Join our conversation as we debunk the taboos surrounding pelvic physical therapy, discuss how pelvic pain affects both men and women, and prove why there is hope for anyone to have a healthier pelvis. Michelle Dela Rosa, PT is the Clinical Director of the Hamilton and Howell locations at Jag Physical Therapy. She has been practicing in the field of pelvic health physical therapy for 20 years. She started in Brooklyn, NY and developed the pelvic PT program, in now NYU Langone Health. She implemented a weekly prenatal exercise class for the community and extended the program to treat pelvic pain in men and post-prostatectomy issues. She has had the privilege of consulting at hospital urogynecology, urodynamics, and orthopedic clinics. Later, she continued to practice in NJ, where she helped to establish the pelvic health PT program at now Penn Medicine Princeton Health. Michelle graduated with a master's in physical therapy from Boston University in 2000. While at BU, she competed at the collegiate level as a member of the university's track and field team. In 2005, she obtained her Doctorate in Physical Therapy from the State University of New York at Stony Brook. As a native of West Windsor, NJ, Michelle opened her own practice, Connect Physical Therapy, in Hamilton in 2009, which had grown to become a leader of pelvic physical therapy in the state. She opened a second location in Howell, NJ in 2015. Their YouTube channel showcasing pelvic health education quickly grew to almost 100K subscribers. Michelle lectures on women's and men's health issues at many community events throughout New Jersey. She also enjoys being a teaching assistant for the Herman and Wallace Pelvic Rehabilitation Institute and earned the Pelvic Rehabilitation Practitioner Certification (PRPC), which identifies her as a specialist in the women's and men's pelvic health fields. This is a recent cassette conversion into mp3 of a message by Rev. Wallace A. Bell, previous pastor at PRPC -for 30 years-, entitled -Personal Evangelism- How to Give the Gospel to Another.- It includes a PDF file that can be opened sharing the diagram he is referencing as it was published in the 1967 Presbyterian Guardian. - There is no date listed on the cassette, but it was probably recorded in the early 1970s based on other tapes on different subjects found in the same storage box with this tape. However, since the accompanying article is dated September 1969 we are giving it that date for the audio message on SA.

This cassette was just discovered in storage at PRPC and converted regarding a special event on January 1, 1971. It hosted guest speaker Angelo Lo Vallo, a former priest speaking against Roman Catholicism.

“You have pain where?!” People have often asked why a healthy person like me would need physical therapy. They are quite surprised to learn about pelvic-floor therapy… and it's not long before I hear “I have pain there too.” What was once a specialty that not even many obstetricians or urologists knew about, Pelvic Floor Physical Therapy is now a rapidly growing field with an outsized demand for new therapists. Treating conditions like urinary and fecal urgency, incontinence, prolapse, nerve entrapment, surgical scarring, pre and postpartum issues, visceral restrictions and sexual dysfunction, it's not hard to see why. When Elizabeth Makous became a physical therapist over 30 years ago there were only a couple of classes teaching pelvic floor therapy. When seeing a patient complaining of vaginal pain “deep inside” she donned a pair of gloves and began treating patients internally. Elizabeth has continued to innovate new treatments in the field applying her unique kind of creativity to the world of medicine. After the home birth of her son she figured out a way to treat her own bathroom issues without surgery. She's developed protocols for successfully treating issues previously thought untreatable (even with surgery). She even approached a company called TheraWand to ask if she could make improvements to an existing vaginal wand product to more efficiently treat the pelvic floor rectally. After spending a year with a heat lamp sculpting plastic into the curves she felt would be more safe and effective… she'd created a brand new trigger wand that can be used to treat both men and women. Maybe it was the way she was raised… spending time in the clinic with her father, a doctor, and her mother, a nurse. Her grandfather was also a doctor in this small-town community who treated blacks for free during the segregation era in the South. Their passion spilled over in such a way that when meeting a patient with a need, Elizabeth says “there is a gift inside me that just looks for a way to treat it.” In this episode we talk about the importance of being your own medical advocate, how sitting for a living can be an occupational hazard, treating patients with lymphedema, pain management (without opioids), and how Covid restrictions didn't stop humans from needing touch. We also speak about being a highly-sensitive (HSP), the need for major reform in the emergency mental healthcare system, and how Elizabeth leans on her Catholic Chrisitan faith (for example: she took two medical mission-trips to Kenya in 2019 and 2021 helping to empower African men and women to alleviate pelvic pain and to treat female victims of female genital mutilation). ⚠ In today's episode we're going to be talking about issues surrounding female health, particular pelvic floor dysfunction. If you usually listen to the podcast episode with little ones listening in, I just wanted to give a heads up that we will be discussing anatomy and having a frank conversation. This is probably one to listen to with earbuds. ⚠ It's a bit of a cultural joke that moms can't jump or laugh without a little bladder leakage. But it's actually no laughing matter. Not only that, but the symptoms associated with a weak pelvic floor can be a lot more painful and embarrassing than just peeing when you laugh. After my own experience with pelvic pain, incontinence, and pelvic prolapse, I became a bit passionate to talk more openly about this neglected topic. And when I started opening up about it, so many mamas started replying! “I'm not the only one?!” was a common reply. When I let folks know about this interview, I probably got more email replies and social media DMs with questions and comments than any other topic! That surprised me, but it probably shouldn't have. We may not be able to get to all those questions today, but I hope this conversation first of all just helps people know they aren't alone. I had felt so embarrassed and ashamed like there was something wrong with my body alone, and it was a relief to realize how common (if hidden) these issues really are. Second, I hope you can come away with some practical tips and solutions to begin your own process of healing. I also hope it encourages other moms to get help as needed. Our bodies are strong and powerful and have nurtured babies and (if you're listening to this podcast) are now educating the next generation. It is worth it to take the time to protect our bodies, to help them heal, and to help them stay strong. Finally, I hope having this conversation just right out in the open, in your earbuds, on the internet, starts to take just a little bit of the sting of the awkward out of this incredibly important conversation. We are all therapists at the end of the day. All of us have received emails and phone calls for people's lives that we have transformed. That's what's most important and that's what brings us to something like continuing education. We are so hungry to learn more, to develop new skills, to expand our knowledge. By nature, therapists are intellectually curious and compassionate people. There is just so much that OT's and PT's have to learn from one another. I personally have trained with some amazing PTs such as Diane Lee- her ISM series has been tremendous for my clients and at the same time PTs have taken my online pelvic health course and felt that it brought such a value to their practice. There's a definite need to cross collaborate and interest in serving our clients while expanding our knowledge base. While I know things are changing- there is a huge gap of knowledge around OT's role within the pelvic health field. Most people associate pelvic health with PTs- so many are surprised to see us – to see ots in this specialty. Although it is within our scope of practice, it is up to each one of us to create our own path to competency to be able to work with our pelvic health clients. I think most of us can agree that with the majority of therapists specialized learning comes post graduation. Both PT's and OT's come to pelvic health from such varied backgrounds that this is what creates a rich individualized experience for our clients. This however also adds to the confusion around what a pelvic health session is like. It's hard to place an experience in a box when we each have our own way of getting there. like OTs take their own paths, pts take their own when to comes to specializing in pelvic health. the APTA even has their own women's health section which is incredible and while there are certifications available out there, such as PRPC through Herrmann and Wallace to streamline the path professionals can take, in most areas of the world these certifications are not required for practicing within pelvic health, again adding to the confusion around who's doing what. however there are more clients in need of this very special area of rehab and there's room for us all. Just as OT's and PT's work side-by-side in other fields of rehab, there's a huge need for us to work collaborating in ph, especially when you consider the sensitive taboo nature of this area of practice. It's been my experience that the general population has such poor understanding of pelvic floor function and particularly how it impacts us from a whole body, whole person perspective that I think the more of us -OT and PT- working together in this field to spread the message, the better. pelvic floor function is a crucial part of a much broader functional tasks of toileting and intimacy, Both of which have broader connections within a person's emotional cognitive and social abilities. It's also important to consider the social implications for people with pelvic floor issues -withdrawal from social and recreational activities, social isolation, disempowerment, lack of self-esteem, anxiety, depression, and the impact on close relationships such as with our spouse, our friends and our children.
