Dr Sian Williams was a familiar face and voice on BBC Breakfast, television news, and Radio 4 for many years, and she's now a presenter on Radio 3 Unwind on BBC Sounds. There she hosts a three hour programme every morning, sharing a restorative selection of music with the aim of supporting your well-being. She also presents Life Changing on Radio 4, interviewing people who have lived through extraordinary events. In 2012, she started to train as a psychologist and was awarded a doctorate in Counselling Psychology four years ago. Since then she has been working in the NHS primarily, with first responders who are experiencing anxiety, stress and trauma. Sarah Fairbairns spent much of her life feeling she was a bit different. Growing up in the 1960's and 70's she had the reputation of a wild child. On a student exchange in the United States she got to dance on stage with the caste of the famous counter-culture musical Hair. In her early 20s she travelled to India with her boyfriend in search of hippy culture, tuning out, dropping out, taking drugs and becoming what was known at the time as a 'freak', a group at the extreme end of the hippy spectrum. And yet all the while she faced bouts of sadness and depression and a confusion as to why that should be. It lead eventually to an attempted suicide and psychiatric treatment. Things improved and stabilised. She married, had children and came to terms with her life, while never really feeling settled. She even trained and qualified as a Psychotherapist. And yet it was only towards the end of her training that she started to connect an event from her childhood with the unsettled life she'd lead and the fragility she felt. That trauma had happened when, at the age of eleven, she had been diagnosed with lateral idiopathic adolescent scoliosis, resulting in curvature of the spine. The result was a period in an orthopaedic hospital away from her family with dramatic surgery on her back and incarceration in a restrictive plaster caste. That long, isolated hospital stay and the process she went through to stabilise her spine was ultimately deemed a success, but the girl that emerged from hospital was more than just a medical success story. In her 70s, and with the threat of further surgery on her back, Sarah began to recognise that a failure to deal with the trauma of that childhood hospitalisation had been a key factor in her state of mind and behaviour throughout her life. She wrote in to Life Changing and told Dr Sian Williams about her slow recognition of her buried and Life Changing childhood trauma, and why confronting and understanding it had provided belated but extraordinary relief.Producer: Tom Alban

In 2013 Nick Hitch found himself at the heart of a violent attack on a Gas facility in Eastern Algeria. It was later revealed that the militiamen were affiliated to Al-Qaeda. As a senior project manager Nick was deliberately targeted, threatened with execution, forced along with his colleagues to sit for hours in fear of detonating explosives to which they had been attached, and ultimately packed into a vehicle alongside a man with a crude suicide bomb on his knee. Thirty-nine foreign workers died during the attack, several of them Nick's close colleagues. Prioritized strategies to improve diagnosis and early management of cerebral palsy for both Māori and non-Māori familiesSian A Williams, Ivana Nakarada-Kordic, Anna H Mackey, Stephen Reay, N Susan StottAbstractAim: To identify prioritized strategies to support improvements in early health service delivery around the diagnosis and management of cerebral palsy (CP) for both Māori and non-Māori individuals.Method: Using a participatory approach, health care professionals and the parents of children with CP attended co-design workshops on the topic of early diagnosis and management of CP. Health design researchers facilitated two 'discovery' (sharing experiences and ideas) and two 'prototyping' (solution-focused) workshops in Aotearoa, New Zealand. A Māori health service worker co-facilitated workshops for Māori families.Results: Between 7 and 13 participants (14 health care professionals, 12 parents of children with CP across all functional levels) attended each workshop. The discovery workshops revealed powerful stories about early experiences and needs within clinician-family communication and service provision. The prototyping workshops revealed priorities around communication, and when, what, and how information is provided to families; recommendations were co-created around what should be prioritized within a resource to aid health care navigation.Interpretation: There is a critical need for improved communication, support, and guidance, as well as education, for families navigating their child with CP through the health care system. Further input from families and health care professionals partnering together will continue to guide strategies to improve health care service delivery using experiences as a mechanism for change.

Dr Sian Williams leads a team charting the story of the Welsh codebreaker whose naked and decomposing body was found inside a bag in his bath at his flat in Central London in August 2010.Mystery still surrounds how and why Gareth Williams, from Anglesey, died - not least how he ended up inside the North Face sports bag in the first place. There were no fingerprints on the bag, on the padlock or in the bath.Gareth's death continues to baffle police with speculation as to the cause ranging from a sex game gone wrong to involvement of a rogue state, possibly Russia.The coroner Fiona Wilcox, who led the inquest into the 31 year old's death, gave a narrative verdict, concluding it was probable Gareth had been unlawfully killed but because there was no clear evidence she couldn't deliver an official verdict of unlawful killing.Gareth Williams normally worked for GCHQ in Cheltenham but had been seconded to the Secret Intelligence Service at MI6 in London. It's still unknown what the exact nature of his work was and evidence by the secret intelligence service at his inquest was minimal.Gareth Williams had not been seen by family or work colleagues for over a week when his body was found and it was only when his sister raised the alarm that MI6 contacted police who carried out a ‘welfare call' at his home in Pimlico. When officers entered his flat they found the heating was on full, (which was unusual for the middle of August) and this may have caused issues for pathologists investigating exactly how Gareth died.In this programme Sian and the team dig deeper into the story.We hear conflicting evidence from experts who attempted up to 300 times to lock themselves inside a North Face sports bag.We delve into some of the evidence not reported widely from the inquest into Gareth's death, we hear more about what his work at MI6 may have actually entailed and we find out some more about Gareth the child genius who went on to be recruited by the intelligence services because of his brilliant mind as a mathematician and codebreaker.Experts give their views on what may have happened to him. Growing up Joe Jaquest Oteng knew very little about his Dad; they struggled to find much common ground. Joe's Dad, Peter, was guarded about his early life in Ghana and the family he had left behind when he emigrated to the UK in the 1960s. When Peter died Joe was left to sort through his belongings. It was September 2000 when Lisa and her 10-year-old brother Gary were taken to the airport. Their Dad said they were going on holiday and that mum Tracey was going to join them later, just as soon as she could get time off work. As the children boarded the plane full of expectations for the trip they could not know how profoundly this moment would shape the rest of their lives – they were soon told Tracey had died and there was no point ever going back to England. Dr Sian Williams hears about Lisa's struggles to adapt to a new life in Pakistan, trying to keep memories of home and her mum alive but falling into despair and loneliness. Meeting her Mum again and returning to England aged 17 comes with a whole new set of challenges. Tracey knew something was wrong the moment she got to the house. The place was empty, the children were gone and so was her husband Taz. Their marriage had broken down and they were going through divorce proceedings. Then she got a text message from him: ‘Gary and Lisa say goodbye forever.' They'd been taken to Pakistan. Tracey enlisted the police, the Foreign Office and Interpol but in the absence of an agreement between Pakistan and England about parental child abduction cases — they were powerless. Tracey was advised not to travel to Pakistan and she had no idea where to start looking for them anyway. Dr Sian Williams hears what Tracey went through in the decade it took for her to find her children, and the complicated aftermath. In the next episode Sian talks to Lisa, Tracey's daughter, to hear the story from her point of view.

What do you do, if falling in love with someone becomes a matter of life or death? In 2013 Aderonke Apata found herself on a coach in the UK, heading for the airport, about to be deported to Nigeria. She'd left her home country years earlier in fear for her life. Her ‘crime' was that she loved another woman. Same-sex couples in Nigeria face jail time from the courts and, in Aderonke's situation, the threat of deadly violence at the hands of mobs. As she got on the coach the documents that Aderonke had painstakingly compiled to stop her deportation were still being furiously faxed to the authorities. At the eleventh hour she was given a reprieve — and so her legal training had begun. She would successfully fight her own case and find herself a new career in the process. Aderonke tells her story to Dr Sian Williams.

Kieran is given a countdown, 3…2…1… and then it happens, he is stabbed — the knife reaching his heart. In surgery, he hears himself flatline. He survives but the wound leaves scars that penetrate deep into his life. It's in the years that follow that the fight for survival really begins. Measuring skeletal muscle morphology and architecture with imaging modalities in children with cerebral palsy: a scoping reviewSîan A Williams 1 2, N Susan Stott 2 3, Jane Valentine 4 5, Catherine Elliott 4 6, Siobhán L Reid 7AbstractAim: To investigate the use of ultrasound and magnetic resonance imaging (MRI) methodologies to assess muscle morphology and architecture in children with cerebral palsy (CP).Method: A scoping review was conducted with systematic searches of Medline, Embase, Scopus, Web of Science, PubMed, and PsycInfo for all original articles published up to January 2019 utilizing ultrasound and/or MRI to determine morphological and architectural properties of lower limb skeletal muscle in children with CP.Results: Eighty papers used ultrasound (n=44), three-dimensional ultrasound (n=16), or MRI (n=20) to measure at least one muscle parameter in children and adolescents with CP. Most research investigated single muscles, predominantly the medial gastrocnemius muscle, included children classified in Gross Motor Function Classification System levels I (n=62) and II (n=65), and assessed fascicle length (n=35) and/or muscle volume (n=35). Only 21 papers reported reliability of imaging techniques. Forty-six papers assessed measures of Impairment (n=39), Activity (n=24), and Participation (n=3).Interpretation: Current research study design, variation in methodology, and preferences towards investigation of isolated muscles may oversimplify the complexities of CP muscle but provide a foundation for the understanding of the changes in muscle parameters in children with CP.What this paper adds: Current evidence is biased towards the medial gastrocnemius muscle and more functionally able children with cerebral palsy (CP). Variations in imaging techniques and joint positioning limit comparisons between studies. Clinimetric testing of parameters of CP muscle is not always considered. Assessment of parameter(s) of muscle with measures of participation is sparse.
