Au Soudan, la guerre continue de faire rage. Depuis deux ans, le conflit a fait des dizaines de milliers de morts et des millions de déplacés à travers tout le pays. Au-delà des appels au cessez-le-feu, ce sont désormais les alertes humanitaires qui se font le plus entendre. En plus de la malnutrition qui s'installe, l'épidémie de choléra s'étend, alors que l'aide humanitaire peine à entrer dans le pays. Entretien avec docteur Rodrigue Alitanou, directeur des opérations de l'ONG Alima. Il répond aux questions de Sidy Yansané.  RFI : À l'occasion de la journée mondiale de l'aide humanitaire mardi 19 août 2025, les Nations Unies indiquent un nombre record de 383 travailleurs humanitaires tués dans le monde l'an dernier. Le Soudan affiche le plus lourd bilan après Gaza. Est-ce qu'on peut dire que la guerre au Soudan est devenue l'une des pires crises humanitaires au monde ? Dr Rodrigue Alitanou : Absolument. Le Soudan aujourd'hui est considéré comme la pire crise humanitaire au monde selon les Nations Unies. Depuis le 15 avril 2023, où cette nouvelle guerre a commencé, elle a déjà fait des dizaines de milliers de personnes tuées et des dizaines de millions de personnes déplacées, avec aussi beaucoup d'humanitaires touchés par cette crise. L'ONU dénonce d'ailleurs « l'expression honteuse de l'inaction et de l'apathie internationales ». C'est un constat que vous partagez ?  Oui, parce que malgré toutes les difficultés déjà que les populations subissent au Soudan, l'acheminement de l'aide humanitaire n'est pas effectif et on a des difficultés à pouvoir envoyer les travailleurs humanitaires [sur le terrain], les approvisionnements pour les hôpitaux, et tout ce qui est nécessaire à ce que l'aide humanitaire soit effective dans les différentes régions. La semaine dernière, c'est l'Union européenne, conjointement avec le Japon, le Royaume-Uni et le Canada, qui appelait à laisser entrer l'aide humanitaire « de toute urgence » et à protéger les civils. Mais le même jour, le général al-Burhan s'engageait à ne faire « ni compromis ni réconciliation, quel qu'en soit le prix » avec les FSR de son adversaire Hemedti. Est-ce qu'il n'y a pas là un aveu d'impuissance de la communauté internationale ?  Sans faire de la politique, oui. Il y a des déclarations, mais il n'y a pas d'actions qui suivent de la part des différents belligérants. Il y a une forme d'impuissance, mais on ne peut pas baisser les bras, on ne doit pas baisser les bras, parce que l'enjeu est de taille. Si on ne fait rien, si l'aide humanitaire n'est pas acheminée dans les différentes régions en proie à la violence au conflit au Soudan, ce sont les populations qui vont en pâtir. En plus d'être sur le podium des États où meurent massivement les travailleurs humanitaires, le Soudan est désormais en tête des pays les plus touchés par le choléra. Comment l'épidémie a progressé aussi vite ? Avec cette crise qui avait jusqu'alors comme résultante l'insécurité alimentaire, la malnutrition, la famine par endroits, la situation est désormais exacerbée par cette épidémie de choléra sans précédent qui s'intensifie de semaine en semaine et touche des adultes comme des enfants, avec une vitesse de propagation assez fulgurante dans les camps de déplacés. J'étais sur place, j'ai vu des arrivées massives des populations dans des camps de déplacés déjà débordés, des populations exténuées par des atrocités et qui ont déjà un niveau d'immunité assez bas. Les populations affamées et assoiffées, parce qu'il y a des ressources très limitées, dorment à même le sol, serrées sous des toits de fortune. Des camps de déplacés caractérisés par une promiscuité totale, sans assainissement et sans suffisamment d'eau. Les gens vivent avec moins de trois litres d'eau par jour. Comme les latrines sont insuffisantes, il y a un phénomène de défécation à l'air libre qui s'est installé. Absolument tout est réuni pour que cette épidémie soit entretenue, spécifiquement dans les États du Darfour et notamment dans le Nord-Darfour, où le conflit a pris une autre dimension depuis mars. Cette épidémie risque d'être hors de contrôle si rien n'est fait. À lire aussiLe Soudan en guerre frappé par l'une des pires épidémies de choléra de son histoire La région du Darfour reste l'épicentre de l'épidémie, mais vos confrères de Médecins sans frontières craignent une propagation, comme vous, bien au-delà des camps de déplacés, au point de créer une combinaison mortelle avec la malnutrition qui frappe aussi le pays.  Effectivement, la situation est préoccupante dans les camps de déplacés, mais également dans les populations hôtes. Aujourd'hui, on parle de milliers de personnes exposées, notamment dans le Nord-Darfour avec 640 000 enfants menacés. Ce sont les chiffres de l'UNICEF de la semaine dernière. Il y a une propagation au-delà des camps qui est déjà effective, plus de 32 États sont aujourd'hui menacés ou atteints du choléra. La situation dans la ville d'El-Facher est particulièrement grave, car elle est sous contrôle de l'armée régulière, mais encerclée par les paramilitaires FSR qui empêchent les convois humanitaires d'y entrer. À votre connaissance, combien de personnes y vivent actuellement et surtout comment survivent-elles ? La ville d'El-Facher est effectivement particulièrement sous tension parce qu'elle est assiégée depuis les affrontements qui ont commencé en avril et qui ont vu migrer de la ville des dizaines de milliers de personnes vers Tawila. Environ un million de personnes sont assiégées dans cette ville où tout manque. Les populations n'ont pas de voies d'approvisionnement, il y a plus de 70% des hôpitaux qui ne fonctionnent pas, l'accès aux soins est délétère. C'est aussi le cas de l'accès aux services de base, tout ce qui est nourriture et eau potable. Des centaines de milliers de personnes sont exposées à cette situation. Et les habitants, les témoignages que RFI a pu recueillir parlent d'enfants qui meurent quotidiennement de malnutrition ou de mauvaises conditions de vie... Exactement. Et c'est la résultante directe de cette guerre. On parle de près de 20 % des enfants souffrant de malnutrition, notamment dans des zones fermées, sans accès aux soins déjà pour la malnutrition. Le choléra vient maintenant s'ajouter et affecte la majorité des personnes, dont les enfants qui sont les couches les plus vulnérables. Vous avez indiqué qu'entre 70 et 80 % des hôpitaux et des centres médicaux du pays sont détruits. Ils sont même systématiquement visés par les belligérants, est-ce que vous arrivez à comprendre pourquoi les belligérants visent systématiquement les infrastructures médicales ? Nous sommes face à une guerre aveugle où les populations, malheureusement, sont devenues des cibles, tout comme les travailleurs humanitaires. Aujourd'hui, plusieurs centres de santé sont attaqués, particulièrement chez Alima. On a été obligés à maintes reprises de relocaliser nos activités, notamment à El-Facher où aujourd'hui, on n'arrive plus à travailler dans l'hôpital général. On s'est déplacé dans un autre endroit, qui a été aussi bombardé à plusieurs reprises, donc on a dû se déplacer de nouveau. Mais ce n'est pas seulement les infrastructures de santé, même les maisons des populations sont touchées. Cela montre clairement la volonté des différents groupes à prendre en étau les populations civiles dans cette guerre aveugle. Si rien n'est fait, le choléra aujourd'hui risque de tuer plus que les armes au Nord-Darfour. Dernièrement, j'ai rencontré sur le terrain une femme enceinte qui a marché 75 km, elle a dû accoucher à son arrivée. Et j'imagine aujourd'hui cette femme qui a pu survivre à toutes ces atrocités, contracter le choléra, fatal aussi bien pour elle que son enfant. Il faut faire tout son possible pour éviter ce risque à tout prix. D'autant que pour résorber le choléra, il faut beaucoup boire ou alors il faut des antibiotiques. Donc ça a l'air d'être un traitement simple, mais pour autant les populations manquent de tout cela… D'une part les financements sont très minimes et d'autre part les voies d'approvisionnement des différentes zones où sévit le choléra aujourd'hui sont coupées par la guerre. C'est un cocktail qui fait que le choléra risque de l'emporter.  Quel a été l'impact du retrait des financements de l'aide américaine d'USAID ? L'impact a été très très sec. Déjà en février, plus de 60 % des organisations humanitaires ont dû arrêter brusquement leurs activités et ça a eu un impact sur le déploiement des quelques aides qui arrivent à venir au Soudan. Depuis cette épidémie, on voit encore plus l'impact puisqu'on n'est que très peu d'organisations pouvant opérer et déployer des soins pour les populations. On a intérêt à tous se mobiliser pour que les financements puissent suivre assez rapidement puisque rien que pour le choléra, nous enregistrons aujourd'hui plus de 1000 morts depuis le début de l'épidémie au Soudan cette année. While writing Old Grandmother's Tree, Joseph took a sabbatical from the U.S. Air Force and taught mathematics to young students for a semester at Holy Family Academy in Gardner Massachusetts.Bolton is of French-Canadian, Native American, Spanish, English, and Irish descent, and is profoundly inspired by the stories of his heritage.
