The sacroiliac joint is a very strong joint that takes the load of the torso from the vertebral column and sends it to the pelvis and lower limbs. It is a synovial joint that allows a little movement and is strongly supported by ligaments. Pain here is often caused by the joint being pulled too far by the large muscles that cross it or that move the pelvis.

Despite its limited range of movement, the sacroiliac joint is prone to instability and is often a source of back pain in patients. On this episode of Back Talk Doc, Dr. Sanjiv Lakhia and Tanner Holden, PT from Carolina Neurosurgery and Spine Associates discuss the SI joint itself, some common causes of SI joint pain, and what the latest science has to offer for management. Patients can experience SI joint pain during or after pregnancy, pain from the joint itself, or autoimmune pain. However, diagnosing SI joint pain is rarely straightforward because medical imaging is not typically helpful in diagnosing SI joint pain. The pain can present in unexpected places and is often best diagnosed with the help of pain provocation tests. Once diagnosed, patients can opt for regular pain-reducing injections for a quick solution. A more holistic treatment plan looks at patient goals and finds ways to improve movement over time. This may also include lifestyle modifications to remove the irritants or activities that cause the pain to flare up. Since SI joint pain sources vary from patient to patient, plans are customized to individual needs. Tanner believes that healthy movement, activity, and strength are key to living an active and pain-free life. He and Dr. Lakhia discuss how this approach puts the patient first and removes the need for unnecessary supports like braces and belts. Instead, a strength-based approach allows patients to take their care into their own hands, resulting in fewer medical visits.

Treatment For Sacroiliac (SI) Joint Pain Today on the podcast, host, Dr. Michael Danko talks about one of the most common causes of low back pain. Tune in to learn about the characteristics of sacroiliac (SI) joint pain and the different treatments that are available. Episode Highlights: The sacroiliac (SI) joint is a common contributor to low back pain. The SI joint is primarily designed for stability; It is a shock absorber for the forces that are transmitted from the spine into the extremities. It has a joint capsule and ligament structure around it.The joint is pretty complex where it is part of the kinetic chain that connects the spine with lower extremities. Dr. Danko explains what primary and secondary pain generator means. How do you experience problems with the SI joint? What are the main causes? Dr. Danko talks about the mechanics of the joint and how arthritis, trauma, and pregnancy are a few contributors. What other things can lead to SI pain? How does SI joint present? If you have a burning quality sensation or numbness and tingling that is most often a sign of nerve root compression or pinched nerve. There are different diagnostic maneuvers they can do in the eam room to determine if the SI joint is the cause of the pain. What medications are used to treat SI joint pain? Physical therapy can be helpful for the condition but if medication and physical therapy do not work, they can do injections to the joint. This is a restorative-y Yin with somatics thrown in for good measure. In my work with students and clients, I find that the sacroiliac joint is quite misunderstood. This really chill practice invites you to explore the anatomy of the hip as well as the movement available to you. It feels so amazing, especially if you get achy in your hips and low back. This is a practice you should bookmark if you often have issues with your hip/low back on one side. You will need a bolster, or a couple of pillows piled up, a couple of blocks and, if you are practicing on a hard floor, a blanket. Mark Laslett is a specialist physiotherapist who has extensive expertise and experience. He explains what the sacroiliac joint is, its function in the body and the varying ways the joint can become inflamed and a cause of pain. He also shares great content on the treatments that are used and the difference between sacroiliac dysfunction and sacroiliac joint pain. This episode is a great opportunity to understand more about the sacroiliac joint and how it can cause pain. KEY TAKEAWAYS The sacroiliac joint is in the centre of the body between the bottom of the spine and the big bones associated with your buttocks. Its sacroiliac joint doesn't have any muscles that move it, the muscles that affect it cause it to clamp together. It's not primarily a movement joint it's a joint that allows force to transferred from the limb to the spine with a shock-absorbing function. As you age the ability of the joint to move and absorb shock decreases as a normal part of the ageing process. There is a fundamental difference in the sacroiliac joint between male and female with women having more movement which increases during pregnancy. We don't know what happens during pregnancy but there can be pain in the sacroiliac joint at this time. In the non-pregnant population, the sacroiliac joint almost never gets sprained except when you experience major trauma. There are certain inflammatory conditions that cause pain in the sacroiliac joint but there are good methods for treating these conditions. There is a difference between sacroiliac joint dysfunction and sacroiliac joint pain. If you have true sacroiliac joint pain then manipulation will not make the pain go away.
