The basis of this interview was the recent publication in the Journal of Science and Medicine in Sport of An Unusual Case of Marathon-related Exercise-associated Collapse: Case Report and Some Considerations for Medical Care at Endurance Mass Participation Events. We will explore the case and some empirically proven treatments that form the mainstay of treatment within EAC. We will also visit some of the reflections on the case and how it has changed Luca's thinking on the list of differentials within EAC. We also examine patient-focused research, intra-speciality collaboration and EAC collated databases. To do this I have Luca Carenzo with me. Luca is an Anaesthetic and Critical Care consultant from Milan, Italy. He previously worked at The Royal London Hospital Adult Critical Care Unit and has taken part in some mission work with Doctors without Borders. He is also a World Extreme Medicine faculty member. In this conversation, we will examine whether point-of-care ultrasound is a reliable predictor of outcome during Traumatic Cardiac Arrest (TCA). We will dig into the recently published systematic review on the topic with Tim Harris, who is one of the authors of the paper. We will examine why the authors decided to look at TCA and POCUS, the number of studies and patients examined, the type of Mechanism Of Injury (MOI) leading to TCA, what the results showed, and how this is relevant to practice. We also look at the other adjunctive markers in the pre-hospital domain to inform the Termination Of Resuscitation (TOR) based on the results found in this paper. We will also dig into another recently published paper related to the diagnostic performance of POCUS in resuscitative outcomes published in April of this year on a systematic review and meta-analysis of 3265 patients and outcomes with POCUS.     To do this I have Tim Harris with me. Tim is a Professor of Emergency Medicine and Programme Director for the Emergency and Resuscitation Medicine programme at the Blizard Institute, Queen Mary University of London. Unlock Your Brain’s Potential: How To Optimize Your Neurotransmitters

When writing about health and fitness, I often weave hormones into the discussion. I'm asked more about hormones than most other topics, as well. But there's another crucial category of communicators that deserves equal attention: neurotransmitters. These chemicals have a massive influence on how you feel, think, and even how you perform. They're often overlooked in mainstream health discussions, but understanding them can be a game-changer for your well-being. In this guide, we will dig deep into neurotransmitters: what they are, how they're different from hormones, and why you should care about them for your overall health. The Difference Between Hormones and Neurotransmitters Hormones are like the body's long-distance messengers. Produced by glands in the endocrine system, they travel through the bloodstream to deliver signals to various tissues and organs.Marieb, E. N., & Hoehn, K. (2018). Human Anatomy & Physiology (11th ed.). Pearson. They're behind a lot of what happens in your body, from controlling your metabolism to regulating your sleep cycles. Neurotransmitters, on the other hand, are more like local couriers. They operate mainly in the brain, helping neurons communicate with each other.Kandel, E. R., Schwartz, J. H., & Jessell, T. M. (2000). Principles of Neural Science (4th ed.). McGraw-Hill. Unlike hormones, they don't travel long distances. They make their impact in the tiny spaces between neurons, called synapses, and their effects are often immediate. Key Differences Speed of Action: Neurotransmitters work fast, almost instantly. Hormones usually take more time to show their effects. Range of Influence: Hormones can affect multiple systems throughout the body. Neurotransmitters usually have a more localized impact, especially within the brain. Regulatory Mechanisms: Hormones are typically regulated by feedback loops involving multiple organs. Neurotransmitters are primarily regulated within the nervous system. How Neurotransmitters Impact Your Health Physical Health Dopamine and norepinephrine aren't just about feeling good; they're about energy optimization. These neurotransmitters act in the brain's arousal system, influencing both wakefulness and alertness. Low levels can lead to symptoms akin to chronic fatigue syndrome, affecting not just your mood but also your physical stamina.Tumilty, S., et al. (2018). The effects of norepinephrine on hemodynamics and oxygen metabolism in hyperdynamic sepsis. Journal of Intensive Care Medicine, 33(6), 357-364. Physical Performance Acetylcholine is a neurotransmitter that you might not have heard of, but it's crucial for anyone interested in physical performance. It acts as the messenger between your nerves and muscles, facilitating the contraction that allows for movement. A deficiency in acetylcholine can lead to muscle weakness and decreased athletic performance.Deschenes, M. R., & Wilson, M. H. (2003). Age-related differences in synaptic plasticity following muscle unloading. The Journals of Gerontology Series A: Biological Sciences and Medical Sciences, 58(8), B678-B687. Mental Health When it comes to mood regulation, serotonin and dopamine are the big players. An imbalance in serotonin levels has been directly linked to mood disorders like depression and anxiety. Dopamine also plays a role in how we perceive pleasure and reward, which impacts our emotional well-being.Wise, R. A. (2004). Dopamine, learning and motivation. Nature Reviews Neuroscience, 5(6), 483-494. Acetylcholine doesn't just affect your muscles; it's also vital for your brain. It plays a significant role in attention, learning, and memory. Studies have found that decreased acetylcholine levels are associated with cognitive impairments, including those seen in Alzheimer's disease.Schliebs, R., & Arendt, T. (2011). The cholinergic system in aging and neuronal degeneration. Behavioural Brain Research, 221(2), 555-563.

Genius doctor, innovative scientist, researcher and inventor Dr. Todd Ovokaitys joins us for a mind-blowing conversation on his laser biophysics research and stem cell protocol using electromagnetic frequency to reverse biological age at the DNA level. By using vibrational frequency and no chemistry whatsoever, he has developed technology that can activate, upregulate and direct stem cells to enhance life force in a system, reducing viral loads and healing disease. He shares his fascinating journey and passion that lies beyond life extension into exploring full human potential. Besides the physical, how does his tech help resolve emotional conflict and enhance consciousness to engineer a new kind of human? With renewed physical vigor and extra biological time, the question then becomes are you up-leveling mentally, emotionally and spiritually? Continuing his work with frequency Dr. Todd created a movement known as Pineal Tones - a system of sounds based on ancient ways of knowing that are designed to assist with human balance, health and expansion of awareness. We also explore how his technology extends to animals and plants with major implications for farming and agriculture, and its potential for producing more high quality and nutrient-dense food. About Dr. Ovokaitys: Dr. Todd was first in his class in High School and at Northwestern University and was accepted only after 2 years of college to the premiere program at Johns Hopkins Medical School. He then did further training at Georgetown in Internal Medicine and Pulmonary and Intensive Care Medicine. He has become a leader in laser biophysics research and has developed a stem cell protocol that has reversed biological age at the DNA level more than any other method in medical history. He has been granted multiple patents in stem cells, nutraceuticals, pharmaceuticals, and even agriculture. Dr. Todd also composes chorale and instrumental symphonic pieces. The transcendental choirs he has created and produced all around the world, with up to 900+ choristers, rejuvenate the body while immensely boosting personal vibrational and awareness sates. The combination of the biomedical and esoteric reflect his passion for the full exploration and expansion of the human potential. Dr. Todd's inventions are bringing a new way of thinking about aging and medicine, based on the body's nutritional needs rather than pure chemical attempts to heal symptoms and issues. His contributions to the science of aging, advanced nutrition, and to solving issues of human disease, are outstanding. MedLink Neurology Podcast is delighted to feature selected episodes from BrainWaves, courtesy of James E Siegler MD, its originator and host. BrainWaves is an academic audio podcast whose mission is to educate medical providers through clinical cases and topical reviews in neurology, medicine, and the humanities, and episodes originally aired from 2016 to 2021.Originally released: April 25, 2019 For patients who survive cardiopulmonary arrest but do not wake up, neurologists are called to the bedside to prognosticate. And the question of “How much will this patient recover?” is not unlike “What is the benefit of aggressive therapy to await possible recovery?” In that way, you might consider neurologists to be the actuaries of hospital medicine. In this week's program, we review the clinical and diagnostic data that neurologists incorporate into their model for outcome prediction following anoxic brain injury. Produced by James E Siegler. Music courtesy of Swelling, Soft and Furious, Rafael Archangel, Lovira, and Dark Room. Sound effects by Mike Koenig and Daniel Simion. Sepsis occurs when the body's immune system responds to an infection and injures its own tissues and organs. It is a medical emergency, requiring early detection and treatment as it can lead to septic shock, multi-organ failure, and death. One in five deaths worldwide is associated with sepsis and 40% of cases are in children under five years of age. Sepsis is the number one cause of death in hospitals, the number one cause of hospital readmissions, and the number one healthcare cost. Laboratory testing is essential in helping prevent, detect, and treat sepsis to minimize injury to the body and the risk of death.   About Our Speaker:  Dr. Lui Forni is a Professor and Consultant Nephrologist and Intensivist at Royal Surrey County Hospital NHS Foundation Trust in Guildford, England. Professor Forni earned a PhD in physical chemistry and subsequently studied medicine, specializing in nephrology and intensive care medicine. His research interests include pre-operative assessment of high-risk surgical candidates, renal replacement therapy, diagnosis, pathophysiology and treatment of acute kidney injury, and predictive modeling in acute medical admissions. He lectures both nationally and internationally and has published widely.  He served as the Past Research Chair and is the Current Secretary of the European Society of Intensive Care Medicine.

"And the good news is that we may or may not have an answer to this fluid debate that's been running now for 25 years..." The Great Fluid Debate is a popular fixture at Evidence Based Perioperative Medicine (EBPOM) and this year that was once again the case. Are we genuinely starting to see evidence based practice emerging and finally knocking "eminence based" notions off the shelf? Presented by Monty Mythen and starting with a short presentation from Professor John A Myburgh, Intensive Care Medicine, University of New South Wales; Director of the Division of Critical Care and Trauma at the George Institute for Global Health and Senior Intensive Care Physician at the St George Hospital, Sydney.
