Ben Taub Hospital, located in the heart of Houston, Texas, is the city's largest hospital for those who cannot afford medical care. Texas, in turn, is the US state with the country's largest uninsured population. Amid chaotic emergency rooms and busy hospital wards serving the most financially and medically vulnerable people, Ricardo Nuila, MD finds meaning and beauty through stories he hears from his patients. In addition to his duties as a hospitalist at Ben Taub Hospital, Dr. Nuila is an associate professor of medicine at Baylor College of Medicine, where he directs the Humanities Expression and Arts Lab, as well as an author whose writings have appeared in the Atlantic, the New Yorker, the New York Times Sunday Review, and more.   Col (ret) Carmack is a fellowship-trained Orthopedic Trauma Surgeon who served in the Air Force.  He has deployment experience and has trained healthcare professionals at Military-Civilian Partnerships at Level 1 Trauma Centers.  He is currently the Physician Director of the Northern Light Orthopedic Service Line in Bangor, Maine.     In this episode, Dr. Carmack discusses his path into orthopedic surgery and how serving as a GMO impacted his career. He also discusses his role in two civilian-military partnerships, how it better prepared him and his team to manage combat casualties, and the importance of maintaining these mil-civ medical relationships to maintain readiness.     Dr. Carmack also discusses his experience in a deployed theatre and his most memorable cases from that time. We ask him for tips for non-orthopedic providers in caring for patients with orthopedic injuries.  He tells us what he thinks are some of the biggest advancements in trauma orthopedic care over the past twenty years. Finally, we discuss his decision to join the Air Force Reserves, what that experience was like, and how he earned an MBA supported by the Reserves.     Col Carmack is a graduate of UC Berkeley for his undergraduate studies, followed by Columbia University for his Medical Degree, where he joined the Air Force through the HPSP program. He subsequently did a General Surgery Internship at Wilford Hall Medical Center. After his internship, he served a tour as a General Medical Officer, serving as the Chief of Flight Medicine at Kelly Air Force Base. This was followed by an Orthopedic Surgery Residency at Wilford Hall Medical Center. He then completed an Orthopedic Trauma Fellowship at Wayne State University. While on active duty, he served in unique roles as an Orthopedic Trauma Surgeon at Ben Taub Hospital in Houston and as part of the C-STARS program at Shock Trauma in Baltimore, Maryland. He deployed to Iraq and Qatar and served at Landstuhl Medical Center. He left Active Duty after 13 years and joined the Air Force Reserves from which he retired in 2014. He previously chaired the Mayor's Red Ribbon Committee to address the Houston Fire Department Emergency Medical Service. He sat on the Hospital Subcommittee of the Mayor's Special Task Force on the Medical Aspects of Disaster. Currently, Dr. Mattox serves as a consultant to the Center for Biologic Evaluation and Research of the FDA. It turns out that I was mistaken about the role of the deputy that responded to the attack.  If you did not listen to the first episode dealing with this, you can listen by clicking here. My initial information was not clear so I assumed that the deputy was in the hospital also as hospital security. Not so. The hospital policy outlines that "security is the responsibility of the agency that brings the inmate to the hospital." The deputy was at the hospital on a detail that was not assigned to Hall. A deputy shot and wounded an inmate receiving care in Houston's Ben Taub Hospital Thursday morning after the patient obtained an 'edged weapon' and held it to a medical student's throat, according to a Houston Chronicle report. Source: Deputy shoots inmate in Ben Taub Hospital who held med student hostage The inmate, Ricky Lynn Hall, was in the hospital for evaluation after having seizures. Mr. Hall seized a female medical student and help a "sharp edged object" to her neck. An off duty deputy working at the hospital heard the medical student screaming from the floor above. Mr. Hall said he would kill the student if the security guard didn't shoot him. The security guard shot him So, this sounds like the inmate wanted "death by cop". The security guard (deputy) shot twice, at least got him once. Mr. Hall went through surgery and is reportedly still alive. Quite a few questions come up from this incident. Why was the medical student alone with an unrestrained inmate? Or, was she? Was any other security or persons in the room with her? How did the inmate get a "sharp edged object"? More questions than this will surface. Hospital violence is on the rise. Do you know what policies and procedures your facility have? Are your security guards armed? Is the training provided adequate? Is the hospital staff comfortable with the training that is provided? See, lots of questions. Violence exists. This is a fact. Instead of being reactionary to it, make a plan, implement the plan and practice the plan. We do not rise to the occasion, we fall to our base level of training. Let's all learn from the Ben Taub Hospital shooting. Stay safe.

She is a mentor to many past, present and future dental students as well.
