International guidelines recommend targeting normocapnia in adults with coma resuscitated after out-of-hospital cardiac arrest. However, normocapnia may be insufficient to restore and maintain adequate cerebral perfusion. Conversely, mild hypercapnia increases cerebral blood flow and may improve neurologic outcomes. Nevertheless, the most effective Paco2 target in adults with coma resuscitated after out-of-hospital cardiac arrest has not been well studied in randomized trials. Therefore, the Targeted Therapeutic Mild Hypercapnia After Resuscitated Cardiac Arrest (TAME) trial was conducted to test the hypothesis that targeted mild hypercapnia would improve neurologic outcomes at 6 months as compared with targeted normocapnia in adults with coma who had been resuscitated after out-of-hospital cardiac arrest.Listen to the podcast for more insight into the hypothesis, methodology and findings from the TAME study. The aim of the ERC-ESICM guidelines on temperature control after cardiac arrest in adults is to provide evidence-based guidance in adults who are comatose after resuscitation from either in-hospital or out-of-hospital cardiac arrest, regardless of the underlying cardiac rhythm. These guidelines replace the recommendations on temperature management after cardiac arrest included in the 2021 post-resuscitation care guidelines co-issued by the European Resuscitation Council (ERC) and the European Society of Intensive Care Medicine (ESICM). In this network meta-analysis, as compared to normothermia (37–37.8 °C), we found that deep hypothermia (31–32 °C), moderate hypothermia (33–34 °C) and mild hypothermia (35–36 °C) may have no effect on survival and functional outcome among comatose survivors of out-of-hospital cardiac arrest. However, both deep and moderate hypothermia were associated with an increased risk of arrhythmia compared to normothermia. Welcome to Finance and Fury, the Furious Friday edition I have identified a thread through history in the emergence of demand side democratic societies. Where there is a centralized authority, there is a need for ever increasing demand of the mobs and its path towards the downfall. Rome is a good example: Centralized authority and the need for increasing demand of the mobs lead to the downfall of Rome It was a kingdom, then a democratic republic, and then a dictatorship During Sulla’s rule, there was a civil war, which made him strip Julia Caesar’s family of their influences Caesar was the High Priest of Jupiter and went into hiding to save his life Caesar joined the army and rose through the ranks, he also knew how to gain public support back in Rome Caesar was kidnapped and ransomed for 20 talents He then came back and had the pirates crucified By 60BC Caesar was elected consul however used this position to gain further power To build favour, he redistributed a lot of land to the poor and the soldiers of his wars The downfall of Rome and the Slave economy – remember that slavery was everywhere in the ancient world The import of free labour destabilised the economy Through the conquest of wars and territory, there were a lot of slaves This introduction of free labour replaced those in paid labour positions Caesar needed to keep slavery around but also keep himself popular with the Roman people, hence the land redistribution tactic. This was funded by his very wealthy friends The Senate then sent Caesar to the border of Gaul for a governorship – so he couldn’t get up to much Caesar has 4 legions under his command and ventured into the unconquered Gallic territory Some Gallic allies were defeated in a battle, so Caesar stepped up and hunted down the tribe As a response, some tribes unifying and arming up – Caesar took that as a sign of aggression and invaded without senate authority At one point, Caesar sold 53,000 people captured into slavery The number of senators that didn’t like him was growing, they thought he was a war criminal, but the Roman army loved him and so did the people The Senate wanted his resignation as governor, however, Caesar knew he would be prosecuted without his immunity as governor Caesar then pursued Pompey and ignited a civil war, eventually capturing Pompey Caesar is then appointed as a dictator for approximately 1 year He saw a need to bring the power back into a central authority The revolt of Italian tribes who wanted Roman citizenship, Caesar defeated them and gave the allies access to citizenship to avoid further wars Caesar had now gained supreme power through the mobilisation of the masses A new constitution was established to accomplish 3 goals To gain the right of citizenship you had to ‘live as the Romans did’ Caesar needed more money, so he opened the treasury Magistrates were no longer representatives of the people, they were representatives of the dictator The senators were involved in their communities and often held accountable for their actions by the people they represent and other senators The Economic side: Rome switched from Supply to Demand slowly over 40 years, everything changed from 1AD onwards Citizens were only really taxed during times of war The wealthy ones got citizenship because they were the ones doing the fighting and could afford the weapons, armour, and horses Each province has to pay a certain amount, someone would pay taxes up front and then go to the people to collect it back When Rome went to an Empire, the leaders need more money for increased spending and more men for the army The tax system was transformed into a system of individual income tax This increased spending led to an economic downturn of Rome The emperors made it illegal to leave Rome, and forced people to work Established family occupations This was the birth of feudalism and a monarchy again The ever spending of the state and increase of taxes led to the decline of Rome Moral of the story? Fueling demand through authoritarian controls never works Indentured servitude is started In the pursuit to stop more civil wars, Caesar ended up creating more They lost what made them great When you have a system that rewards popularity by demand, you fail Supply side isn’t some uncaring process, it gives the people choice The market decides, based around our wants and demands. Until regulation and controls influences come about, then there are unfair competition increases The emperors of Rome couldn’t afford not to spend money, their power was amassed by making the people happy Thank you for listening today. Gabriele Salerno aka T kode bern in the early 90’ in Rome (IT), approaches into music when he was teenage, every day he needed to hear music feel enveloped by a sound, a melody, a rhythm was something new and he liked, and when something you like doing. Soon he discovers the Electronic music starts high school attending partys in italy, but also in Europe only a few years before the aged, even up to live in Sydney, Australia where he lived for a year. The project started by Gabriele Salerno on March 2015 called “ T Kode “ was born from the idea of the music codes. The music have no rules and can’t have no rules, the music is your rhythm, is how you think it is, words, air, noise, there are no rules to compose it or play it, there are methods that everyone applies to his way of making music, to see music and play it. T kode as taken part in many events like resident or headliner not only in Rome’s clubs and Italians clubs but also foreign as Hungary (Budapest) at Dürer Kert, Spain (Madrid) at Sala Pirandello and Germany (Berlin) at Trésor Club (for wich came out madly in love) and Suicide Circus. T Kode in his own production start to do a careful selection of sounds that reflect the realities of Detroit and Chicago scene, also with references to the old school acid, with modern grooves arrangements. In the set of T Kode you can easily feel influenze by the productions of European and World renown artists such as: FUSE (Richie Hawtin), Belleville Three (Juan Atkins, Derrik May and Kevin Saunderson), Fanon Flowers, Developer, Blotnik Brothers, Mike Parker, Takaaki Itho and more.. Influenced also by the oldest productions of the early 90’s until the middle of 2000, research and selection of techno records with high bpm and frantic groove replicated in the productions of the eastern world techno scene. The first project ever has released in the second half of 2015 from “Blackwater Label” which open completely the doors to the business music marketing with the first Ep on vinyl W&P by T Kode featuring Lunatik called “Tunnel Ep” out September 29, 2015. During the end of 2015 and the beginning of 2016, after many collaborations on the ep’s and remixes made with the close friend Lunatik, T Kode started to written and performed his music by himself, 100% with his ideas and points of views and concept of Techno music. In the beginning of May 2016 T Kode announces the opening of its independent record label called "Stratosphera Records” and he has immediatly released a Digital Ep to lunching his new own label “Stratosphera Records" entitled “Spherical Ep" consists of two tracks: "Sphere1.0" and " Sphere2.0 “. From here T Kode imprinted Stratosphera Records totally with his concept of UNDERGROUND business about vinyls with limited edition copies, but sometimes there may be the opportunity to make even the digital release. The groove is the thing that T Kode loves and take care of more and we like to hear. T Kode also imprinted Stratosphera Records to the tracks with references of the past '90 years synths techno sound but modern Stratosphera Records only release Various Artists album, Various Artists album Remixes and the Ep’s, focus just on make vinyls releases but sometimes there may be the opportunity to make even the digital release. During May 2016 T Kode has released another track called Amorphiæ included in “PUNTI DI VISTA Various Artists Vol1” exclusive and limited project pressed only on Vinyl, sold out of stock and already repressed.
