Abstract: Since the first report of a new pneumonia disease in December 2019 (Wuhan, China) up to now WHO reported more than 50 million confirmed cases and more than one million losses, globally. The causative agent of COVID-19 (SARS-CoV-2) has spread worldwide resulting in a pandemic of unprecedented magnitude. To date, no clinically safe drug or vaccine is available and the development of molecules to combat SARS-CoV-2 infections is imminent. A well-known strategy to identify molecules with inhibitory potential against SARS-CoV-2 proteins is the repurposing of clinically developed drugs, e.g., anti-parasitic drugs. The results described in this study demonstrate the inhibitory potential of quinacrine and suramin against SARS-CoV-2 main protease (3CLpro). Quinacrine and suramin molecules present a competitive and non-competitive mode of inhibition, respectively, with IC50 and KD values in low M range. Using docking and molecular dynamics simulations we identified a possible binding mode and the amino acids involved in these interactions. Our results suggested that suramin in combination with quinacrine showed promising synergistic efficacy to inhibit SARS-CoV-2 3CLpro. The identification of effective, synergistic drug combinations could lead to the design of better treatments for the COVID-19 disease. Drug repositioning offers hope to the SARS-CoV-2 control. Prion-Erkrankungen sind tödlich verlaufende, neurodegenerative Erkrankungen, die sporadischen, hereditären oder infektiösen Ursprungs sein können. Die Missfaltung des zellulären Prion-Proteins (PrPC) spielt eine zentrale Rolle bei der Pathogenese. In der vorliegenden Arbeit wurden zwei verschiedene Themenbereiche bearbeitet: zum einen die Analyse der Faltung und Maturierung krankheitsassoziierter Mutanten und zum anderen die Wirkung einer Anti-Prion-Substanz auf die Biogenese von PrPC und die Propagierung von infektiösem PrPSc. Hierfür wurden verschiedene humanpathogene Mutanten, sowie andere Punkt- und Deletionsmutanten in Maus-Neuroblastomzellen untersucht. Im ersten Teil dieser Arbeit konnte gezeigt werden, dass zwei Mutationen, die Prion-Erkrankungen beim Menschen hervorrufen (T183A und F198S), die Faltung und Biogenese von PrP beeinträchtigen. Im Gegensatz zu PrPC, nehmen diese Mutanten im ER spontan eine missgefaltete Konformation an und erhalten weder eine komplexe Glykosilierung noch einen GPI-Anker. Die missgefalteten Mutanten werden nicht durch eine ER-assoziierte Degradierung eliminiert, sondern in ihrer High Mannose Form sekretiert. Darüber hinaus können die sekretierten Formen von benachbarten Zellen wieder aufgenommen werden. Diese Studie leistet einen wichtigen Beitrag zur Klärung der Missfaltung von T183A und F198S, welche offenbar mit einer Destabilisierung des hydrophoben Cores von PrP assoziiert ist und verdeutlicht außerdem, dass ein Zusammenhang zwischen der Faltung und den posttranslationalen Modifikationen von PrP besteht. Der zweite Teil der Arbeit konzentriert sich auf den Wirkungsmechanismus der Anti-Prion-Substanz Suramin. Es konnte gezeigt werden, dass der Effekt von Suramin auf einer Entfernung von PrPC von der Zelloberfläche beruht. So induziert Suramin eine Missfaltung von PrPC an der Plasmamembran, was eine anschließende Internalisierung und rasche Degradierung zur Folge hat. Dabei sind Missfaltung und Internalisierung zwei aufeinander folgende Prozesse, die von unterschiedlichen Domänen von PrPC vermittelt werden. Während die Suramin-induzierte Missfaltung vom proximalen Bereich des C-Terminus abhängt, ist der N-Terminus für die Endozytose verantwortlich. Diese Analyse verdeutlicht, dass in neuronalen Zellen eine Qualitätskontrolle zur Entfernung missgefalteter PrP-Formen von der Zelloberfläche besteht und liefert außerdem einen entscheidenden Ansatz für die Entwicklung effizienterer Anti-Prion-Strategien.

Zwei deutsche Tropenreisende erkrankten nach einer zweitägigen Safari im Akagera-Nationalpark in Ruanda an ostafrikanischer Trypanosomiasis (Schlafkrankheit). Leitsymptome waren Fieber, Lymphadenopathie und eine typische Primärläsion (Trypanosomenschanker). Die Diagnose konnte durch den Nachweis von Trypanosomen im peripheren Blut gesichert werden. Das Zentralnervensystem war in beiden Fällen nicht beteiligt. Unter dem Einfluß einer Therapie mit Suramin, 1 g pro Woche intravenös über 6 Wochen, bildeten sich die Symptome und die Parasitämie rasch zurück. Nach der Zahl der seit 1970 berichteten Fälle ergibt sich für deutsche Tropenreisende ein Infektionsrisiko von 0,3 pro 100 000. Aufgrund der teilweise erheblichen Wiederzunahme der Schlafkrankheit in einigen afrikanischen Ländern kann mit einer Zunahme des Infektionsrisikos auch für Touristen gerechnet werden.

Bei einem 45jährigen Patienten wurde im April 1986 ein rechtsseitiges Nebennierenrindenkarzinom (978 g) entfernt und das Tumorbett postoperativ mit 40 Gy bestrahlt. Wegen multipler Lungenmetastasen wurde im Januar und im Juni 1987 eine Polychemotherapie mit Cisplatin, Etoposid und Bleomycin durchgeführt, ohne daß eine Befundbesserung erreicht wurde. Auch eine Therapie mit Mitotan (Lysodren®) blieb wirkungslos und mußte wegen schwerer Nebenwirkungen beendet werden. Im August 1987 wurde eine Therapie mit Suramin (Germanin®) begonnen. Nach einer Aufsättigungsdosis von 10,7 g über 6 Wochen kam es zu einer nahezu vollständigen Rückbildung der Lungenmetastasen. Während einer niedrig dosierten Erhaltungstherapie mit Suramin wurden im Januar 1988 erneut Lungenmetastasen nachweisbar. Eine Dosissteigerung führte zu Wachstumsstillstand, nicht jedoch zur Rückbildung der Metastasen. Der Patient starb im April 1988 überraschend an akutem Kreislaufversagen. Die Suramin-Therapie war 6 Wochen zuvor bei Bronchopneumonie und verschlechtertem Allgemeinzustand beendet worden. Nebenwirkungen der Suramin-Therapie waren Thrombozytopenie, Gerinnungsstörungen und eine mäßiggradige Proteinurie.
