Send us a text if you want to be on the Podcast & explain why!A burning leg at night, a “herniated disc” on the MRI, and years of mixed messages—sound familiar? We unpack a real client story and show how to trade fear for a plan using assessment-first coaching, language that calms the system, and progressions that actually work.We start by debunking outdated ortho beliefs that frame backs as fragile and people as broken. Then we map a simple screening process to pinpoint intolerances—flexion, extension, rotation, lateral flexion, or compression—and use those findings to build a zero-discomfort movement menu. You'll hear why stretching into nerve symptoms often flares sciatica-like pain, when sciatic nerve glides help, and how to reframe “pain” as “discomfort” to reduce threat and increase buy-in.From there we get practical. Learn how to coach precise planks, side planks, and bridges; when to choose TRX rows over bent-over rows; how to modify pressing with landmine variations; and why bilateral pulling beats asymmetrical loading early on. We cover bracing and drawing-in cues, pacing volume to track responses, decompression options for compression-sensitive clients, and small lifestyle levers like sleep setup, hydration, and nutrition quality. We also explain imaging context, disc reabsorption, and the power of daily low back routines that rebuild trust. Finally, we share networking tactics to team up with pro-movement physios and chiros so your client always has a path forward. If you coach people with back issues—or you're working through them yourself—you'll leave with a step-by-step framework: listen, screen, find zero-discomfort ranges, progress deliberately, and keep the wins going for life. In this episode, we are joined by Dr. Stuart M. McGill, distinguished professor, emeritus of Spine Biomechanics at the University of Waterloo. With over 40 years of research and 245+ peer-reviewed publications, he has helped everyone from Olympic athletes to everyday people overcome back pain. Vikas & Dr. McGill discussed in detail about spine biomechanics, how the body works as an elastic system for running, why most runners don't need heavy deadlifts, and practical strategies to build a resilient, pain-free back. Here are some key takeaways:Distance runners are elastic athletes, not strength athletes - Great runners store and recover energy through tuned stiffness in their fascia and tendons, not by building maximum muscle strength. Deadlifts may actually hurt your running performance.Core endurance matters more than core strength - Maintaining good form throughout a marathon requires core endurance to prevent energy leaks, not maximal strength. Train your core.Sitting is damaging your back and your running - Eight hours sitting at a desk, builds cumulative damage that can't be undone by hard training at night. Take frequent micro-breaks at home or in office, to maintain back health.Barefoot running teaches optimal mechanics - Running without shoes forces you to find the sweet spot between absorbing shock and preserving elastic energy. Too much cushioning makes you slower and loads your tendons more.Sciatica can often be resolved without surgery - Most sciatica cases respond to understanding the pain mechanism and following proper movement patterns. Surgery should be considered only after conservative approaches have been thoroughly tried.About Vikas Singh:Vikas Singh, an MBA from Chicago Booth, worked at Goldman Sachs, Morgan Stanley, APGlobale, and Reliance before coming up with the idea of democratizing fitness knowledge and helping beginners get on a fitness journey. Does your sciatica seem to vanish for a few days, giving you hope that you are finally "fixed," only to return with a vengeance the moment you try to resume your normal life? This cycle of relief followed by relapse is incredibly common and deeply frustrating. In this session, we explain the critical difference between symptom relief (the absence of pain) and structural healing (the repair of the tissue). Understanding this distinction is the key to breaking the cycle of recurring back pain and preventing future flare-ups.Using the analogy of a grazed eyelid, we explore a vital concept: just because you can see perfectly clearly (function is 100%), it doesn't mean the wound on your eyelid has healed (structure is still damaged). In the same way, your back can often move pain-free while the disc is still fragile. We discuss the "Scab Phase"—that dangerous window where you feel capable but are clinically vulnerable. Learn why "testing" your back during this phase is exactly like picking a scab, and how to navigate this timeline safely to build true, long-term spinal resilience.

Sciatica pain stems from irritation of your sciatic nerve, the long nerve running from your lower back to your foot, and causes sharp pain, numbness, or tingling that makes even simple movements difficult Movement — not rest — is the key to recovery, because gentle activity increases blood flow, reduces inflammation, and helps the nerve heal faster Short walks, swimming, and simple stretches like knee-to-chest or the cobra pose relieve pressure on the nerve and prevent stiffness during flare-ups Daily habits such as limiting sitting time, improving posture, strengthening your core, and supporting your spine during sleep help stop sciatica from returning Most people recover within weeks when they stay active, use heat and cold strategically, and build strength in their hips and core to protect their lower back

Today's livestream starts with a deep dive into what “engaging your core” really means when you're rehabbing an L4/L5 or L5/S1 injury. Mike contrasts a lazy, bicycle-style dead bug with the properly braced version, shows how to use diaphragmatic breathing to brace without moving the spine, and explains why filming yourself from the side is essential to spot the tummy rising and then creeping back up as soon as the legs move. He stresses progressively tightening the brace as the legs travel further away, using the dead bug, marching bridge and everyday tasks as a way to practise variable core engagement rather than just going through the motions. In the Q&A, he tackles a run of common rehab sticking points: why Roman chair back extensions are essentially an expensive, less safe hip hinge, when to move from Phase One into Phase Two, and why single-leg raises and extreme seated hip hinges are usually a bad trade for people with disc injuries. There's detailed guidance on when to start rehab after a herniation or discectomy (basically now, but at an appropriate level), how to progress from low-hold squats and hinges with dumbbells towards barbell work, and how to troubleshoot hip hinges with bands when hamstrings or fear are in the way. He also addresses anxiety about MRIs, anterior pelvic tilt and cauda equina, uses his own eye injury as an analogy for why pain is a terrible measure of healing, and finishes by clarifying that Phase One is about mastering movement and reducing daily aggravation first, so that genuine strengthening in Phase Three and Four is both safe and effective.Highlights

Why Stretching Is Making Sciatica Worse?

This live session dives into a common problem for people with sciatica and lower back pain: why hamstring stretching can sometimes make everything feel worse. Mike explains how long-standing nerve irritation at L5/S1 and L4/5 can drive powerful hamstring tightness, how that tightness forces the lower back to round during everyday movements and stretches, and why the real issue is often weakness and loss of control rather than the stretch itself. You'll learn how to keep hamstring work aggravation-free using pain ratings, neutral spine and hand support, and why building spine stability with exercises such as the dead bug, marching bridge, squats and hip hinges is the foundation for better flexibility. He also answers member questions on step-up technique, whether squats are “bad” for the spine, how to reintroduce sitting, whether swimming helps, what to make of findings like lumbalisation and bone spurs, and how much pain is acceptable during rehab. Oscar's story of progressing to loaded squats and long, pain-free sitting shows what consistent, well-coached strength work can achieve over a few months. Broadcast from KSQD, Santa Cruz on 10-23-2025: Dr. Dawn opens with a passionate plea about E-bike safety after observing riders ignoring stop signs and wearing inadequate helmets in Santa Cruz. She explains the physics of collisions, noting that force equals mass times acceleration, and a car hitting an E-bike rider at 20 mph delivers impact equivalent to falling from a two-story building. She emphasizes that 97% of bike fatalities in New York involved helmetless riders, and brain injuries result from the brain striking the skull twice during impact - once on the impact side and again on the opposite side during deceleration. She urges drivers to honk at helmetless riders and calls for stricter helmet law enforcement. An emailer asks about hydroxyapatite in toothpaste. Dr. Dawn traces its origins to NASA research in the 1960s by Dr. Bernard Rubin studying crystal growth for preventing bone and tooth mineral loss in astronauts. Japanese company Sangi acquired the patent and created the first hydroxyapatite toothpaste by 1980, receiving official anti-cavity recognition in 1993. Studies show it matches fluoride's cavity prevention effectiveness by filling microscopic cracks where bacteria take root. It also relieves temperature sensitivity by sealing micro-fractures in enamel that expose the dentin layer, making it especially helpful for people who clench their jaws. Researchers from UC Berkeley and the Allen Institute used electrodes and lasers to study how mouse brains process optical illusions like the Kanizsa triangle. They discovered specialized IC encoder neurons in the visual cortex that fill in missing information, creating complete shapes from partial cues. When these pattern-completing circuits activate inappropriately, they may trigger hallucinations in conditions like schizophrenia. Dr. Dawn explains that illusions occur when the brain perceives something different from actual visual input, while hallucinations create perceptions with no external stimulus. She discusses frontotemporal dementia where visual hallucinations result from protein deposits in the occipital cortex, and notes that a 2021 British Journal of Psychiatry study found hallucination rates varying from 7% in young people to 3% in those over 70. An emailer describes unbearable chronic lumbar pain with degenerative disc disease shown on MRI. Dr. Dawn emphasizes that MRI findings don't necessarily correlate with pain levels, citing shopping mall studies showing equal degenerative changes in people with and without back pain. She stresses checking for sciatica symptoms including leg pain below the knee, sensory differences between sides, calf size asymmetry, and ability to walk on tiptoes and heels. Without these red flags, the degenerative disease likely isn't causing the pain. She warns against unnecessary surgery citing frequent "failed back" syndrome when surgery for a disk image doesn't "fix" the pain. She recommends water jogging with a ski vest, McKenzie exercises, abdominal strengthening, ergonomics, removing wallets from back pockets, and alternating heat and ice therapy. She discusses mindfulness meditation and cognitive behavioral therapy for pain management. A caller references Daniel Levitin's book "Your Brain on Music," discussing research using functional MRIs showing distinct brain activation patterns in musicians versus non-musicians due to integrated auditory, visual, and kinesthetic training. Dr. Dawn describes how infant brains develop from three to six layers with increasingly complex synaptic connections resembling circuit boards. She highlights a blindfold study where college students' visual cortices began responding to sound within two weeks as the auditory cortex expanded. She shares her husband's remarkable recovery demonstrating adult brain plasticity through intensive rehabilitation. Learning new musical instruments helps dementia patients by activating multiple brain regions simultaneously and improving standard cognitive test performance. A caller describes an eight-day chest cold with thick white phlegm. Dr. Dawn recommends guaifenesin as a mucus-thinning expectorant to prevent bacterial growth in respiratory secretions that serve as "bacteria chow." She emphasizes the importance of current flu, COVID, and RSV vaccinations. Secondary bacterial infections develop when bacteria colonize viral-induced mucus in the lungs and invade tissues. She advises aggressive hydration and chicken soup, which research shows helps clear mucus. Another caller provides additional information about Daniel Levitin as a musicologist, neurologist, and musician who runs the Music Perception, Cognition and Expertise laboratory at McGill University.

I was listening to your podcast Can you cheat death (ep. 872) and you say your doctor should check your cholesterol, crp levels and homocysteine levels.. could you please tell us what the ranges are? Normal healthy levels? Please and thank you and happy healing to everyone                                      Melanie: I have the Essure device implanted for Birth control sterilization. They have since recalled it. I would like to get it removed but it is a risky procedure. I don't have specific problems from it but I was wondering if it could be contributing to some health problems I don't realize?                                                                       Geoff: Hello! What would you do to heal as quickly as possible after a gum procedure? I'm having my gum recession fixed and have a few things lined up to aid the healing process. Here's what I'm planning on using: hydrogen water tablets (3x/day on an empty stomach), the "Healing like Wolverine" peptide stack (BPC-157, TB-500, GHK-Cu, Ipamorelin), along with daily infrared sauna sessions. I also have some C60 oil that I'm considering. What else would you do? Thank you so much for taking the time to answer my question!! You and your team are the best! Hope you have a great day!   Mohamed: Good day Doctor Cabral. I am big fan of your work and have utilized many of equilifes products. I also regularly watch your YouTube videos and podcasts. Your work is incredible. My question is regarding bilateral sciatica pain (Nerve pain) which lead to pondylitic changes with nerve root impingement. How can I address this. What do I need to avoid or utilize. I know B1 is good for Nerve related issues. Please weigh in. Thanks                                                                        Sheena:  Hi  Dr Cabral! Thank you so much for answering my question. I'm 45 and starting to feel the effects of perimenopause. Hot flashes everyday and night, cant stay asleep because of it, bad break outs, and irregular prolonged periods. Fortunately no vaginal dryness but I don't doubt it will come soon! My endocrinologist suggested I could take low-dose birth control pill and says it'll help. I'm hesitant in taking something synthetic everyday as I know it could come with long term effects. I have been taking this organic tincture for menopause that includes sage, black cohosh, burdock, chasetree, dongquai and motherwort, it was really helping for about a year, but not as much anymore. Which is why I'm considering birth control pill. What are your thoughts and recommendations? Then, they shift to the nighttime betrayals of the body: the mysterious transformation from human being to fragile pile of orthopedic complaints, where simply “sleeping” can result in injuries typically reserved for Olympic gymnastics.Along the way, we get hot tips from Tommy's therapist (shout-out to Bonnie!), dream science from the BBC, and Pete's personal musculoskeletal origin story—which involves surprise vertebra fusion, leg-length inequality, and an orthotic insert named Steve. There's also a powerful case made for the humble foam roller, a pony running off with Tommy's Mazda, and a rare Greek myth about moon-gazing lovers that ends, predictably, in a curse. If you've ever woken up more injured than you were when you went to bed, if you've ever texted your group chat just to be out-sympathied by someone who dislocated their shoulder sneezing, or if you've ever been haunted by the ghost of your own plantar fascia, this one's for you.Support the show at allthefeelings.fun and become a Feeling Friend today—for early episodes, bonus content, and a front-row seat to Pete's ever-expanding sneaker collection.

Sciatic nerve pain can be caused by a protruded disc, scar tissue on the nerve root, arthritis, an injury, or extended periods of sitting. Poor posture, excess driving, and aging can also contribute to the problem.Inversion uses the weight of your upper body to pull down on your lower back, creating negative pressure and increasing blood flow. This also breaks up adhesions and scar tissue. For the quickest lower back pain relief, try intermittent inversion traction. This involves inverting your body for 1 to 2 minutes and then repeatedly returning to an upright position. It can significantly reduce sciatic nerve pain by targeting the nerve, disc, scar tissue, and blood flow all at once.Research has shown that inversion therapy increases flexibility, reduces the risk of surgery for sciatica, and can relieve lower back pain better than medication. If you have high blood pressure, glaucoma, or heart problems, check with your doctor before doing inversion therapy. Start slow with inversion therapy. Begin with a gentle angle of 15–30 degrees for just 1–2 minutes, once or twice a day for the first week. Gradually work up to 5 minutes at a time (in 1-minute intervals) at a 60-degree angle over the next 1–2 weeks. After 3–4 weeks, you can try full inversion for 15–20 minutes, broken into 2-minute intervals, three to four times per week.Intermittent inversion traction will leave you feeling taller with less sciatic nerve pain. You'll also feel like you've had a fantastic stretch! Other ways to eliminate sciatica pain include:•Following a low-carb ketogenic diet•Take 20,000 to 30,000 IU of vitamin D3 and 400 mg of magnesium glycinate (include vitamin K2)•Eliminate omega-6 seed oils from your diet•Perform myofascial stretching You can also perform sciatica stretches without an inversion table! Try the child's pose with blankets, a wall-assisted leg-up stretch, or lying spinal decompression with a stability ball for sciatic nerve pain relief. Swimming can also be very therapeutic.
