helping to reshape the healthcare landscape.   -=+=-=+=-=+= Are you a doctor struggling to provide the best care for your patients while dealing with financial and caregiving matters out of the scope of your practice? She highlights the continued need for such conferences to foster community, support, and collaboration among women physicians. Despite significant progress, Dr. Rotering emphasizes that disparities between male and female physicians persist, particularly in leadership opportunities, compensation, and work-life balance. She underscores the importance of building inclusive environments where women can show up authentically, unburdened by the need to hide parts of themselves. Conferences like this provide a space for women to connect, share experiences, and advocate for change. By creating a culture of support and mentorship, these events play a critical role in advancing gender equity in medicine and shaping a more holistic and inclusive medical culture. -=+=-=+=-=+= Are you a doctor struggling to provide the best care for your patients while dealing with financial and caregiving matters out of the scope of your practice? Many physicians experience a significant disconnect between the idealized vision of their careers and the stark realities of day-to-day practice. Dr. Goins is a powerful example of this struggle, illustrating the transformative power of facing these challenges head-on. Trained at prestigious institutions such as Rutgers, Emory, and Yale, Dr. Goins found herself immersed in a demanding professional environment that drained her passion for medicine. Despite her deep love for the field, the overwhelming demands led to chronic burnout and severe health issues, manifesting in distressing physical symptoms. The relentless pressure of academia gradually turned her dream into a nightmare, filled with sleepless nights and debilitating anxiety. A pivotal moment occurred in a neurologist's office when a simple prescription highlighted the dire reality she was determined to escape. Dr. Goins immediately recognized the gravity of her situation, understanding that her struggle with burnout was deeply connected to her core values, not merely a medical issue. This realization spurred her to take deliberate, courageous steps toward change. With the guidance of a coach and through reflective journaling, she began to unravel her limiting beliefs and confront the societal pressures that had shaped her path. -=+=-=+=-=+= Are you a doctor struggling to provide the best care for your patients while dealing with financial and caregiving matters out of the scope of your practice?
