The preliminary accident report on the August 9, 2024 crash of a Voepass ATR72 near Sao Paolo, Brazil has just been issued. John Goglia, Todd Curtis and Greg Feith apply their expertise to examine the findings and critical aviation safety issues. The report has detailed facts, including a summary of the flight, but several key pieces of information are missing. One big issue – no detailed transcript of what was said in the cockpit during the flight. The report focuses on the aircraft's deicing and anti-icing systems, including its apparent malfunctions. This system was first activated after the crew received an alert from the aircraft's ice detection system, and was turned off less than a minute later. The crew did not react appropriately after turning off the deicing system warning. Rather than leaving the altitude where the icing was occurring or disengaging the autopilot, the crew did neither.Based on the portions of the crew's conversation during the flight in the report, John concludes that the crew was not paying enough attention to flying the airplane. They did not address warnings from the aircraft. There is a potential conflict of interest that may impede getting all the details of this accident. The Brazilian Air Force runs CENIPA, the aviation accident investigative authority, and the Brazilian air traffic control organization. Greg and John share their firsthand experiences with investigators dealing with outside influences using the example of the 1994 Roselawn, Indiana ATR72 accident.Key takeaway here: pilots, especially professional pilots, need to educate themselves about how icing affects their aircraft.Related documents are available at the Flight Safety Detectives website. What led to the deadly crash of a Voepass Linhas Aereas in August? Video of the flight's final moments show the aircraft rapidly descending in a flat spin. Early reports on the accident mention that icing conditions were present in the area at the time of the crash.Recovering from a spin in a large airliner is a significant challenge for pilots. Understanding how to avoid situations that lead to stalls and spins is the best way to avoid these tragedies.Flight training typically does not require pilots to experience actual spins. Even full-motion simulators do not provide the full range of physical experience on an actual airplane. Pilots need to understand their aircraft's stall and spin characteristics and the situations that make an aircraft's wings more prone to stalling.John Goglia shares his experience as an NTSB Board member investigating a 1994 icing-related crash of an ATR72. That investigation included an FAA test involving another ATR72 in controlled icing conditions, which revealed that certain icing conditions could result in icing that the ATR72's deicing system could not control.Todd Curtis and John also delve into an incident at Boston Logan Airport where the pilot of a Brazil-registered Embraer Phenom 300E who had difficulties communicating with an air traffic controller. The pilot could not follow several ATC requests, resulting in altitude and speed deviations during a landing approach. English is the language used by ATC at international airports, but pilots from countries where English is not the language used to communicate can have difficulties. The English used for air traffic control is not the same English used in normal conversations. Even native English speakers have challenges when communicating with ATC, particularly those performing a single-pilot IFR flight. The Airports Authority of India successfully conducted a light trial using the GAGAN satellite navigation system for the landing of an ATR72 aircraft belonging to IndiGo at the Kishangarh Airport in Rajasthan last week. GAGAN is a system jointly developed by the Airports Authority of India and the Indian Space Research Organisation (ISRO) in collaboration with US defence contractor Raytheon at an estimated cost of Rs 774 crores. It provides a very accurate and high-level of satellite signals for precision air navigation over the entire Indian airspace, with the capability of expanding to nearby regions. It is capable of providing navigation services for departure, en-route and landing operations to equipped planes. The DGCA had issued a mandate, directing that all aircraft registered in India after July 1st 2021 to be fitted with GAGAN equipment. Simply speaking, GAGAN is a Satellite Based Augmentation System or SBAS, which is a regional network of ground stations and satellites that provide GPS signal corrections, giving a better position accuracy. GPS is the most prevalent Global Navigation Satellite System (GNSS) and is owned by the US government. GAGAN is the fourth such SBAS system that has been operationalised after the US' WAAS, European Union's EGNOS an d Japan's MSAS and it is interoperable with the other three. The GAGAN system consists of 15 earth-based reference stations, two master control centres, three land uplink stations and three geostationary satellites. The reference stations gather GPS satellite data and the master control centres collect data from reference stations and create GPS correction messages. Through this, errors caused by ionospheric disturbances, satellite orbit errors and inaccurate clocks are corrected. And through the uplink stations, the corrected messages are sent to the geostationary satellites which then broadcast them to the aircraft. India is the first country in the Asia Pacific Region to trial indigenous SBAS for landing. According to SV Satish, former executive director (Air Traffic Management), AAI, GAGAN opens a gateway for all airports to have low visibility approaches. It will bring down diversions, save fuel and boost efficiency. He says, GAGAN can serve smaller airports effectively, and older aircraft have to be upgraded with GAGAN receivers. GAGAN will help airports which are currently devoid of precision approach capability equipment and have higher visibility requirements. It will reduce flight delays, save fuel, and improve flight safety. Now, aircraft will be able to land at airports not equipped with expensive Instrument Landing Systems, which include many small regional airports. At present, IndiGo, SpiceJet, Air India, Go First and AirAsia India have aircraft that are capable of carrying out these satellite-based procedures. As GAGAN's footprint expands from Africa to Australia, India is in a position to offer its service to neighbouring countries. Though primarily meant for aviation, GAGAN's capabilities can be utilised in many other user segments such as intelligent transportation, maritime, highways, railways, surveying and the telecom industry. GAGAN Message Service (GMS) can relay alerts to deep-sea fishermen, farmers, and disaster affected people at the time of natural calamities. Watch video

Drones assist with solar energy production, the satellite industry sees UAS opportunities, a quadcopter approaches a commercial flight, NYPD is developing a plan to counter weaponized drones, and more from the NBAA panel discussion on UAS. News These Drones With Little Brooms Keep Solar Panels Clean Large solar panel farms are increasingly being used to generate electricity. You can capture a lot of solar power in the desert, but the panels need to be cleaned to remove the dust that accumulates. Startup Aerial Power Limited is testing brush-carrying UAVs that gently keep the solar panels clean. Different Industries Debate the Potential of UAVs and the Need for Satellite UAVs often depend on satellite-based technology, and the satellite industry sees business opportunity in that. By their nature, satellite data transmissions are global, and that means international standards are needed. Quadcopter drone 'deliberately flown at passenger airliner' over Essex The UK Airprox Board has released its report on the May 2014 incident where a quadcopter came close to an ATR72 on approach to runway 06 at Southend Airport. The report (No. 2014073) states: “As the aircraft was about to intercept the ILS Glide-Slope, the pilot saw a remote-controlled quadcopter very close to the right wing-tip. The aircraft captain did not see the quadcopter but the sighting was reported to Southend ATC. The co-pilot formed the impression that the quadcopter had been flown deliberately close to the AT72 because he had seen it around 100m away as it approached from the right-hand side and made a turn to fly in the opposite direction to his aircraft, around 25m away and at the same level.” The quadcopter operator could not be traced. Beyond that… nothing to be done. NYPD: Threat Of Terrorists With Drones Is A Growing Concern New York police have been concerned about the possibility of an attack by drone. While they don't have any intelligence about a specific threat, they want to be prepared. The NYPD is consulting with the military and they are working on a plan to counter weaponized drones.
