The sacroiliac joint is an area that remains contentious despite all of the research that's taken place and in this episode, we are chatting to Wilfried Simon, an osteopath and physio from Belgium about the misconceptions, if it can be a cause of pain and what to do if it is KEY TAKEAWAYS It's a connection point but there isn't much movement, only 3 degrees and its deep within the spine If you want the patient to be able to treat themselves and achieve autonomy they have to understand what's wrong When you have manipulation it's about what it creates overall including how the person thinks about it Identify the movement that is painful and work with the patient to make the movement less sensitive through changing the axis of the movement and building the confidence of the patient to complete movements Trust the future and the process, not very day is the same, not every day is pain free but that's what make the patient stronger and you stronger BEST MOMENTS ‘It's important to treat the person not the problem' ‘There is aways a combination of what's happening mechanically to the joint and how the patient perceives and feels about what's happening' RESOURCES FOR THIS EPISODE https://www.instagram.com/the.spine.physio/?hl=en THE BACK PAIN PODCAST PROVIDER MAP - FINDING SOMEONE TO HELP YOU WITH YOUR BACK PAIN https://thebackpainpodcast.com/index.php/members-map/ VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast website The Back Pain Podcast recommended products affiliate link Our Rode Mixer https://amzn.to/3waU8bx Our Microphones https://amzn.to/3rzSZ9Z Second Microphone https://amzn.to/2ObKMeA XLR Cable https://amzn.to/3rBL8ZB Studio Headphones https://amzn.to/3u082LE Laptophttps://amzn.to/3dhfafT Our webcam https://amzn.to/31uUefQ ABOUT THE HOSTS Dave Elliott Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK. Adam Dobson, physiotherapist, returns to the podcast to talk about the Back to Health Programme, a residential rehabilitation programme for people suffering from persistent back painIn this episode hear not only from Adam but also from Rebecca whose experience on the programme changed her life. They discuss the challenges of being told many different things, medication and why the programme is life-changing KEY TAKEAWAYS You can go to various physiotherapists and be told many different things, it can become a shopping list given to patients without any real idea of what it means to manage the problem I was pushed out of my comfort zone in many different ways but I was in an environment with people who understood and shared their experiences, it was enlightening in many ways and restored my faith in human nature In the first days of the course there will be many accounts of the shopping list of problems people have been told about their bodies When a professional tells you something you believe them because they are the expert in that area. The programme has taken on a lot of feedback and has evolved, with lots of previous patients contributing to the programme It should be a front-ended option for people with persistent pain who want to learn deeply to take control and get happy again To know with confidence that doing things isn't going to cause any more damage has made me stronger than I've ever been The course gives you the confidence to understand how the body is working, what it's telling you and what's dangerous and what isn't BEST MOMENTS‘There wasn't one thing that was important, the whole course is important every aspect of it is important'‘Everyone's experiences had put them in a dark place'‘He led us through those grotty times and showed us where we could be today'‘It's given me back my confidence and I do more than I've ever done before,'I no longer believe the pain is causing damage' THE BACK PAIN PODCAST PROVIDER MAP - FINDING SOMEONE TO HELP YOU WITH YOUR BACK PAINhttps://thebackpainpodcast.com/index.php/members-map/ VALUABLE RESOURCESThe Back Pain PodcastThe Back Pain Podcast websiteThe Back Pain Podcast recommended products affiliate linkOur Rode Mixer https://amzn.to/3waU8bxOur Microphones https://amzn.to/3rzSZ9ZSecond Microphone https://amzn.to/2ObKMeAXLR Cable https://amzn.to/3rBL8ZBStudio Headphones https://amzn.to/3u082LELaptophttps://amzn.to/3dhfafTOur webcam https://amzn.to/31uUefQ ABOUT THE HOSTSDave ElliottDave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK. Dave and Rob are having a behind the treatment room door chat about their working week as chiropractors They discuss the importance of exercise plans and how increasing functional capacity is an important element of any treatment plan and share a great analogy to help understanding KEY TAKEAWAYS Any conservative or passive care doesn't necessarily decrease pain from issues such as osteoarthritis People assume massage is aimed at reducing pain but its about increasing functional capacity If we improve the capacity of someone's knee health then it means they can do more Exercise increases the capacity and your threshold so you are increasing functional capacity You have to look at the bigger picture, if you knee or back is stopping you doing the things you want in life then surgery may be the option to consider If you are utilising exercise as part of your treatment plan you need to look at usage and function as well as lessening pain levels Having an expectation of no pain can result in less resilience BEST MOMENTS ‘You may be able to do more with the same amount of pain' ‘Pain is a very normal aspect of society' ‘We can usually increase your capacity but this may not lessen the level of pain' THE BACK PAIN PODCAST PROVIDER MAP - FINDING SOMEONE TO HELP YOU WITH YOUR BACK PAIN https://thebackpainpodcast.com/index.php/members-map/ VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast website The Back Pain Podcast recommended products affiliate link Our Rode Mixer https://amzn.to/3waU8bx Our Microphones https://amzn.to/3rzSZ9Z Second Microphone https://amzn.to/2ObKMeA XLR Cable https://amzn.to/3rBL8ZB Studio Headphones https://amzn.to/3u082LE Laptophttps://amzn.to/3dhfafT Our webcam https://amzn.to/31uUefQ ABOUT THE HOSTS Dave Elliott Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK. Dave and Rob are talking about diagnostic labels and particularly those for lower back pain Lower back pain is a complex and multifactorial condition and a specific diagnosis may not reflect the underlying causes or mechanisms for the pain so does a specific label help or hinder? This is a learning episode where the discussion is a great opportunity to carefully consider what you do as a clinician with the evolution of diagnostic labelling KEY TAKEAWAYS The research is now saying a diagnostic label is not as important as previously thought There is back pain that needs a specific diagnosis but this is not the case for the majority of non-specific back pain With non-specific back pain we know it's nothing sinister or serious and what's left is a combination of other things Many things can be contributing to the pain and the treatments and outcomes are similar for non-specific back pain Understanding the multifactorial and compounding aspect of non-specific back pain means there are a multitude of things you can do to help yourself The vast majority of patients will be better in 12 weeks It makes sense for us to stop using these labels and be better at explaining BEST MOMENTS ‘Our previous notion of a single answer to find is a bit immature' ‘A more holistic approach may lead to better outcomes' ‘This is evolving and changing our diagnostic labelling in line with current research' THE BACK PAIN PODCAST PROVIDER MAP - FINDING SOMEONE TO HELP YOU WITH YOUR BACK PAIN https://thebackpainpodcast.com/index.php/members-map/ VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast website The Back Pain Podcast recommended products affiliate link Our Rode Mixer https://amzn.to/3waU8bx Our Microphones https://amzn.to/3rzSZ9Z Second Microphone https://amzn.to/2ObKMeA XLR Cable https://amzn.to/3rBL8ZB Studio Headphones https://amzn.to/3u082LE Laptophttps://amzn.to/3dhfafT Our webcam https://amzn.to/31uUefQ ABOUT THE HOSTS Dave Elliott Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK. In this episode, Rob is with Dr Sophie Newton talking about the medication both prescribed and over the counter that is used to treat back pain What is appropriate for one person may not be the right thing for another but this is an opportunity to find out more about how different types of medication work and the reasons why specific treatments are used KEY TAKEAWAYS Short-term, acute pain treatments include anti-inflammatory medication which works on damping down the inflammation Milder opioids like codeine are often given in conjunction with co-codamol Opiates work on opioid receptors that trigger the pain response Medications can be very effective but they do have side effects and it's important that people know about these People need to become active to repair the back and it's about the medication that can help support this Antidepressants are now recommended for chronic pain because they work on neurotransmitters The topical gels and patches do really work and for many people, it's a much better option as the side effects profile is less BEST MOMENTS ‘The smallest dose is the best dose' ‘We need you to get moving and you need pain relief onboard to get moving because this is what will be the biggest help' ‘GPs are always assessing the risk and the benefits of any medication' RESOURCES FOR THIS EPISODE Dr Sophie Newton: Youtube Channelhttps://www.youtube.com/channel/UCEUJz86eCtUsXBMssYoC6pA Instagram https://www.instagram.com/drsophiegp/?hl=en Twitter https://twitter.com/drsophiegp?lang=en THE BACK PAIN PODCAST PROVIDER MAP - FINDING SOMEONE TO HELP YOU WITH YOUR BACK PAIN https://thebackpainpodcast.com/index.php/members-map/ VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast website The Back Pain Podcast recommended products affiliate link Our Rode Mixer https://amzn.to/3waU8bx Our Microphones https://amzn.to/3rzSZ9Z Second Microphone https://amzn.to/2ObKMeA XLR Cable https://amzn.to/3rBL8ZB Studio Headphones https://amzn.to/3u082LE Laptophttps://amzn.to/3dhfafT Our webcam https://amzn.to/31uUefQ ABOUT THE HOSTS Dave Elliott Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK. In this episode, Dave shares a detailed lower back pain case history and takes specific questions from the audience in the journey to a diagnosis This is a fascinating insight into the approach that begins from the moment the clinician meets the patient through to the treatment journey and the factors that influence recovery KEY TAKEAWAYS Whatever your discipline the tenets are the same when you talking about pain As osteopaths and chiropractors, we are talking the same words just in a different language A disc doesn't have to present with radicular symptoms initially Back or cervical disc issues have to be considered When the range of motion is restricted isolating the areas and specific groups of muscles, joints and nerves can help with the diagnosis If the specific pain is on a full extension this is a classic sign of a set joint irritation If you experience pain over a period of time fear avoidance can become a part of the individual's experience The biggest test can be the basic test of ‘poking it' Everything is testing and a provocative test can be seeing the patient coming into the room A test doesn't have to have a name and make it onto your notes BEST MOMENTS ‘The basic tenets of good care should be what you are going by' ‘Identifying which one is the head honcho and ruling the show is what we are looking for' ‘Your provocative test starts when you greet that patient' THE BACK PAIN PODCAST PROVIDER MAP - FINDING SOMEONE TO HELP YOU WITH YOUR BACK PAIN https://thebackpainpodcast.com/index.php/members-map/ VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast website The Back Pain Podcast recommended products affiliate link Our Rode Mixer https://amzn.to/3waU8bx Our Microphones https://amzn.to/3rzSZ9Z Second Microphone https://amzn.to/2ObKMeA XLR Cable https://amzn.to/3rBL8ZB Studio Headphones https://amzn.to/3u082LE Laptophttps://amzn.to/3dhfafT Our webcam https://amzn.to/31uUefQ ABOUT THE HOSTS Dave Elliott Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK. Ian Gatt is a physiotherapist with extensive experience and expertise and is an upper limb injury specialist. In this episode Ian shares advice on tennis and golfers' elbow from the symptoms to the causes and what you can do to be able to get on with your life in the way you want to. KEY TAKEAWAYS It's something that affects a tendon and it impacts daily activities Tennis elbow tends to be on the outside and golfers the inside of the elbow It can be linked to who you are as a person and the environment you are in Having a conversation and understanding how the person feels is important and the first step You can be chasing a diagnosis but understanding the function of the person is vital so you know what they need to do and can do It's not posture, it's technique and the repetition of a technique can cause the issue It's about planning, taking a step back when you experience the symptoms and doing things in a different way If you are feeling something, listen to your body and give yourself a breather BEST MOMENTS ‘It's important to understand the nature of the symptoms' ‘We just need to understand what the person can and can't do with the focus on the can' ‘I use the formula of prognosis equals diagnosis plus function' RESOURCES FOR THIS EPISODE Ian Gatt Instagram THE BACK PAIN PODCAST PROVIDER MAP - FINDING SOMEONE TO HELP YOU WITH YOUR BACK PAIN https://thebackpainpodcast.com/index.php/members-map/ VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast website The Back Pain Podcast recommended products affiliate link Our Rode Mixer https://amzn.to/3waU8bx Our Microphones https://amzn.to/3rzSZ9Z Second Microphone https://amzn.to/2ObKMeA XLR Cable https://amzn.to/3rBL8ZB Studio Headphones https://amzn.to/3u082LE Laptophttps://amzn.to/3dhfafT Our webcam https://amzn.to/31uUefQ ABOUT THE HOSTS Dave Elliott Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK. Hello, podcast friends and family. And welcome back to another episode of the Back Pain Podcast. So continuing our journey beyond just back pain into the wider realm of musculoskeletal injuries, today we're talking about a very rare cause of shoulder pain. Now Parsonage turn syndrome is a rare cause of shoulder pain and weakness. Also known as brachial neuritis or neuralgic. Amyotrophic is characterised by very, very intense shoulder pain, which lasts for a matter of weeks, which then fades away but is replaced by severe muscle wasting and weakness. Now, this can last for a long period of time, up to two, maybe even three years. And in rare occurrences, in rare instances, it can continue to reoccur. In this episode physio, Andrew Goodall goes through ACL tears from how you are likely to injure your anterior cruciate ligament through the likely treatments and the phases of recovery and rehabilitation As one of the major ligaments inside the knee, it plays an important role in stabilising the knee joint and helps to control the movement of the lower leg but is a common sports injury and Andrew shares great informative content that will help anyone to know what to expect from recovery and how to make sure their treatment is the right one for them KEY TAKEAWAYS The mechanism, force and speed at which ASL happens means it's likely there will also be other damage Calming the knee down means that it's possible to get a good quality image and reduce the pain Getting the quadricep thigh muscles working efficiently as quickly as possible will bring stability Initially, it's about doing less but maintaining strength and function For most surgery is part of the process and in the UK 80% of people have surgery Taking a section of the hamstring tendon and using it as an ASL graft is the most common approach in the UK It's bespoke surgery for your needs, your situation and taking into account the factors that have predisposed you to being injured Rehab is likely to be around a year with anything less being exceptional progress Phase one is calming down, phase two is the building of load and phase three will be moving closer to training The final phase is preventative activities and a return to training BEST MOMENTS ‘In some sports movements, the rotation through the leg translates as an inwards movement of the knee' ‘I'm essentially trying to come out of the session with a diagnosis and a plan' ‘If you can rapidly reduce swelling and pain the person is more likely to be able to use the knee and keep muscle function' ‘Stability is dynamic and controlled by lots of systems, it's the integrity of the ligament that is crucial' RESOURCES FOR THIS EPISODE https://www.instagram.com/sportskneephysio/?hl=en https://www.instagram.com/puresportsmed/?hl=en https://www.completeaclrehabcourse.com/ THE BACK PAIN PODCAST PROVIDER MAP - FINDING SOMEONE TO HELP YOU WITH YOUR BACK PAIN https://thebackpainpodcast.com/index.php/members-map/ VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast website The Back Pain Podcast recommended products affiliate link Our Rode Mixer https://amzn.to/3waU8bx Our Microphones https://amzn.to/3rzSZ9Z Second Microphone https://amzn.to/2ObKMeA XLR Cable https://amzn.to/3rBL8ZB Studio Headphones https://amzn.to/3u082LE Laptophttps://amzn.to/3dhfafT Our webcam https://amzn.to/31uUefQ ABOUT THE HOSTS Dave Elliott Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK. There are lots of different things that patients think are causes of sciatica and it's the role of the health care professional to explain when it is sciatica, what it is, what it is not and what you can do about it In this episode, Rob discusses vascular claudication, a less common cause of leg pain but due to its risk factors, it's one that healthcare professionals stay alert to. He shares the diagnosis of intermittent vascular claudication with a patient and explains the crucial indicators that led to the diagnosis along with the treatment KEY TAKEAWAYS Intermittent pain in both legs can have several causes Intermittent peripheral arterial disease is to do with the blood supply. When you are walking muscles in the legs require more blood supply and can't get it Patients often describe a cramping or burning sensation in the thighs Vascular risk factors including smoking, weight, diabetes and age increase the likelihood of intermittent peripheral arterial disease What he told us about the pain was crucial in making a clinical decision. The type and duration of the pain was an important indicator High blood pressure raises the likelihood of it being vascular A significant reduction in blood flow in the legs will be an indicator of a vascular issue Patients with vascular claudication have a much higher risk factor for an acute vascular episode BEST MOMENTS ‘As health professionals, we often provoke pain to understand the source' ‘Changes can take time but will make a difference' ‘It's one we have to stay alert for as health care professionals' THE BACK PAIN PODCAST PROVIDER MAP - FINDING SOMEONE TO HELP YOU WITH YOUR BACK PAIN https://thebackpainpodcast.com/index.php/members-map/ VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast website The Back Pain Podcast recommended products affiliate link Our Rode Mixer https://amzn.to/3waU8bx Our Microphones https://amzn.to/3rzSZ9Z Second Microphone https://amzn.to/2ObKMeA XLR Cable https://amzn.to/3rBL8ZB Studio Headphones https://amzn.to/3u082LE Laptophttps://amzn.to/3dhfafT Our webcam https://amzn.to/31uUefQ ABOUT THE HOSTS Dave Elliott Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK. Rob takes a look at not only the role of the chiropractor but the elements that any good healthcare professional will be offering to their patients He also looks in detail at the ways the words chosen and used by professionals can impact not only on recovery but also have long-term implications and explores what a good professional and patient relationship looks like and how it facilitates positive recovery, listen in and hear more KEY TAKEAWAYS Around one-third of the population will suffer from lower back pain How we explain back pain is a vital part of the process The approach of different professionals should be similar and quality care should include pain education and activity advice A package of care will focus on cognitive care and manual intervention and a good assessment should include a detailed history Patients want is a good connection with their health care practitioner and a choice in the treatment to get better The words we chose to use can impact recovery and have far-reaching implications We want patients to leave the treatment room feeling positive not damaged broken and fragile Understanding what the patient wants to achieve is a vital element in a positive recovery journey Patients want a choice so understanding their expectations is crucial in creating treatment options that will work for them BEST MOMENTS ‘No matter what the profession we should be hitting the same high points when it comes to managing lower back pain ‘As health care professionals we are in a hugely powerful position' ‘We need to show our patients they are resilient, robust and strong not weak, fragile and broken' THE BACK PAIN PODCAST PROVIDER MAP - FINDING SOMEONE TO HELP YOU WITH YOUR BACK PAIN https://thebackpainpodcast.com/index.php/members-map/ VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast website The Back Pain Podcast recommended products affiliate link Our Rode Mixer https://amzn.to/3waU8bx Our Microphones https://amzn.to/3rzSZ9Z Second Microphone https://amzn.to/2ObKMeA XLR Cable https://amzn.to/3rBL8ZB Studio Headphones https://amzn.to/3u082LE Laptophttps://amzn.to/3dhfafT Our webcam https://amzn.to/31uUefQ ABOUT THE HOSTS Dave Elliott Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK. Rob and Dave use a real-life example to explore how they would treat and manage a patient who comes to them having already experienced conflicting advice They go through the steps that are covered in an initial appointment and discuss the points at which a scan is the right choice along with what sorts of manual therapy can be a good choice Practitioners approach patients in different ways but you should always expect a very individualised treatment plan and this is a great opportunity to find out about what you should expect and why KEY TAKEAWAYS The first step should be taking a really detailed history and the patient's thoughts and beliefs are a vital part of the conversation It provides insight and an understanding that helps with building a personalised plan It's the two-way conversation between the practitioner and the patient With sciatica, it's about calming down the most aggravated part first Sometimes over testing can make it more difficult to assess, move up to the pain, it's not about aggravating it Patients can be fearful and avoid particular movements but it can be used as part of the methodology of building toward graded exposure The most important exercise is the one the patient does You need to look at an individual patient's tolerance to pain and make any exercise personalised A lot of changes seen on a scan are perfectly normal It's about focusing on the structural pathology There are always the red flag questions that result in a scan being the right approach Manual therapy provides a window of opportunity to feel better BEST MOMENTS ‘People who are fearful and worried about their pain tend to have longer term back pain' ‘If a patient believes something should be done it's important to explore it' ‘If something is painful it doesn't mean it's being damaged' THE BACK PAIN PODCAST PROVIDER MAP - FINDING SOMEONE TO HELP YOU WITH YOUR BACK PAIN https://thebackpainpodcast.com/index.php/members-map/ VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast website The Back Pain Podcast recommended products affiliate link Our Rode Mixer https://amzn.to/3waU8bx Our Microphones https://amzn.to/3rzSZ9Z Second Microphone https://amzn.to/2ObKMeA XLR Cable https://amzn.to/3rBL8ZB Studio Headphones https://amzn.to/3u082LE Laptophttps://amzn.to/3dhfafT Our webcam https://amzn.to/31uUefQ ABOUT THE HOSTS Dave Elliott Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK. Rob and Dave are both seeing patients every day in their clinics and in this episode they take a look at how things have changed over the last 10 years They share the learning that has changed how they work with their patients and discuss the vital importance of movement and how evolving to work with patients as someone who is supporting that individual's journey to recovery, not the hero clinician builds positive and productive relationships Listen in to hear Rob and Dave reflect on how their practice has evolved and why it's always important to stay on top of new developments if you want to be a great clinician KEY TAKEAWAYS It's important to take a considered approach and look at emerging clinical research and trends. It's updating, evolving, and staying on top of it Worry and anxiety about your posture is only going to potentially exacerbate your back pain It's not the specific exercises that make a difference it's about keeping the movement going We start with the basics of good quality movement and then fine tune Some movement is important it doesn't have to be a specific granular program Conversations in the clinic have changed from ‘I'm the therapist here to save you' The clinician is no longer the hero fixing you, the patient is now the hero with the clinic providing a helping hand It's not about waiting for someone else to do it for you, it's about taking responsibility for yourself and your recovery supported by the clinician You will have a much better long-term relationship if you are happy to give the praise away to the patient Having difficult conversations is a skill that needs to be developed Talking to patients about their fears is a crucial element in building a relationship and understanding the patient BEST MOMENTS ‘We can't be blinkered we need to be looking for the newest research and turning points in the science ‘When they are as knowledgeable as possible that's when the magic happens' ‘I understand now the power of better movement and communication' RESOURCES FROM THIS EPISODE https://podcasts.apple.com/gb/podcast/what-is-cauda-equina-syndrome-rob-tyer/id1511515312?i=1000506119396 https://www.cpdo.net/Lederman_The_fall_of_the_postural-structural-biomechanical_model.pdf THE BACK PAIN PODCAST PROVIDER MAP - FINDING SOMEONE TO HELP YOU WITH YOUR BACK PAIN https://thebackpainpodcast.com/index.php/members-map/ VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast website The Back Pain Podcast recommended products affiliate link Our Rode Mixer https://amzn.to/3waU8bx Our Microphones https://amzn.to/3rzSZ9Z Second Microphone https://amzn.to/2ObKMeA XLR Cable https://amzn.to/3rBL8ZB Studio Headphones https://amzn.to/3u082LE Laptophttps://amzn.to/3dhfafT Our webcam https://amzn.to/31uUefQ ABOUT THE HOSTS Dave Elliott Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK. Rob and Dave are looking at some of the most frequently asked questions from the communities they work with and in this episode share great content in answer to a question on the difference between chronic and acute They also discuss what Kinesio tape and foam rolling actually do and what part of the routine where Kinesio tape and foam rollers are used is really important, listen in and hear the answers KEY TAKEAWAYS Acute and chronic are used to refer to the time frame, the amount of time you have had the pain Chronic refers to pain for over 3 months anything under that is described as acute The skill of any practitioner is going from medical language to patient language Part of our journey as healthcare practitioners is understanding the impact of the language we use Tape changes the sensation from a painful area to a non-painful one giving your brain something else to focus on There are lots of claims about what foam rollers can do but they are in reality a way to self-massage The action of getting up and moving to complete physical actions is what's important Warming up is a vital part of the exercise you are Walking increases blood flow far more than rolling around on a Victorian torture device BEST MOMENTS ‘It's elasticated tape at the end of the day, it doesn't do anything magical ‘I will warm up for the session doing squats, I won't spend twenty minutes flapping around on a foam roller and getting into weird positions' ‘Never do anything that actively causes pain' THE BACK PAIN PODCAST PROVIDER MAP - FINDING SOMEONE TO HELP YOU WITH YOUR BACK PAIN https://thebackpainpodcast.com/index.php/members-map/ VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast website The Back Pain Podcast recommended products affiliate link Our Rode Mixer https://amzn.to/3waU8bx Our Microphones https://amzn.to/3rzSZ9Z Second Microphone https://amzn.to/2ObKMeA XLR Cable https://amzn.to/3rBL8ZB Studio Headphones https://amzn.to/3u082LE Laptophttps://amzn.to/3dhfafT Our webcam https://amzn.to/31uUefQ ABOUT THE HOSTS Dave Elliott Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK. Rob and Dave talk with Martin Christensen, a physiotherapist who joins them from Norway Martin has gained a huge following for his Friday Physio Confessions and in this episode talks about the importance of building relationships and discusses why it's vital that patients understand that physios are human and like all humans will make mistakes. If you want to know what a good clinician and patient relationship can be like, listen in and find out more KEY TAKEAWAYS A good healthcare professional makes the experience of the time matter more A patient should feel listened to and that their concerns are taken seriously There needs to be a rapport between the patient and the healthcare professional The healthcare professional and patient need to communicate effectively and honestly about what the goals are Asking the right questions is a crucial element in building rapport A great practitioner can bridge the gap and build rapport with any patient, making the patient feel comfortable by adapting to them and meeting them on their individual level We need to acknowledge that professionals are not machines but human Because practitioners are human, like all humans they will make mistakes Being authentic and being honest is about sometimes saying we need to wait and see Building a relationship with a patient means it's possible to say when you make a mistake BEST MOMENTS ‘It's the dynamics of the relationship between two people, getting along and finding out what works' ‘A good healthcare professional is someone who treats you like a person' THE BACK PAIN PODCAST PROVIDER MAP - FINDING SOMEONE TO HELP YOU WITH YOUR BACK PAIN https://thebackpainpodcast.com/index.php/members-map/ VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast website The Back Pain Podcast recommended products affiliate link Our Rode Mixer https://amzn.to/3waU8bx Our Microphones https://amzn.to/3rzSZ9Z Second Microphone https://amzn.to/2ObKMeA XLR Cable https://amzn.to/3rBL8ZB Studio Headphones https://amzn.to/3u082LE Laptophttps://amzn.to/3dhfafT Our webcam https://amzn.to/31uUefQ RESOURCES FOR THIS EPISODE Friday Physio Confessions - @Martin_nekkolai ABOUT THE HOSTS Dave Elliott Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK. Dave and Rob are joined by Adam Dobson regular on the podcast and Annina Schmid, physiotherapist and neuroscience researcher who leads the Neuromusculoskeletal Health and Science Lab at the University of Oxford. They discuss in detail radiculopathy, what it is and how it is diagnosed. With the neurological examination a key clinical diagnosis this is a great opportunity to hear about how Annina effectively uses sensory examination and what the results mean. Learn about radiculopathy and the sensory exam with great content in this informative episode KEY TAKEAWAYS Nerve related pain is usually a radiating pain, not a pain that can be pinpointed to one spot Always compare left and right talking to the patient about how the affected side feels compared to the other side We need to establish a normative area, you cannot just compare left and right because you may miss a sensory deficit Test sensation using something neutral like tissue or cotton wool The key to neurological examination is understanding single items in isolation have no relevant value it's putting together the pieces You have to make the pieces fit together to understand the picture Studies use the reference testing of MRI imaging or inter surgical findings which don't tell us anything about the loss of function, the definition of radiculopathy Radiculopathy is a clinical diagnosis that includes the neurological examination The reference standard should be the clinical diagnosis because that's what radiculopathy is For a nerve to be affected it doesn't need something to be directly encroaching or deforming it We are imaging macro structure but a nerve can be affected on the inside so we should be looking further are the inside of the nerve Severe motor deficit or progressive motor deficit will be a referral We do not need to be specialists but we do need an awareness of what can masquerade as radiculopathy and refer to the right professional BEST MOMENTS ‘If the patient has lower back pain and it radiates into the buttock it's enough for me to do an examination' ‘With radiculopathy, the reference standard should be the neurological examination' ‘If there is a pain and it radiates this would trigger a neurological examination' THE BACK PAIN PODCAST PROVIDER MAP - FINDING SOMEONE TO HELP YOU WITH YOUR BACK PAIN https://thebackpainpodcast.com/index.php/members-map/ VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast website The Back Pain Podcast recommended products affiliate link Our Rode Mixer https://amzn.to/3waU8bx Our Microphones https://amzn.to/3rzSZ9Z Second Microphone https://amzn.to/2ObKMeA XLR Cable https://amzn.to/3rBL8ZB Studio Headphones https://amzn.to/3u082LE Laptophttps://amzn.to/3dhfafT Our webcam https://amzn.to/31uUefQ RESOURCES FOR THIS EPISODE Annina Twitter https://twitter.com/AnninaBSchmid Annina Website http://www.neuro-research.ch/ ABOUT THE HOSTS Dave Elliott Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK. Dave is going solo and shares the advice and reminders that everyone venturing back out into the garden really needs to hear because it's this time of the year that many clinicians are seeing a very specific patient – the gardener There's lots you can do from strength work throughout the year through to changing it up with different tasks and knowing when to stop As the sun starts to shine listening to this is a great way to get back into the garden the best way- without back pain KEY TAKEAWAYS It's a seasonal activity so not doing anything and then doing lots can be challenging Gardening doesn't have a graded exposure mentality Lightweight activities completed over an extended time period may not be ideal Gardening is weather dependant and this can lead to excessive activity load in a short time frame There is a steep exposure when you get back into gardening in the spring Repetitive exercise over the year can help as graded exposure activity making you better prepared for gardening Switch up your tasks to break up long term exposure, break the activities down into manageable chunks Set yourself specific targets and monitor how you feel building before extending the task target BEST MOMENTS ‘That 20kg bag of compost still weighs 20kg and you may not have been maintaining your 20kg lifting muscles over the winter ‘Stay active and strong throughout the year' Don't fall into the ‘just a little bit more trap' THE BACK PAIN PODCAST PROVIDER MAP - FINDING SOMEONE TO HELP YOU WITH YOUR BACK PAIN https://thebackpainpodcast.com/index.php/members-map/ VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast website The Back Pain Podcast recommended products affiliate link Our Rode Mixer https://amzn.to/3waU8bx Our Microphones https://amzn.to/3rzSZ9Z Second Microphone https://amzn.to/2ObKMeA XLR Cable https://amzn.to/3rBL8ZB Studio Headphones https://amzn.to/3u082LE Laptophttps://amzn.to/3dhfafT Our webcam https://amzn.to/31uUefQ ABOUT THE HOSTS Dave Elliott Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK. Rob is a guest on the Knowledge Exchange podcast talking about misconnections and what good quality care looks like He discusses the power of the words and language used by practitioners and explains why he is an advocate for clear and simple communication with patients. Practitioners need to understand where patients are coming from and acknowledge their beliefs working with them to support them in achieving their goals. Being a high-level communicator and sharing strengths is crucial for success for any practitioner who is working with patients KEY TAKEAWAYS MRIs are a valuable tool the challenges are in the reporting An MRI shows everything, they need to be explained carefully and used in conjunction with your medical history and consultation Posture is ingrained in our culture but can be misunderstood The problem isn't the position you sit in it's the movement you make Postural devices don't help with posture Communication is key and we always want to communicate with patients in a clear and easily understood way A summary at the end of consultation can really help to make sure the patient leaves with the right information and understanding Acknowledge the patient's beliefs and work with them to change the narrative Building trust with the patient and understanding their goals and beliefs is crucial As healthcare professionals, we have a responsibility to make sure we are sharing correct and factual information We have the opportunity to educate and inform people, we are primarily patient-facing and want to put out clear simple information for people We want patients to know what to expect when they meet with their healthcare practitioner Having groups means we can understand more about what people are going through and is the opportunity to share and signpost potential support Passive care is when something is done to the patient and active care is something the patient does for themselves BEST MOMENTS ‘MRIs don't cause back pain, it's about the reporting ‘For patients, it's their story' ‘You have to meet patients where they are at and begin from there' THE BACK PAIN PODCAST PROVIDER MAP - FINDING SOMEONE TO HELP YOU WITH YOUR BACK PAIN https://thebackpainpodcast.com/index.php/members-map/ VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast website The Back Pain Podcast recommended products affiliate link Our Rode Mixer https://amzn.to/3waU8bx Our Microphones https://amzn.to/3rzSZ9Z Second Microphone https://amzn.to/2ObKMeA XLR Cable https://amzn.to/3rBL8ZB Studio Headphones https://amzn.to/3u082LE Laptophttps://amzn.to/3dhfafT Our webcam https://amzn.to/31uUefQ RESOURCES FOR THIS EPISODE The Knowledge Exchange Podcast ABOUT THE HOSTS Dave Elliott Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK. As they reach the milestone hundredth episode of The Back Pain Podcast, hosts Dave and Rob reflect on their guests and how the knowledge they have shared has impacted their own learning and professional practice They look back at some of their favourite and most significant episodes and share some great moments of advice and learning that will help anyone to understand more about mindset, the brain and how it perceives pain, cumulative load, modulation, and the journey to recovery. Listen in, celebrate, learn, and hear about Dave and Robs exciting plans for the future of The Back Pain Podcast as it moves forward KEY TAKEAWAYS Communication is a critical element and Jared Hall talked about being able to talk to people using the right analogies to help them understand on their level All of our job is about communication and being able to explain things in a way that's easily understood is vital Nick Metcalf, talked about moderation of activity based on stressors in the previous days. You can effectively use modulation in your everyday life and adapt to take into account changes Julia Gover talked about pain, why things hurt and how pain is related to threat and shared a great analogy about snakes, sticks and how the brain processes pain The brain uses past experiences and can both increase and dampen experiences of pain but pain doesn't mean you have to stop and do nothing Tom Jesson, physio, and sciatica expert explained how nerves can take time to heal and become less irritated. Recovery is not a linear journey, think about your recovery on a week to week basis Frequent guest Adam Dobson talked in detail about the value of MRIs when they are used in conjunction with your medical history and consultation The skill with MRIs is the correct reporting and clear explanation to the patient Learning a better language to explain back pain can make a significant impact on the patient Donald Robertson discussed the ways your mindset plays a pivotal role in pain and how you perceive and manage it BEST MOMENTS ‘This podcast over the last two years has probably changed my practice more so than anything else I've done in the last ten years' ‘Little things add up and a sudden change or spike in load can be a trigger for pain or injury ‘It's recognising the cumulative load and understanding the factors that are impacting' ‘At the end of a consultation, a recap can be really helpful for the client' THE BACK PAIN PODCAST PROVIDER MAP - FINDING SOMEONE TO HELP YOU WITH YOUR BACK PAIN https://thebackpainpodcast.com/index.php/members-map/ VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast website The Back Pain Podcast recommended products affiliate link Our Rode Mixer https://amzn.to/3waU8bx Our Microphones https://amzn.to/3rzSZ9Z Second Microphone https://amzn.to/2ObKMeA XLR Cable https://amzn.to/3rBL8ZB Studio Headphones https://amzn.to/3u082LE Laptophttps://amzn.to/3dhfafT Our webcam https://amzn.to/31uUefQ RESOURCES FOR THIS EPISODE Episode 89 Sticks and Stones, How Language and Words Can influence your Pain- Jarod Hall Episode 65 - Why be Resilient, When You Can be Anti-fragile Episode 53 – The Biggest Myth in Back Pain -Julia Gover Episode 40 - What is Cauda Equina? Dave explains why it's important that your recovery is not just measured by an improvement in your pain levels but also by progress in the function and movement you want to achieve from being able to run a marathon through to being able to pick up your child Pain maybe the tipping point that led to you seeking intervention but your ‘why' the motivation and goals you have are crucial in your journey to recovery and the function and movement that matters to you KEY TAKEAWAYS Understanding the motivation of the patient and their goals for health and movement is vital Everyone wants to get out of their pain Pain is a major motivator – no one wants to be in pain and its often the first thing that's mentioned What function and movement do you want to have when you are pain-free? Pain is easily measured and quantified as you progress but shouldn't be the only measure Pain and recovery are not linear so if pain is the only measure seeing progress clearly can be challenging Pain may remain in the background and is very subjective. It's felt differently depending on many variables Let your clinician know your goals and the activities you want to return to Function goals can be hugely positive in the journey of recovery Additional data about what you want to do means your rehabilitation plan can be very specific BEST MOMENTS ‘A real and personal goal can be the motivator you need' ‘Pain can be the tipping point ‘ THE BACK PAIN PODCAST PROVIDER MAP - FINDING SOMEONE TO HELP YOU WITH YOUR BACK PAIN https://thebackpainpodcast.com/index.php/members-map/ VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast website The Back Pain Podcast recommended products affiliate link Our Rode Mixer https://amzn.to/3waU8bx Our Microphones https://amzn.to/3rzSZ9Z Second Microphone https://amzn.to/2ObKMeA XLR Cable https://amzn.to/3rBL8ZB Studio Headphones https://amzn.to/3u082LE Laptophttps://amzn.to/3dhfafT Our webcam https://amzn.to/31uUefQ ABOUT THE HOSTS Dave Elliott Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK. It's a sign your migraine is about to happen The typical migraine headache is one-sided, unilateral, and generally over the eye and side of the head When experiencing a migraine headache, it is difficult to function Activation of the nervous system causes an expression of a neuropeptide and the protein creates a sensitivity in part of the brain Triptans stop the pain of the headache because they stop the dilation of blood vessels if taken in time CGRP blocks the neuropeptide and can stop the headache from beginning TAC headaches are always on one side and the extreme pain can result in people feeling very agitated The hypothalamus is involved in cluster headaches so the headaches can often happen seasonally at the same time of the day BEST MOMENTS ‘Visual auras are very common' ‘Most people will need to find a quiet dark room and wait for the storm to pass' ‘When you have experienced an aura repeatedly it becomes a familiar sign that a migraine is beginning' THE BACK PAIN PODCAST PROVIDER MAP - FINDING SOMEONE TO HELP YOU WITH YOUR BACK PAIN https://thebackpainpodcast.com/index.php/members-map/ VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast website The Back Pain Podcast recommended products affiliate link Our Rode Mixer https://amzn.to/3waU8bx Our Microphones https://amzn.to/3rzSZ9Z Second Microphone https://amzn.to/2ObKMeA XLR Cable https://amzn.to/3rBL8ZB Studio Headphones https://amzn.to/3u082LE Laptophttps://amzn.to/3dhfafT Our webcam https://amzn.to/31uUefQ RESOURCES FROM THIS EPISODE Watford Osteopaths ABOUT THE HOSTS Dave Elliott Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK. In this episode, Dr Louis Howe researcher and strength and conditioning coach discusses lumbar spine flexion and getting out of neutral He explains the current research and how in real life spinal flexion can't be stopped but can be managed with the right strategies to avoid end range motion This is a fascinating episode for both professional practitioners and individuals who want to understand more about spinal flexion and what getting out of neutral really means KEY TAKEAWAYS Neutral alignment is seen as being optimal in sharing the load The research indicates that the spine flexes away from the neutral posture Whilst flexion can't be stopped there are strategies that can be used to avoid end range motion and lower or increase those values When you perform lifting you are flexing and moving out of neutral We see more flexion occurring at the lower lumbar spine than at the upper lumbar spine When you are lifting the spine is both flexing and being exposed to compressive load Flexion has the potential to result in pathologies of the spine All of the tissues in our bodies can adapt to loading There is potential for managing the position of the spine and affecting injury risk Avoiding end range position and flexion can be important in training Coaching can encourage kinesthetics awareness to help avoid end range positions Programming strategies and the way overload is created in exercise is valuable as an injury risk management tool Years of strength training will bring architectural changes and spinal flexion can put muscles in a better position for lifting BEST MOMENTS ‘The concept of neutral is attractive to practitioners because it puts the spine in a position where the load is spread' ‘It's a variable that we can manipulate' ‘When they perform the tasks they do it in such a way that they are able to cope with the loads they are exposed to' THE BACK PAIN PODCAST PROVIDER MAP - FINDING SOMEONE TO HELP YOU WITH YOUR BACK PAIN https://thebackpainpodcast.com/index.php/members-map/ VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast website The Back Pain Podcast recommended products affiliate link Our Rode Mixer https://amzn.to/3waU8bx Our Microphones https://amzn.to/3rzSZ9Z Second Microphone https://amzn.to/2ObKMeA XLR Cable https://amzn.to/3rBL8ZB Studio Headphones https://amzn.to/3u082LE Laptophttps://amzn.to/3dhfafT Our webcam https://amzn.to/31uUefQ RESOURCES FROM THIS EPISODE Getting Out of Neutral: the risks and rewards of lumbar spine flexion during lifting exercises - Louis Howe with Greg Lehman ABOUT THE HOSTS Dave Elliott Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK. Dave talks with Tracey ‘The Sleep Coach' about what leads to poor sleep and insomnia along with how you can change things to achieve good sleep A good night's sleep is the foundation for feeling good and being able to do what you want to do during the day so good sleep is always important Tracey shares practical tips on what you can do to optimise your sleep so that the sleep you get during the night is ‘a good night's sleep' making you feel good and able to do what you need and want to do during the day KEY TAKEAWAYS Sleep is very individual; some people require more sleep than others Optimise your sleep by creating the right environment There is no need to try to ‘catch up' on missed sleep It's important to keep doing the good things in life to take your attention away from any sleep issues A relaxed approach to sleep is the best way to achieve it An uncluttered room, a comfortable bed, a cooler environment, and a good wind-down routine are the starting points for good sleep Good sleepers can't tell you how they sleep they just go to bed and sleep well Tracking sleep patterns only gives you information to hang anxiety on so stop clock watching your sleep and ditch the wearables There is not one position that is preferable for sleeping, it's very individual and depends on several factors Sleep is an emotional process as well as a physical one and it's not always linear Sometimes we lose our connection or get in the way of it but you can reconnect with your natural ability to sleep it's something we never lose CBTi gives you a set of skills mental, emotional, and behavioural that you can use for the rest of your life BEST MOMENTS ‘When you stop worrying about sleep it lowers your arousal level and you will experience better sleep' ‘There isn't a return for trying to get more sleep than you need‘ ‘Accepting that you may not sleep well takes the anxiety away' THE BACK PAIN PODCAST PROVIDER MAP - FINDING SOMEONE TO HELP YOU WITH YOUR BACK PAIN https://thebackpainpodcast.com/index.php/members-map/ VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast website The Back Pain Podcast recommended products affiliate link Our Rode Mixer https://amzn.to/3waU8bx Our Microphones https://amzn.to/3rzSZ9Z Second Microphone https://amzn.to/2ObKMeA XLR Cable https://amzn.to/3rBL8ZB Studio Headphones https://amzn.to/3u082LE Laptophttps://amzn.to/3dhfafT Our webcam https://amzn.to/31uUefQ RESOURCES FOR THIS EPISODE Tracey The Sleep Coach Tracy The Sleep Coach Instagram Sound Sleep Strategies Group Facebook ABOUT THE HOSTS Dave Elliott Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK. Rob and Dave are sharing invaluable content today as they explain some common imaging findings and how they would discuss them with their own patients Having someone explain your scan to you is really important because, as Rob and Dave explain any diagnosis is made from a combining of the information from your medical history, your physical examination alongside any imaging There are normal age-related changes that happen to everyone and the skill of the clinician is working out which changes are normal and what may be contributing to or causing your pain, listen in and find out more KEY TAKEAWAYS The diagnosis your clinician makes is based on a combination of your medical history, a physical examination and this is then combined with your imaging An MRI, X-ray or CT scan can't see your pain, they are a snapshot of how you look at that point in time Back pain is very common without there being any changes on a scan Interventions can be about modulating and reducing the pain If your scan comes back as normal it doesn't mean you don't have pain It's a win and a great starting block if your scan comes back as normal Normal age-related changes (NARC) happen to everyone and are expected over time If you've been told you have a degenerative disc disorder it doesn't mean your pain will get worse Focusing on incidental findings can mean focusing on the wrong areas, and not achieving the proper outcome Changes happen to everyone with or without pain. It's working out if the changes are relevant or irrelevant Disc bulges can occur without pain and often it's the location that's more important Disc bulges happen very slowly over time and the disc doesn't have to go back in for the pain to go away A scan is just a snapshot in time to be used alongside other information to discover the likely cause of your pain BEST MOMENTS ‘The job of a good clinician is to describe the changes and explain them to you' ‘A scan is another tool in the arsenal to finally land on the conclusion' ‘There are often words in reports that can make you think the worst when not understood in the right context' THE BACK PAIN PODCAST PROVIDER MAP - FINDING SOMEONE TO HELP YOU WITH YOUR BACK PAIN https://thebackpainpodcast.com/index.php/members-map/ VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast website The Back Pain Podcast recommended products affiliate link Our Rode Mixer https://amzn.to/3waU8bx Our Microphones https://amzn.to/3rzSZ9Z Second Microphone https://amzn.to/2ObKMeA XLR Cable https://amzn.to/3rBL8ZB Studio Headphones https://amzn.to/3u082LE Laptophttps://amzn.to/3dhfafT Our webcam https://amzn.to/31uUefQ ABOUT THE HOSTS Dave Elliott Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK. Should I stop bending over? We are taught early on about how bending or lifting must be done in a particular way to protect our backs but is there a right way to do it and is bending a trigger for back pain? In this episode, Rob shares great content and the answers to the questions people are always asking about bending and its connection to stress and pain. He also explains how to build and prepare your body to help it adapt, listen in, and find out more KEY TAKEAWAYS People often blame bending as a trigger for their back pain Your back can be hurt in the same way as any other part of your body It can be easy to become fearful about hurting your back There are always many factors to consider if you have back pain There is not a correct way to lift, although there may be an optimum Apply the rule of twos - Are you lifting too much too often after lifting too little for too long Bodies are amazing at adapting to stresses if they are prepared Stress is how the body grows and adapts It's all about building your body to cope with increased load BEST MOMENTS ‘Our bodies really don't like sudden change' ‘If you never bend from your back you can't expect to be great at bending from your back, you have to prepare' THE BACK PAIN PODCAST PROVIDER MAP - FINDING SOMEONE TO HELP YOU WITH YOUR BACK PAIN https://thebackpainpodcast.com/index.php/members-map/ VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast website The Back Pain Podcast recommended products affiliate link Our Rode Mixer https://amzn.to/3waU8bx Our Microphones https://amzn.to/3rzSZ9Z Second Microphone https://amzn.to/2ObKMeA XLR Cable https://amzn.to/3rBL8ZB Studio Headphones https://amzn.to/3u082LE Laptophttps://amzn.to/3dhfafT Our webcam https://amzn.to/31uUefQ ABOUT THE HOSTS Dave Elliott Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK. Dave and Rob discuss a topic that reoccurs often in their Facebook community – MRI scans when to have them when they are not needed and using the right language to discuss them This episode is a great reminder about the importance of explanation and the right language around MRI's and how they are an amazing tool but only one piece of the puzzle when it comes to the diagnosis of back pain. Listen in and hear great informative information KEY TAKEAWAYS Patients can often expect that an MRI is going to show them where their pain is An MRI shows everything that is happening under your skin A clear and skilful explanation of an MRI is vital Language is really important when talking to patients about their pathway to recovery Changes on your spine seen on an MRI do not corollate with pain and your spine deteriorating over time There are a thousand different reasons why back pain can flare up An MRI is looked at along with your medical history, pain, examination, and all other information An MRI is just another piece of the puzzle It's a very good tool but not something that needs to be used as the first step with back pain unless there are red flags BEST MOMENTS ‘An MRI works really well when used with the right patient at the right time' ‘MRIs are seriously cool and some of the best diagnostic imaging we have' THE BACK PAIN PODCAST PROVIDER MAP - FINDING SOMEONE TO HELP YOU WITH YOUR BACK PAIN https://thebackpainpodcast.com/index.php/members-map/ VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast website The Back Pain Podcast recommended products affiliate link Our Rode Mixer https://amzn.to/3waU8bx Our Microphones https://amzn.to/3rzSZ9Z Second Microphone https://amzn.to/2ObKMeA XLR Cable https://amzn.to/3rBL8ZB Studio Headphones https://amzn.to/3u082LE Laptophttps://amzn.to/3dhfafT Our webcam https://amzn.to/31uUefQ ABOUT THE HOSTS Dave Elliott Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK. In this ‘ask us anything' episode hear about pelvic tilt, asymmetry, and the answers to whether massage guns help and if core bracing is beneficial Rob shares great content and clear explanations about what actions will make a difference and discusses the importance of movement in this informative episode KEY TAKEAWAYS Pelvic tilt is commonly blamed by people for their pain Asymmetry is perfectly normal in the human body and doesn't correlate with increased pain The body adapts best to being outside of neutral comfort zones Our bodies are designed to bend, lift, stretch and twist moving beyond the neutral zone The back benefits from load and this can reduce pain Massage guns help you to undertake soft tissue work Soft tissue work can help reduce pain for some people changing the input from a painful area of the brain If you want to increase blood flow to an area moving is the most effective way to achieve this is by moving Often core bracing is used in the short term Bracing for the first few days is fine but you should be exploring movement Often patients with back pain think they have a weak core but in reality, patients with back pain have an overactive core Deep breathing and movement can help with relaxing the core BEST MOMENTS ‘Sometimes bracing your core can help you to initially achieve a movement with less pain' ‘What people have been avoiding can be the key to unlocking recovery and improvement' THE BACK PAIN PODCAST PROVIDER MAP - FINDING SOMEONE TO HELP YOU WITH YOUR BACK PAIN https://thebackpainpodcast.com/index.php/members-map/ VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast website The Back Pain Podcast recommended products affiliate link Our Rode Mixer https://amzn.to/3waU8bx Our Microphones https://amzn.to/3rzSZ9Z Second Microphone https://amzn.to/2ObKMeA XLR Cable https://amzn.to/3rBL8ZB Studio Headphones https://amzn.to/3u082LE Laptophttps://amzn.to/3dhfafT Our webcam https://amzn.to/31uUefQ RESOURCES FOR THIS EPISODE Adam Meakins Blog - Leg Length Differences https://www.thesports.physio/the-long-and-short-of-leg-length-differences/ Mehmet Gem - Twitter - LLD and Pelvic Tilts https://twitter.com/thehipphysio/status/1483178257319174145?s=21 Greg Lehman - Core Stability Episode https://podcasts.apple.com/gb/podcast/the-back-pain-podcast/id1511515312?i=1000500403667 ABOUT THE HOSTS Dave Elliott Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK. Communication isn't just words, it affects the way we think and feel and in this, not to be missed, episode Jarod Hall, physical therapist, and co-author of ‘Making Sense of Pain' talks with Dave and Rob about the importance of communication and the way it impacts on us all differently according to our personal history, experiences and beliefs He shares great content for clinicians about the ways to explain injuries and symptoms effectively and empathetically, along with great advice for patients about understanding more about their pain and personal journey of recovery. KEY TAKEAWAYS Human beings are a social species and we communicate both verbally and in other ways When we use language we convey a lot of information Language and the way we speak to people has a huge impact on the way they feel Your physiology can change according to the words being used towards you You should communicate in a way that is helpful, not harmful Your thoughts affect your physiology and what people say to you affects the way your body functions and in turn the way you feel Exposure to painful stimuli and building over time with graded exposure can lessen the pain Clinicians need varied anecdotes to help different patients understand the processes of recovery Everyone is different and every interaction between clinician and patient means each person is bringing their experience and history and expectations to that individual situation It's about working out the best way to interact with each other in the intersubjective space to achieve the goals Clinicians need to be able to assess when people are ready to hear what they have to say It doesn't matter how great your stories are if you don't have an open and respectful dialogue You need to ask permission to share your explanations as a clinician BEST MOMENTS ‘When we communicate with someone in pain we need to be even more aware of the language we use' ‘As we age we all get wrinkles on the inside' ‘His book looks at many of the analogies and stories surrounding pain and how the language used impacts' THE BACK PAIN PODCAST PROVIDER MAP - FINDING SOMEONE TO HELP YOU WITH YOUR BACK PAIN https://thebackpainpodcast.com/index.php/members-map/ VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast website The Back Pain Podcast recommended products affiliate link Our Rode Mixer https://amzn.to/3waU8bx Our Microphones https://amzn.to/3rzSZ9Z Second Microphone https://amzn.to/2ObKMeA XLR Cable https://amzn.to/3rBL8ZB Studio Headphones https://amzn.to/3u082LE Laptophttps://amzn.to/3dhfafT Our webcam https://amzn.to/31uUefQ RESOURCES FOR THIS EPISODE Jarod's Book - Making Sense of Pain (Sticks and Stones) https://amzn.to/3JKSjc0 Jarod Instagram https://www.instagram.com/drjarodhalldpt/?hl=en Modern Pain Care https://modernpaincare.com/?doing_wp_cron=1641367942.7520160675048828125000 ABOUT THE HOSTS Dave Elliott Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK. Rob and Dave take a look at the question of the best sleep position to avoid back pain and explain what everyone can do to get the best quality of sleep Ultimately good quality sleep plays an important role in any recovery so this should be the focus, getting the sleep is always more important than how you sleep, listen in and hear more KEY TAKEAWAYS The best position to sleep in is the one that doesn't cause you pain There is not a best position to sleep in to avoid back pain long term There is no evidence that adopting a specific position will help to improve back pain A big factor in any recovery is the quality of sleep You don't often wake up in the same position as when you went to sleep because you move during the night If a position causes you discomfort then changing it for a short period can help A big factor in recovery is good quality sleep It's all dependant on you and what's comfortable for you as an individual Clinicians should be helping people to self-manage Your preferences may change over time in the same way your anatomy will change over time If you are thinking about a new mattress try out firm and soft toppers to find what works for you BEST MOMENTS ‘Whatever makes you comfortable is the position for you' ‘Sleep is more important than how you sleep' ‘Find what's comfortable for you whether it's firm, soft, hay, straw or rocks!' Rob and Dave continue their discussion around what they talk most about with patients on a daily basis. In this episode, they bust the myths and misconceptions about whether your back is fragile and weak and look at the narrative around ‘putting your back out' They share great information and content in a way that is easy to understand and explain how they work with their clients towards successful recovery building strength, resilience and adaptability KEY TAKEAWAYS Your back isn't fragile, the back is one of the strongest parts of the body Pain back can be some of the worst pain experienced so people become fearful of hurting their backs Our backs put up with a huge amount of stress on a daily basis The type of stress we put our backs through is a good thing The stress we place on our bodies is how we grow and get stronger, our muscles and joints require stress and work to grow Your back will become weak if you don't use it at all because you won't be exposing it to anything new Exposure to load is a really important part of recovery Breaking down a movement that is a trigger for back pain into small parts is part of effective recovery You gradually expose the spine and body combining the movements and adding load and as a result, get stronger Your back and body are inherently strong and adaptable if you put gradual and increasing stress on them False beliefs and misconceptions can mean that people become afraid of doing certain movements because they think it will put their back out Clinicians and therapists should be helping clients to improve their resilience and helping them to achieve improvement BEST MOMENTS ‘We are strong, resilient and adaptable and put up with stresses very well on a day to day basis' ‘The stress and work we go through is what helps our backs to recover from injury' ‘It's become a common narrative in our culture to see the back as being weak' THE BACK PAIN PODCAST PROVIDER MAP - FINDING SOMEONE TO HELP YOU WITH YOUR BACK PAIN https://thebackpainpodcast.com/index.php/members-map/ VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast website The Back Pain Podcast recommended products affiliate link Our Rode Mixer https://amzn.to/3waU8bx Our Microphones https://amzn.to/3rzSZ9Z Second Microphone https://amzn.to/2ObKMeA XLR Cable https://amzn.to/3rBL8ZB Studio Headphones https://amzn.to/3u082LE Laptophttps://amzn.to/3dhfafT Our webcam https://amzn.to/31uUefQ ABOUT THE HOSTS Dave Elliott Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK. Join your host Rob and Laura Rathbone, a pain specialist and clinician who in this episode talks about what it's like to have chronic pain and how Acceptance and Commitment therapy can play an important role in being able to move forward. There isn't universal biology for pain, it's a nuanced experience created in the individual. Pain has psychological experiences within it along with biological processes and physical experiences. Acceptance and Commitment therapy offers an approach that is focused on acknowledging the struggle and looking at changing a dimension of the experience. Listen in and learn more to find out if it may be the approach you need to help with your pain KEY TAKEAWAYS You recognise the expertise of the person experiencing pain, they know their pain It's about looking at the complete landscape of evidence to see if there is anything that can be used to help them with their pain The first conversation is a discovery session and beginning the process of working collaboratively Acceptance and Commitment therapy is developed as a third way of CBT and is a compassion-focused process-based therapy ACT is about acknowledging the struggle and looking at changing a dimension of the experience Pain has psychological experiences within it along with biological processes and physical experiences ACT has a structure and uses a person's ability to self-observe their present moment, recognise where there are obstacles and take action We are using ACT to see if there is a way to create room for change It's focusing on the principle of rehabilitation and moving flexibly with what life offers and staying connected to what's important ACT is inviting people on a journey of exploration setting their own boundaries as to how they want to live well with the experience of pain and understanding that the pain may remain It's about being willing to have the feeling to get to the stuff you want ACT is about trying to stay true to someone's value-based action and stay in that moment of turning towards it An ACT informed approach to physical therapy is using the principle of rehabilitation in the moment with the added skill of managing the emotional distress BEST MOMENTS ‘They know their pain more than I can ever know their pain' ‘I'm an MSK trained therapist looking to use psychological perspectives and skillsets within my therapy' ‘Pain is changeable it's just by how much and how it is done' THE BACK PAIN PODCAST PROVIDER MAP - FINDING SOMEONE TO HELP YOU WITH YOUR BACK PAIN https://thebackpainpodcast.com/index.php/members-map/ VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast website The Back Pain Podcast recommended products affiliate link Our Rode Mixer https://amzn.to/3waU8bx Our Microphones https://amzn.to/3rzSZ9Z Second Microphone https://amzn.to/2ObKMeA XLR Cable https://amzn.to/3rBL8ZB Studio Headphones https://amzn.to/3u082LE Laptophttps://amzn.to/3dhfafT Our webcam https://amzn.to/31uUefQ RESOURCES FROM THIS EPISODE Laura Rathbone website Pain Geeks ABOUT THE HOSTS Dave Elliott Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK. Dave and Rob are joined again by specialist back pain physiotherapist, Adam Dobson and are discussing spinal stenosis They talk about what lumber spinal stenosis is and why for some it creates no problems and for others, it will mean treatment to alleviate symptoms and improve quality of life Adam shares great information and advice on what to do if it does cause you problems and explains how exercise therapy and surgical decompression are used based on clinical assessment. This is a must-listen with great answers for people with lumbar spinal stenosis KEY TAKEAWAYS Lumbar spinal stenosis refers to the lower back. The symptoms are defined as being positionally influenced or exertionally influenced Three-quarters of people over the age of forty are expected to have some degree of spinal space narrowing 11% of the population present with these problems but it's not a sign of the body wearing out Some masquerading problems may not be related to the back at all Walking even with a degree of leg pain is not harmful if it's tolerable Bending the back for therapeutic effect should be encouraged There can be forms of general exercise, sleep and weight management that can help For most people, lumbar spinal stenosis is a long term problem The focus should be on quality of life function and exercise is a good first-line treatment It's not about trying to change the tunnel narrowing it's about influencing the biology, how things feel and inflammation around the nerve roots. If the pain is acceptable then it's fine to continue and working with a clinician can help to establish your personal baseline Surgery is often reserved for those with high degrees of disability and those who have already been through physiotherapy, it's rarely essential so preference plays an important role BEST MOMENTS ‘There will be softer signs for spinal stenosis' ‘Narrowing doesn't mean you will have problems' ‘I'm a big advocate of getting the back bending' ‘We are trying to make things feel better and improve the condition' THE BACK PAIN PODCAST PROVIDER MAP - FINDING SOMEONE TO HELP YOU WITH YOUR BACK PAIN https://thebackpainpodcast.com/index.php/members-map/ VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast website The Back Pain Podcast recommended products affiliate link Our Rode Mixer https://amzn.to/3waU8bx Our Microphones https://amzn.to/3rzSZ9Z Second Microphone https://amzn.to/2ObKMeA XLR Cable https://amzn.to/3rBL8ZB Studio Headphones https://amzn.to/3u082LE Laptophttps://amzn.to/3dhfafT Our webcam https://amzn.to/31uUefQ RESOURCES FROM THIS EPISODE Spinal Stenosis Booklet https://www.southtees.nhs.uk/content/uploads/Lumbar-Spinal-Stenosis-Booklet.pdf South Tees NHS patient information booklets https://www.southtees.nhs.uk/services/back-pain-triage-and-treat/patient-information-educational-resources/ Adams Twitter https://twitter.com/adamdobson123 ABOUT THE HOSTS Dave Elliott Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK. Rob talks with Diarmuid Denneny a lead physiotherapist, Chair of the Physiotherapist Pain Association, and member of the guideline development committee for the NICE chronic pain guidelines in an episode that shares great informative content about the topic that is currently very high profile – the recommendations around prescribing opioids for back pain. Diarmuid explains what the recommendations are and discusses with Rob the vital elements of a collaborative approach and informed decisions along with alternative strategies in the effective treatment of pain where it's all about starting small and building KEY TAKEAWAYS The NICE guidelines are evidence-based recommendations for health and care The advice is to not initiate a prescription for opioid medication for chronic pain Opioids have a place in the treatment of shorter-term pain and for end of life care It's about making sure that people who are taking opioids are fully aware of the potential side effects If someone is taking opioids at a low dose and is seeing benefits it's about a shared care plan and the individual being fully aware of the risks It's about informed choice and shared decision making with the prescribing professional There are many factors to consider and the conversation with the clinician at the beginning is key in a personalised approach It's important to consider alternative strategies It's always a collaborative approach between the patient and the prescribing clinician A range of strategies should be considered around the pain being experienced and what the individual wants to achieve There are factors that influence pain that are beyond your control such as the environment Pain is complex and there are elements we are aware of and many that we are less aware of and have less control over It's about medication helping with the management and augmentation of pain BEST MOMENTS ‘People expect that because they are called painkillers they will stop the pain but for most it's a reduction in pain' It's not about stopping opioids for everyone ‘It's starting small and building gradually THE BACK PAIN PODCAST PROVIDER MAP - FINDING SOMEONE TO HELP YOU WITH YOUR BACK PAIN https://thebackpainpodcast.com/index.php/members-map/ VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast website The Back Pain Podcast recommended products affiliate link Our Rode Mixer https://amzn.to/3waU8bx Our Microphones https://amzn.to/3rzSZ9Z Second Microphone https://amzn.to/2ObKMeA XLR Cable https://amzn.to/3rBL8ZB Studio Headphones https://amzn.to/3u082LE Laptophttps://amzn.to/3dhfafT Our webcam https://amzn.to/31uUefQ RESOURCES FROM THIS EPISODE Opioids Aware Live Well With Pain Footsteps Festival 2021 Facebook ABOUT THE HOSTS Dave Elliott Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK. Dr Mark Laslett is a specialist muscular-skeletal physiotherapist based in New Zealand with a particular interest in predicting the structure that is the source of pain. Adam Meakins is a physiotherapist and co-founder of the educational company ‘The Better Clinician Project' In this much-anticipated discussion they talk at length about low back pain, Adam's back injury, his recovery, deadlifting and spinal flexion along with lots, lots more This is an episode with great content that you will want to listen to more than once KEY TAKEAWAYS Our ability to assess and diagnose specific structures is challenging An injury with an acute phase, followed by easing off of pain and then the development of radicular pain syndrome is a common picture You manipulate the probability based on other factors Your diagnosis can change as you receive more information from the patient and as symptoms change The question is whether sometimes self-management should be augmented There could be another reason why someone has a mechanical restriction in a certain direction other than having a disc pathology I had differential diagnosis's that were changing in the acute stages with it becoming more certain as time evolved You can't get the extension back unless you correct the shift Using the mechanism of injury as a treatment is about strengthening those areas through exposure to the forces and stimuli that you will experience Flexion will temporarily reduce the amount of bulging but if you continue the bulging will increase When discs are hydrated there is a greater risk of herniation Flexion has not been seen consistently to be a causative factor to disc structure deterioration You cannot live a life without flexion and you have to restore it as part of your rehab BEST MOMENTS ‘The evidence shows that on average people are 75% better in 3 to 6 months' ‘You should start flexion when it's safe to do so' ‘Sometimes things need time just to calm down' ‘Sedentariness is a risk for disc deterioration' THE BACK PAIN PODCAST PROVIDER MAP - FINDING SOMEONE TO HELP YOU WITH YOUR BACK PAIN https://thebackpainpodcast.com/index.php/members-map/ VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast website The Back Pain Podcast recommended products affiliate link Our Rode Mixer https://amzn.to/3waU8bx Our Microphones https://amzn.to/3rzSZ9Z Second Microphone https://amzn.to/2ObKMeA XLR Cable https://amzn.to/3rBL8ZB Studio Headphones https://amzn.to/3u082LE Laptophttps://amzn.to/3dhfafT Our webcam https://amzn.to/31uUefQ RESOURCES FROM THIS EPISODE Adam Meakins Website - https://www.thesports.physio/ Twitter - https://twitter.com/AdamMeakins Instagram - https://www.instagram.com/adammeakins/ Mark Laslett Twitter - https://twitter.com/marklaslett_NZ ABOUT THE HOSTS Dave Elliott Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK.
